Abstract

Birnbaum and Saunders introduced a two-parameter lifetime distribution to model fatigue life of a metal, subject to cyclic stress. Since then, extensive work has been done on this model providing different interpretations, constructions, generalizations, inferential methods, and extensions to bivariate, multivariate and matrix-variate cases. More than two hundred papers and one research monograph have already appeared describing all these aspects and developments. In this paper, we provide a detailed review of all these developments and at the same time indicate several open problems that could be considered for further research.

4. Analysis of Type-II hybrid censored competing risks data

Abstract

Kundu and Gupta (2007, Metrika, 65, 159 - 170) provided the analysis of Type-I hybrid censored competing risks data, when the lifetime distribution of the competing causes of failures follow exponential distribution. In this paper we consider the analysis of Type-II hybrid censored competing risks data. It is assumed that latent lifetime distributions of the competing causes of failures follow independent exponential distributions with different scale parameters. It is observed that the maximum likelihood estimators of the unknown parameters do not always exist. We propose the modified estimators of the scale parameters, which coincide with the corresponding maximum likelihood estimators when they exist, and asymptotically they are equivalent. We obtain the exact distribution of the proposed estimators. Using the exact distributions of the proposed estimators, associated confidence intervals are obtained. The asymptotic and bootstrap confidence intervals of the unknown parameters are also provided. Further, Bayesian inference of some unknown parametric functions under a very flexible Beta-Gamma prior is considered. Bayes estimators and associated credible intervals of the unknown parameters are obtained using Monte Carlo method. Extensive Monte Carlo simulations are performed to see the effectiveness of the proposed estimators and one real data set has been analyzed for the illustrative purposes. It is observed that the proposed model and the method work quite well for this data set.

5. Active mechanical cloaking for unsupervised damage resilience in programmable elastic metamaterials.

Abstract

Owing to the architected void-filled low-density configurations, metamaterials are prone to defects during the complex manufacturing process, or damages under operational conditions. Recently mechanical cloaking has been proposed to shield the effect of such disorders in terms of homogenized mechanical responses. The major drawback in these studies are that the damage location should be known a priori, and the cloak is designed around that damaged zone before manufacturing. Such postulation does not allow unsupervised damage resilience during the manufacturing and service life of metamaterials by active reconfiguration of the stress field depending on the random and unpredictable evolution of damage. Here, we propose a radically different approach by introducing piezoelectric lattices where the effect of random appearance of any single or multiple disorders and damages with complex shapes, sizes and distributions can be shielded through active multi-physically controlled cloaks by voltage-dependent modulation of the stress fields within the cloaking region. Notably, this can be achieved without breaking periodicity and any additional material in the cloaking region unlike earlier studies concerning mechanical cloaks. The proposed active class of elastic metamaterials will bring a step-change in the on-demand mechanical performance of critically important structural components and unsupervised damage resilience for enhanced durability and sustainability. Abstract

Recently Jammalamadaka and Mangalam [2003. Non-parametric estimation for middle censored data. J. Nonparametric Statist. 15, 253-265] introduced a general censoring scheme called the "middle-censoring" scheme in non-parametric set up. In this paper we consider this middle-censoring scheme when the lifetime distribution of the items is exponentially distributed and the censoring mechanism is independent and non-informative. In this set up, we derive the maximum likelihood estimator and study its consistency and asymptotic normality properties. We also derive the Bayes estimate of the exponential parameter under a gamma prior. Since a theoretical construction of the credible interval becomes quite difficult, we propose and implement Gibbs sampling technique to construct the credible intervals. Monte Carlo simulations are performed to evaluate the small sample behavior of the techniques proposed. A real data set is analyzed to illustrate the practical application of the proposed methods. © 2008 Elsevier B.V. All rights reserved.

16. Influence of seedling age and nitrogen application on photosynthesis and yield of rice (Oryza sativa) grown under waterlogged condition

Abstract

Rice seedlings of three different age groups, i.e.,30, 45 and 60 days old were grown with three different N levels, i.e.,0, 20 and 40 kg ha−1for two consecutive years under waterlogged condition. The highest yield was obtained with 40 kg N ha−1in crop grown with 60 day old seedlings in both the years. Higher dosage of N application as mud ball showed higher stomatal conductance, net photosynthesis and transpiration. The response of N was better in crop grown with older seedlings. Grain yield showed significant positive relationship with net photosynthesis rate (r= 0.82**) and carboxylation efficiency (r= 0.81**) than with stomatal conductance (r= 0.66), and could be critical determinant for better yield in crop grown with older seedlings at higher N level under waterlogged condition.

Abstract

BackgroundIn locations where few people have received COVID-19 vaccines, health systems remain vulnerable to surges in SARS-CoV-2 infections. Tools to identify patients suitable for community-based management are urgently needed.MethodsWe prospectively recruited adults presenting to two hospitals in India with moderate symptoms of laboratory-confirmed COVID-19 in order to develop and validate a clinical prediction model to rule-out progression to supplemental oxygen requirement. The primary outcome was defined as any of the following: SpO2 < 94%; respiratory rate > 30 bpm; SpO2/FiO2 < 400; or death. We specified a priori that each model would contain three clinical parameters (age, sex and SpO2) and one of seven shortlisted biochemical biomarkers measurable using near-patient tests (CRP, D-dimer, IL-6, NLR, PCT, sTREM-1 or suPAR), to ensure the models would be suitable for resource-limited settings. We evaluated discrimination, calibration and clinical utility of the models in a temporal external validation cohort.Findings426 participants were recruited, of whom 89 (21·0%) met the primary outcome. 257 participants comprised the development cohort and 166 comprised the validation cohort. The three models containing NLR, suPAR or IL-6 demonstrated promising discrimination (c-statistics: 0·72 to 0·74) and calibration (calibration slopes: 1·01 to 1·05) in the validation cohort, and provided greater utility than a model containing the clinical parameters alone.InterpretationWe present three clinical prediction models that could help clinicians identify patients with moderate COVID-19 suitable for community-based management. The models are readily implementable and of particular relevance for locations with limited resources.FundingMédecins Sans Frontières, India.RESEARCH IN CONTEXTEvidence before this studyA living systematic review by Wynants et al. identified 137 COVID-19 prediction models, 47 of which were derived to predict whether patients with COVID-19 will have an adverse outcome. Most lacked external validation, relied on retrospective data, did not focus on patients with moderate disease, were at high risk of bias, and were not practical for use in resource-limited settings. To identify promising biochemical biomarkers which may have been evaluated independently of a prediction model and therefore not captured by this review, we searched PubMed on 1 June 2020 using synonyms of “SARS-CoV-2” AND [“biomarker” OR “prognosis”]. We identified 1,214 studies evaluating biochemical biomarkers of potential value in the prognostication of COVID-19 illness. In consultation with FIND (Geneva, Switzerland) we shortlisted seven candidates for evaluation in this study, all of which are measurable using near-patient tests which are either currently available or in late-stage development.Added value of this studyWe followed the TRIPOD guidelines to develop and validate three promising clinical prediction models to help clinicians identify which patients presenting with moderate COVID-19 can be safely managed in the community. Each model contains three easily ascertained clinical parameters (age, sex, and SpO2) and one biochemical biomarker (NLR, suPAR or IL-6), and would be practical for implementation in high-patient-throughput low resource settings. The models showed promising discrimination and calibration in the validation cohort. The inclusion of a biomarker test improved prognostication compared to a model containing the clinical parameters alone, and extended the range of contexts in which such a tool might provide utility to include situations when bed pressures are less critical, for example at earlier points in a COVID-19 surge.Implications of all the available evidencePrognostic models should be developed for clearly-defined clinical use-cases. We report the development and temporal validation of three clinical prediction models to rule-out progression to supplemental oxygen requirement amongst patients presenting with moderate COVID-19. The models are readily implementable and should prove useful in triage and resource allocation. We provide our full models to enable independent validation.

38. A case of anterior scleritis in association with posterior scleritis - a diagnostic riddle

Abstract

We herein report a case of a young female presenting with multiple nodular scleral abscesses mimicking infective scleritis with exudative retinal detachment. Repeated diagnostic scraping for microbiological and histopathological analysis was inconclusive. The patient's systemic and collagen disease work-up was non-contributory. She was treated with multiple surgical debridement and de-roofing of the abscesses along with antibiotic irrigation. Topical and oral steroid was stopped on worsening of the condition, and the patient was maintained on topical and oral antibiotics. Complete resolution of scleritis and exudative detachment was noted at 1 month follow-up and no recurrence was noted until 6 months.

Abstract

OBJECTIVES: The value of the "trace" result in Xpert Ultra for diagnosing active tuberculosis (TB) remains unclear. Our study evaluated the diagnostic accuracy of Xpert MTB/RIF Ultra (Xpert Ultra) (Cepheid, Sunnyvale, USA) over Xpert MTB/RIF (Xpert) (Cepheid, Sunnyvale, USA) and mycobacterial culture when compared with a composite reference standard (CRS). METHODS: A retrospective single-center observational study was conducted in a tertiary care hospital in South India. Over three months, patients (aged ≥15 years) data on Xpert Ultra tests and mycobacterial culture of pulmonary and extrapulmonary samples were extracted from their electronic medical records. Patients were defined as TB cases based on the CRS criteria. Sensitivity, specificity, positive and negative predictive values of diagnostic tests were calculated by comparing them to the CRS. RESULTS: Xpert Ultra was more sensitive (87.8%) than Xpert (72.1%) and culture (44.1%). The specificity of Xpert Ultra was lower (98.1%) than those of Xpert (100%) and culture (100%). The sensitivity (92%) and specificity (100%) of Xpert Ultra were highest when performed on pus samples. CONCLUSIONS: Xpert Ultra with the trace category is superior to the conventional Xpert, and mycobacterial culture in identifying TB.

Abstract

Soil stability, yield decline and higher carbon decomposition are the key issues in intensive agriculture. Long-term integrated nutrient management (INM) influences carbon (C) dynamics and maintains the soil aggregate stability, which driven by the type of cropping system and climatic zone. Therefore, the specific effects of rice-wheat-jute cropping system on soil aggregate distribution, C-stability, and trade-off between C-storage and decomposition rate were studied of a long-term INM trial in the sub-tropical zone. We investigated the effect of four long-term nutrient management practices that include, control, N (nitrogen (N) and no phosphorus (P), and potassium (K)), NPK (N, P and K) and INM (N, P, K + farmyard manure (FYM)). Results showed that the addition of FYM with NPK improved macro-aggregates by 71%, and associated C-pools by 30%. The INM enhanced C-storage (20%), however, the decomposition rate was also higher (28%) over control at 35 °C at field capacity. A trade-off exists between the soil-aggregate stabilization and C-decomposition rate. The proportion of C-storage was higher than decomposition in INM. Further, the C-sequestration varied (0.17 to 0.26 t ha−1 y−1) under different treatments and were found highest in INM, indicating long-term soil stability and yield sustainability in the rice-wheat-jute system. INTRODUCTION In locations where few people have received Covid-19 vaccines, health systems remain vulnerable to spikes in SARS-CoV-2 infections. Triage tools, which could include biomarkers, to identify patients with moderate Covid-19 infection suitable for community-based management would be useful in the event of surges. In consultation with FIND (Geneva, Switzerland) we shortlisted seven biomarkers for evaluation, all measurable using point-of-care tests, and either currently available or in late-stage development. METHODS We prospectively recruited unvaccinated adults with laboratory-confirmed Covid-19 presenting to two hospitals in India with moderate symptoms, in order to develop and validate a clinical prediction model to rule-out progression to supplemental oxygen requirement. Moderate disease was defined as oxygen saturation (SpO2) ≥ 94% and respiratory rate < 30 breaths per minute (bpm), in the context of systemic symptoms (breathlessness or fever and chest pain, abdominal pain, diarrhoea, or severe myalgia). All patients had clinical observations and blood collected at presentation, and were followed up for 14 days for the primary outcome, defined as any of the following: SpO2 < 94%; respiratory rate > 30 bpm; SpO2/fraction of inspired oxygen (FiO2) < 400; or death. We specified a priori that each model would contain three easily ascertained clinical parameters (age, sex, and SpO2) and one of the seven biomarkers (C-reactive protein (CRP), D-dimer, interleukin-6 (IL-6), neutrophil-to-lymphocyte ratio (NLR), procalcitonin (PCT), soluble triggering receptor expressed on myeloid cells-1 (sTREM-1), or soluble urokinase plasminogen activator receptor (suPAR)), to ensure the models would be implementable in high patient-throughput, low-resource settings. We evaluated the models’ discrimination, calibration, and clinical utility in a held-out external temporal validation cohort. ETHICS Ethical approval was given by the ethics committees of AIIMS and CMC, India, the Oxford Tropical Research Ethics Committee, UK; and by the MSF Ethics Review Board. Furthermore, they provided greater utility than a model containing the clinical parameters alone (c-statistic = 0.66; calibration slope = 0.68). The inclusion of either NLR or suPAR improved predictive performance such that the ratio of correctly to incorrectly discharged patients increased from 10:1 to 23:1 or 25:1 respectively. Including IL-6 resulted in a similar proportion (~21%) of correctly discharged patients as the clinical model, but without missing any patients requiring supplemental oxygen. CONCLUSION We present three clinical prediction models that could help clinicians identify patients with moderate Covid-19 suitable for community-based management.
