To systematically map the current evidence about the characteristics of health systems, providers and patients to design rehabilitation care for post coronavirus disease 2019 (COVID-19) condition.We conducted a scoping review by searching the databases: MEDLINE®, Embase®, Web of Science, Cochrane COVID-19 Registry and Cochrane Central Register of Controlled Trials, from inception to 22 April 2022. The search strategy included terms related to (i) post COVID-19 condition and other currently known terminologies; (ii) care models and pathways; and (iii) rehabilitation. We applied no language or study design restrictions. Two pairs of researchers independently screened title, abstracts and full-text articles and extracted data. We charted the evidence according to five topics: (i) care model components and functions; (ii) safe delivery of rehabilitation; (iii) referral principles; (iv) service delivery settings; and (v) health-care professionals.We screened 13 753 titles and abstracts, read 154 full-text articles, and included 37 articles. The current evidence is conceptual and expert based. Care model components included multidisciplinary teams, continuity or coordination of care, people-centred care and shared decision-making between clinicians and patients. Care model functions included standardized symptoms assessment, telehealth and virtual care and follow-up system. Rehabilitation services were integrated at all levels of a health system from primary care to tertiary hospital-based care. Health-care workers delivering services within a multidisciplinary team included mostly physiotherapists, occupational therapists and psychologists.Key policy messages include implementing a multilevel and multiprofessional model; leveraging country health systems' strengths and learning from other conditions; financing rehabilitation research providing standardized outcomes; and guidance to increase patient safety.Recenser systématiquement les données probantes actuelles concernant les caractéristiques des systèmes de santé, prestataires et patients afin de définir un modèle de réadaptation pour les affections consécutives à la maladie à coronavirus 2019 (COVID-19).Nous avons mené une analyse exploratoire en étudiant plusieurs bases de données: MEDLINE®, Embase®, Web of Science, Registre Cochrane des études sur la COVID‐19 et Registre central Cochrane des essais contrôlés, depuis le début et jusqu'au 22 avril 2022. La stratégie de recherche consistait à inclure des termes en lien avec (i) les affections post-COVID-19 et d'autres terminologies connues à ce jour; (ii) les modèles et parcours de soins; et enfin, (iii) la réadaptation. Nous n'avons appliqué aucune restriction de langue ou de format d'étude. Deux binômes de chercheurs ont passé au crible les titres, résumés et articles complets, indépendamment l'un de l'autre, pour ensuite en extraire des informations. Nous avons établi une grille de données probantes autour de cinq thèmes: (i) éléments et fonctions des modèles de soins; (ii) fourniture de services de réadaptation sûrs; (iii) principes de référence; (iv) lieux de prestation des services; et enfin, (v) professionnels de la santé.Nous avons examiné 13 753 titres et résumés, lu 154 articles complets et inclus 37 d'entre eux. Les données probantes actuelles reposent sur des concepts et expertises. Les éléments des modèles de soins impliquent des équipes multidisciplinaires, une continuité ou une coordination des soins, des prestations axées sur l'humain et une prise de décision partagée entre médecins et patients. Parmi les fonctions des modèles de soins figurent l'évaluation standardisée des symptômes, la télémédecine et les soins virtuels, ainsi que le mécanisme de suivi. La estrategia de búsqueda incluyó términos relacionados con (i) afección posterior a la COVID-19 y otras terminologías que se conocen en la actualidad; (ii) modelos y vías de atención; y (iii) rehabilitación. No se aplicó ninguna restricción de idioma o de diseño del estudio. Dos pares de investigadores revisaron de forma independiente el título, los resúmenes y los artículos de texto completo y extrajeron los datos. Se clasificó la evidencia de acuerdo con cinco temas: (i) componentes y funciones del modelo de atención; (ii) prestación segura de la rehabilitación; (iii) principios de derivación; (iv) entornos de prestación de servicios; y (v) profesionales sanitarios.Se revisaron 13 753 títulos y resúmenes, se leyeron 154 artículos de texto completo y se incluyeron 37 artículos. La evidencia actual es conceptual y de especialistas. BACKGROUND The underuse or overuse of knowledge products leads to waste in health care, and primary care is no exception. OBJECTIVE This study aimed to characterize which knowledge products are frequently implemented, the implementation strategies used in primary care, and the implementation outcomes that are measured. METHODS We performed a systematic review (SR) of SRs using the Cochrane systematic approach to include eligible SRs. The inclusion criteria were any primary care contexts, health care professionals and patients, any Effective Practice and Organization of Care implementation strategies of specified knowledge products, any comparators, and any implementation outcomes based on the Proctor framework. We searched the MEDLINE, EMBASE, CINAHL, Ovid PsycINFO, Web of Science, and Cochrane Library databases from their inception to October 2019 without any restrictions. We searched the references of the included SRs. Pairs of reviewers independently performed selection, data extraction, and methodological quality assessment by using A Measurement Tool to Assess Systematic Reviews 2. Data extraction was informed by the Effective Practice and Organization of Care taxonomy for implementation strategies and the Proctor framework for implementation outcomes. We performed a descriptive analysis and summarized the results by using a narrative synthesis. RESULTS Of the 11,101 records identified, 81 (0.73%) SRs were included. Of these 81, a total of 47 (58%) SRs involved health care professionals alone. Moreover, 15 SRs had a high or moderate methodological quality. Most of them addressed 1 type of knowledge product (56/81, 69%), common clinical practice guidelines (26/56, 46%) or management, and behavioral or pharmacological health interventions (24/56, 43%). Mixed strategies were used for implementation (67/81, 83%), predominantly education-based (meetings in 60/81, 74%; materials distribution in 59/81, 73%; and academic detailing in 45/81, 56%), reminder (53/81, 36%), and audit and feedback (40/81, 49%) strategies. Education meetings (P=.13) and academic detailing (P=.11) seemed to be used more when the population was composed of health care professionals alone. Improvements in the adoption of knowledge products were the most commonly measured outcome (72/81, 89%). The evidence level was reported in 12% (10/81) of SRs on 62 outcomes (including 48 improvements in adoption), of which 16 (26%) outcomes were of moderate or high level. CONCLUSIONS Clinical practice guidelines and management and behavioral or pharmacological health interventions are the most commonly implemented knowledge products and are implemented through the mixed use of educational, reminder, and audit and feedback strategies. There is a need for a strong methodology for the SR of randomized controlled trials to explore their effectiveness and the entire cascade of implementation outcomes.

ContextMore than 18M people worldwide (150K Canadians) are living with Long COVID resulting in debilitating sequalae and disabilities that impact their quality of life and capacity to return to work. A new care model is needed for persons living with this complex and multi-systemic disease.ObjectivesWhat is the best-available evidence about care models for persons living with Long COVID?DesignRapid Living Systematic Review.MethodWe systematically searched seven electronic databases (MEDLINE, Embase, Web of Science, COVID-END, L-OVE, CDRS and WHO Ovid) on May 27th, 2021. Two independent reviewers screened titles, abstracts and full text. We included studies reporting on 1- persons living with Long COVID and 2- proposing a specific care model (i.e., dedicated clinic, care pathway). We extracted characteristic of studies (e.g., countries, study design, age group), referral pathways targeted (e.g., hospitalized, community), reporting of the care model implementation with number of patients, clinical settings of care model (e.g., primary care), healthcare professions included in the care model, care model principles (e.g., person-centred care) and care model components (e.g., standardized symptoms assessment). We used descriptive statistics and frequency count.ResultsWe screened 2181 citations, read 65 full text and included 12 eligible articles reporting on care models for Long COVID. Half studies were from the United Kingdom. 7 out of 12 models reported conceptual models without a description of implementation. All but one model was designed for discharge and long-term follow-up of hospitalized patients and half models were designed for non- hospitalized or patients who lived with the disease only in the community. Nine out of 12 care models included primary care, 8 out of 12 included specialized clinics and all studies included rehabilitation services. A total of 30 healthcare professions and medical specialties were proposed for staffing Long COVID services. More than half studies proposed multidisciplinary teams, integrated/coordination of care, evidence-based care and patient-centred care as key care model principles. Standardized symptom assessment, follow-up system and virtual care were the most frequent care model components.ConclusionThe implementation of care models for Long COVID is underway in several countries. Care models need to include both hospitalized and non-hospitalized patients. A complete care model for this population appears to design a care pathway integrating primary care, rehabilitation services and specialized clinics for medical assessment. The entry into care pathways is likely possible through a centralized referral system. It is possible to design sustainable and equitable care pathways for Long COVID in Canada integrated in current infrastructure.Protocol/Topic RegistrationCRD42021282266SummaryAn estimated 150K Canadians, mostly women, are facing debilitating sequalae and disabilities from Long COVID that impact their quality of life and capacity to return to work.
