Simultaneous recordings from large populations of individual neurons across distributed brain regions over months to years will enable new avenues of scientific and clinical development. The use of flexible polymer electrode arrays can support long-lasting recording, but the same mechanical properties that allow for longevity of recording make multiple insertions and integration into a chronic implant a challenge. Here is a methodology by which multiple polymer electrode arrays can be targeted to a relatively spatially unconstrained set of brain areas. The method utilizes thin-film polymer devices, selected for their biocompatibility and capability to achieve long-term and stable electrophysiologic recording interfaces. The resultant implant allows accurate and flexible targeting of anatomically distant regions, physical stability for months, and robustness to electrical noise. The methodology supports up to sixteen serially inserted devices across eight different anatomic targets. As previously demonstrated, the methodology is capable of recording from 1024 channels. Of these, the 512 channels in this demonstration used for single neuron recording yielded 375 single units distributed across six recording sites. Importantly, this method also can record single units for at least 160 days. This implantation strategy, including temporarily bracing each device with a retractable silicon insertion shuttle, involves tethering of devices at their target depths to a skull-adhered plastic base piece that is custom-designed for each set of recording targets, and stabilization/protection of the devices within a silicone-filled, custom-designed plastic case. Also covered is the preparation of devices for implantation, and design principles that should guide adaptation to different combinations of brain areas or array designs.

The brain is a massive neuronal network, organized into anatomically distributed sub-circuits, with functionally relevant activity occurring at timescales ranging from milliseconds to years. Current methods to monitor neural activity, however, lack the necessary conjunction of anatomical spatial coverage, temporal resolution, and long-term stability to measure this distributed activity. Here we introduce a large-scale, multi-site, extracellular recording platform that integrates polymer electrodes with a modular stacking headstage design supporting up to 1,024 recording channels in freely behaving rats. This system can support months-long recordings from hundreds of well-isolated units across multiple brain regions. Moreover, these recordings are stable enough to track large numbers of single units for over a week. This platform enables large-scale electrophysiological interrogation of the fast dynamics and long-timescale evolution of anatomically distributed circuits, and thereby provides a new tool for understanding brain activity.

Understanding the detailed dynamics of neuronal networks will require the simultaneous measurement of spike trains from hundreds of neurons (or more). Currently, approaches to extracting spike times and labels from raw data are time consuming, lack standardization, and involve manual intervention, making it difficult to maintain data provenance and assess the quality of scientific results. Here, we describe an automated clustering approach and associated software package that addresses these problems and provides novel cluster quality metrics. We show that our approach has accuracy comparable to or exceeding that achieved using manual or semi-manual techniques with desktop central processing unit (CPU) runtimes faster than acquisition time for up to hundreds of electrodes. Moreover, a single choice of parameters in the algorithm is effective for a variety of electrode geometries and across multiple brain regions. This algorithm has the potential to enable reproducible and automated spike sorting of larger scale recordings than is currently possible.

The flux-weighted cross-sections of the
nat Yb(γ, xn)175,169,167 Yb reactions were measured at the bremsstrahlung end-point energies of 12, 14, 16, 60, 65, and 70 MeV by the activation and off-line γ-ray spectrometric technique using the 20 MeV electron linac (ELBE) at Helmholtz-Zentrum Dresden-Rossendorf (HZDR), Dresden, Germany, and 100 MeV electron linac at the Pohang Accelerator Laboratory (PAL), Korea. Thenat Yb(γ, xn)175,169,167 Yb reaction cross-sections as a function of photon energy were also calculated theoretically using the TALYS 1.9 code. The flux-weighted average values at different end-point energies were obtained from the literature as well as from the theoretical values reported in the TALYS library based on mono-energetic photons. They were compared with the flux-weighted values based on the present experimental data and were found to be in general agreement. It was also found that the experimental and theoretical cross-section data increased from the threshold values to a certain energy, where other reaction channels opened, which highlights the role of excitation energy. After a certain value, the individual reaction cross-sections decrease with an increase in bremsstrahlung energy owing to the opening of other reaction channels, which indicates the partitioning of energy in different reaction channels. - Abstract
Palliative care is a type of medical care given to patients with a serious or life-threatening illness, such as cancer. Palliative medicine addresses the needs of the entire patient: mind, body, and spirit. Palliative medicine intends to alleviate suffering, including physical, psychosocial, spiritual, and existential symptoms for those whose illness impacts their daily lives. Total pain is defined here as well nonpharmacological interventions that may be helpful in addressing the care of these patients. To comprehensively address these holistic domains, an interprofessional approach to care is crucial. This chapter discusses the unique emotional, spiritual, and cultural challenges of the cancer patient.

The spectrum-averaged cross-sections of nat Gd(gamma,xn)159,153 Gd reactions induced by the bremsstrahlung end-point energies of 12, 14, 16. 60, 65, and 70 MeV were measured by activation and off-line gamma-ray spectrometric technique using the 20 MeV electron linac (ELBE) at HZDR, Dresden, Germany, and 100 MeV electron linac at Pohang Accelerator Laboratory, Korea. The TALYS 1.9 code was also used to calculate the theoretical nat Gd(gamma, xn)159,153 Gd reaction cross-sections as a function of photon energy. The spectrum-averaged values at various end-point energies were calculated from the literature data as well as theoretical values based on the TALYS 1.9 code, which is for mono-energetic photons. They were found to be in good agreement with the flux-weighted values of the current experimental data. It was also observed that the experimental and theoretical cross-sections increase from the threshold values to a certain energy, at which point another reaction channel opens, indicating the role of excitation energy. Individual reaction cross- sections decrease after a certain value as bremsstrahlung energy increases due to the opening of other reactions, indicating energy shearing among the different reaction channels.

While end-of-life (EOL) care has been a relatively common option for patients with terminal cancer, the utilization of EOL care in Alzheimer disease and other dementias has become available more recently. By the time end-stage dementia is present, the clinicians and caregivers become faced with multiple clinical issues—their inability to provide subjective complaints of pain and discomfort, behavioral symptoms, delirium, food refusal, and so on. In addition to providing quality EOL care to the patients, clinicians need to work with their families in an open and empathic manner, assuring that their loved ones will receive supportive measures to keep them comfortable.

With the advancements in treatments for previously terminal conditions, more Americans are now living with chronic serious illnesses for longer periods of time. Not only are there often associated symptoms of the disease itself, but often treatments for these chronic disease states have significant and debilitating side effects. Palliative medicine and hospice are subspecialties that focus on management of pain, symptoms, and stress for those living with a serious chronic disease. Both provide an interdisciplinary focus and a layer of support for patients and their families. Despite the rise in the availability of palliative medicine and hospice, many communities remain disproportionately underserved. There are many challenges for community-based palliative care programs including financial, staffing, ethical, and mental health issues. However, despite these challenges, there are opportunities for growth. As the healthcare entities move to a more patient-centered healthcare delivery system tied across the community, community-based palliative care programs are well positioned to bridge across the continuum of care settings and deliver a person-centered interprofessional approach to people dealing with serious illnesses and conditions.

Care of the dementia patient continues to be challenging. It is a terminal condition that many times goes undiagnosed leading to improper evidence-based interventions. Healthcare professionals (HCPs) should initiate goals of care conversations early with patients and their families in order to align treatment preferences. Early integration of palliative medicine is an important intervention that can lead to better manage symptoms and lessen the strain on loved ones. The spectrum-averaged cross-sections of
nat Gd(γ, xn)159,153 Gd reactions induced by the bremsstrahlung end-point energies of 12, 14, 16. 60, 65, and 70 MeV were measured by activation and off-line γ-ray spectrometric technique using the 20 MeV electron linac (ELBE) at HZDR, Dresden, Germany, and 100 MeV electron linac at Pohang Accelerator Laboratory, Korea. The TALYS 1.9 code was also used to calculate the theoreticalnat Gd(γ, xn)159,153 Gd reaction cross-sections as a function of photon energy. The spectrum-averaged values at various end-point energies were calculated from the literature data as well as theoretical values based on the TALYS 1.9 code, which is for mono-energetic photons. They were found to be in good agreement with the flux-weighted values of the current experimental data. It was also observed that the experimental and theoretical cross-sections increase from the threshold values to a certain energy, at which point another reaction channel opens, indicating the role of excitation energy. Individual reaction cross-sections decrease after a certain value as bremsstrahlung energy increases due to the opening of other reactions, indicating energy shearing among the different reaction channels. The hippocampus is diversely interconnected with other brain systems along its axis. Cycles of theta-frequency activity are believed to propagate from the septal to temporal pole, yet it is unclear how this one-way route supports the flexible cognitive capacities of this structure. We leveraged novel thin-film microgrid arrays conformed to the human hippocampal surface to track neural activity two-dimensionally in vivo. All oscillation frequencies identified between 1–15 Hz propagated across the tissue. Moreover, they dynamically shifted between two roughly opposite directions oblique to the long axis. This predominant propagation axis was mirrored across participants, hemispheres, and consciousness states. Directionality was modulated in a participant who performed a behavioral task, and it could be predicted by wave amplitude topography over the hippocampal surface. Our results show that propagation directions may thus represent distinct meso-scale network computations, operating along versatile spatiotemporal processing routes across the hippocampal body. New microgrid recordings on the human hippocampal surface reveal that oscillations travel in reversing directions. The route of travel at a given moment was related to behavior and topographic patterns of activity strength, suggesting directions may be biomarkers of hippocampal cognitive processes.

From animal experiments by Cohen and Suzuki et al. in the 1960s to the first-in-human clinical trials now in progress, prosthetic electrical stimulation targeting semicircular canal branches of the vestibular nerve has proven effective at driving directionally appropriate vestibulo-ocular reflex eye movements, postural responses, and perception. That work was considerably facilitated by the fact that all hair cells and primary afferent neurons in each canal have the same directional sensitivity to head rotation, the three canals’ ampullary nerves are geometrically distinct from one another, and electrically evoked three-dimensional (3D) canal-ocular reflex responses approximate a simple vector sum of linearly independent components representing relative excitation of each of the three canals. In contrast, selective prosthetic stimulation of the utricle and saccule has been difficult to achieve, because hair cells and afferents with many different directional sensitivities are densely packed in those endorgans and the relationship between 3D otolith-ocular reflex responses and the natural and/or prosthetic stimuli that elicit them is more complex. As a result, controversy exists regarding whether selective, controllable stimulation of electrically evoked otolith-ocular reflexes (eeOOR) is possible. Using micromachined, planar arrays of electrodes implanted in the labyrinth, we quantified 3D, binocular eeOOR responses to prosthetic electrical stimulation targeting the utricle, saccule, and semicircular canals of alert chinchillas. Stimuli delivered via near-bipolar electrode pairs near the maculae elicited sustained ocular countertilt responses that grew reliably with pulse rate and pulse amplitude, varied in direction according to which stimulating electrode was employed, and exhibited temporal dynamics consistent with responses expected for isolated macular stimulation.NEW & NOTEWORTHY As the second in a pair of papers on Binocular 3D Otolith-Ocular Reflexes, this paper describes new planar electrode arrays and vestibular prosthesis architecture designed to target the three semicircular canals and the utricle and saccule. With this technological advancement, electrically evoked otolith-ocular reflexes due to stimulation via utricle- and saccule-targeted electrodes were recorded in chinchillas. Results demonstrate advances toward achieving selective stimulation of the utricle and saccule.

Simultaneous recordings from large populations of individual neurons across distributed brain regions over months to years will enable new avenues of scientific and clinical development. The use of flexible polymer electrode arrays can support long-lasting recording, but the same mechanical properties that allow for longevity of recording make multiple insertions and integration into a chronic implant a challenge. Here is a methodology by which multiple polymer electrode arrays can be targeted to a relatively spatially unconstrained set of brain areas. The method utilizes thin-film polymer devices, selected for their biocompatibility and capability to achieve long-term and stableelectrophysiologic recording interfaces. The resultant implant allows accurate and flexible targeting of anatomically distant regions, physical stability for months, and robustness to electrical noise. The methodology supports up to sixteen serially inserted devices across eight different anatomic targets. As previously demonstrated, the methodology is capable of recording from 1024 channels. Of these, the 512 channels in this demonstration used for single neuron recording yielded 375 single units distributed across six recording sites. Importantly, this method also can record single units for at least 160 days. This implantation strategy, including temporarily bracing each device with a retractable silicon insertion shuttle, involves tethering of devices at their target depths to a skull-adhered plastic base piece that is custom-designed for each set of recording targets, and stabilization/protection of the devices within a silicone-filled, custom-designed plastic case. Also covered is the preparation of devices for implantation, and design principles that should guide adaptation to different combinations of brain areas or array designs.

With the increasing interest in apolipoprotein E (APOE) genetic testing to estimate the risk of developing late-onset Alzheimer disease, new educational tools are needed to help people make the best decision for themselves about whether to undergo this test. This study evaluated an online tool to assist in this decision process. A prototype decision aid was studied in a two-part survey that collected data from participants before and after they examined the decision aid. Both surveys had multiple-choice options and opportunities for open-ended responses, yielding quantitative and qualitative information. The responses before and after use of the aid were compared for each participant. A total of 1,262 individuals completed both surveys. The overall effectiveness of the decision aid was shown by three measures: 94% found the decision aid very helpful or somewhat helpful; general knowledge was increased; and some people changed their minds about APOE genetic testing, with 35% shifting to a higher likelihood of undergoing the test and 20% to a lower likelihood. Suggestions for improvements were noted and incorporated into the online tool. This decision aid can provide useful educational assistance to many individuals as they consider APOE genetic testing as well as facilitate further discussions with their health-care providers. The brain is a massively interconnected neuronal network, organized into specialized circuits consisting of large ensembles of neurons distributed across anatomically connected regions. While circuit computations depend upon millisecond timescale interactions, the structure of the underlying networks are remodeled on timescales ranging from seconds to months. Current approaches lack the combination of resolution, spatial coverage, longevity, and stability to measure the detailed dynamics of these networks. Here we describe a large-scale, multisite recording platform that integrates polymer electrodes with a modular stacking headstage design supporting up to 1024 channels of recording in freely-behaving rats. We show that the integrated system can yield months-long recordings from hundreds of well-isolated units across multiple regions. Moreover, the recordings are stable enough to track a substantial fraction of single units for over a week. This platform enables large-scale electrophysiological interrogation of the function and evolution of distributed circuits throughout an animal9s adult life.
