Abstract

We introduce a new flexible one-dimensional photochemical model named Photochemical and RadiatiOn Transport model for Extensive USe (PROTEUS), which consists of a Python graphical user interface (GUI) program and Fortran 90 modules. PROTEUS is designed for adaptability to many planetary atmospheres, for flexibility to deal with thousands of or more chemical reactions with high efficiency, and for intuitive operation with GUI. Chemical reactions can be easily implemented into the Python GUI program in a simple string format, and users can intuitively select a planet and chemical reactions on GUI. Chemical reactions selected on GUI are automatically analyzed by string parsing functions in the Python GUI program, then applied to the Fortran 90 modules to simulate with the selected chemical reactions on a selected planet. PROTEUS can significantly save the time for those who need to develop a new photochemical model; users just need to write chemical reactions in the Python GUI program and just select them on GUI to run a new photochemical model.

Abstract

Planetary climates are strongly affected by planetary orbital parameters such as obliquity, eccentricity, and precession. In exoplanetary systems, exo-terrestrial planets should have various obliquities. High-obliquity planets would have extreme seasonal cycles due to the seasonal change of the distribution of the insolation. Here, we introduce the Non-hydrostatic ICosahedral Atmospheric Model(NICAM), a global cloud-resolving model, to investigate the climate of high-obliquity planets. This model can explicitly simulate a three-dimensional cloud distribution and vertical transports of water vapor. We simulated exo-terrestrial climates with high resolution using the supercomputer FUGAKU. We assumed aqua-planet configurations with 1 bar of air as a background atmosphere, with four different obliquities ($0^{\circ}$, $23.5^{\circ}$, $45^{\circ}$, and $60^{\circ}$). We ran two sets of simulations: 1) low-resolution (~ 220 km-mesh as the standard resolution of a general circulation model for exoplanetary science) with parametrization for cloud formation, and 2) high-resolution (~ 14 km-mesh) with an explicit cloud microphysics scheme. Results suggest that high-resolution simulations with an explicit treatment of cloud microphysics reveal warmer climates due to less low cloud fraction and a large amount of water vapor in the atmosphere. It implies that treatments of cloud-related processes lead to a difference between different resolutions in climatic regimes in cases with high obliquities., Comment: 18 pages, 9 figure, 2 tables. Abstract

Global characteristics of the small-scale gravity wave (GW) field in the Martian atmosphere obtained from a high-resolution general circulation model (GCM) are presented for the first time. The simulated GW-induced temperature variances are in a good agreement with available radio occultation data in the lower atmosphere between 10 and 30 km. The model reveals a latitudinal asymmetry with stronger wave generation in the winter hemisphere, and two distinctive sources of GWs: mountainous regions and the meandering winter polar jet. Orographic GWs are filtered while propagating upward, and the mesosphere is primarily dominated by harmonics with faster horizontal phase velocities. Wave fluxes are directed mainly against the local wind. GW dissipation in the upper mesosphere generates body forces of tens of m~s$^{-1}$~sol$^{-1}$, which tend to close the simulated jets. Abstract

First high-altitude observations of gravity wave (GW)-induced CO$_2$ density perturbations in the Martian thermosphere retrieved from NASA's NGIMS instrument on board the MAVEN satellite are presented and interpreted using the extended GW parameterization of Yi\u{g}it et al. [2008] and the Mars Climate Database as an input. Observed relative density perturbations between 180-220 km of 20-40 % demonstrate appreciable local time, latitude, and altitude variations. Modeling for the spatiotemporal conditions of the MAVEN observations suggests that GWs can directly propagate from the lower atmosphere to the thermosphere, produce appreciable dynamical effects, and likely contribute to the observed fluctuations. Modeled effects are somewhat smaller than the observed but their highly variable nature is in qualitative agreement with observations. Abstract

Objectives We evaluated the association between anti-ribosomal P antibody (anti-RibP) titres and disease activity in Japanese SLE patients. Methods Eighty patients admitted and treated in Niigata University Hospital for new-onset or flare-up of SLE were included in this retrospective cross-sectional study. Clinical data were obtained from medical records at admission. The anti-RibP index, and cytokine and tryptophan metabolite levels were determined by ELISA. Results Of the 80 SLE patients, 30 had anti-RibP. Anti-RibP presence was associated with a greater prevalence of skin rash and more severe inflammatory responses, demonstrated by higher inflammatory cytokine levels, hypocomplementemia, and accelerated tryptophan metabolism, in younger patients. The serum anti-RibP index was correlated with age at diagnosis, clinical indicators, initial prednisolone dose, and cytokines and tryptophan metabolite levels in univariate analysis. Multivariate analysis showed that the anti-RibP index was independently associated with the initial prednisolone dose and the prevalence of skin rash. The anti-RibP IgGs were mainly the IgG2 and IgG3 subclasses, and anti-RibP IgG3 was associated with hypocomplementemia, higher DAS, accelerated kynurenine pathway activity, and higher proinflammatory cytokine production. The coexistence of anti-dsDNA IgG and anti-RibP IgG2 or IgG3 accompanied higher IL-10 and IFN-α2 levels; furthermore, anti-RibP IgG3 coexistence with anti-dsDNA antibody contributed to the requirement for higher initial prednisolone doses and accelerated kynurenine pathway activity. Conclusion Anti-RibP was associated with clinical manifestations and parameters in SLE, and its index might be a useful indicator of disease severity. 22. Comparison of Prasugrel and Clopidogrel in Thrombotic Stroke Patients with Risk Factors for Ischemic Stroke Recurrence: An Integrated Analysis of PRASTRO-I, PRASTRO-II, and PRASTRO-III.

Abstract

Introduction: Patients with stroke are at a high risk of recurrence, and although they receive antiplatelet therapies such as clopidogrel for secondary prevention of non-cardioembolic stroke, the recurrence rate remains high. Three phase 3 trials (PRASTRO-I/II/III) were conducted to determine the efficacy of prasugrel in preventing recurrent stroke. Here, we performed an integrated analysis of these studies to confirm the generalizability of the PRASTRO-III findings and to supplement the small sample size of the study. Methods: Patients from PRASTRO-I, PRASTRO-II, and PRASTRO-III with ischemic stroke (large-artery atherosclerosis or small-artery occlusion) and at least one of the following were included: hypertension, dyslipidemia, diabetes mellitus, chronic kidney disease, or ischemic stroke history. The primary efficacy endpoint was the composite incidence of ischemic stroke, myocardial infarction (MI), and death from other vascular causes in the intention-to-treat population. Bleeding events (life-threatening bleeding, major bleeding, and clinically relevant bleeding) were evaluated as the primary safety endpoint. Cumulative incidences and 95% confidence intervals (CIs) were calculated for the study outcomes using the Kaplan-Meier method. Hazard ratios (HRs) and 95% CIs were calculated using the Cox regression model. Results: The data of 2,184, 274, and 230 patients from PRASTRO-I, PRASTRO-II, and PRASTRO-III, respectively, were analyzed (N = 2,688; prasugrel, N = 1,337; clopidogrel, N = 1,351). Stroke at enrollment was classified as large-artery atherosclerosis in 49.3% of patients and small-artery occlusion in 50.7% of patients. The primary efficacy endpoint composite incidence (prasugrel vs. clopidogrel) was 3.4% versus 4.3% (HR: 0.771, 95% CI: 0.522–1.138). The incidence of each component of the primary efficacy endpoint for prasugrel versus clopidogrel was 3.1% (n = 41) versus 4.1% (n = 55) for ischemic stroke, 0.3% (n = 4) versus 0.2% (n = 3) for MI, and no events of death from other vascular causes. For the primary safety endpoint, bleeding events were reported in 6.0% of patients in the prasugrel group versus 5.5% of patients in the clopidogrel group (HR: 1.074, 95% CI: 0.783–1.473). Conclusions: This integrated analysis supports the findings of PRASTRO-III. Prasugrel is a promising treatment that results in a numerical reduction in the composite incidence of ischemic stroke, MI, and death from other vascular causes in patients with ischemic stroke who are at a high risk of stroke recurrence. Abstract

Background:In PENDULUM mono, Japanese patients with high bleeding risk (HBR) received short-term dual antiplatelet therapy (DAPT) followed by single antiplatelet therapy (SAPT) with prasugrel after percutaneous coronary intervention (PCI). One-year data from PENDULUM mono showed better outcomes with prasugrel monotherapy after short-term DAPT compared with matched patients in the PENDULUM registry with longer DAPT durations according to guidelines at that time. This study presents 2-year results.Methods and Results: We compared 24-month data from PENDULUM mono (n=1,107; de-escalation strategy group) and the PENDULUM registry (n=2,273; conventional strategy group); both were multicenter, non-interventional, prospective registry studies, using the inverse probability of treatment weighting (IPTW) method. In the PENDULUM mono group, the cumulative incidence of clinically relevant bleeding (CRB) at 24 months post-PCI (primary endpoint) was 6.8%, and that of major adverse cardiac and cerebrovascular events (MACCE) was 8.9%. After IPTW adjustment, the cumulative incidence of CRB was 5.8% and 7.2% in PENDULUM mono and the PENDULUM registry, respectively (hazard ratio [HR] 0.77; 95% confidence interval [CI] 0.57-1.04; P=0.086), and that of MACCE was 8.0% and 9.5%, respectively (HR 0.77; 95% CI 0.59-1.01; P=0.061)., Conclusions:Japanese PCI patients with HBR prescribed prasugrel SAPT after short-term DAPT had a lower ischemic event risk than those prescribed long-term DAPT, and this was particularly relevant for ischemic events after 1 year.

Abstract

Objectives: The incidence of femoral localized periosteal thickening (LPT), which can precede atypical femoral fracture (AFF), is not low (1-10%) in Japanese patients with autoimmune inflammatory rheumatic diseases (AIRDs). We explored the associations between underlying AIRDs and the prevalence of LPT. Methods: We conducted post hoc analyses of two cohorts that included a total of 280 Japanese women, 105 of whom had AIRDs and had been taking bisphosphonate (BP) and prednisolone (PSL) and 175 of whom had rheumatoid arthritis (RA). Results: LPT was detected in a total of 18 patients (6.4%) and 3 (1.1%) developed AFFs. RA was negatively correlated with LPT. A disease other than RA requiring glucocorticoid treatment, BP use ≥5 years, PSL use ≥7 years, and a PSL dose ≥5.5 mg/day were positively correlated with LPT. After adjusting for age, diabetes mellitus, and BP duration or daily PSL dose, RA was no longer associated with LPT. Conclusions: LPT in Japanese patients with AIRDs was associated with BP and glucocorticoid treatment rather than underlying AIRDs. When PSL dose ≥5.5 mg/day is required long-term [typically combined with long-term BP treatment (≥5 years)], clinicians need to pay particular attention in cases LPT and AFF as well as glucocorticoid-induced osteoporosis. Abstract

Background: The risks of bleeding and cardiovascular events in high bleeding risk (HBR) Japanese patients undergoing percutaneous coronary intervention (PCI) while receiving single-antiplatelet therapy (SAPT) remains unknown. We aimed to evaluate the frequency of bleeding and cardiovascular events associated with prasugrel monotherapy after short-term dual-antiplatelet therapy (DAPT) in Japanese HBR patients after PCI. Methods and Results: The PENDULUM mono study was a multicenter, non-interventional, prospective registry (n=1,173). The primary endpoint was the cumulative incidence of clinically relevant bleeding (CRB; Bleeding Academic Research Consortium types 2, 3, and 5) from 1 to 12 months after PCI. Secondary endpoints included major adverse cardiac and cerebrovascular events (MACCE). The proportion of patients who received prasugrel monotherapy at 12 months after PCI was 79.7%, and no cases of stent thrombosis were observed among these patients. The cumulative incidence of CRB was 3.2% from 1 to 12 months after PCI; that of MACCE was 3.8%. Severe anemia, chronic kidney disease, oral anticoagulant use at discharge, and heart failure were significantly associated with CRB. Conclusions: Among HBR patients undergoing PCI who were not suitable for concomitant aspirin and were scheduled for prasugrel monotherapy, most patients were on prasugrel monotherapy after DAPT. Cumulative incidences of CRB and MACCE after periprocedural period were 3.2% and 3.8%, respectively, and no cases of stent thrombosis were reported. SAPT might be a suitable alternative to DAPT.

Abstract

Wavelike perturbations in the Martian upper thermosphere observed by the Neutral Gas Ion Mass Spectrometer (NGIMS) onboard the Mars Atmosphere and Volatile EvolutioN (MAVEN) spacecraft have been analyzed. The amplitudes of small-scale perturbations with apparent wavelengths between approx. 100 and approx. 500 km in the Ar density around the exobase show a clear dependence on temperature (T(sub 0)) of the upper thermosphere. The average amplitude of the perturbations is approx. 10% on the dayside and approx. 20% on the nightside, which is about 2 and 10 times larger than those observed in the Venusian upper thermosphere and in the low-latitude region of Earths upper thermosphere, respectively. The amplitudes are inversely proportional to T(sub 0), suggesting saturation due to convective instability in the Martian upper thermosphere. After removing the dependence on T(sub 0), dependences of the average amplitude on the geographic latitude and longitude and solar wind parameters are found to be not larger than a few percent. These results suggest that the amplitudes of small-scale perturbations are mainly determined by convective breaking saturation in the upper thermosphere on Mars, unlike those on Venus and Earth.
