Background The most common complaint following cementation is the occurrence of post cementation sensitivity. The sensitivity to cold stimulus after cementation can be indicative of a gap present below the crown or a marginal gap connecting to opened tubules which emerge into the pulp. A pulp fluid outflow occurs as a result of the contraction of dentinal fluid in this gap which causes postoperative sensitivity. Thus luting agents should form complete seal between fabricated restoration and prepared tooth.Objectives The current study was conducted to clinically evaluate post-cementation hypersensitivity of bioactive luting cement in a randomized clinical control trial using resin modified glass ionomer cement RMGIC as the control.Methods According to standard tooth preparation protocol the teeth were prepared to receive Porcelain fused to metal PFM bridges on both sides of the mouth. PFM final restorations were cemented using Bioactive and RMGIC luting cement. Sensitivity was tested by using compressed airbiting pressure tests and cold water and was evaluated by using Schiff scale with 0-3 scores before cementation soon after cementation one week and three months post cementation.Results Almost all the patients responded to hypersensitivity at pre-cementation and baseline with compressed airbiting pressure tests and cold water. The sensitivity scores reduced over time one week and three months for both the luting agents.Conclusion Based on the results it can be concluded that the bioactive luting cement exhibited lesser sensitivity scores when compared to resin modified GIC luting cement.

This is due to the advancement of Technology and increase in online transaction resulting in frauds causing huge financial loss. Therefore, there is need for effective methods to reduce the loss. In addition, fraudsters find ways to steal the credit card information of the user by sending fake SMS and calls, also through masquerading attack, phishing attack and so on. This paper aims in using the multiple algorithms of Machine learning such as support vector machine (SVM), k-nearest neighbor (Knn) and artificial neural network (ANN) in predicting the occurrence of the fraud. Further, we conduct a differentiation of the accomplished supervised machine learning and deep learning techniques to differentiate between fraud and non-fraud transactions.

Human to human transmitted disease is the game of coronavirus disease (COVID-19) transmission and it had been declared an emergency global pandemic that caused major disastrous in the respiratory system to more than five million people and killing more than half a billion deaths across the globe. Besides lower acute respiratory syndrome, there is damage to the alveolar with severe inflammatory exudation. COVID-19 patients often have lower immunosuppressive CD4+ T and CD8+ T cells and most patients in intensive care units (ICU) need mechanical ventilation, hence longer stay in hospitals. These patients have been discovered to develop fungal co-infections. COVID-19 patients develop what is known as mucormycosis a black fungal infection that is deadly leading to loss of sight and hearing and eventually death. This chapter will focus on mucormycosis, a black fungus caused during post covid complications.

A 36 year old gentleman with no significant past medical history but had recent COVID-19 exposure presented to the hospital with the chief complaints of fever, shortness of breath, cough and pain abdomen. He was unfortunately found to SARS-CoV-2 positive. Laboratory findings showed S. amylase, S. lipase along with elevated inflammatory markers and unremarkable creatinine, bilirubin, cardiac troponin level. The cause of his elevated S. amylase and lipase was discovered to be due to COVID-19 and no evidence of other common etiology like gallstones, viral, bacterial infections, strenuous exercise, non-traumatic exertional etiology. He received aggressive fluid with antibiotics throughout his hospitalization course. His pain abdomen and breathlessness improved with treatment and he was found to maintain stable hemodynamics and was subsequently discharged home.

Presenting a rare case of amoebic liver abcess with post covid pneumonia

Familial dyskeratotic comedones (FDC) is a rare autosomal dominant inherited condition, characterized by widespread, symmetrically scattered, comedone-like, hyperkeratotic papules, which are cosmetically unappealing. Lesions show a predilection to involve the trunk, arms and face and appear around puberty. The lesions are usually asymptomatic and worsen gradually with time. Histology shows invagination of the epidermis with a lamellar keratinous plug and focal evidence of dyskeratosis. This condition is generally refractory to therapy. We report here a case with this rare disorder.

Introduction: Hemostasis is a fundamental management issue post-operatively in minor oral surgical procedures. To ensure safety and therapeutic efficacy in patients, under oral anti coagulant therapy, is complicated by necessity for frequent determination of prothrombin time or international normalised ratio. Aim: The aim of the study was to determine whether early hemostasis achieved by using Hemcon Dental Dressing (HDD) will affect post-operative care and surgical healing outcome in minor oral surgical procedures. Materials and Methods: A total of 30 patients, aged 18 years to 90 years, except those allergic to seafood, who consented to participate, were enrolled into this study. Patients were required to have two or more surgical sites so that they would have both surgical and control sites. All patients taking Oral Anticoagulation Therapy (OAT) were included for treatment in the study without altering the anticoagulant regimens. Institutional Review Board approval was obtained for the same. The collected data was subjected to statistical analysis using unpaired t-test. Results: All HDD surgically treated sites achieved hemostasis in 1.49 minutes and control wounds in 4.06 minutes (p < 0.001). Post-operative pain at HDD treated sites (1.87,1.27 on 1st and 3rd day respectively) was significantly lower than the control sites (4.0,1.87 on 1st and 3rd day respectively) p-value (0.001, 0.001 respectively). BACKGROUND This study aims to determine the predictive power of baseline C-reactive protein (CRP) value in cirrhotic patients with ascites, without overt infection, that might lead to spontaneous bacterial peritonitis (SBP)/ cellullitis. METHODS 152 consecutive cirrhotic patients with ascites, without overt infection were included in the study, after measuring the baseline CRP value. All patients were followed up for a duration of one year, or till development of SBP/cellulitis. RESULTS Baseline CRP was elevated in 76.8% of the patients. Development of infection was observed in 78 (51.3%) patients. SBP was diagnosed in 54 patients, cellulitis was documented in 15 patients. 9 patients had simultaneous SBP and cellulitis Baseline CRP was 10.2 ± 6.34 mg/dL in the group who developed infection, it was 4.81 ± 4.41 mg/dL in the group who did not develop infection (p = 0.002). Baseline CRP > 9.5 mg/dL, serum albumin < 2.8 g/dL and a previous history of infection were independent predictors of developing SBP/cellulitis. CONCLUSIONS Along with low serum albumin and previous history of infection, CRP can be used as a predictive tool for early detection of infection, thus enabling to reduce the morbidity and mortality.

Currently there are few contraindications to transcatheter closure of secundum atrial septal defects (ASDs). Known device-related complications of percutaneous ASD closure are worsening of aortic regurgitation and cardiac erosion. Interference of a septal occluder with anatomic variant coronaries is a rare but important complication that can result in post-interventional fatalities. We report a woman with ASD and anomalous left circumflex artery arising from the right sinus of Valsalva with a retro-aortic course. Device closure was not attempted in her due to concerns about coronary compression by the device occluder and hence she underwent surgical closure.Learning objective: Adequate imaging studies to detect coronary variants in patients scheduled to undergo transcatheter atrial septal defect (ASD) device closure are essential. Identifying anomalies that are at risk for compression from the occluder device prevents post-procedural complications. This case study highlights the importance of thorough imaging, including coronary angiogram and computed tomography angiogram to delineate coronary anomalies associated with ASD..

However, when we engage in true process improvement, we seek to learn what causes things to happen in a process and to use better technology to reduce product variation, remove activities that contribute no value to the product or service produced, and improve customer satisfaction. To examine all of the factors affecting the process in related with men, machine and materials are main responsible for the transformation of materials in to a product or service. Lean manufacturing is originated and developed in Japan which is considered as a business strategy to reduce and eliminate wastes occurred during manufacturing process, thus it leads to improve the productivity by giving quality products with higher accuracy especially for small and medium scale enterprises (SMEs) To implementing lean approach, VSM (value stream mapping) and kaizen principles are most effective in nature among all other lean practices. In this research work, a case study on implementing VSM and kaizen as the lean manufacturing initiative is clearly reported. And the same lean tools are implemented in pump assembly of medium scale enterprises to identify and reduce the wastes during this case study. Here initially process path is visualized by drawn current value stream mapping (CVSM), after tracking the entire process wastes affecting the cycle time are identified and analyzed. In the next stage, the application of Kaizen principle to reduce the wastes in the process, then future state map is developed to analyze the previous cause for the waste development. This capability will definitely enable the considering enterprise to achieve cycle time reduction, competitive strength and productivity.
