The role of pathological biomineralization (PBM) as a prognostic and diagnostic marker in thyroid cancer is continuously debated among investigators. Detection of pathological biomineralization in the lymph nodes of the neck is an alarming signal for clinicians. Typically, a lymph node with signs of calcification is a symptom of papillary thyroid cancer. The lymph node contains such a form of calcification as psammoma bodies in such cases. Psammoma bodies of relatively large size (more than 200 μm) can be detected by ultrasound. Our study aims to study the crystal-chemical and phase characteristics of calcifications of metastatic lymph nodes in thyroid cancer to develop promising methods of early diagnosis. Materials and methods. Several complex research methods have been conducted for a deeper understanding of the pathological biomineralization of metastatic lymph nodes in thyroid cancer, such as macroscopic examination, ultrasound diagnostics, detection of strict lymph node, histological, histochemical, and electron microscopic (scanning electron microscopy, X-ray diffraction, and transmission electron microscopy). Our study found that the main component of pathological biomineral deposits is calcium phosphate compounds. The Ca / P ratio corresponds to the characteristic features of hydroxyapatite. A significant proportion of β-tricalcium magnesium phosphate was also detected in some cases. The specific plate structure and the known phase and crystal-chemical composition of psammoma bodies can be the point of application of searches for their early detection using the latest diagnostic methods with high resolution. Conclusion. Our study demonstrated the significance of PBM in lymph nodes as a diagnostic symptom of papillary thyroid cancer patients. The presence and extent of PBM in the lymph nodes should be considered a metastasis of papillary thyroid cancer. The study of the structure, physicochemical, phase composition of lymph node calcifications, and visualization features is promising given the possible practical application for early diagnosis of metastases of papillary thyroid cancer. Вступ. Рак щитоподібної залози посідає дев’яте місце в структурі злоякісних новоутворень. З 1980-х років у більшості країн світу спостерігалося швидке зростання рівня захворюваності та відносно стабільний або навіть знижувальний рівень смертності. Мета нашого дослідження – проаналізувати тенденції захворюваності та смертності від раку щитовидної залози в Сумській області протягом 2014–2021 рр., вивчити поширеність гістологічних варіантів та статево-вікових характеристик та порівняти їх із загальноукраїнськими та світовими даними. . Результати дослідження. За даними Національного канцер-реєстру України, з 2014 по 2021 рік на рак щитовидної залози виявлено 24 626 осіб, з них 4 556 чоловіків (18,5%) і 2 0070 жінок (81,5%). У середньому за рік діагностовано 3078,25 ± 136,15 осіб. Рівень захворюваності серед жінок у 3,8 рази вищий, ніж серед чоловіків. Середній показник захворюваності в Україні за 2014–2021 роки становив 6,5 на 100 тис. населення, у Сумській області – 8,9 на 100 тис. населення. Середні показники смертності жителів Сумської області та України не відрізнялися і становили 0,4 на 100 тис. населення. Особливістю є нижчий порівняно із загальноукраїнськими показниками показник чоловічої та жіночої смертності в Сумській області (0,2 проти 0,3 на 100 тис. населення та 0,6 проти 0,4 на 100 тис. населення відповідно). Вивчення гістологічних типів злоякісних новоутворень щитоподібної залози показало, що найчастіше зустрічається папілярний варіант (70,5-81,9%). Питома частка фолікулярного раку становила від 12,8 до 19,2 %. Значно рідше зустрічався медулярний варіант – у 1,5-4,6% випадків, тоді як недиференційований і плоскоклітинний рак зустрічався вкрай рідко (0,2-3,3% випадків). У Буринському районі, Великописарівському районі, Конотопському районі, Краснопільському районі, Кролевецькому районі, Лебединському районі, Недригайлівському районі, Путивльському районі, Сумському районі, Тростянецькому районі та м. Суми спостерігався значно вищий рівень захворюваності. Найчастіше в Україні хворіють жінки віком 50–64 роки та чоловіки віком 55–69 років. Найвищий рівень смертності спостерігається у віковому діапазоні 70–84 років як для чоловіків, так і для жінок. Висновки. Протягом 2014–2021 років показники захворюваності та смертності населення України залишалися на стабільному рівні та були нижчими порівняно зі світовими показниками. З 2017 року в Сумській області спостерігається зниження рівня захворюваності, що відповідає європейським тенденціям. У Сумській області найпоширенішим гістологічним варіантом залишається папілярний рак щитоподібної залози. Загалом частки папілярного, фолікулярного, медулярного та анапластичного раку відповідають європейським показникам. Загалом вікові тенденції в Україні та інших країнах світу схожі. Introduction. Thyroid cancer ranks ninth in the structure of malignant neoplasms. Since the 1980s, most countries in the world have seen a rapid increase in the incidence rate and a relatively stable or even decreasing mortality rate. The objective of our study was to analyze the thyroid cancer morbidity and mortality trends in the Sumy region during 2014–2021, to study the prevalence of histological variants and sex-age-specific characteristics, and to compare them with all-Ukrainian and global data. Study results. According to the National Cancer Registry of Ukraine, 24,626 people were diagnosed with thyroid cancer between 2014 and 2021, including 4,556 men (18.5%) and 2,0070 women (81.5%). On average, 3078.25 ± 136.15 people were diagnosed per year. The incidence rate among women is 3.8 times higher than among men. The average incidence rate in Ukraine in 2014–2021 was 6.5 per 100,000 population, while in the Sumy region – 8.9 per 100,000 population. The average mortality rates among residents of the Sumy region and Ukraine did not differ and amounted to 0.4 per 100,000 population. A distinctive feature was represented by the lower male and higher female mortality rates in the Sumy region compared to all-Ukrainian indicators (0.2 versus 0.3 per 100,000 population and 0.6 versus 0.4 per 100,000 population, respectively). The study of histological types of thyroid malignancies demonstrated that papillary variant was most common (70.5 to 81.9%). The specific share of follicular cancer ranged from 12.8 to 19.2%. The medullary variant occured much less often – in 1.5 to 4.6% of cases, while undifferentiated and squamous cell cancer was extremely rare (0.2 to 3.3% of cases). In Buryn district, Velyka Pysarivka district, Konotop district, Krasnopillia district, Krolevets district, Lebedyn district, Nedrygailiv district, Putyvl district, Sumy district, Trostyanets district and the city of Sumy, a significantly higher incidence rate was observed. Women aged 50–64 and men aged 55–69 are most often affected in Ukraine. The highest mortality rate is reported for the age range of 70–84 years for both males and females. Conclusions. During 2014–2021, the incidence and mortality rates among the population of Ukraine remained at a stable level and were lower compared to global indicators. Since 2017, the Sumy region has been presenting with a decrease in the incidence rate, which corresponds to European trends. In the Sumy region, papillary thyroid cancer remains the most common histological variant. In general, the shares of papillary, follicular, medullary, and anaplastic cancers are consistent with the European figures. In general, the age trends in Ukraine and other countries of the world are similar.

Вступ. Лікування хворих на рак щитоподібної залози (РЩЗ) із клінічно невиявленими метастазами у регіонарні лімфатичні вузли залишається невирішеною проблемою сучасної онкології. Пошук надійних методів діагностики, на яких грунтується рішення про проведення адекватної лімфаденектомії при РЩЗ, є одним з пріоритетних напрямків онкохірургії. Мета дослідження – пошук ефективного методу інтраопераційного контрастування сторожових лімфатичних вузлів (СЛВ) у хворих на РЩЗ за допомогою толуїдинового синього та оцінка його ефективності. Методи. Проведене дослідження 123 хворих на РЩЗ, яким проводили оперативне лікування із застосуванням контрастно-візуального методу у обсязі тиреоїдектомії з центральною та двосторонньою селективною шийною лімфодисекцією (рівні VI; IIA; III; IV). Під час оперативного втручання у пухлину за допомогою шприца вводили 1% розчин тулуїдинового синього. Оперативне лікування виконували у два етапи. На першому – проводилася тиреоїдектомія у блоці з центральним лімфоколектором шиї (рівень IV), який включав видалення паратрахеальної, преларингеальної, навколощитоподібної клітковини шиї. Проводили термінове інтраопераційне гістологічне дослідження пухлини щитоподібної залози та віддалених СЛВ. На другому етапі операції виконували шийну ліфодисекцію з видаленням клітковини IIA; III; IV рівнів шиї, з обов'язковою ревізією рівня VB. Після операції проводили остаточне гістологічне дослідження видаленого препарату з пухлиною та усіх лімфатичних вузлів. Результати. Після застосування методики контрастування СЛВ були виявлені у 120 (97,6%) хворих. Їх кількість варіювала від 2 до 12. Ці лімфатичні вузли в основному локалізувались у центральному колекторі шиї на боці первинної пухлини (центральні, іпсилатеральні СЛВ) у 91 (75,8%) хворого. На підставі результатів інтраопераційного дослідження СЛВ метастази у них були виявлені у 33 (27,5 %) з 120 хворих. У переважній більшості випадків (83,3%) СЛВ визначалися в центральному лімфоколекторі шиї (VI рівень). У 76 (63,3%) хворих відбулося фарбування лімфатичних вузлів III рівня шиї (середній іпсилатеральний югулярний лімфоколектор), у 11 (9,2%) СЛВ були виявлені лише на III, IV та VB рівнях шиї. Досить рідко (5,8%), контрастувалися лімфатичні вузли передньо-верхнього середостіння. Висновки. Метод контрастної візуалізації регіонарного колектора у хворих на РЩЗ за допомогою 1% розчину толуїдинового синього є високоінформативним дослідженням, що дозволяє визначати уражені метастазами регіонарні лімфатичні вузли та диференційовано підходити до вибору об'єму лімфодисекції на ранніх стадіях пухлинного процесу. Метод простий у виконанні, не вимагає великих фінансових витрат і може бути рекомендованим для використання у онкологічних клініках. Introduction. Treatment of patients with thyroid cancer (TС) with clinically undetected metastases to regional lymph nodes remains an unsolved problem of modern oncology. The search for reliable diagnostic methods, which underlie the decision to perform an adequate lymphadenectomy for TC, is one of the priorities of oncosurgery. The aim of the research: to find an effective method of intraoperative contrasting of sentinel lymph nodes (SLN) in patients with TC using toluidine blue and to evaluate its effectiveness. Methods. A study of 123 patients with TC who underwent surgical treatment using the contrast-visual method in the amount of thyroidectomy with central and bilateral selective cervical lymph node dissection (levels VI; IIA; III; IV). During surgery, a 1% solution of toluidine blue was injected into the tumor using a syringe. Surgical treatment was performed in two stages. At the first stage – thyroidectomy was performed in a block with a central lymphatic collector of the neck (level IV), which included removal of paratracheal, prelaryngeal, parathyroid tissue of the neck. Urgent intraoperative histological examination of thyroid tumor and distant SLN was performed. In the second stage of the operation performed cervical lymph node dissection with removal of fiber IIA; III; IV levels of the neck, with mandatory revision of the level of VB. After the operation, the final histological examination of the removed specimen with the tumor and all lymph nodes was performed. Results. After the application contrasting technique, SLN were found in 120 (97.6%) patients. Their number varied from 2 to 12. These lymph nodes were mainly localized in the central collector of the neck on the side of the primary tumor (central, ipsilateral SLN) in 91 (75.8%) patients. Based on the results of an intraoperative study of SLN, metastases were found in 33 (27.5%) of 120 patients. In most cases (83.3%) SLNs were detected in the central lymphatic collector of the neck (level VI). In 76 (63.3%) patients, staining of the lymph nodes of the III level of the neck (middle ipsilateral jugular lymphatic collector) occurred, in 11 (9.2%) SLNs were found only at levels III, IV and VB of the neck. Very rarely (5.8%), the lymph nodes of the anterior-superior mediastinum were contrasted. Conclusions. The method of contrast visualization of the regional collector in patients with TC using a 1% solution of toluidine blue is a highly informative study that allows to determine the regional lymph nodes affected by metastases and to differentiate the choice of the volume of lymph node dissection in the early stages of tumors. The method is simple to implement, does not involve considerable costs and can be recommended for use in oncology clinics.

Злоякісні новоутворення гортані в останнє десятиріччя виходять на провідні позиції онкологічних захворювань верхніх дихальних шляхів, за даними Національного канцер-реєстру у 2015 році було зареєстровано 1967 нових випадків раку гортані на Україні. Серед основних етіологічних чинників: паління, шкідливі чинники навколишнього середовища, інфікування папілома-вірусом. Спостерігається прогресивна тенденція до зростання захворюваності на рак гортані, що спонукає до пошуку нових методик лікування та післяопераційної реабілітації.

«Золотим стандартом» в лікуванні ІІІ-ІV стадії раку гортані залишається саме ларингектомія. У післяопераційному періоді спостерігається велика кількість різноманітних ускладнень, але на наш погляд, найбільш важливими залишаються саме респіраторні порушення.

Laryngeal malignant neoplasms is getting in last decade into leading position among oncological diseases of upper respiratory tracts in the last decade. To main etiological triggers belong: smoking, hazardous environmental impacts, papilloma-virus infection. A progressive tendency increasing towards laryngeal cancer incidence is observed that motivates to look for new methods of therapy and post-surgery rehabilitation, especially considering the large number of post-surgery complications.
