Introduction: As the average lifespan becomes longer, the number of cases of chronic shin ulcers is rising, which is slowly becoming a societal problem. Currently, there no methods for objective evaluation of ulcer healing process. Aim of the study: The aim of the study was to assess the use of high-frequency ultrasound to monitor the healing of shin ulcers subjected to laser biostimulation. Material and methods: The study included 8 patients (4 men and 4 women) aged from 46 to 81 years with 9 shin ulcers in whom the healing process did not progress within 8 weeks despite effective causal treatment. The ulcers were subjected to laser biostimulation. The induced changes were monitored using high-frequency ultrasound and photographic records. Ultrasound examination was used to assess the depth of ulceration and the thickness of granulation tissue, migrating epidermis, subepidermal low-echogenic band (SLEB) and epidermis in the immediate vicinity. The wound severity index and ulcer vascularity were also assessed. Results: As a result of laser therapy, in 7 out of 9 cases the ulcers healed completely and in the remaining 2 cases ulcer size was reduced. During the ulcer healing process induced by laser therapy, ultrasound examination revealed decreasing ulcer depth, wound severity index and SLEB thickness. Granulation tissue increased in thickness and there was an epidermal formation effect (newly formed epidermis was much thicker than the one surrounding the wound and became thicker as a result of laser therapy sessions). During the healing of ulcers induced by laser therapy no significant changes were observed in ulcer vascularity and thickness of the surrounding epidermis. Conclusions: High-frequency ultrasound is a useful technique for the imaging of leg ulcers. It allows one to monitor their healing process induced by laser biostimulation. Further research on the subject needs to be pursued.

Methods: Decision modeling was used to estimate the probability of cow’s milk allergic infants developing tolerance to cow’s milk by 18 months. The model also estimated the cost to the NFZ and parents (Polish Zloty [PLN] at 2013–2014 prices) for managing infants over 18 months after starting one of the formulas as well as the relative cost-effectiveness of each of the formulas. Results: The probability of developing tolerance to cow’s milk by 18 months was higher among infants who were fed eHCF + LGG (0.82) compared with those fed eHCF alone (0.53) or an AAF (0.22). An infant who is initially managed with eHCF + LGG is expected to consume fewer health care resources than infants managed with the other formulas. Hence, the estimated total health care cost incurred by the NFZ for initially feeding infants with eHCF + LGG (PLN 5,693) was less than that of feeding infants with eHCF alone (PLN 7,749) or an AAF (PLN 24,333). However, the total cost incurred by parents for initially feeding infants with an AAF (PLN 815) was marginally less than that of feeding with eHCF + LGG (PLN 993), which was less than that of feeding with eHCF alone (PLN 1,226). Conclusion: Using eHCF + LGG instead of eHCF alone or an AAF for first-line management of newly diagnosed infants with cow’s milk allergy affords a cost-effective use of NFZ-funded resources, since it improves outcome for less cost. Whether eHCF + LGG would be viewed as being cost-effective by parents is dependent on their willingness to pay an additional cost for additional tolerance acquisition to cow’s milk. The thesis is a polemic against Jan Mukařovský’s method of structural analysis of acting. The author accuses him of excluding the actor’s subjectivity as well as the psychological factor. Instead, he is proposing to extend the reasoning with the theory of communication psychology — a field of study dealing with the relationship between emotions and their articulation. In the first part, Mukařovsky’s conclusions are confronted with communication psychology. The second one describes the representation of female hysteria in film as an example of acting that is resistant to structural analysis.

PurposeThe ongoing lack of data standardization severely undermines the potential for automated learning from the vast amount of information routinely archived in electronic health records (EHRs), Radiation Oncology Information Systems (ROIS), treatment planning systems (TPSs), and other cancer care and outcomes databases. The effort sought to create a standardized ontology for clinical data, social determinants of health (SDOH), and other radiation oncology concepts and interrelationships.Methods and materialsThe American Association of Physicists in Medicine's (AAPM's) Big Data Science Committee (BDSC) was initiated July of 2019 to explore common ground from the stakeholders' collective experience of issues that typically compromise the formation of large inter- and intra- institutional databases from EHRs. The BDSC adopted an iterative, cyclical approach to engaging stakeholders beyond its membership to optimize the integration of diverse perspectives from the community.ResultsWe developed the Operational Ontology for Oncology (O3) which identified 42 key elements, 359 attributes, 144 value sets, and 155 relationships ranked in relative importance of clinical significance, likelihood of availability in EHRs, or the ability to modify routine clinical processes to permit aggregation. Recommendations are provided for best use and development of the O3 to four constituencies: device manufacturers, centers of clinical care, researchers, and professional societies.ConclusionsO3 is designed to extend and interoperate with existing global infrastructure and data science standards. The implementation of these recommendations will lower the barriers for aggregation of information that could be used creating large, representative, findable, accessible, interoperable and reusable (FAIR) datasets supporting the scientific objectives of grant programs. The construction of comprehensive "real world" datasets and application of advanced analytic techniques, including artificial intelligence (AI), holds the potential to revolutionize patient management and improve outcomes by leveraging increased access to information derived from larger, more representative datasets.

This article is devoted to the practical application of CFD Computational Fluid Dynamics software, as a tool helpful in the explosive hazardous area classification. The practical examples will be discussed, which show that use of computer software for the simulation of gas emissions or the simulation of vapors of flammable liquids, allows not only visualization, but also improvement the quality of following conclusions received during the explosive hazardous area classification. The Computational Fluid Dynamics is a branch of physics which deals with solving transport equations for given boundary conditions using numerical methods. The solution is found by dividing the analyzed space into a specified number of small control volumes (cells). At the same time, time period is divided into short steps. Then, for each of the cells at each time step, the transport equations are converted into the system of linear equations. Such systems are solved using appropriate numerical algorithms.

Introduction: Right heart changes and their association with exercise capacity, including sex differences, are still being investigated. We analysed right heart structure and its relationship with exercise capacity parameters in amateur cyclists.Materials and methods: A cross-sectional study involving 215 consecutive adult amateur cyclists, who underwent resting transthoracic echocardiography and a cardiopulmonary exercise test (CPET) to exhaustion was performed.Results: The median age of participants was 29 years (IQR 24–37), 71% of them were men. The mean training time was 6 h/week, and 90% participated in vigorous or moderate physical activity. Men had larger right ventricular diameters (basal - RVD1, mid-cavity - RVD2 and longitudinal - RVD3) compared to women (40.9 vs. 37.6 mm, p = 0.0005, 28.7 vs. 26.3 mm, p = 0.03, 92.2 vs. 81.9 mm, p < 0.0001). Indexing for body surface area revealed comparable right atrial volume (RAVI) between sexes (24.1 vs. 22.7 mL/m2). Men achieved higher peak exercise capacity parameters [O2 pulse, oxygen consumption (VO2) and workload] in CPET. Multivariate linear regression models revealed a positive association between peak VO2, workload and O2 pulse with RAVI in women but not with RVD1 or RVD3. Conversely, these parameters showed a positive association with RVD3 and RVD1 but not with RAVI in men.Conclusion: Sex differences exist in the relationship between right heart structural parameters and peak exercise capacity descriptors in amateur cyclists. Better exercise capacity during CPET to exhaustion is associated with greater RAVI in women but a greater RVD1 and RVD3 in men. These findings suggest different mechanisms of right heart adaptation to training in men and women.

Radiation-induced gastrointestinal (GI) dose constraints are still a matter of concern with the ongoing evolution of patient outcomes and treatment-related toxicity in the era of image-guided intensity-modulated radiation therapy (IMRT), stereotactic ablative radiotherapy (SABR), and novel systemic agents. Small bowel (SB) dose constraints in pelvic radiotherapy (RT) are a critical aspect of treatment planning, and prospective data to support them are scarce. Previous and current guidelines are based on retrospective data and experts’ opinions. Patient-related factors, including genetic, biological, and clinical features and systemic management, modulate toxicity. Omic and microbiome alterations between patients receiving RT to the SB may aid in the identification of patients at risk and real-time identification of acute and late toxicity. Actionable biomarkers may represent a pragmatic approach to translating findings into personalized treatment with biologically optimized dose escalation, given the mitigation of the understood risk. Biomarkers grounded in the genome, transcriptome, proteome, and microbiome should undergo analysis in trials that employ, R.T. Bioinformatic templates will be needed to help advance data collection, aggregation, and analysis, and eventually, decision making with respect to dose constraints in the modern RT era.

Abstract A new method for the determination of main glucocorticoids (cortisol, cortisone, and corticosterone) in hair by liquid chromatography-tandem mass spectrometry was developed. Glucocorticoids were extracted from hair shafts using methanol followed by solid-phase extraction. A validation test was performed using hair from three species of wild mammals with different body size (0.2–800 kg), lifestyle (terrestrial, burrowing and arboreal species), social organization (living in herds or solitary), and different predicted type of hair glucocorticoids: European bison (Bison bonasus), European hamster (Cricetus cricetus), and Eurasian red squirrel (Sciurus vulgaris). Abstract Accumulation of medicinally important flavones and acteoside was evaluated in Scutellaria lateriflora hairy root cultures subjected to different experimental strategies – feeding with precursors of phenolics biosynthesis (phenylalanine, cinnamic acid, and sodium cinnamate), addition of elicitors (chitosan, jasmonic acid) and Amberlite XAD-4 and XAD-7 resins and permeabilization with dimethyl sulfoxide (DMSO) and methanol. The production profile of S. lateriflora cultures changed under the influence of the applied strategies. Hairy roots of S. lateriflora were found to be a rich source of wogonoside or wogonin, depending on the treatment used. The addition of sodium cinnamate (1.0 mg/L) was the most effective approach to provide high production of flavonoids, especially wogonoside (4.41% dry weight /DW/; 566.78 mg/L). Permeabilization with DMSO (2 µg/ml for 12 h) or methanol (30% for 12 h) resulted in high biosynthesis of wogonin (299.77 mg/L and 274.03 mg/L, respectively). The obtained results provide new insight into the selection of the optimal growth conditions for the production of in vitro biomass with a significant level of flavone accumulation. The data may be valuable for designing large-scale cultivation systems of hairy roots of S. lateriflora with high productivity of bioactive compounds – wogonin or wogonoside.

In this study, based on the identification of similarity factors of the national business cultures of Poland, Lithuania and Ukraine, the hypothesis was formulated and verified concerning the formation of premises for a new management style – Eastern European management as a synthesis of Western and Eastern management. Using the measurement tools of modern economic comparative analysis, an analysis of the business cultures of these countries was conducted. The feature system was built by comparing western and eastern conceptual management approaches and practices. It has been proven that the great proximity (similarity) of the national business cultures of Poland, Lithuania and Ukraine creates conditions for optimal combination of the advantages of Western and Eastern management.

Abstract European rivers are disconnected by more than one million man-made barriers that physically limit aquatic species migration and contribute to modification of freshwater habitats. Here, a Conceptual Habitat Alteration Model for Ponding is developed to aid in evaluating the effects of impoundments on fish habitats. Fish communities present in rivers with low human impact and their broad environmental settings enable classification of European rivers into 15 macrohabitat types. These classifications, together with the estimated fish sensitivity to alteration of their habitat are used for assessing the impacts of six main barrier types (dams, weirs, sluices, culverts, fords, and ramps). Our results indicate that over 200,000 km or 10% of previously free-flowing river habitat has been altered due to impoundments. Although they appear less frequently, dams, weirs and sluices cause much more habitat alteration than the other types. Their impact is regionally diverse, which is a function of barrier height, type and density, as well as biogeographical location. This work allows us to foresee what potential environmental gain or loss can be expected with planned barrier management actions in rivers, and to prioritize management actions.

Biomarkers for resistance in Glioblastoma multiforme (GBM) are lacking, and progress in the clinic has been slow to arrive. CD133 (prominin-1) is a membrane-bound glycoprotein on the surface of cancer stem cells (CSCs) that has been associated with poor prognosis, therapy resistance, and tumor recurrence in GBM. Due to its connection to CSCs, to which tumor resistance and recurrence have been partially attributed in GBM, there is a growing field of research revolving around the potential role of CD133 in each of these processes. However, despite encouraging results in vitro and in vivo, the biological interplay of CD133 with these components is still unclear, causing a lack of clinical application. In parallel, omic data from biospecimens that include CD133 are beginning to emerge, increasing the importance of understanding CD133 for the effective use of these highly dimensional data sets. Given the significant mechanistic overlap, prioritization of the most robust findings is necessary to optimize the transition of CD133 to clinical applications using patient-derived biospecimens. As a result, this review aims to compile and analyze the current research regarding CD133 as a functional unit in GBM, exploring its connections to prognosis, the tumor microenvironment, tumor resistance, and tumor recurrence.

Abstract Median sternotomy is the surgical method of choice for many procedures where one of the main problems is the long post-operative wound healing process leading to sternal dehiscence and the development of infection. This leads to prolonged hospital stay and increased mortality due to post-operative complications. A promising solution seems to be the use of allogeneic chondrocytes for wound treatment, whose properties in the field of cartilage reconstruction are widely used in medicine, mainly in orthopedics. In the present study, we investigated the effect of local delivery of allogeneic chondrocytes on the biological response and healing of the sternum after sternotomy. We optimized the culture conditions for the isolated chondrocytes, which were then applied to the sternal incision wound. Chondrocytes in the culture were assessed on the basis of the presence of chondrocyte-specific genes: Sox9, Aggrecan and Collagen II. In turn, the histopathological and immunohistochemical evaluation was used to assess the safety of implantation. In our work, we demonstrated the possibility of obtaining a viable culture of chondrocytes, which were successfully introduced into the sternal wound after sternotomy. Importantly, implantation of allogeneic chondrocytes showed no significant side effects. The obtained results open new possibilities for research on the use of allogeneic chondrocytes in the process of accelerating wound healing after median sternotomy.

Due to the hostilities, the largest losses in the agricultural sector of Ukraine’s economy were caused by the loss of agricultural machinery – 20.9% of it was destroyed or damaged. Therefore, ensuring the effective functioning of the market for agricultural machinery is a key factor in restoring the material and technical base of agricultural enterprises. The research aims to investigate the current state of the agricultural machinery market and the impact of military operations on it and to identify effective ways of its development. The following methods were used in the study: dialectical method of scientific knowledge, monographic, statistical, and economic, economic and mathematical, expert evaluation, and graphical. The general trend towards a decrease in the share of households with certain types of machinery in 2015-2022 is determined. Disproportions between large and small forms of management in the provision of machinery, as well as restrictions on access to financial resources for small agricultural enterprises, have been identified. The study identified a general decline in the size of the Ukrainian machinery market due to the loss of production capacity in the east and south of the country. Imports of agricultural machinery – tractors by 57.0%, combine harvesters by 30.0%, seeders by 33.8%, disc harrows by 45.1%, and cultivators by 38.9% – also decreased significantly in quantitative terms. The analysis of state support for agricultural machinery and agricultural enterprises is carried out. A forecast of the development of the agricultural machinery market for crop production under positive and negative scenarios, including risks and challenges that may arise during and after military operations, is made. Recommendations for the development of the market of agricultural machinery for crop production have been developed, including implementation of the state’s protectionist policy; preservation of human resources; evacuation of agricultural machinery enterprises from the war zone; simplification of business to stimulate investment activity; state support for small farms, etc. The implementation of these recommendations will help restore the effective functioning of the agricultural machinery market and the logistics of agricultural production in Ukraine

Acute respiratory infections are the most common reason to seek medical attention in primary care. The infections are mostly of viral aetiology, and preparations containing herbal substances are used in symptomatic treatment. This paper reviews single and combination herbal medicinal products used for dry and wet (nonproductive and productive) cough. The therapeutic effects of individual herbal substances are discussed regarding the presence of specific biologically active compounds, taking into account the results of clinical and application studies.
