2. Construction of Differentially Private Summaries over Fully Homomorphic Encryption

Abstract

Cloud computing has garnered attention as a platform of query processing systems. However, data privacy leakage is a critical problem. Chowdhury et al. proposed Crypt(epsilon), which executes differential privacy (DP) over encrypted data on two non-colluding semi-honest servers. Further, the DP index proposed by these authors summarizes a dataset to prevent information leakage while improving the performance. However, two problems persist: 1) the original data are decrypted to apply sorting via a garbled circuit, and 2) the added noise becomes large because the sorted data are partitioned with equal width, regardless of the data distribution. To solve these problems, we propose a new method called DP-summary that summarizes a dataset into differentially private data over a homomorphic encryption without decryption, thereby enhancing data security. Furthermore, our scheme adopts Li et al.'s data-aware and workload-aware (DAWA) algorithm for the encrypted data, thereby minimizing the noise caused by DP and reducing the errors of query responses. An experimental evaluation using torus fully homomorphic encryption (TFHE), a bit-wise fully homomorphic encryption library, confirms the applicability of the proposed method, which summarized eight 16-bit data in 12.5 h. We aimed to evaluate the impact of the pandemic on the diagnosis, treatment, and consultation methods for patients with hepatocellular carcinoma (HCC).Patients and Methods: We conducted a survey among 27 hospitals from 14 Asia-Pacific countries, collecting hospital-level information on the number of newly diagnosed HCC cases during a pre-pandemic period (February to May 2019) and for the same period during the pandemic (February to May 2020). Information was also collected on delays in diagnosis and treatment, changes in treatment modalities and complication rates, changes in patient enrollment in clinical trials, and modes of patient consultation. The information was stratified by the Barcelona Clinic Liver Cancer (BCLC) stage.Results: The survey included cohorts of 2789 and 2045 patients newly diagnosed with HCC during the pre- and pandemic period, respectively. A decline of 26.7% in new HCC cases was reported during the pandemic compared to the pre-pandemic. A sizable proportion of institutions reported delays in diagnosis (48.2% in BCLC 0/A/B and 51.9% in BCLC C), delays in treatment (66.7% in BCLC 0/A/B and 63.0% in BCLC C), changes in treatment modality (33.3% in BCLC 0/A/B and 18.5% in BCLC C), an increase in treatment complications (about 15% across all BCLC stages), and no growth in clinical trial enrollments during the pandemic. Furthermore, there was a decline of 27.3% in face-to-face patient consultations and an increase of 18.3% in video/telephonic consultations during the pandemic. A considerable variation in changes in HCC management was observed among countries.Conclusion: The COVID-19 pandemic has significantly impacted the management of HCC among Asia-Pacific countries. The impact varies according to the disease stage and country. Each type of physical activity was assigned a metabolic equivalent (MET), and the subjects were classified into sedentary, low or high groups using the “METS*T” (METs × hours per week) value of 5 or 21 as a threshold.Results: Among the PNPLA3 G/G genotype carriers, the high and low METS*T groups had a lower risk of NAFLD than the sedentary METS*T group (odds ratio [95% confidence interval]: 0.14 [0.02– 0.99] and 0.16 [0.03– 1.04], respectively). Furthermore, the PNPLA3 C/C or C/G genotype carriers showed no significant difference in the risk of NAFLD among the three METS*T groups.Conclusion: The PNPLA3 rs738409 genotype may be associated with the beneficial effects of physical activity on the risk of NAFLD among elderly Japanese individuals. Abstract

BackgroundHepatocellular carcinoma (HCC) is a complicated condition influenced by multiple confounding factors, making optimum patient management extremely challenging. Ethnicity, stage at diagnosis, comorbidities and tumour morphology affect outcomes and vary from region to region, and there is no common language to assess patient prognosis and make treatment recommendations. Despite recent efforts to reduce the incidence of HCC, most patients present with unresectable disease. Non-surgical treatments include ablation, transarterial chemoembolisation and the multikinase inhibitor, sorafenib, but their effects in all patient subgroups are not known and further information is needed to optimise the use of these treatments.AimsThe Global Investigation of Therapeutic DEcisions in Hepatocellular Carcinoma and Of its Treatment with SorafeNib (GIDEON) study (ClinicalTrials.gov identifier NCT00812175; http://clinicaltrials.gov/) is an ongoing global, prospective, non-interventional study of patients with unresectable HCC who are eligible for systemic therapy and for whom the decision has been taken to treat with sorafenib under real-life practice conditions. The aim of this study is to evaluate the safety and efficacy of sorafenib in different subgroups, especially Child-Pugh B where data are limited.DiscussionThis study will recruit 3000 patients from > 40 countries and follow them for approximately 5 years to compile a large and robust database of information that will be used to analyse local, regional and global differences in baseline characteristics, disease aetiology, treatment practice patterns and treatment outcomes, with a view to improve the knowledge base used to guide physician treatment decisions and to improve patient outcomes.

Abstract

This study aims to shed light on the differences in the way in which an environmental issue is represented by the news media in Western and non-Western society. It also examines the degree to which the previous theories of media and environment, which have been primarily based on examination of domestic news coverage of environmental issues in Western society, can account for these differences. It employs a case study approach, which comparatively examines news texts of the whaling conflict drawn from three Australian newspapers and two Japanese newspapers. The tension between these two nations dramatically increased in 2005-2006 in the wake of Japan's decision to more than double the quota of its research whaling in the Southern Ocean, and the case study is designed to analyse the newspaper articles published during this period. The method employed in this study combines quantitative and qualitative analysis techniques to investigate four aspects of press coverage of the whaling conflict: fluctuation in the number of articles; diversity of quoted news sources; the articles' coverage of the positions put by the Australian and Japanese governments; and differences in the way in which the newspapers' views of the whaling conflict were reflected in their news reporting. The analyses of the quantitative aspects of the news texts show that Australian newspapers carried more information about the conflict than Japanese newspapers, and they gave a similar amount of coverage to the official positions of Australia and Japan, unlike Japanese papers whose coverage mainly reiterated the arguments presented by the Japanese Government. The qualitative discourse analysis demonstrates that while the suspicion of Australian newspapers and their nation's political elites about Japan's whaling policy was noticeably reflected in the news reports, Japanese newspapers did not show their scepticism of the anti-whaling nations' positions in their news reports in an explicit way. The study shows how some theories of environment and media fail to give a thorough explanation of the differences in press coverage of the whaling conflict - not only the differences between Australian and Japanese papers but also the differences among the newspapers in the same nation. This suggests that the interaction of internal and external factors concerning news production, such as news culture, social perception of the issues and news companies' views on the issues, which in combination generate the diversity in news coverage of environmental issues, is more complex than commonly understood.

We present a rare case of a primary myxoid liposarcoma in the foot of a 63-year-old man that was diagnosed by fine needle aspiration cytology (FNAC). The patient presented to our hospital with a 10-year history of a slow-growing mass on the left lateral ankle. On physical examination, the 60 mm ×60 mm mass, was found to be soft and elastic, causing poor mobility without pain. Magnetic resonance imaging of the mass revealed high signal intensity on T1-weighted images and heterogeneously high signal intensity on T2-weighted images without fat suppression. As a result of the physical examination and imaging, the mass was suspected to be a lipoma or ganglion cyst. However, the FNAC procedure revealed atypical small, round, short spindle cells, or foamy cells with a myxoid background, suggesting myxoid liposarcoma. Histology using an open biopsy showed a proliferation of atypical spindle and asteroid cells with a few foamy lipoblasts in a capillary-rich and myxoid background. The atypical lipoblasts were slightly positive for S-100. These findings were consistent with myxoid liposarcoma. The patient was treated with a resection of the mass. Liposarcoma is considered to be the second most common soft-tissue sarcoma. It usually involves the lower extremities, particularly the thigh, and is very rare in the adult foot. Recent observational advances enabled by the Interface Region Imaging Spectrometer (IRIS) have revolutionized our view of the critical role this highly dynamic interface between the photosphere and corona plays in energizing and structuring the outer solar atmosphere. Despite these advances, a major impediment to better understanding the solar atmosphere is our lack of empirical knowledge regarding the direction and strength of the magnetic field in the upper chromosphere. Such measurements are crucial to address several major unresolved issues in solar physics: for example, to constrain the energy flux carried by the Alfven waves propagating through the chromosphere (De Pontieuet al., 2014), and to determine the height at which the plasma β = 1 transition occurs, which has important consequences for the braiding of magnetic fields (Cirtainet al., 2013; Guerreiroet al., 2014), for propagation and mode conversion of waves (Tian et al., 2014a; Straus et al., 2008) and for non-linear force-free extrapolation methods that are key to determining what drives instabilities such as flares or coronal mass ejections (e.g., De Rosa et al., 2009). The most reliable method used to determine the solar magnetic field vector is the observation and interpretation of polarization signals in spectral lines, associated with the Zeeman and Hanle effects. Magnetically sensitive ultraviolet spectral lines formed in the upper chromosphere and transition region provide a powerful tool with which to probe this key boundary region (e.g., Trujillo Bueno, 2014). Probing the magnetic nature of the chromosphere requires measurement of the Stokes I, Q, U and V profiles of the relevant spectral lines (of which Q, U and V encode the magnetic field information).
