Abstract Background Social isolation and loneliness can co-occur; however, they are distinct concepts. There is discrepancy as some people feel lonely in social isolation, while others do not. This study sought to enhance our understanding of this discrepancy between social isolation and loneliness by investigating its related factors, with a specific focus on mental status and personality traits. Methods This study adopted a cross-sectional study design and utilized data from the 2016 and 2018 waves of the University of Michigan Health and Retirement Study. The participants were community dwellers aged 50 years and older. The outcome measurement was defined as the discrepancy between social isolation, based on six criteria, and loneliness, assessed using the three-item version of the Revised UCLA Loneliness Scale. Multinomial logistic regression models were conducted to examine the factors associated with the discrepancy. Results Participants with fewer depressive symptoms and higher extraversion were associated with the only social isolation group and the only loneliness group rather than the group consisting of those who felt lonely with social isolation. In addition, lower neuroticism was associated with the only social isolation group. Participants with fewer depressive symptoms, lower neuroticism, and higher extraversion were more likely not to feel lonely even with social isolation, compared to feeling lonely even in the absence of isolation. Conclusions Mental status and personality traits may be closely related to the discrepancy between social isolation and loneliness. This study suggests that incorporating social, mental, and psychological factors may be essential for interventions in social isolation and loneliness.

Abstract Background Frailty is a multifactorial syndrome; through this study, we aimed to investigate the physiological, psychological, and social factors associated with frailty and frailty worsening in community-dwelling older adults. Methods We conducted a cross-sectional and longitudinal study using data from the “Community Empowerment and Well-Being and Healthy Long-term Care: Evidence from a Cohort Study (CEC),” which focuses on community dwellers aged 65 and above in Japan. The sample of the cross-sectional study was drawn from a CEC study conducted in 2014 with a total of 673 participants. After excluding those who were frail during the baseline assessment (2014) and at the 3-year follow-up (2017), the study included 373 participants. Frailty assessment was extracted from the Kihon Checklist, while social relationships were assessed using the Social Interaction Index (ISI). Variable selection was performed using Least Absolute Shrinkage and Selection Operator (LASSO) regression and their predictive abilities were tested. Factors associated with frailty status and worsening were identified through the Maximum-min Hillclimb algorithm applied to Bayesian networks (BNs). Results At baseline, 14.1% (95 out of 673) participants were frail, and 24.1% (90 out of 373) participants experienced frailty worsening at the 3-years follow up. LASSO regression identified key variables for frailty. For frailty identification (cross-sectional), the LASSO model’s AUC was 0.943 (95%CI 0.913–0.974), indicating good discrimination, with Hosmer–Lemeshow (H–L) test p = 0.395. For frailty worsening (longitudinal), the LASSO model’s AUC was 0.722 (95%CI 0.656–0.788), indicating moderate discrimination, with H–L test p = 0.26. The BNs found that age, multimorbidity, function status, and social relationships were parent nodes directly related to frailty. It revealed an 85% probability of frailty in individuals aged 75 or older with physical dysfunction, polypharmacy, and low ISI scores; however, if their social relationships and polypharmacy status improve, the probability reduces to 50.0%. In the longitudinal-level frailty worsening model, a 75% probability of frailty worsening in individuals aged 75 or older with declined physical function and ISI scores was noted; however, if physical function and ISI improve, the probability decreases to 25.0%. Conclusion Frailty and its progression are prevalent among community-dwelling older adults and are influenced by various factors, including age, physical function, and social relationships. BNs facilitate the identification of interrelationships among these variables, quantify the influence of key factors. However, further research is required to validate the proposed model.

BackgroundSocial communication is a key factor in maintaining cognitive function and contributes to well-being in later life. ObjectiveThis study will examine the effects of “Photo-Integrated Conversation Moderated by Application version 2” (PICMOA-2), which is a web-based conversational intervention, on cognitive performance, frailty, and social and psychological indicators among community-dwelling older adults. MethodsThis study is a randomized controlled trial with an open-label, 2-parallel group trial and 1:1 allocation design. Community dwellers aged 65 years and older were enrolled in the trial and divided into the intervention and control groups. The intervention group receives the PICMOA-2 program, a web-based group conversation, once every 2 weeks for 6 months. The primary outcome is verbal fluency, including phonemic and semantic fluency. The secondary outcomes are other neuropsychiatric batteries, including the Mini-Mental State Examination, Logical Memory (immediate and delay), verbal paired associates, and comprehensive functional status evaluated by questionnaires, including frailty, social status, and well-being. The effect of the intervention will be examined using a mixed linear model. As a secondary aim, we will test whether the intervention effects vary with the covariates at baseline to examine the effective target attributes. ResultsRecruitment was completed in July 2023. A total of 66 participants were randomly allocated to intervention or control groups. As of January 1, 2024, the intervention is ongoing. Participants are expected to complete the intervention at the end of February 2024, and the postintervention evaluation will be conducted in March 2024. ConclusionsThis protocol outlines the randomized controlled trial study design evaluating the effect of a 6-month intervention with PICMOA-2. This study will provide evidence on the effectiveness of social interventions on cognitive function and identify effective target images for remote social intervention. Abstract BackgroundAsking questions is common in conversations, and while asking questions, we need to listen carefully to what others say and consider the perspective our questions adopt. However, difficulties persist in verifying the effect of asking questions on older adults’ cognitive function due to the lack of a standardized system for conducting experiments at participants’ homes. ObjectiveThis study examined the intervention effect of cognitive training moderated by robots on healthy older adults. A focus on the feasibility of the intervention at participants’ homes was also maintained. Feasibility was evaluated by considering both the dropout rate during the intervention and the number of questions posed to each participant during the experiment. MethodsWe conducted a randomized controlled trial with 81 adults older than 65 years. Participants were recruited through postal invitations and then randomized into 2 groups. The intervention group (n=40) received sessions where participants listened to photo-integrated stories and posed questions to the robots. The control group (n=41) received sessions where participants listened to photo-integrated stories and only thanked the robots for confirming participation. The participants participated in 12 dialogue sessions for 2-3 weeks. Scores of global cognitive functioning tests, recall tests, and verbal fluency tasks measured before and after the intervention were compared between the 2 groups. ResultsThere was no significant intervention effect on the Telephone Interview for Cognitive Status-Japanese scores, recall tests, and verbal fluency tasks. Additionally, our study successfully concluded with no participant dropouts at follow-up, confirming the feasibility of our approach. ConclusionsThere was no statistically significant evidence indicating intervention benefits for cognitive functioning. Although the feasibility of home-based interventions was demonstrated, we identified areas for improvement in the future, such as setting up more efficient session themes. Further research is required to identify the effectiveness of an improved cognitive intervention involving the act of asking questions.

The efficient separation of solid and liquid phases of soil under reductive conditions is of the utmost importance to study soil chemistry and to predict the mobility and bioavailability of nutrients and toxic contaminants in waterlogged reduced soils (WRSs). However, there is no established method for efficiently separating the solid and liquid phases of WRS within a short time while maintaining its reductive conditions. This study aimed to verify the applicability of a simple centrifugal filtration method (CFM) for the efficient separation of solid and liquid phases of a WRS and examine the CFM-extracted soil solution to confirm that the reductive condition was maintained during the solid–liquid separation process. Incubation experiments were performed under reductive conditions with or without ethanol/molasses used as additional organic material (OM), while the soil solution was collected by both a suction method and CFM at different centrifugation speeds (700, 2760, and 11,000 rpm) and times (1–7 min). The results showed that the soil pH increased with time while the Eh decreased, indicating that its reducing state was enhanced during the incubation experiments. The addition of OM promoted the reductive conditions in the first days of the experiments. Centrifugation speed, rather than time, was found to be the key to extract the maximum amount of soil solution, while a higher centrifugation speed (11,000 rpm), which represents the permanent wilting point, was found to be most effective for extracting the maximum amount of soil solution. The results exhibited no significant difference in solute (As, Fe(II), and Mn) concentrations when varying amounts of CFM-extracted soil solution were measured. The statistical analysis also indicated no significant (p > 0.05) difference between the solute concentrations in the CFM-extracted soil solution and the solute concentrations in the soil solution extracted by the suction method, confirming that the reductive condition was maintained during solid–liquid separation by CFM. This study suggests that CFM operating at a higher centrifugation speed could potentially be employed as a simple and highly effective technique to efficiently separate the solid and liquid phases of WRS (sandy clay loam) within a short time while maintaining its reductive conditions.

BackgroundSocial activity is a key component in the prevention of cognitive decline. However, face-to-face social intervention has limited accessibility. To address this issue, we developed the “Photo-Integrated Conversation Moderated by Application” (PICMOA), a home-based group conversation intervention using smartphones. This paper introduces the PICMOA intervention and the protocol of the ongoing randomized controlled trial (RCT), which aims to evaluate the effects of PICMOA on the cognitive functioning and psychological well-being of Japanese community dwelling older adults at the risk of cognitive function decline.MethodsThis study uses an RCT design in two parallel group trials with 1:1 allocation. The participants are community dwelling older adults aged 65 years and above, living in an urban city in Japan, with subjective cognitive concerns. In total, 81 participants were allocated to the intervention or control groups. The intervention group receives 30 min of weekly PICMOA sessions at their home for 12 weeks. The PICMOA intervention consists of (1) a photo preparation period before the session and (2) a structured group conversation session talking about the photos that participants took according to a specific theme. The control group receives 30 min of weekly health education videos on a tablet device. The primary outcome is cognitive functioning at pre- and post-phases of the 12-week intervention measured using the Telephone Interview for Cognitive Status in Japanese, semantic and phonemic fluency tests, and the Digit Span Forward and Backward tests. The secondary outcomes are psychological and social aspects including mental status, well-being, loneliness, and social support.DiscussionInterest is growing in internet-based activities for preventing social isolation. However, the effect of remote conversation interventions on cognitive functioning remains unclear. Summary:. Sternal osteomyelitis is a rare but devastating complication of median sternotomy. To achieve good outcomes, it should be diagnosed early and treated appropriately. Standard treatment involves antibiotics, debridement, and reconstruction with flaps. To prevent flap complications and recurrence, the wound bed must be prepared carefully. One approach, a recent development, is negative pressure wound therapy with instillation and dwell time (NPWTi-d), where suction cycles are interspersed with wound instillation with solutions. NPWTi-d is currently cautioned against for large trunk wounds and cavities because it might alter core body temperature. Here, we report a new NPWTi-d dressing technique that is associated with successful reconstruction in two severe sternal osteomyelitis cases with wound sizes of 29 × 10 and 28 × 8 cm. This “delay-dressing technique” involves manually pulling the wound edges together; inserting a thin strip of dressing foam; applying dressing film strips from one side of the chest wall to the other, thus placing strong stretching tension on the normal skin around the wound; and then applying NPWTi-d. In our cases, we used the V.A.C. Ulta system for 20 and 17 days. The successful reconstruction in both cases may reflect good wound bed preparation and flap preconditioning due to the mechanical stress imposed by NPWTi-d. Thus, this dressing technique with the V.A.C. Ulta system may be an effective treatment option for sternal osteomyelitis cases.

Abstract Background Social relationships may be the key to successful aging among older adults. However, little is known about the variability of social relationships among community-dwelling older people. This study aimed to describe the patterns of social relationships and examine the differences in sociodemographic characteristics and mental and physical health status among these patterns. Methods We obtained the data from a questionnaire survey in 2017 for older adults aged 65 and above who lived in a suburban area in Japan. The Index of Social Interaction (ISI) was used to evaluate social relationships. The final sample comprised 964 people who were independently mobile and answered at least one item of the ISI. To clarify the patterns of social relationships, latent class analysis was performed with five subscales of ISI treated as indicator variables. Multinomial logistic regression was conducted to examine the factors associated with the patterns of social relationships. Results The patterns of social relationships were classified into three classes: “Active” (73.6%), “Socially isolated” (14.7%), and “Less motivated” (11.7%). Persons who had depressive symptoms were more likely to be allocated to the “Socially isolated” (Odds Ratio [OR] 1.80, 95% Confidence Interval [CI] 1.13–2.86) or the “Less motivated” groups (OR 1.69, 95% CI 1.00–2.85) compared to the “Active” group. In addition, men (OR 1.72, 95% CI 1.07–2.76) and those living alone (OR 3.07, 95% CI 1.43–6.61) were more likely to be allocated to the “Socially isolated” group. Moreover, those who were dependent, according to the instrumental activities and daily living functions, were more likely to be assigned to the “Socially isolated” (OR 2.19, 95% CI 1.21–3.97) or “Less motivated” (OR 6.29, 95% CI 3.47–11.39) groups. Conclusion This study revealed the patterns of social relationships in older adults and suggested that there may be variations of social relationships among community dwellers. The results also indicated the necessity of assessing individual patterns of social relationships and devising strategies for each pattern in public health practice.

Abstract Background The prevalence of osteopenia and osteoporosis is higher in patients with chronic kidney disease than that in the general population. Although physical exercise prevents bone loss in hemodialysis (HD) patients, previous studies have not focused on peritoneal dialysis (PD) patients. Therefore, we aimed to evaluate the effects of home-based exercise on bone mineral density (BMD) in patients with PD. Methods Stable outpatients undergoing PD were randomly assigned to the intervention group (n = 26; male, 20; median age, 66 years) or usual-care group (n = 27; male, 21; median age, 64 years). Patients in the intervention group performed home-based exercises (resistance exercise, stretching, and aerobic exercise such as walking) for 6 months, whereas those in the usual-care group performed stretching and their usual physical activity. Based on dual X-ray absorptiometry, the primary outcomes were the BMD data of the lumbar spine and proximal femoral neck. Secondary outcomes included physical function and physical activity. Pre- and post-intervention values were compared. Results There was no significant within-group change in the BMD of the lumbar spine, femoral neck, and hip after 6 months of the exercise program. The intervention group had significantly improved 30-s chair-stand test, 6-min walk test, and physical activity results. Conclusions Home-based exercises in patients with PD did not improve BMD at any of the sites evaluated. Improvement in physical function and physical activity may reduce the risk of falls in patients with PD. Trial registration UMIN Clinical Trials Registry, UMIN000041678 . Background: Lack of male partners’ postpartum support can cause postpartum depression (PPD), a serious mood disorder, in women. Thus, to prevent PPD, it is important to clarify the factors influencing male partners’ postpartum support. Methods: In total, 62 male partners of postpartum women at 1 week–6 months after delivery, living in South Jakarta, were invited by assistance from community volunteers. Focus group interviews were employed; participants were grouped depending on their residences and asked about their behaviors in supporting postpartum women. Interviews were documented using two voice recorders and two video recorders. The content analysis method and Bertalanffy’s system theory of internal and external factors of health behaviors was utilized to develop units and codes. Results: The analysis identified five codes of individual-level factors: (1) knowledge; (2) belief; (3) social and life adaptation skills; (4) psychological disposition; and (5) physiological condition. Three codes of community-level factors included: (1) social support; (2) media; and (3) socio-cultural and economic. Two codes of social policy-level factors included (1) health care system and; (2) Indonesian societal laws and regulations. Conclusions: Various internal and external factors influence Indonesian male partners’ postpartum support. Different interventions for these factors might be impactful in preventing PPD.

Social interaction might prevent or delay dementia, but little is known about the specific effects of various social activity interventions on cognition. This study conducted a single-site randomized controlled trial (RCT) of Photo-Integrated Conversation Moderated by Robots (PICMOR), a group conversation intervention program for resilience against cognitive decline and dementia. In the RCT, PICMOR was compared to an unstructured group conversation condition. Sixty-five community-living older adults participated in this study. The intervention was provided once a week for 12 weeks. Primary outcome measures were the cognitive functions; process outcome measures included the linguistic characteristics of speech to estimate interaction quality. Baseline and post-intervention data were collected. PICMOR contains two key features: 1) photos taken by the participants are displayed and discussed sequentially; and 2) a robotic moderator manages turn-taking to make sure that participants are allocated the same amount of time. Among the primary outcome measures, one of the subcategories of cognitive functions, verbal fluency significantly improved in the intervention group. Among the process outcome measures, a part of the subcategories of linguistic characteristics of speech, the amount of speech and richness of words, proportion of providing topics, questions, and answers in total utterances were larger for the intervention group. This study demonstrated for the first time the positive effects of a robotic social activity intervention on cognitive function in healthy older adults via RCT. The group conversation generated by PICMOR may improve participants’ verbal fluency since participants have more opportunity to provide their own topics, asking and answering questions which results in exploring larger vocabularies. Objectives: To translate and study the factor structure of a Japanese version of the Stressors in Nursing Students scale. Methods: The Stressors in Nursing Students scale was translated into Japanese and administered to a large cohort (N = 1298) of female Japanese nursing students across five universities and across all four years of the nursing programme. The data were analysed using exploratory factor analysis. Results: Exploratory factor analysis revealed four factors: ‘Clinical’; ‘Conflict and confidence’; ‘Education’; and ‘Free time’. The relationship between the total score on the Stressors in Nursing Students scale and the demographic aspects of the sample was weak but there was a significant increase in the perception of stressors between first and second year students. Conclusions: There were similarities and differences between the perception of stressors by nursing students in Japan compared with, for example, the United Kingdom and China mainland. While a four-factor structure was found here, the distribution of items in the Stressors in Nursing Students scale differed from previous studies in the United Kingdom and China with the ‘Free time’ factor being unique to this study. Stress is an issue for nursing students and is related to the specific stressors they encounter in the process of their work and study. Findings can inform the development of strategies to reduce such stressors among nursing students in multiple, global contexts. Keywords: Nursing students, Factor analysis, Japan, Stressors

Background: Since limited, specific assessments of social environments have previously been used, with the effect of a change in social interaction proving rare in previous studies, this study sought to clarify the specific association between changes in social interaction and symptoms of dementia. Methods: The participants were elderly people, aged > 60 years, from a suburban community. All the participants formed part of the investigation from 2008 to 2011. The Index of Social Interaction was used to measure social interaction among elderly people. Data of dementia were obtained from Health and Welfare Center of local government. The chi-square test and a logistical regression analysis were used to examine the relationship between changes in social interaction and dementia. Results: There were 321 participants without missing data. The results showed that “reading newspapers” was a positive change in social interaction in relation to the risk of dementia (odds ratio=3.45, confidence interval: 1.04–11.45). Conclusion: This study focused on changes in social interaction and found that a positive change could help prevent dementia. Specifically, this study indicated that elderly people's engagement with the social environment and in intellectual activities would prevent dementia.

Objectives: This study aimed to evaluate food consumption frequency in 1-6-year-olds as an eating behaviour-related predictor of behavioural problems over a span of six years. Eating behaviour in early childhood serves as a foundation for future health outcomes. Diet patterns can have long-term beneficial or adverse effects on social behaviour development. Methods: This longitudinal study was performed based on information obtained between July 2011 and August 2017 provided from a project named 'Community Empowerment and Care for Wellbeing and Health Longevity' initiated in 1991; the current study involved 124 mother-child dyads from the project. Children aged 1-6 years were studied in July 2011, with a follow-up assessment in August 2017. The primary exposure examined was the frequency of food items intake. The primary outcome was behaviour problems as assessed by the Strengths and Difficulties Questionnaire (SDQ). Results: The adjusted results suggested that a higher intake of leafy green and light-coloured vegetables were significantly associated with decreased odds of conduct problems and prosocial behaviour problems in Japanese children. However, no associations were observed among fruits, milk, small fish, eggs, soybeans, seaweed and any SDQ subscales. Conclusion: This study shows that eating leafy green and light-coloured vegetables may have a protective effect on a child's conduct and against prosocial behaviour problems. Due consideration should be given to children's eating habits in the early stages of their lives to ensure better mental health. [ABSTRACT FROM AUTHOR]

Background: Social relationships might be a key factor for preventing the decline of instrumental activities of daily living (IADL). In the current study, we examined the association between social relationships and IADL decline, and how this association differed by age. Methods: The study employed a prospective cohort design by introducing complete sampling. Questionnaires were distributed in 2011 and 2017 to all residents over 65 years old living in a suburban area of central Japan. Analysis included 501 residents who were independent in IADL in 2011 and who participated after 6 years without missing data. Social relationships were evaluated by the Index of Social Interaction, and IADL were measured by the Tokyo Metropolitan Institute of Gerontology Index of Competence. Multiple logistic regression analysis was used to examine the association between social relationships and IADL. An age-stratified analysis was also conducted. Results: One hundred three participants showed IADL decline after 6 years. Age (OR = 1.18) and social relationships (OR = 0.79) were significantly associated with IADL decline. In the age-stratified analysis, age (OR = 1.21) and having a musculoskeletal disease (OR = 5.09)were significantly associated with IADL decline among individuals 65-74 years old. Among individuals 75 years old or older, age (OR = 1.31) and social relationships (OR = 0.58) were significantly associated with IADL decline. Conclusion: Social relationships might prevent IADL decline in older people. Furthermore, their effects might differ by age group.
