- Abstract
Objectives: This study examined the effects of sacubitril/valsartan on N-terminal pro–B-type natriuretic peptide (NT-proBNP) levels and determined patient characteristics associated with favorable NT-proBNP reduction response. Background: NT-proBNP levels reflect cardiac wall stress and predict event risk in patients with acute decompensated heart failure (ADHF). Methods: Post-hoc analysis of the TRANSITION (Comparison of Pre- and Post-discharge Initiation of Sacubitril/Valsartan Therapy in HFrEF Patients After an Acute Decompensation Event) study, including stabilized ADHF patients with reduced ejection fraction, randomized to open-label sacubitril/valsartan initiation in-hospital (pre-discharge) versus post-discharge. NT-proBNP was measured at randomization (baseline), discharge, and 4 and 10 weeks post-randomization. A favorable NT-proBNP response was defined as reduction to ≤1,000 pg/ml or >30% from baseline. Results: In patients receiving sacubitril/valsartan in-hospital, NT-proBNP was reduced by 28% at discharge, with 46% of patients obtaining favorable NT-proBNP reduction response compared with a 4% reduction and 18% favorable response rate in patients initiated post-discharge (p < 0.001). NT-proBNP was reduced similarly in patients initiating sacubitril/valsartan pre- and post-discharge (reduction at 4 weeks: 25%/22%; 10 weeks: 38%/34%) with comparable favorable response rates (46%/42% and 51%/48% at 4 and 10 weeks, respectively). NT-proBNP favorable response at 4 weeks was associated with lower risk of first heart failure (HF) rehospitalization or cardiovascular death through 26 weeks (hazard ratio: 0.57; 95% confidence interval [CI]: 0.38 to 0.86; p = 0.007). 7. Early Palaeozoic initial-rift volcanism in the Central European Variscides (the Kaczawa Mountains, Sudetes, SW Poland): evidence from SIMS dating of zircons

Early Palaeozoic volcanic suites are widespread throughout the Variscan Belt, and have commonly been ascribed to incipient rifling along the northern periphery of Gondwana in Cambrian to Ordovician times. Their distribution across Europe defines the present-day extent of Gondwana-derived terranes and constrains the timing of their separation from the Gondwanan margin. The Kaczawa Mountains in the West Sudetes, at the eastern termination of the Variscan Belt, include bimodal rift-related rocks, but their protolith age and, hence, their significance have been highly uncertain. We have applied secondary ionization mass spectrometry zircon geochronology to a metarhyodacite and a metatrachyte from this suite, yielding ages of 502.4 [+ or -] 2.6 Ma and 485.7 [+ or -] 1.6 Ma, respectively. This constrains the initial rift magmatism to c. 500-485 Ma in this part of the European Variscides. The rift-related Early Palaeozoic volcanism thus seems to have been broadly synchronous throughout the peri-Gondwanan terranes of Europe.

New U-Pb monazite and zircon data combined with a study of their internal morphologies significantly clarifies the controversy surrounding the Palaeozoic geodynamic evolution of the Sudetes Mountains in the northeastern Bohemian Massif. Variably thin rims of secondary magmatic overgrowth on zircons from an anatectic granite in the Gory Sowie block are dated by a U-Pb zircon lower intercept age of 353 [+ or -] 61 Ma, by magmatic monazite growth at 378 [+ or -] 2 Ma, and by xenotime crystallization at 383-370 Ma in a late syntectonic pegmatite, reflecting the anatectic event associated with a single-cycle Variscan orogeny in this area. The still highly disputable terrane boundaries in the Sudetic segment of the Variscan belt mostly seem to follow major strike-slip faults and shear zones. Their kinematics, expected to place important constraints on the regional structural models, is discussed in some detail. The most conspicuous is the WNW-ESE Intra-Sudetic Fault Zone, separating several different structural units of the West Sudetes. It showed ductile dextral activity and, probably, displacement magnitude of the order of tens to hundreds kilometres, during late Devonian(?) to early Carboniferous times. In the late Carboniferous (to early Permian?), the sense of motion on the Intra-Sudetic Fault was reversed in a semi-brittle to brittle regime, with the left-lateral offset on the fault amounting to single kilometres. The north-south trending Niemcza and northeast-southwest Skrzynka shear zones are left-lateral, ductile features in the eastern part of the West Sudetes. Similarly oriented (northeast-southwest to NNE-SSW) regional size shear zones of as yet undetermined kinematics were discovered in boreholes under Cenozoic cover in the eastern part of the Sudetic foreland (the Niedzwiedz and Nysa-Brzeg shear zones). One of these is expected to represent the northern continuation of the major Stare Mesto Shear Zone in the Czech Republic, separating the geologically different units of the West and East Sudetes. The Rudawy Janowickie Metamorphic Unit, assumed in some reconstructions to comprise a mostly strike-slip terrane boundary, is characterized by ductile fabric developed in a thrusting regime, modified by a superimposed normal-slip extensional deformation. Thrusting-related deformational fabric was locally reoriented prior to the extensional event and shows present-day strike-slip kinematics in one of the sub-units. The Sudetic Boundary Fault, although prominent in the recent structure and topography of the region, was not active as a Variscan strike-slip fault zone. The reported data emphasize the importance of syn-orogenic strike-slip tectonics in the Sudetes. The recognized shear sense is compatible with a strike-slip model of the northeast margin of the Bohemian Massif, in which the Kaczawa and Gory Sowie Units underwent late Devonian-early Carboniferous southeastward long-distance displacement along the Intra-Sudetic Fault Zone from their hypothetical original position within the Northern Phyllite Zone and the Mid-German Crystalline High of the German Variscides, respectively, and were juxtaposed with units of different provenance southwest of the fault. The Intra-Sudetic Fault Zone, together with the Elbe Fault Zone further south, were subsequently cut in the east and their eastern segments were displaced and removed by the younger, early to late Carboniferous, NNE-SSW trending, transpressional Moldanubian-Stare Mesto Shear Zone.

13. Geochemical evidence for progressive, rift-related early Palaeozoic volcanism in the western Sudetes

The volcanigenic rocks of the Kaczawa Mts (western Sudetes, Poland), in the eastern Variscides, show changing geological and geochemical evolution during early Palaeozoic time. The lower part of the succession (Cambrian (?)-Ordovician) has three components. 1. Shallow marine to subaerial metabasalts, associated with limestones and volcaniclastics. The lavas are dominantly of a transitional tholeiitic-alkaline type and their trace element patterns typically represent a rift-related environment. They pass laterally (and upwards ?) into more depleted basalts resembling enriched MORB, with Nd-isotopic characteristics indicating contamination by continental crust. 2. Interlayered rhyodacitic lavas and volcaniclastics which show negative Epsilon(sub Nd) values, suggesting formation of the original magma by crustal melting. 3. An overlying alkaline bimodal suite of lavas and volcaniclastic rocks, as well as alkaline metabasites of shallow-intrusive character. The geochemistry of the latter resembles oceanic island volcanics, but they may well have been emplaced in the same evolving rift environment. The upper part of the Kaczawa succession, Ordovician-Silurian (?) in age, comprises a thick and monotonous sequence of deep-marine pillowed and massive metabasalts, associated with black shales and cherts. These lavas exhibit MORB trace element characteristics, with minor evidence of crustal contamination. During this stage of rifting, true oceanic crust probably formed. It is thus suggested that the studied part of the Kaczawa succession developed in a progressively evolving rift, initially within an ensialic environment, and finally reaching the stage of a basin underlain by oceanic-type crust. Together with similar Cambrian-Ordovician volcanic-sedimentary associations, widely distributed in western Europe, from Portugal, through France and Germany, they represent a record of the Early Palaeozoic rifting in the northern periphery of Gondwana.

Background Evolocumab is a monoclonal antibody that inhibits proprotein convertase subtilisin–kexin type 9 (PCSK9) and lowers low-density lipoprotein (LDL) cholesterol levels by approximately 60%. Whether it prevents cardiovascular events is uncertain. Methods We conducted a randomized, double-blind, placebo-controlled trial involving 27,564 patients with atherosclerotic cardiovascular disease and LDL cholesterol levels of 70 mg per deciliter (1.8 mmol per liter) or higher who were receiving statin therapy. Patients were randomly assigned to receive evolocumab (either 140 mg every 2 weeks or 420 mg monthly) or matching placebo as subcutaneous injections. The primary efficacy end point was the composite of cardiovascular death, myocardial infarction, stroke, hospitalization for unstable angina, or coronary revascularization. The key secondary efficacy end point was the composite of cardiovascular death, myocardial infarction, or stroke. The median duration of follow-up was 2.2 years. Results At 48 weeks, the least-squares mean percentage reduction in LDL cholesterol levels with evolocumab, as compared with placebo, was 59%, from a median baseline value of 92 mg per deciliter (2.4 mmol per liter) to 30 mg per deciliter (0.78 mmol per liter) (P

- Abstract
BACKGROUND Evolocumab is a monoclonal antibody that inhibits proprotein convertase subtilisin-kexin type 9 (PCSK9) and lowers low-density lipoprotein (LDL) cholesterol levels by approximately 60%. Whether it prevents cardiovascular events is uncertain. METHODS We conducted a randomized, double-blind, placebo-controlled trial involving 27,564 patients with atherosclerotic cardiovascular disease and LDL cholesterol levels of 70 mg per deciliter (1.8 mmol per liter) or higher who were receiving statin therapy. Patients were randomly assigned to receive evolocumab (either 140 mg every 2 weeks or 420 mg monthly) or matching placebo as subcutaneous injections. The primary efficacy end point was the composite of cardiovascular death, myocardial infarction, stroke, hospitalization for unstable angina, or coronary revascularization. The key secondary efficacy end point was the composite of cardiovascular death, myocardial infarction, or stroke. The median duration of follow-up was 2.2 years. RESULTS At 48 weeks, the least-squares mean percentage reduction in LDL cholesterol levels with evolocumab, as compared with placebo, was 59%, from a median baseline value of 92 mg per deciliter (2.4 mmol per liter) to 30 mg per deciliter (0.78 mmol per liter) (P<0.001). Relative to placebo, evolocumab treatment significantly reduced the risk of the primary end point (1344 patients [9.8%] vs. 1563 patients [11.3%]; hazard ratio, 0.85; 95% confidence interval [CI], 0.79 to 0.92; P<0.001) and the key secondary end point (816 [5.9%] vs. 1013 [7.4%]; hazard ratio, 0.80; 95% CI, 0.73 to 0.88; P<0.001). The results were consistent across key subgroups, including the subgroup of patients in the lowest quartile for baseline LDL cholesterol levels (median, 74 mg per deciliter [1.9 mmol per liter]). International audience; Ca. 500 Ma orthogneisses and bimodal suites are widespread along the northern part of the Bohemian Massif (central European Variscides) and are interpreted to document intense magmatism during a continental break-up episode along the northern periphery of Gondwana. Based on geological setting, and geochemical and isotopic evidence, these felsic igneous rocks record the generation of: (1) magmas of pure or predominantly crustal derivation, represented by minor extrusives and much more voluminous orthogneisses similar to S-type granitoids; (2) subordinate magmas of exclusively mantle origin (ranging from within-plate alkali trachytes to oceanic plagiogranites) corresponding to felsic derivatives of associated basalts; and (3) magmas of hybrid origin, produced either as a result of large degrees of contamination of mantle-derived magmas ascending through the crust, or alternatively, generated by partial melting of mixed sources, such as interlayered sediments and mafic rocks or graywackes containing a juvenile component. The high-temperature dehydration melting process responsible for the generation of the most abundant rock-types necessitated the advection of mantle heat, in a context of continental lithosphere extension, as documented by broadly coeval basaltic magmatism at the scale of the igneous province. The large volumes of felsic magmas generated during the 500-Ma anorogenic event are interpreted to result from the combination of a hot extensional tectonic regime with the widespread availability in the lower crust of fertile lithologies, such as metagraywackes. This in turn reflects the largely undifferentiated nature of the crustal segment accreted some 50–100 m.y. earlier during the Cadomian orogeny.

Multidisciplinary studies of geotransects across the North European Plain and Southern North Sea, and geological reexamination of the Variscides of the North Bohemian Massif, permit a new 3-D reassessment of the relationships between the principal crustal blocks abutting Baltica along the Trans-European Suture Zone (TESZ). Accretion was in three stages: Cambrian accretion of the Bruno–Silesian, Lysogory and Malopolska terranes; end-Ordovician/early Silurian accretion of Avalonia; and early Carboniferous accretion of the Armorican Terrane Assemblage (ATA). Palaeozoic plume-influenced metabasite geochemistry in the Bohemian Massif explains the progressive rifting away of peri-Gondwanan crustal blocks before their accretion to Baltica. Geophysical data, faunal and provenance information from boreholes, and dated small inliers and cores confirm that Avalonian crust extends beyond the Anglo-Brabant Deformation Belt eastwards to northwest Poland. The location and dip of reflectors along the TESZ and beneath the North European Plain suggest that Avalonian crust overrode the Baltica passive margin, marked by a high-velocity lower crustal layer, on shallowly southwest-dipping thrust planes forming the Heligoland–Pomerania Deformation Belt. The “Variscan orocline” of southwest Poland masks two junctions between the Armorican Terrane Assemblage (ATA) and previously accreted crustal blocks. To the east is a dextrally transpressive contact with the Bruno–Silesian and Malopolska blocks, accreted in the Cambrian, while to the north is a thrust contact with easternmost Avalonia, deeply buried beneath younger sedimentary cover. In the northeast Bohemian and Rhenohercynian Massifs Devonian “early Variscide” deformation dominated by WNW and NW-directed thrusting, records closure of Ordovician–Devonian seaways between detached “islands” of the ATA and Avalonia.

- Abstract
Background: The length of hospital stay in patients with acute myocardial infarction and ST-segment elevation (STEMI) has been shortened in recent years with corresponding savings in costs, but there is limited available data on its implementation in clinical practice. The aim of this trial was to determine whether early discharge in selected patients after STEMI is feasible and safe., Methods: 151 patients with STEMI successfully treated with primary percutaneous coronary intervention (PCI) who fulfilled the inclusion criteria of low risk were randomly assigned to two groups on a 1:1 ratio: early (within 48-72 h of admission) and standard (after 72 h) discharge. The primary end point was the composite of death, myocardial infarction (MI), unstable angina, stroke, unplanned rehospitalization, repeated target vessel revascularization and stent thrombosis at 90 days after discharge. The study is registered with ClinicalTrials.gov (identifier NCT02023983)., Results: The primary end point occurred in 5 patients in the early group and 6 in the standard group (6.6% vs. 8.0%, P=0.765). There were no significant differences in the incidence of individual components of the primary end point at 90 days. The length of hospital stay was significantly shorter in the intervention group (60.8 ± 8.5 vs. 92.1 ± 12.1 h, P<0.0001)., Conclusion: This study confirms that early discharge within 48-72 h in selected low risk patients after STEMI treated with successful primary PCI is feasible and safe, with outcomes comparable to the later discharge. This strategy applies to more than a quarter of all STEMI patients.

- Abstract
Purpose: To compare local tumor control after percutaneous tumor ablation by interstitial laser therapy (ILT) or CT-guided brachytherapy (CTGB)., Patients and Methods: In a matched pair analysis including 18 patients with 36 liver metastases of colorectal primary, both ILT and CTGB were performed in different lesions. The following matching factors were considered: (i) tumor size < or = 5 cm, and (ii) execution of chemotherapy after tumor ablation. Primary endpoint was local tumor control., Results: Treated lesions were identical in terms of tumor size and all matching criteria were fulfilled in all patients except for the performance of adjuvant chemotherapy. Median follow-up was 14 months (3-24 months) for both groups. Only five of 18 patients (28%) demonstrated local tumor progression after CTGB, whereas in ten of 18 patients (56%) tumor progression was found after ILT. Differences encountered were significant for all patients (p = 0.04), whereas in those who fulfilled all matching criteria (n = 14) the level of statistical significance was not reached (p = 0.23)., Conclusion: CTGB demonstrated superior local tumor control compared to ILT in long-term follow-up.
