Artikkelissa käsitellään tiedolla johtamista sosiaalipalveluissa keskittyen tietojärjestelmiin merkittävinä tiedontuotannon ja -hallinnan välineinä. Tutkimuksessa tarkastellaan tiedolla johtamisen prosessimallin avulla, miten tietojärjestelmät tukevat tiedolla johtamista sosiaalipalveluissa. Empiiristä kyselyaineistoa (n = 145) analysoidaan määrällisesti kuvailevilla menetelmillä ja moniluokitteluanalyysillä sekä näitä täydentävällä laadullisella analyysillä. Tulokset paljastavat merkittäviä puutteita tietojärjestelmien tietotuotteissa ja -palveluissa sekä sen, ettei sosiaalialan johtajien tietotarpeita ole huomioitu riittävästi. Julkisella sektorilla tietojärjestelmän tuki tiedolla johtamiselle havaittiin yleisesti heikommaksi verrattuna yksityiseen ja kolmanteen sektoriin. Tietojärjestelmän käytettävyyden ja tiedolla johtamisen tuen välillä havaittu yhteys korostaa käytettävyyden merkitystä sosiaalipalveluiden tietojärjestelmien kehittämisessä.

Background: Antibiotics predispose travellers to acquire multidrug-resistant bacteria, such as extended-spectrum beta-lactamase-producing Enterobacterales (ESBL-PE). Although widely used in antimalarial prophylaxis, doxycycline has scarcely been studied in this respect. Methods: We explored the impact of doxycycline on rates of traveller's diarrhoea (TD), ESBL-PE acquisition and, particularly, doxycycline co-resistance among travel-acquired ESBL-PE in a sample of 412 visitors to low- and middle-income countries. We reviewed the literature on traveller studies of doxycycline/tetracycline resistance among stool pathogens and the impact of doxycycline on TD rates, ESBL-PE acquisition, and doxycycline/tetracycline resistance. Results: The TD rates were similar for doxycycline users (32/46; 69.6%) and non-users (256/366; 69.9%). Of the 90 travel-acquired ESBL-PE isolates, 84.4% were co-resistant to doxycycline: 100% (11/11) among users and 82.3% (65/79) among non-users. The literature on doxycycline's effect on TDwas not conclusive nor did it support a recent decline in doxycycline resistance. Although doxycycline did not increase ESBL-PE acquisition, doxycycline-resistance among stool pathogens proved more frequent for users than non-users. Conclusions: Our prospective data and the literature review together suggest the following: 1) doxycycline does not prevent TD; 2) doxycycline use favours acquisition of doxy/tetracycline-co-resistant intestinal bacteria; 3) although doxycycline does not predispose to travel-related ESBL-PE acquisition per se, it selects ESBL-PE strains co-resistant to doxycycline; 4) doxycycline resistance rates are high among stool bacteria in general with no evidence of any tendency to decrease.

Artikkelissa tarkastellaan sosiaalialan korkeakoulutettujen ammattilaisten arvioita käyttämistään asiakastietojärjestelmistä. Artikkelissa arvioidaan Sote-tieto hyötykäyttöön strategian tavoitteiden toteutumista sosiaalialan ammattilaisten näkökulmasta ja tuotetaan tuotemerkkikohtaista tietoa käyttäjäkokemuksista kehittämisen tueksi. Artikkelin tutkimuskysymyksenä oli: Minkälaisia arvioita sosiaalialan korkeakoulutetut ammattilaiset antavat käyttämilleen asiakastietojärjestelmille vuonna 2020 käyttäjäkokemuksiinsa perustuen? Tutkimusaineistona hyödynnettiin STePS 3.0 -hankkeessa syys-lokakuussa 2020 ammattiliittojen kanssa yhteistyössä kerättyä tutkimusaineistoa (n=990), josta tähän artikkeliin valittiin ne vastaajat, joilla oli käytössä sähköinen asiakastietojärjestelmä (n=974). Aineistoa analysoitiin kuvailevin menetelmin hyödyntämällä suoria jakaumia ja ristiintaulukointia. Kouluarvosanoista esitettiin keskiarvot, keskihajonnat, keskiarvojen 95% luottamusvälit sekä heikkojen ja kiitettävien arvosanojen osuudet. Tulokset osoittavat, että sosiaalihuollossa käytössä olevien tuotemerkkien välillä on vaihtelua toimivuudessa, käytettävyydessä, työn ja yhteistyön tuessa sekä koetuissa hyödyissä. Tuotemerkkien arvosanojen keskiarvot vaihtelivat kohtalaisesta (6) hyvään (8). Parhaimmat arviot saivat Nappula ja Domacare, joita käytetään yleensä rajatummissa sosiaalihuollon toimintaympäristöissä. Laajasti sosiaalihuollossa käytössä olevista asiakastietojärjestelmistä parhaimman arvion sai Pro Consona (tyydyttävä). Asiakastietojärjestelmien suunnittelu ja mukauttaminen pienemmän ja rajatumman käyttäjäryhmän tarpeisiin on luonnollisesti yksinkertaisempaa ja järjestelmän käytön oppiminen nopeampaa verrattuna suurten käyttäjäryhmien ja monipuolisten toiminnallisuuksien asiakastietojärjestelmiin. Kaikkien asiakastietojärjestelmien kehittämiskohteiksi tunnistettiin yhteistyön ja tiedonvaihdon tukeminen, asiakkaan palvelukokonaisuuden hahmottaminen sekä asiakkaan sähköisen asioinnin kehittäminen. Paikannetut kehittämiskohteet ovat merkittäviä tulevia hyvinvointialueita ajatellen. Asiakastietojärjestelmillä olisi potentiaalia tukea sote-uudistukselle asetettuja tavoitteita, mikäli niitä kehitetään tulevaisuudessa vastaamaan paremmin ammattilaisten tarpeisiin., This article examines the experiences of client information systems (CISs) used by social welfare professionals with at least a bachelor-level education. The purpose of the article is to evaluate the realisation of the objectives of the eHealth and eSocial Strategy 2020 from the perspective of social welfare professionals and to produce brand-specific information about user experiences to support future development of CISs. The research question is: What kind of assessments do social welfare professionals with at least a bachelor-level education provide about the CISs they used in 2020, based on their user experiences? The study utilises survey data (n=990) gathered in the STePS 3.0 project in September–October 2020 in cooperation with trade unions, using a subpopulation of respondents with an electronic CIS in use (n = 974). The data was analysed by descriptive methods using direct distribution and cross-tabulation. Mean values, standard deviations, 95% confidence intervals of the means and the proportions of poor and commendable grades were presented for school grades 4–10. The results show that there is variance between the brands used in social care in terms of functionality, usability, support for work and cooperation and the perceived benefits. The average ratings of the brands ranged from moderate (6) to good (8). Nappula and Domacare, which are generally used in the limited social care settings, received the best ratings. Of the CISs widely used in social welfare, Pro Consona received the best rating (satisfactory). Designing and adapting CISs to the needs of a smaller and more limited user groups are naturally more straightforward, and users are able to learn a CIS system faster compared to CISs with large user groups and versatile functionalities. The development needs for all CISs were identified as support for cooperation and information exchanges, as well as support in understanding client services and improvements of electronic client services. The identified development areas are significant for future welfare regions. CISs have the potential to support the established objectives for social and welfare reform if they are developed in the future to better meet the needs of professionals.

Asiakas- ja potilastietojärjestelmät ovat sairaanhoitajien työssä keskeisiä työvälineitä. Artikkelissa selvitettiin, missä määrin eri asiakas- ja potilastietojärjestelmät tukevat sairaanhoitajien työtä eri toimintaympäristöissä. Tavoitteena oli tuottaa tietoa kansallisen tiedonhallinnan strategian täyttymisestä ja sairaanhoitajien käyttäjäkokemuksista palautteeksi tietojärjestelmätoimittajille. Tiedonkeruu sairaanhoitajien tietojärjestelmäkokemuksista toteutettiin keväällä 2020 kyselynä. Kyselyyn vastasi 3912 sairaanhoitajaa, joista pääosa työskenteli julkisessa sairaalassa, terveyskeskuksessa, sosiaalialalla tai yksityissektorilla. Yleisimmin käytettyjä asiakas- ja potilasjärjestelmiä olivat yleisyysjärjestyksessä Lifecare asiakas- ja potilastietojärjestelmä (Lifecare), Uranus, Pegasos, Apotti (tuotemerkki: Epic), Effica terveydenhuolto (Effica), Mediatri, Esko, DynamicHealth ja DomaCare. Kyselyn väittämistä raportoitiin teemoja, jotka koskevat tietojärjestelmien kykyä tukea vuorovaikutusta (4), hyötyjä (4), käytettävyyttä (2), teknistä toimivuutta (3) sekä paperin ja faksin käyttöä (1), tietojärjestelmille annettavia kouluarvosanoja (1) ja järjestelmätoimittajille suoraan annettavaa palautetta (30). Kysely tehtiin sairaanhoitajille toista kertaa, ja pohdinnassa tarkasteltiin muutoksia vuoteen 2017 nähden. Tulosten mukaan tietojärjestelmien tuki sairaanhoitajien työlle parantui tai pysyi ennallaan monien muuttujien osalta. Tulokset vaihtelivat asiakas- ja potilastietojärjestelmien tuotemerkkien ja toimintaympäristöjen välillä ja myös samojen tuotemerkkien välillä eri toimintaympäristöissä. Sairaanhoitajat olivat tyytyväisiä etenkin tietojärjestelmien kykyyn tukea yhteistyötä ja tiedonkulkua sairaanhoitajien kesken organisaation sisällä sekä sairaanhoitajien ja lääkärien välillä. He myös kokivat, että tietojärjestelmät tuottavat hyötyjä asiakkaiden ja potilaiden hoitoprosessille. Toisaalta organisaatioiden välisessä tiedonkulussa turvauduttiin edelleen paperiin ja faksiin. Lisäksi vastaajat tunnistivat asiakas- ja potilastietojärjestelmissä enemmän työtä hankaloittavia kuin hyvin toimivia ominaisuuksia. Vaikka tietojärjestelmien kehityssuunta vaikutti positiiviselta, eivät ne kaikilta osin edelleenkään tukeneet sairaanhoitajien työtä riittävällä tasolla ja näin ollen tietojärjestelmien systemaattista seurantaa on syytä jatkaa., Information systems are key tools for nursing care. The article examines the extent to which different client information systems and electronic health records support the work of nurses in specific work environments. The aim is to provide information on the achievement of the national strategy for information management and on nurses’ user experiences of individual systems as feedback to the system suppliers. The data collection was carried out as a survey on nurses' system experiences in the spring of 2020. The survey was answered by 3,912 nurses. Most of the respondents worked in public hospitals, community health centers, social services or private sector. The most commonly used client information systems and electronic health records were in order of frequency: Lifecare, Uranus, Pegasos, Apotti (system brand: Epic), Effica Healthcare, Mediatri, Esko, DynamicHealth and DomaCare. The questions that were reported in the study were related to information systems’ ability to support interaction (4), benefits (4), usability (2), and technical functionality (3) and the use of paper and fax (1), information systems’ evaluation on a numeric scale (1) and direct feedback to system suppliers (30). The survey for nurses was conducted for the second time, so it was possible to examine the changes since 2017. According to the results, it appeared that the information systems’ ability to support work improved or remained unchanged for the most part. The results varied by information system brands and between the work environments as well as between the same brands in different work environments. Nurses appeared to be particularly satisfied with the information systems' ability to support collaboration and information flow between nurses within their own organization and between nurses and physicians. They also felt that the information systems provided benefits to the care process of clients and patients. On the other hand, the information flow between organizations was still dependent on paper and fax. The respondents also identified more problems than well-functioning features in the information systems. Thus, although the development trend of the information systems appeared to be positive, the information systems did not fully support the work of nurses in all respects. Therefore the systematic monitoring of the information systems is worth continuing.

Background: Antibiotics are no longer the primary approach for treating all travellers' diarrhoea (TD): most cases resolve without antibiotics and using them predisposes to colonization by multidrug-resistant bacteria. Data are accumulating on increasing resistance among TD pathogens, yet research into the most common agents, diarrhoeagenic Escherichia coli (DEC), remains limited. Methods: A total of 413 travellers to the (sub)tropics were analyzed for travel-acquired diarrhoeal pathogens and ESBL-PE. To identify ESBL-producing DEC, ESBL-producing E. coli (ESBL-EC) isolates were subjected to multiplex qPCR for various DEC pathotypes: enteroaggregative (EAEC), enteropathogenic (EPEC), enterotoxigenic (ETEC), enteroinvasive (EIEC) and enterohaemorrhagic (EHEC) E. coli. For a literature review, we screened studies among travellers and locals in low- and middle-income countries (LMICs) on the frequency of ESBL-producing DEC, and among travellers, also DEC with resistance to ciprofloxacin, azithromycin, and rifamycin derivatives. Results: Our rate of ESBL-EC among all DEC findings was 2.7% (13/475); among EAEC 5.7% (10/175), EPEC 1.1% (2/180), ETEC 1.3% (1/80) and EHEC (0/35) or EIEC 0% (0/5). The literature search yielded three studies reporting ESBL-EC frequency and thirteen exploring resistance to TD antibiotics among travel-acquired DEC. For EAEC and ETEC, the ESBL-EC rates were 10-13% and 14-15%, resistance to fluoroquinolones 0-42% and 0-40%, azithromycin 0-29% and 0-61%, and rifaxim in 0% and 0-20%. The highest rates were from the most recent collections. Proportions of ESBL-producing DEC also appear to be increasing among locals in LMICs and even carbapenemase-producing DEC were reported. Conclusion: ESBL producers are no longer rare among DEC, and the overall resistance to various antibiotics is increasing. The data predict decreasing efficacy of antibiotic treatment, threatening its benefits, for disadvantages still prevail when efficacy is lost.

Background: End-user participation is essential to the development of health information systems (HIS) that are useful for clinicians and support their routine work. However, few studies have investigated end users’ experiences with HIS development and their preferred ways of participation in it. Objectives: This study examined the participation experiences of physicians and nurses with HIS development. Methods: National cross-sectional surveys on end users’ experiences with HIS development were conducted in Finland among physicians in 2010, 2014, and 2017 and nurses in 2017. For the purposes of this study, we selected and analyzed the statements concerning participation and end users’ experiences on HIS development and their preferred ways of participation in it. Results: A total of 3013 physicians and 2685 nurses working in public hospitals and health centers were included in this study. In total, 48.4 % of physicians and 45.4 % of nurses reported that they had participated in HIS development; however, 85.1% of respondents regarded that software vendors are not interested in end users’ viewpoints and development ideas. Most respondents (53.4 %) preferred to participate by communicating with a person responsible for HIS development within the organization. Few participants reported that the proposed improvements took place in the desired manner (10.0 %) or quickly enough (6.9 %). Younger clinicians were more willing to participate in HIS development than older clinicians. During the follow-up period (2010, 2014, 2017), the physicians’ experiences did not improve. Conclusions: While physicians and nurses are willing to participate in HIS development, suitable methods to effectively include them and their feedback seem to be lacking or underutilized. Crucially, physicians and nurses, who make up the largest groups of end users, are not able to influence HIS development in their preferred ways. Healthcare organizations must recognize the importance of clinician participation; these clinicians should have the opportunity to continue clinical work.

Tietojärjestelmien kehittämistyön lähtökohtana tulee olla vahva ymmärrys käyttäjien tarpeista, tehtävistä sekä toimintaympäristöstä. Lisäksi suunnitteluratkaisuja tulee arvioida käyttäjien kanssa kehittämisprosessin aikana ja käyttöönoton jälkeen. Vaikka kansallisesti kohdennetaan runsaasti resursseja tietojärjestelmäpalveluiden kehittämiseen, sosiaalialalta tutkimusta aiheesta löytyy niukasti. Sosiaalialan ammattilaisten kokemuksia asiakastietojärjestelmien kehittämistyöstä kartoitettiin osana keväällä 2019 toteutettua valtakunnallisesta käyttäjäkokemuskyselyä. Kysymyksissä käsiteltiin kokemuksia yhteistyöstä tietojärjestelmätoimittajan kanssa, osallistumisesta kehittämistyöhön ja osallistumistavoista tietojärjestelmien kehitykseen. Tuloksia tarkasteltiin asiakastietojärjestelmän tuotemerkeittäin. Puolet vastaajista ei ollut osallistunut lainkaan kehittämistyöhön, mutta vain 19% ei ollut halukkaita osallistumaan ollenkaan. Suosituimmat osallistumistavat olivat ongelmien raportointi järjestelmien kehittämisen vastuuhenkilölle ja oman työn esittely kehittäjille; sen sijaan tietojärjestelmätoimittajien yleisimmin käyttämät tavat eli palautteen kirjaaminen verkkosivustolle tai sähköpostiin eivät olleet käyttäjien mieleen. Suurin osa tiesi, miten ongelmatilanteista ja kehitysehdotuksista raportoidaan. Sosiaalialan ammattilaiset ovat kiinnostuneita osallistumaan kehittämistyöhön, mutta he eivät ole tyytyväisiä mahdollisuuksiin ja tapoihin osallistua käyttämiensä järjestelmien kehittämiseen. Järjestelmätoimittajien ja käyttäjien väliltä puuttuvat sopivat toimintatavat. Tulokset ovat samansuuntaisia kuin vastaavissa lääkäreille ja sairaanhoitajille tehdyissä tutkimuksissa. On myös mahdollista, että organisaatiot eivät hyödynnä innokasta kehittäjäpotentiaalia. Kehittäjien tulee jalkautua kentälle ja organisaatioiden palkata tietojärjestelmäkehityksestä vastaavia ammattilaisia tulkeiksi järjestelmätoimittajan ja käyttäjien välille. Vertaisarvioitu. English summary. Lähtökohdat : Lääkärien arvioita potilastietojärjestelmistä on tutkittu vuosina 2010, 2014 ja 2017, mutta lääkärien käyttäjäkokemuksia tietojärjestelmistä johtamisen tukena ei ole aiemmin laajasti raportoitu. Menetelmät : Tutkimus perustuu ylilääkärien ja muiden johtavassa asemassa olevien lääkärien arvioihin tietojärjestelmien käytöstä johtamisen tukena. Tiedot kerättiin osana vuosien 2014 (n = 723) ja 2017 (n = 727) Potilastietojärjestelmät lääkärin työvälineenä -tutkimusta. Tulokset : Puolet vastaajista käyttää johtamisen tietojärjestelmiä päivittäin, terveyskeskuksissa muita harvemmin. Suurin osa koki joutuvansa hakemaan johtamiseen tarvitsemiaan tietoja monesta järjestelmästä. Tietojärjestelmien antama tuki toiminnan tehostamiselle oli parantunut kolmessa vuodessa. Tyytyväisimpiä johtamisen työkaluihinsa olivat yksityisen sektorin vastaajat. Päätelmät : Suuri osa johtamisasemassa olevista lääkäreistä käyttää johtamisen tietojärjestelmiä päivittäin, mutta kokee tiedon olevan hajallaan ja laadultaan epäluotettavaa. Lääkärien arviot pysyivät ennallaan vuodesta 2014 vuoteen 2017.

Travellers’ diarrhoea (TD) is the most common disease encountered by travellers to the (sub)tropics: it affects 20–70% of travellers annually. Although the disease is mostly mild and seldom requires hospitalisation, among children living in developing regions, the same pathogens may cause growth retardation and cognitive impairment and attribute to over 500,000 deaths each year. While the aetiology of TD has been widely studied, the results of these investigations are not fully reliable since with traditional culture-based methods the aetiology remains open in up to half of travellers with TD. Diarrhoeagenic Escherichia coli are the most common pathogens, but they often remain unidentified. The introduction of PCR-based diagnostic methods into TD research have brought a need to reassess our knowledge based on older investigations and new studies are needed. We developed a novel multiplex real-time quantitative reverse transcription PCR (RT-qPCR) assay that identifies the following diarrhoeal pathogens: enterotoxigenic (ETEC), enteroaggregative (EAEC), enteropathogenic (EPEC), enterohaemorrhagic (EHEC) and enteroinvasive (EIEC) E. coli as well as Campylobacter, Shigella, Salmonella, Yersinia and Vibrio cholerae. The study groups in this thesis included 459 travellers recruited at a travel clinic pre-travel consultation and a group of 45 travelling together to Benin, West Africa. The travellers provided stool samples and questionnaires before and after travel. The aim of this thesis was to apply the novel molecular method on the aetiology of TD in various geographic regions, compare the impact of different definitions for TD and No TD on pathogen findings and analyse the association of pathogen findings to experienced symptoms. A pathogen was detected in 17% of stool samples when employing culture-based methods while the multiplex RT-qPCR assay detected a pathogen in 76%. Moreover, multiple pathogen findings were common: among the group travelling to Benin, two or more pathogens detected in 78% of stools. Furthermore, in the group of 459 travellers recruited at the Travel Clinic, a pathogen was detected in half of those remaining asymptomatic and those with antibiotic use during travel. In earlier studies, ETEC has been the most frequently identified pathogen in all regions other than Southeast Asia where Campylobacter predominate. In our study, EAEC and EPEC outnumbered these pathogens in all regions. This difference is mainly explained by the better coverage and higher sensitivity of the method applied. In TD research, either classical or WHO definition have been used for TD. Our results showed that both definitions are applicable. By contrast, defining control groups had greater impact: If the classical definition was used, control groups may have included not only the fully asymptomatic, but also those with mild or resolved symptoms. Moreover, if the control group included travellers with mild and resolved symptoms, the difference between those with TD and the control group would not have appeared significant for EAEC and Campylobacter. This thesis applied a novel PCR-based method with broad coverage to study the aetiology of TD. The results have changed our perception on the pathogen findings and their role in TD. When designing new studies on TD special emphasis should be given on defining control groups. PCR-based methods not only open up new possibilities into TD research but also show that the role of each pathogen should be re-evaluated. Ripulitaudit ovat yleisiä lasten kuoleman syitä kehittyvissä maissa. Matkaripuli on myös subtropiikkiin ja tropiikkiin matkailevan tavallisin sairaus: 20–70% turisteista sairastuu siihen vuosittain. Aikaisemmissa tutkimuksissa, joissa on käytetty viljelyyn perustuvia menetelmiä, taudinaiheuttaja on jäänyt tunnistamatta jopa puolella sairastuneista. Tässä väitöskirjassa sovellettiin uutta, kattavaa geenimonistukseen ( PCR; polymeraasiketjureaktio) perustuvaa menetelmää matkaripulin aiheuttajien tunnistamiseen. Suurimmalta osalta matkailijoista löytyi jokin taudinaiheuttaja. Aikaisemmin tavallisimpana ripulinaiheuttajana pidettiin ETEC-bakteeria (enterotoxigeeninen Escherichia coli), mutta uusilla menetelmillä voitiin vahvistaa että tätäkin yleisempiä ovat EAEC (enteroaggregatiivinen E. coli) ja EPEC (enteropatogeeninen E. coli). Tulokset osoittivat myös, että näitä ripulinaiheuttajia löytyy myös täysin oireettomilta, tosin ei yhtä usein kuin oireisilta. Toisaalta samalta ripuloivalta matkailijalta saattoi lisäksi löytyä jopa useampia muita taudinaiheuttajia. Bakteerin löytyminen ulosteesta ei siis aina tarkoita sitä, että se aiheuttaisi oireita, puhumattakaan siitä, että löydös edellyttäisi antibioottihoitoa. Uudet diagnostiset menetelmät ovat herkkiä ja kattavia ja ne muuttavat pitkään vallinneita käsityksiä matkaripulin aiheuttajista ja löydösten todellisesta merkityksestä. Kunkin taudinaiheuttajan merkitys matkaripulin aiheuttajana tulisi tutkia uudelleen konaiskuvan tarkentuessa. Uusien menetelmien vaikutukset ulottuvat paitsi sairastuneiden matkailijoiden hoitoon käytännön työssä, ne myös tarjoavat aiempaa huomattavasti tarkemmat välineet tutkimustyölle eri puolilla maailmaa ja vaikuttavat mm. rokotteiden kehitystyöhön.

Background: Problems in the usability of health information systems (HISs) are well acknowledged, but research still lacks a validated questionnaire for measuring and monitoring different dimensions of usability of HISs. Such questionnaires are needed not only for research but also for developing usability of HISs from the viewpoint of end-user experiences. Objective: This study aimed to develop and test the validity of the questionnaire measuring the National Usability-Focused HIS-Scale (NuHISS) among a nationally representative sample of Finnish physicians. Methods: We utilized 2 cross-sectional data collected from a random sample of Finnish physicians in 2014 (N=3781; of which 2340 [61.9%] were women) and 2017 (N=4018; of which 2604 [64.8%] were women). Exploratory and confirmatory factor analyses (structural equation modeling [SEM]) were applied to test the structural validity of the NuHISS. As the concurrent validity measure, we used the self-reported overall quality of the electronic health record system (school grade) provided by the participants using marginal structural models. Introduction: Timely, complete and accurate patient data is needed in care decisions along the continuum of care. To access patient data from other organizations, there are three types of regional health information exchange systems (RHIS) in use In Finland. Some regions use multiple RHISs while others do not have a RHIS available. The recently introduced National Patient Data Repository (Kanta) is increasingly used for health information exchange (HIE). Objectives: The purpose of this study was to assess usage of paper, RHISs and Kanta by context in 2017; evolution of paper use over the years; and predictors of paper use in 2017 among Finnish physicians for HIE system development. Methods: Data from national electronic health record (EHR) usage and user experience surveys were taken from 2010 (prior to ePrescription system implementation), 2014 (prior to implementation of Kanta) and 2017 (Kanta was in full use in the public sector and in large private organizations). The web-based surveys were targeted to all physicians engaged in clinical work in Finland. Results: Kanta was the most frequently used means of HIE in 2017. Paper use had reduced significantly from 2010 to 2014. The trend continued in 2017. Still, up to half of the physicians reported using paper daily or weekly in 2017. There were great variations in paper use by healthcare sector, available RHIS type and EHR system used. In multivariable analysis (with all other variables constant), predictors of more frequent use of paper than electronic means for HIE were: private sector or hospital, access to Master Patient Index RHIS (type 1), multiple RHIS (type 4) or no RHIS (type 5), two particular EHR systems, older age, less experience, operative, psychiatric or diagnostic specialties, and male gender. Conclusions: Usability of HIE systems including EHRs as access points to HIE need to be improved to facilitate usage of electronic HIE. Usage ensures more timely and complete patient data for safe, coordinated care. Specialty-specific needs and requirementscall for more user participation in HIE design. Especially older professionals need training to better exploit HIS for HIE.

The interest towards monitoring and guiding the development of healthcare information systems on a national level is increasing. In this paper, we report results from the three cross-sectional surveys on physicians' experiences on usability of their electronic health record (EHR) systems in Finland. The research question was: How have physicians' experiences on usability of their EHR systems evolved between 2010 and 2017? The data consists of responses to six usability statements from Finnish physicians working in public healthcare centres and hospitals. Among physicians working in healthcare centres, results between 2010 and 2017 show change for the worse. Among their colleagues in hospitals, results indicate slight improvement only in the domain of ease of use of the systems. In general, contrary to general expectations, the results do not show improvements between the years 2010, 2014 and 2017. In the future, we will continue the monitoring work in Finland on a national level from the viewpoint of physicians and other professional groups. © 2019 American Psychological Association Inc.
