Prolonged exposure to environmental respirable toxicants can lead to the development and worsening of severe respiratory diseases such as asthma, chronic obstructive pulmonary disease (COPD) and fibrosis. The limited number of FDA-approved inhaled drugs for these serious lung conditions has led to a shift from in vivo towards the use of alternative in vitro human-relevant models to better predict the toxicity of inhaled particles in preclinical research. While there are several inhalation exposure models for the upper airways, the fragile and dynamic nature of the alveolar microenvironment has limited the development of reproducible exposure models for the distal lung. Here, we present a mechanistic approach using a new generation of exposure systems, the Cloud α AX12. This novel in vitro inhalation tool consists of a cloud-based exposure chamber (VITROCELL) that integrates the breathing AXLung-on-chip system (AlveoliX). The ultrathin and porous membrane of the AX12 plate was used to create a complex multicellular model that enables key physiological culture conditions: the air-liquid interface (ALI) and the three-dimensional cyclic stretch (CS). Human-relevant cellular models were established for a) the distal alveolar-capillary interface using primary cell-derived immortalized alveolar epithelial cells (AXiAECs), macrophages (THP-1) and endothelial (HLMVEC) cells, and b) the upper-airways using Calu3 cells. Primary human alveolar epithelial cells (AXhAEpCs) were used to validate the toxicity results obtained from the immortalized cell lines. To mimic in vivo relevant aerosol exposures with the Cloud α AX12, three different models were established using: a) titanium dioxide (TiO2) and zinc oxide nanoparticles b) polyhexamethylene guanidine a toxic chemical and c) an anti-inflammatory inhaled corticosteroid, fluticasone propionate (FL). Our results suggest an important synergistic effect on the air-blood barrier sensitivity, cytotoxicity and inflammation, when air-liquid interface and cyclic stretch culture conditions are combined. To the best of our knowledge, this is the first time that an in vitro inhalation exposure system for the distal lung has been described with a breathing lung-on-chip technology. The Cloud α AX12 model thus represents a state-of-the-art pre-clinical tool to study inhalation toxicity risks, drug safety and efficacy.

PURPOSE To systematically assess test performance of patient-adapted D-dimer cut-offs for the diagnosis of venous thromboembolism (VTE). METHODS Systematic review and analysis of articles published in PubMed, Embase, ClinicalTrials.gov, and Cochrane Library databases. Investigations assessing patient-adjusted D-dimer thresholds for the exclusion of VTE were included. A hierarchical summary receiver operating characteristic model was used to assess diagnostic accuracy. Risk of bias was assessed by QUADAS-2. RESULTS 68 studies involving 141,880 patients met the inclusion criteria. The standard cut-off revealed a sensitivity of 0.99 (95% confidence interval [CI] 0.98 - 0.99) and specificity of 0.23 (95% CI 0.16 - 0.31). Sensitivity was comparable to the standard cut-off for age-adjustment (0.97 [95% CI 0.96 - 0.98]) and YEARS algorithm (0.98 [95% CI 0.91 - 1.00]) but lower for pre-test probability (PTP)-adjusted (0.95 [95% CI 0.89 - 0.98) and COVID-19-adapted thresholds (0.93 [95% CI 0.82 - 0.98]). Specificity was significantly higher across all adjustment strategies (age: 0.43 [95% CI 0.36 - 0.50]; PTP: 0.63 [95% CI 0.51 - 0.73]; YEARS algorithm: 0.65 [95% CI 0.39 - 0.84]; and COVID-19: 0.51 [95% CI0.40 - 0.63]). The YEARS algorithm provided the best negative likelihood ratio (0.03 [95% CI 0.01 - 0.15]), followed by age-adjusted (both 0.07 [95% CI 0.05 - 0.09]), PTP (0.08 [95% CI 0.04 - 0.17), and COVID-19-adjusted thresholds (0.13 [95% CI 0.05 - 0.32]). CONCLUSIONS This study indicates that adjustment of D-dimer thresholds to patient-specific factors is safe and embodies considerable potential for reduction of imaging. However, robustness, safety, and efficiency vary considerably between different adjustment strategies with a high degree of heterogeneity. This article is protected by copyright. In der deutschen Papierindustrie werden jedes Jahr etwa 17 Millionen Tonnen Altpapier als Rohstoff zur Produktion von 22,6 Millionen Tonnen Papier und Pappe eingesetzt. Dies entspricht einer Altpapier-Einsatzquote von 75 %. Altpapier ist damit der wichtigste Rohstoff der deutschen Papierindustrie. Auch im internationalen Vergleich liegt Deutschland mit seinem Altpapiereinsatz auf einem Spitzenplatz. In Europa wird Altpapier gemäß der Standardsortenliste EN 643 in eine große Zahl verschiedener Klassen eingeteilt. Seit der Revision im Jahre 2013 enthält diese Norm einige grundsätzliche Verbesserungen, insbesondere in Hinblick auf die sortenspezifischen Toleranzgrenzen für papierfremde Bestandteile und eine detaillierte Beschreibung der einzelnen Sorten. Nichtsdestotrotz reichen diese Informationen nicht aus, um die Qualität der einzelnen Altpapiersorten im Sinne ihres Potentials zur Herstellung neuer Papier- und Pappe-Produkte abzuleiten. Papierprodukte, und damit auch die Qualität von Altpapier, unterliegen einer Vielzahl von Veränderungen und Innovationen. Die Auswirkungen dieser Innovationen machen sich im Recyclingkreislauf bemerkbar. Häufig wird dies erst nach einer Zeit der Anreicherung sichtbar. Beispielsweise führt der Trend zu hohem Druckglanz dazu, dass grafische Papiere mittlerweile oft gestrichen sind, was den Anteil an mineralischen Bestandteilen in Altpapieren der grafischen Sortengruppen deutlich erhöht. Bei Verpackungspapieren wiederum ist ein Trend zu höherwertigen Produkten mit weißen, dem Verbraucher zugewandten Außenlagen zu beobachten. Dies führt tendenziell zu hochwertigen Zellstofffasern in den Sortengruppen des Verpackungsaltpapiers. Solche und andere Entwicklungen bei Papierendprodukten verändern die Altpapierqualität laufend. Um diese dynamischen Prozess zu erfassen ist eine kontinuierliche Messung von Qualitätseigenschaften notwendig. Aus diesem Grund führt das Institut für Papierfabrikation und Mechanische Verfahrenstechnik etwa alle 10 Jahre eine Studie zur aktuellen Altpapierqualität in Deutschland durch. Aufgrund der Limitierungen in Zeit und finanziellen Mitteln sowie des großen Aufwands durch die große Zahl an durchgeführten Untersuchungen und gemessenen Parametern an den einzelnen Proben können nicht alle Sorten der Standardsortenliste untersucht werden. Daher beschränken sich die Untersuchungen auf die drei wichtigsten Sorten 1.02.00 (gemischtes Altpapier), 1.04.00 (Verpackungen aus Papier und Karton) und 1.11.00 (Deinkingware), die zusammen bereits einen großen Teil des in Deutschland gehandelten Altpapiers ausmachen. Diese Sorten geben daher ein gutes Bild der aktuellen Altpapierqualität in Deutschland. Außerdem wurde für die aktuelle Untersuchung noch die Sorte 3.10.00 (Multidruck) erstmalig untersucht. Gemessen werden neben der Zusammensetzung insbesondere auch Suspensionseigenschaften, mechanische Eigenschaften und chemische Inhaltsstoffe der genannten Altpapiersorten. Um ein umfassendes Bild der aktuellen Altpapierqualität in ganz Deutschland zu erhalten und möglichen regionalen Besonderheiten keine zu hohe Gewichtung zu geben, wurden Proben von insgesamt 9 Standorten untersucht, die über ganz Deutschland verteilt liegen. Bei den Standorten handelt es ich sowohl um Papierfabriken als auch um Papiersortieranlagen und Altpapierhändler. Die Probenmenge der Einzelproben wurde so gewählt, dass die Zusammensetzung repräsentativ für eine Altpapierlieferung ist. Wie dies zu erreichen ist, wird in der Literatur beschrieben. Je nach Sorte lag der Umfang einer Probe zwischen 40 kg und 100 kg. Im Wesentlichen bestätigten die gemessenen Werte die Ergebnisse der Untersuchung von 2010. Trotz der geschlosseneren Recyclingkreisläufen kam es zu keinen drastischen Einbrüchen in den gemessenen Qualitätsparametern. Ein „Recycling-Kollaps“ ist auf Grundlage der vorliegenden Zahlen und Trends nicht zu erwarten. Ein besonderes Augenmerk lag in diesem Projekt auf der Bestimmung der Zusammensetzung der Altpapierproben. Bisher gibt es bisher kein automatisches Messverfahren, dass es ermöglicht, Altpapierproben hinsichtlich ihrer Zusammensetzung nach den Vorgaben der EN643 zu analysieren. Aus diesem Grund wurden Altpapierproben, bei denen eine genaue Bestimmung der Zusammensetzung notwendig war, in der Vergangenheit immer manuell ausgezählt. Dieser Vorgang ist besonders zeitintensiv. Abhängig von der Probenzusammensetzung kann die Auszählung von 10 kg Probenmaterial bis zu 3 Stunden in Anspruch nehmen. Im Rahmen dieses Projekts wurde daher die Entwicklung eines automatischen Messsystems vorangetrieben, dass den zeitlichen und personellen Aufwand der Probenanalyse signifikant reduziert. Mit Hilfe von künstlicher Intelligenz können die von Sensoren gemessenen charakteristischen Merkmale von Papierobjekten dazu genutzt werden, diese in verschiedene Papierklassen einzuordnen. Hierfür wurde das bisher halbautomatische Messsystem u. a. um einen Roboter erweitert, um den Prozess der Probenanalyse vollständig zu automatisieren. Des Weiteren wurden neue Sensoren in das Messsystem integriert und die Auswertealgorithmen verbessert. Dadurch konnte die Erkennungsrate der zu unterscheidenden Papierklassen deutlich verbessert werden.

In ProCycle wurde die Gesundheitsrelevanz von Stäuben beurteilt, die bei der werkstofflichen oder thermischen Verwertung von Nanokompositen entstehen. In diesem Bericht werden die Arbeiten vorgestellt, die hinsichtlich einer thermischen Verwertung von nanomaterialhaltigen Kunststoffen durchgeführt wurden. Das umfasst einen Überblick zur Kunststoffverwertung in Deutschland, Grundlagenuntersuchungen zu den eingesetzten Nanomaterialien (TiO2, CoO, CNTs), sowie Abbranduntersuchungen der Kunststoff-Matrizes und Nanokomposite. Die Versuche wurden im Labormaßstab durchgeführt, was den Vorteil bietet, das verschiedene Parameter flexibel geändert und an erhaltene Erkenntnisse angepasst werden konnten. Die eingesetzte Messtechnik umfasst Partikelanalysatoren, wie Mobilitätsanalysator und elektrischer Niederdruckimpaktor, Mikroskopie, wie Trans- und Rasterelektronenmikroskopie und Filterproben zur offline Analyse. Zusätzlich zur umfassenden physiko-chemischen Charakterisierung der entstehenden Verbrennungsaerosole wurde ein Online Expositionssystem an den Versuchsaufbau angekoppelt, mit dem menschliche Lungenzellen an der Gas-Flüssigkeits-Grenzschicht gegenüber Aerosolen exponiert werden konnten. Dabei wird das Verbrennungsaerosol auf 37 °C und 85 % relative Feuchte konditioniert und anschließend über die Zellen geleitet, wo sich im Aerosol enthaltene Partikel abscheiden können. Die Dosis, die auf die Zellen aufgebracht wird, wurde mithilfe der Partikelanalysatoren bestimmt.

Background Ship engine emissions are important with regard to lung and cardiovascular diseases especially in coastal regions worldwide. Known cellular responses to combustion particles include oxidative stress and inflammatory signalling. Objectives To provide a molecular link between the chemical and physical characteristics of ship emission particles and the cellular responses they elicit and to identify potentially harmful fractions in shipping emission aerosols. Methods Through an air-liquid interface exposure system, we exposed human lung cells under realistic in vitro conditions to exhaust fumes from a ship engine running on either common heavy fuel oil (HFO) or cleaner-burning diesel fuel (DF). Advanced chemical analyses of the exhaust aerosols were combined with transcriptional, proteomic and metabolomic profiling including isotope labelling methods to characterise the lung cell responses. Results The HFO emissions contained high concentrations of toxic compounds such as metals and polycyclic aromatic hydrocarbon, and were higher in particle mass. These compounds were lower in DF emissions, which in turn had higher concentrations of elemental carbon (“soot”). Common cellular reactions included cellular stress responses and endocytosis. Reactions to HFO emissions were dominated by oxidative stress and inflammatory responses, whereas DF emissions induced generally a broader biological response than HFO emissions and affected essential cellular pathways such as energy metabolism, protein synthesis, and chromatin modification. Conclusions Despite a lower content of known toxic compounds, combustion particles from the clean shipping fuel DF influenced several essential pathways of lung cell metabolism more strongly than particles from the unrefined fuel HFO. This might be attributable to a higher soot content in DF. Thus the role of diesel soot, which is a known carcinogen in acute air pollution-induced health effects should be further investigated. Exposure to air pollution resulting from fossil fuel combustion has been linked to multiple short-term and long term health effects. In a previous study, exposure of lung epithelial cells to engine exhaust from heavy fuel oil (HFO) and diesel fuel (DF), two of the main fuels used in marine engines, led to an increased regulation of several pathways associated with adverse cellular effects, including pro-inflammatory pathways. In addition, DF exhaust exposure was shown to have a wider response on multiple cellular regulatory levels compared to HFO emissions, suggesting a potentially higher toxicity of DF emissions over HFO. In order to further understand these effects, as well as to validate these findings in another cell line, we investigated macrophages under the same conditions as a more inflammationrelevant model. An air-liquid interface aerosol exposure system was used to provide a more biologically relevant exposure system compared to submerged experiments, with cells exposed to either the complete aerosol (particle and gas phase), or the gas phase only (with particles filtered out). Data from cytotoxicity assays were integrated with metabolomics and proteomics analyses, including stable isotope-assisted metabolomics, in order to uncover pathways affected by combustion aerosol exposure in macrophages. Through this approach, we determined differing phenotypic effects associated with the different components of aerosol. The particle phase of diluted combustion aerosols was found to induce increased cell death in macrophages, while the gas phase was found more to affect the metabolic profile. In particular, a higher cytotoxicity of DF aerosol emission was observed in relation to the HFO aerosol. Furthermore, macrophage exposure to the gas phase of HFO leads to an induction of a pro-inflammatory metabolic and proteomic phenotype. These results validate the effects found in lung epithelial cells, confirming the role of inflammation and cellular stress in the response to combustion aerosols.
