Introduction and Aim: Cystic echinococcosis (CE) is the most common parasitic disease in Kyrgyzstan, which can affect all organs and organ systems of the body, but its most common localization is the liver and lungs. This study aimed to describe the possibilities and main stages of videolaparoscopy in the surgical treatment of hepatic CE. Materials and Methods: Twenty-five patients aged 17-64 years who were under observation were admitted to the surgical department of our hospital from 2018-2022, were diagnosed with hepatic CE, and underwent surgical treatment by videolaparoscopy. Results: The variant of the surgical technique was based on the use of three trocars including paraumbilical (basic – 10mm), epigastric (introduction and removal of a rubber sac with a chitinous sheath – 10 mm), and hypochondrium (on the right – 5mm). Only in three cases, it became necessary to include the 4th trocar mesogastric (on the right – 5mm), as demonstrated. The operation duration was 60±5 minutes. Drainage in the abdominal cavity was installed according to the location of the cavity and removed for 2-3 days. Conclusion: Laparoscopic methods of treatment of hepatic CE provide greater efficiency when performed with proper surgical manipulations by considering the size and localization of cysts.

Introduction and Aim: Apoptosis, autophagy, and necrosis are the main mechanisms of neuron death in acute ischemic stroke (AIS). This study aimed to evaluate the expression of apoptosis and autophagy biomarkers in peripheral blood of patients with AIS. Materials and Methods: Sixty-eight patients (32 men and 36 women) aged 30-60 years with AIS underwent a clinical and neurological examination on the 1st, 7th, and 14th days after the disease onset. The expression of apoptosis and autophagy biomarkers in peripheral blood was evaluated by flow cytometry and compared with the severity of neurological deficit and injury on the 1st, 7th, and 14th days, using correlation analysis. Results: There is a statistical significance compared with the control group and an increase in the expression of key biomarkers of apoptosis and autophagy was revealed. Increased expression levels of annexin A5 and caspase-3 positively correlate with the severity of neurological deficit and injury on the 1st and 7th days from the onset of the disease. Conclusion: A direct correlation was revealed between elevated levels of apoptosis and autophagy biomarkers in peripheral blood and severity of neurological deficit and injury on the 1st, 7th, and 14th days from the onset of AIS.

Despite the availability of a safe and effective vaccine, measles remains an endemic in many countries and is the main cause of morbidity and mortality among young children. Therefore, the main objective of this study was to describe the most important aspects of measles that allow clinicians to identify suspected cases for timely diagnosis and treatment, which are essential to avoid inappropriate interventions and prevent complications. The measles virus is highly contagious, is transmitted through fomites and respiratory secretions, and remains active in the environment or on surfaces for several hours. Diagnosing measles can be difficult because most clinicians are unfamiliar as there have been few confirmed cases in recent years. Taking a complete medical history, while considering the clinical phases of measles, and a thorough physical examination can help guide the diagnosis, as the main characteristics of measles (fever and rash) can be seen in both infectious and non-infectious diseases. Treatment consists of three fundamental aspects: supportive care (management of fever and hydration), identification and treatment of associated complications, and prevention of disease spread through patient and family group education. It is important to ensure compliance with immunization policies and strategies globally to control the re-emergence of measles and increase in the burden of disease caused by the measles virus.

Introduction and Aim: We examined the effect of pre- and/or post-infection doxycycline on human nasal epithelial cell viability and SARS-CoV-2 (clinical strain IHUMI-3) replication in vitro. Materials and Methods: Human nasal epithelial cells, an in vivo SARS-CoV-2 target, were derived from healthy donor nasal epithelial stem/progenitor cells via in vitro differentiation. The cells were exposed to doxycycline at 0, 0.1, 0.5, 1, 5, 10, 50, and 100 ?M before and/or after IHUMI-3 inoculation to determine the optimal inhibitory concentration. Viral replication was evaluated using quantitative reverse-transcription PCR, and doxycycline 50% cytotoxic concentration (CC50) and half-maximal effective concentration (EC50) were calculated. The peak serum concentration (Cmax) resulting from typical oral (100 or 200 mg) or intravenous (100 mg) doxycycline doses was estimated, and the Cmax/EC50 ratio was calculated as an index of potential clinical utility. Results: Doxycycline exhibited low cytotoxicity (CC50 > 100 ?M) in human nasal epithelial cells and inhibited SARS-CoV-2 replication (EC50: 5.2 ± 3.3 ?M) in a dose-dependent manner when administered pre- and/or post-infection. Reasonable oral or intravenous doses will help achieve effective concentrations in vivo. Conclusion: Early administration of this well-characterized, safe, and accessible drug may limit person-to-person transmission and prevent progression to severe coronavirus disease.
