Abstract

To compare the effectiveness, safety and patient satisfaction of a double balloon catheter (DB) with a synthetic osmotic cervical dilator (OD) for pre-induction cervical ripening in an outpatient setting. This is a prospective, dual-center pilot study including 94 patients with an unripe cervix (Bishop Score <6) near term; 50 patients received the DB and 44 patients the OD. The primary outcomes were the difference in BishopScore (BS) and cervical shortening. Pain perception at insertion and during the cervical ripening period was evaluated by a visual analogue scale and patient satisfaction by a predefined questionnaire. The use of DB was associated with a significantly higher increase in BS (median 3) compared to OD (median 2; p=0.002) and resulted in significantly greater cervical shortening (median −14 mm vs. −9 mm; p=0.003). There were no serious adverse events at placement of devices or during the cervical ripening. There were no significant differences in perinatal outcomes. Pain perception during cervical ripening was significantly higher (p<0.001), and patient satisfaction regarding sleep, relaxing time and performing desired daily activities were significantly lower in patients with DB compared to patients with OD (p<0.001). DB was superior to OD regarding cervical ripening based on BS and on sonographic measurement of the cervical length. However, CVE remain the leading cause for morbidity and mortality in patients after NTX. Nevertheless, kidney transplant candidates are less likely to receive cardiovascular diagnostics. So far, the best cardiovascular diagnostic regime for kidney transplant candidates is unknown. Objective: Aim of this study was to analyze the application of cardiovascular diagnostics in NTX candidates and the occurrence of CVE after NTX at Uniklinik RWTH Aachen. Methods: Charts of 212 patients after NTX at Uniklinik RWTH Aachen between 2005 and 2011 were analyzed retrospectively. Relevant data were collected in a data base. Statistical analysis was performed using descriptive and inductive data analysis. Study-relevant end points were analyzed via Kaplan-Meier survival curves and Cox regression analysis. Results: In 22.6 % of the patients a CVE occurred after NTX and 23.8 % of death events resulted from CVE. Abstract

JUSTIFICATIVA E OBJETIVOS: No âmbito das afecções do aparelho locomotor, a dor é o sintoma mais frequente e, além da alta prevalência, sua importância decorre por estar presente nas doenças que causam maior impacto negativo na qualidade de vida e na produtividade dos indivíduos acometidos. O objetivo deste estudo foi investigar tópicos relevantes sobre a influência da dor osteoarticular crônica na capacidade funcional no idoso.CONTEÚDO: Foi realizado um estudo bibliográfico mediante consulta às principais publicações sobre o tema. O material foi identificado com auxílio das bases eletrônicas de dados da Medline, LILACS, Scielo e Pubmed. Foram empregados os descritores "dor crônica", "doenças osteoarticulares" e "capacidade funcional".CONCLUSÃO: Deve haver maior atenção às políticas públicas de implementação de programas que visem principalmente a prevenção e o controle das doenças crônicas osteoarticulares, promovendo funcionalidade e maior qualidade de vida para essa população. BACKGROUND AND OBJECTIVES: Among locomotor system diseases, pain is the most frequent symptom and, in addition to its high prevalence, it is important because it is present in diseases causing the highest negative impact on quality of life and productivity of affected individuals. This study aimed at investigating relevant topics on the influence of chronic osteoarticular pain on the functional capacity of the elderly.CONTENTS: A bibliographic study was carried out by querying major publications on the subject. Material was identified with the aid of electronic databases Medline, LILACS, Scielo and Pubmed. Keywords used were "chronic pain", "osteoarticular diseases" and "functional capacity".CONCLUSION: Further attention should be given to public policies implementing programs aiming especially at preventing and controlling chronic osteoarticular diseases, promoting functionality and better quality of life for this population.
