Methods: In this study, 100 patients aged 18-70 years were randomised to Group cTLIP (conventional TLIP block with 20 mL of 0.25% ropivacaine with GA), Group mTLIP (modified TLIP block with 20 mL of 0.25% ropivacaine with GA), and Group C (only GA using multimodal analgesia). The primary outcome was to assess the total peri-operative opioid consumption in the first 24 h. The secondary outcomes were to assess pain score upon arriving in the post-anaesthesia care unit, time to first analgesic need after surgery, post-operative opioid consumption in 24 h, and incidence of nausea and vomiting. Results: The total peri-operative opioid consumption in Group cTLIP (507.58 (258.55) μg) and Group mTLIP (491.67 (165.39) μg) was significantly lower than that in Group C (1225.4 (237.03) μg); (P < 0.001). However, it was comparable between groups cTLIP and mTLIP (P = 0.767). Pain score was comparable in groups cTLIP and mTLIP. It was significantly lower than Group C (P = 0.001). Rescue analgesia was needed in all (100%) patients of Group C but in only 15.2% of patients of the cTLIP and mTLIP groups. No patient in groups cTLIP and mTLIP complained of nausea and vomiting in the first 24 h, whereas it was significantly higher (61.8%) in Group C (P = 0.001). Conclusion: The analgesic effect of the modified TLIP block was not superior to the conventional TLIP block. Both techniques provided the same intra-operative and post-operative analgesia for lumbar disc surgeries. Erector spinae plane (ESP) block is a recently described technique. We planned to compare the efficacy and safety of ultrasound-guided continuous ESP and TPV blocks for postoperative analgaesia following MRM. Methods: Sixty-six patients belonging to American Society of Anaesthesiologists physical status I and II, aged 25-85 years, undergoing MRM were enrolled and randomly allocated into two groups. Ipsilateral block was given preoperatively at T3 or T4 level with 20 ml of 0.5 ropivacaine and 50 μg fentanyl. Infusion of 0.5 and 0.2 ropivacaine with fentanyl 2 μg/ml at a rate of 5 ml/hr was continued during intraoperative and postoperative period, respectively. Pain was assessed using visual analogue scale (VAS) till 24 hours. Block performance time, time to first rescue analgaesia, total amount of rescue analgaesic consumed, the incidence of procedure-related and postoperative complications, failure rate and patient satisfaction score were also recorded. Data collected were analysed using the Chi-square test or Student&apos;s t-test with the help of SPSS 22.0. Results: Demographics, baseline vitals, VAS scores both at rest and on movement, block performance time, time to first rescue analgaesia, the total amount of rescue analgaesia and patient satisfaction score were comparable in both groups (P value > 0.05). No complications were observed in either group. Conclusion: In patients undergoing MRM, continuous catheter technique ESP block is as efficacious and safe as TPV block for providing prolonged postoperative analgaesia. Propofol using bispectral index (BIS)/target-controlled infusion (TCI) system can be an optimal method of sedation, as it combines objective measurement of sedation using BIS along with maintenance of a steady plasma concentration of propofol with the TCI device. The aim of this study was to ascertain the dose and safety of propofol using BIS/TCI system for sedation in patients undergoing surgeries under neuraxial anaesthesia. Methods: One hundred and seven adult patients, undergoing elective surgical procedures under spinal or combined spinal epidural anaesthesia, were recruited. Propofol infusion was started with TCI at an initial target plasma concentration (Cpt) of 1.2 μg/mL, and after equilibration between Cpt and effect site concentration (Ce), propofol was then adjusted in increments and/or decrements of 0.2 μg/mL in order to maintain a BIS value between 60 and 80. The average time to reach BIS = 80 after starting infusion was 7.32 ± 3.13 minutes. The objective was to calculate mean Cpt value maintaining BIS between 60 and 80 and to observe recovery time and complications. Results: Mean Cpt value was 1.13 ± 0.17 mg/mL with 95 confidence interval (1.10-1.16 μg/mL). In 85 of patients, a BIS value of 60-80 was maintained at Cpt ≤ 1.2 μg/mL. No patient had severe complications requiring stoppage of infusion. Conclusion: Propofol sedation using BIS/TCI system can provide safe and convenient sedation during neuraxial anaesthesia at very low plasma concentration, Cpt ≤ 1.2 μg/mL in majority of patients. In this article, we have demonstrated WO3/NiO nanocomposite (NC) based UV photodetector (PD) synthesized using RF sputtering technique. The fabricated WO3/NiO NC were porous and crystalline in nature with average diameter of ~ 29.8 nm. This fabricated WO3/NiO NC photodetector exhibited a low dark current value of 0.32 µA at -1 V applied bias. Moreover, the device showed high photo sensitivity (36), and responsivity (2.24 mA/W) without any applied bias demonstrating its self-powered application. A high detectivity of 3.82 × Jones along with low noise equivalent power (NEP) of 88 pW at 0 V was obtained under the illumination of UV light. The WO3/NiO NC detector also showed fast switching response with rise time of 0.65 s and fall time of 0.33 s (at 0 V) demonstration its application as a low power photodetector. ABSTRACT: Context: Anganwadi workers (AWWs) require technical skills for growth monitoring of under-five children. Anthropometric skills help Anganwadi workers assess any changes or deterioration in the normal growth of children. Aims: To determine the effect of a structured training program on the anthropometric skills of Anganwadi workers in rural areas of Raebareli district in Uttar Pradesh. Settings and Design: A community-based interventional study, was conducted among 162 AWWs, 73 from Salon and 89 from Rahi development blocks of Raebareli district. Methods and Material: The skills of AWWs in anthropometric measurement domains (weight, height/length, and mid-upper arm circumference) were assessed using a pre-tested observatory checklist both before and after the training sessions. The skills for each domain were assessed on four points i.e. standard procedure applied to the child, assurance of safety precautions, observer protocol during anthropometry, and precision and accuracy in recording measurement. Statistical Analysis Used: The McNemar Chi-square test was used for paired categorical data analysis. P < 0.05 was considered to be statistically significant. Results: The mean age of the study population was 43.30 ± 9.68 years. The mean years of their job was 16.68 + 7.38 years. About one-fourth (26.5%) of AWWs had received training within the last three years. Inadequate/non-functional equipment (9.8%), low honorarium (29.4%), interruption in timely supply of supplementary food (6.3%), non-cooperation of the community (84.6%), insufficient space (58.0%), and availability of periodic job aids (16.8%) were the major challenges that were faced by ICDS workers. The anthropometric skills of the AWWs were found to improve significantly in almost all the domains. The proportion of AWWs following standard procedure, assurance of safety precautions, observer protocol during anthropometry, precision, and accuracy on recording measurement were found to increase significantly after a structured training program. (P < 0.05). Conclusions: A structured training programme focusing anthropometric skills on play an important role in increasing the skills of AWWs in growth monitoring. In this work, we report on the beta-gallium oxide (β-Ga2O3) monolithic bidirectional switch. The as-fabricated switch works on enhancement mode operation with a threshold voltage of ∼4 V. Maximum drain current density of 1.93 mA/mm is obtained with a drain voltage of 5 V. The bidirectional switch in a bidirectional mode has an ON/OFF current ratio of ∼107 with ON-resistance of 1.11 and 1.09 kΩ · mm in forward and reverse direction, respectively. However, in diode mode, the device shows an ON/OFF current ratio of 1.6 × 108 and 1.4 × 108 in forward and reverse conduction modes, respectively. The fabricated β-Ga2O3 monolithic bidirectional switch is then used to chop a 60 Hz Alternating Current signal at a chopping frequency of 1 kHz, indicating its potential applications in a range of converters. Solid state semiconductor based neutron detectors have the potential to be energy efficient and compact, making them suitable for applications where low power consumption and size constraints are important considerations. Herein, neutron detection devices based on PIN structures consisting of BGaN/GaN superlattice (SL) are demonstrated. These SL structures enable to incorporate significant boron (B) content and achieve good crystalline quality epilayers crucial for better neutron detection. Further, by leveraging the built‐in electric field generated by the PIN structure, these devices can be operated without any applied bias, simplifying overall operation and enabling a more compact size system for detection. Their performance is evaluated by measuring real‐time current response under neutron irradiation (IN) and without it (ID). The neutron induced current density (ΔJ = JN − JD) is determined, reaching an impressive value of 0.67 pA cm−2 (two times JD) under thermal neutron flux of 1.2 × 104 n cm−2 s−1 without biasing, demonstrating their self‐powered capability. They exhibit a linear response to varying thermal neutron flux levels. Additionally, the detectors successfully detect low thermal neutron fluxes down to 300 n cm−2 s−1, showcasing their potential for diverse applications, including in low neutron environments, screening nuclear warheads, and preventing illegal trafficking of radiological materials. This study demonstrates pseudo-source-gated beta-gallium oxide (β-Ga2O3) metal oxide semiconductor field effect transistors (MOSFETs). The proposed pseudo-source gated transistor (pseudo-SGT) architecture has a thin (∼11 nm) recessed channel design, effectively emulating conventional SGT characteristics without significantly compromising on-current. The fabricated devices exhibit remarkable intrinsic gain of 104, low output conductance of 10−8 S/mm, transconductance of 10−3 S/mm, and drain saturation voltage of ∼1.5 V, while maintaining a drain current of 1.3 mA/mm. These enhanced performance metrics significantly expand the potential of β-Ga2O3 MOSFETs for the development of Ga2O3 monolithic power integrated circuits. [ABSTRACT FROM AUTHOR]

Background and Aims: None of the clinical difficult airway predictors are 100% sensitive and specific. Ultrasound is being used for airway assessment, but there is still no established parameters or model to predict difficult laryngoscopy. This observational study was planned to determine the predictive ability of clinical and sonography-based airway assessment parameters for difficult laryngoscopy and intubation. Material and Methods: A total of 130 patients of 18–60 years of age undergoing elective intubation were included. The distribution of Cormack–Lehane (CL) grade and intubation difficulty scale (IDS) was correlated with the clinical and sonographic screening parameters for difficult airways. Results: The prevalence of difficult laryngoscopy and difficult intubation in our study was 17.6% and 11.5%, respectively. Mallampati grade (MMG), upper lip bite test (ULBT), neck circumference, hyomental distance ratio (HMDR), tongue thickness (TT), skin to epiglottis/epiglottis to vocal cord distance (SED/E-VC), and mandibular condylar mobility (MCM) had significant association with the difficult laryngoscopy and MMG, neck circumference, SED, SED/E-VC; MCM had significant association with the difficult intubation. The combination of these predictors showed better diagnostic ability for difficult airways. Model 1 based on ultrasound parameters showed an area under the curve (AUC) of 0.848 (CI- 0.748-0.947, P value < 0.0001) and model 2 based on combined clinical and ultrasound parameters showed an AUC of 0.755 (95% CI- 0.631-0.879, P value < 0.0001). Conclusions: Ultrasound-based airway predictors can help in predicting difficult laryngoscopy and intubation along with the clinical parameters. Dogs usually suffer from numerous parasitic diseases and among which tick-borne haemoparasitic infection is one of the major concerns. Common tick-borne haemoparasites which cause disease in dogs are Babesia canis, B. gibsoni, Hepatozoon canis and rickettsial parasites viz. Ehrlichia canis and Anaplasma platys. In the present study, the prevalence of tick-borne haemoparasites was assessed in dogs in Agartala, Tripura. Blood samples were collected from a total of 1776 dogs irrespective of breed, age, sex and categories for a period of one calendar year starting from March 2022 to February 2023. The overall prevalence of tick-borne haemoparasites in dogs was found 59.46 %(1056/1776) with only 11 cases of mixed infection. The study on the basis of breed, sex, age, season and category showed the highest prevalence of tick-borne haemoparasites in German Spitz (77.20%), male dogs (63.57%), adult dogs (60.71%), monsoon season (73.83%) and stray dogs (68.45%). The high prevalence of tick-borne haemoparasitic infection in dogs in the study area demands the strategic control of tick population and effective management practices necessary to control tick-borne haemoparasitic infections.
