2. Percutaneous Transluminal Angioplasty for Atherosclerotic Stenosis of Vertebral Artery Origin.

Abstract

Background: In patients with vertebral artery origin (VAO) stenosis and concomitant stenoses of other cerebral feeding arteries, data on the risk of percutaneous transluminal angioplasty (PTA) alone and with stent placement (PTAS) for VAO stenosis are limited. We aimed to determine how the presence of polystenotic lesions in other cerebral feeding arteries and concomitant carotid artery stenting (CAS) affect the periprocedural risk and long-term effect of PTA/S for atherosclerotic VAO stenosis. Methods: In a retrospective descriptive study, consecutive patients treated with PTA/S for ≥70% VAO stenosis were divided into groups with isolated VAO stenosis and multiple stenoses. We investigated the rate of periprocedural complications in the first 72 h and the risk of restenosis and ischemic stroke (IS)/transient ischemic attack (TIA) during the follow-up period. Results: In a set of 66 patients aged 66.1 ± 9.1 years, polystenotic lesions were present in 56 (84.8%) patients. 21 (31.8%) patients underwent endovascular treatment for stenosis of one or more other arteries in addition to VAO stenosis (15 underwent CAS). During the periprocedural period, no patient suffered from an IS or died, and, in the polystenotic group with concomitant CAS, there was one case of TIA (1.6%). During a mean follow-up period of 36 months, we identified 8 cases (16.3%) of ≥50% asymptomatic VA restenosis, and, in the polystenotic group, 4 (8.9%) cases of IS. Conclusion: The presence of severe polystenotic lesions or concomitant CAS had no adverse effect on the overall low periprocedural risk of PTA/S of VAO stenosis or the risk of restenosis during the follow-up period. Abstract

Background and ObjectivesCOVID-19–related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19.MethodsThis was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection. With a doubly robust model combining propensity score weighting and multivariate regression, we studied the association of COVID-19 with intracranial bleeding complications and clinical outcomes. Subgroup analyses were performed according to treatment groups (IVT-only and EVT).ResultsOf a total of 15,128 included patients from 105 centers, 853 (5.6%) were diagnosed with COVID-19; of those, 5,848 (38.7%) patients received IVT-only and 9,280 (61.3%) EVT (with or without IVT). Patients with COVID-19 had a higher rate of symptomatic intracerebral hemorrhage (SICH) (adjusted OR 1.53; 95% CI 1.16–2.01), symptomatic subarachnoid hemorrhage (SSAH) (OR 1.80; 95% CI 1.20–2.69), SICH and/or SSAH combined (OR 1.56; 95% CI 1.23–1.99), 24-hour mortality (OR 2.47; 95% CI 1.58–3.86), and 3-month mortality (OR 1.88; 95% CI 1.52–2.33). Patients with COVID-19 also had an unfavorable shift in the distribution of the modified Rankin score at 3 months (OR 1.42; 95% CI 1.26–1.60).DiscussionPatients with AIS and COVID-19 showed higher rates of intracranial bleeding complications and worse clinical outcomes after revascularization treatments than contemporaneous non–COVID-19 patients receiving treatment. Current available data do not allow direct conclusions to be drawn on the effectiveness of revascularization treatments in patients with COVID-19 or to establish different treatment recommendations in this subgroup of patients with ischemic stroke. Abstract

The authors present an overview of the trend in endovascular procedure rates for ischemic stroke during the last six years. Mechanical thrombectomy (MT) gained the highest level of evidence in 2015 when positive results of several studies comparing the clinical outcome of MT with intravenous thrombolysis using recombinant tissue plasminogen activator were published. MT has become the most frequent endovascular therapeutic procedure on cerebral vessels. In order to provide this therapy, there has been an active geographical system of qualified cerebrovascular centers at two levels in the Czech Republic since 2010. The authors give a survey of possibilities how to increase the effectiveness of the existing system in terms of both quantitative and qualitative aspects. [ABSTRACT FROM AUTHOR]

Abstract

Background and Purpose- We aimed to determine the safety and mortality after mechanical thrombectomy in patients taking vitamin K antagonists (VKAs) or direct oral anticoagulants (DOACs). Methods- In a multicenter observational cohort study, we used multiple logistic regression analysis to evaluate associations of symptomatic intracranial hemorrhage (sICH) with VKA or DOAC prescription before thrombectomy as compared with no anticoagulation. The primary outcomes were the rate of sICH and all-cause mortality at 90 days, incorporating sensitivity analysis regarding confirmed therapeutic anticoagulation. Additionally, we performed a systematic review and meta-analysis of literature on this topic. Results- Altogether, 1932 patients were included (VKA, n=222; DOAC, n=98; no anticoagulation, n=1612); median age, 74 years (interquartile range, 62-82); 49.6% women. VKA prescription was associated with increased odds for sICH and mortality (adjusted odds ratio [aOR], 2.55 [95% CI, 1.35-4.84] and 1.64 [95% CI, 1.09-2.47]) as compared with the control group, whereas no association with DOAC intake was observed (aOR, 0.98 [95% CI, 0.29-3.35] and 1.35 [95% CI, 0.72-2.53]). Sensitivity analyses considering only patients within the confirmed therapeutic anticoagulation range did not alter the findings. A study-level meta-analysis incorporating data from 7462 patients (855 VKAs, 318 DOACs, and 6289 controls) from 15 observational cohorts corroborated these observations, yielding an increased rate of sICH in VKA patients (aOR, 1.62 [95% CI, 1.22-2.17]) but not in DOAC patients (aOR, 1.03 [95% CI, 0.60-1.80]). Conclusions- Patients taking VKA have an increased risk of sICH and mortality after mechanical thrombectomy. The lower risk of sICH associated with DOAC may also be noticeable in the acute setting. The paper deals with a relatively new methode of treatment - Thrombolysis in neurology. It's composed of a theoretical partand a practical one. In the theoretical part there is golbal information about this methode, its indications, its contraindications, atc. Thrombolysis is a methode of treatment used at acute phase of illness - ischeamic stroke. That's why the characteristic of these illness, its diagnostics, its treatment, atc. are included in this part. The practical part is based on quantitative research. Explored group was composed of patients suffering from stroke and hospitalisated on Intensive Care Unit on Neurological Clinic in Fakultní nemocnice v Hradci Králové. In research there was explored how many times the thrombolysis had been aplied, the reason why it hadn't been aplied and the amount. There were explored the average age and the average lenght of hospitalisation too.

Abstract

The thesis is focused on clients who had a cerebrovascular accident and on their relatives who care for them. It describes the changes which clients and their relatives experience. It explains the terms "need", "social services", "nursing care". The importance of taking care for clients at home is emphasized. The research was carried out by the method of questionnaires with family members of hospitalized patients. The aim of the investigation was to establish the possibilities of home nursing of patients with a cerebrovascular accident. The investigation was executed at the neurological part of the multi-branch intensive care unit at the Regional Hospital in Karlovy Vary, at the hospital in Sokolov.

Abstract

The benefit of mechanical thrombectomy over intravenous thrombolysis was definitively proved by several studies in 2015. It is relevant for proximal, large artery occlusions in the anterior circulation with onset of clinical symptoms lasting 6 hours. Two trials published in 2018 showed that in patients who are selected using CT perfusion or MR diffusion weighted imaging, thrombectomy significantly improves outcome even up to 24 hours from onset. The benefit of the vertebrobasilar circulation, where occlusion of large arteries occurs in 7 % cases, has not been proved by randomised studies yet. However, mechanical thrombectomy is performed in this arterial territory because of very poor prognosis of nonrecanalized occlusions there. Patients with acute ischemic stroke should be transferred to comprehensive stroke centres. These centres should provide parenchymal CT imaging and CT arteriography. These two modalities provide sufficient information to determine eligibility for endovascular treatment in the first 6 hours from stroke onset. Physicians performing endovascular therapy of stroke must be properly trained in clinical neuroscience, neuroimaging and neurointerventions. Success rate of this therapy depends on its organization which includes fast transport of patients, fast clinical and diagnostic evaluation, fast decisions, and availability of trained interventional team.
