Introduction We investigated whether living in immigrant-dominated neighbourhoods constituted a risk factor for COVID-19 infection and hospitalisation among healthcare workers (HCWs) in Sweden, and if so, whether such exposure exacerbated the risk of COVID-19 among immigrant HCWs.Methods We used population-based register data from HCWs aged 20–62 years (N=86 187) resident in 14 Swedish municipalities (3 of which are Sweden’s largest metropolitan cities) on 1 January 2020. Residential neighbourhoods of the HCWs were categorised into three groups: Swedish-dominated, mixed and immigrant-dominated. Multilevel mixed-effects survival regression was used for the association analyses, with control for relevant confounding variables. The results are reported as HRs, with 95% CIs.Results From 1 January 2020 to 30 September 2022, we recorded 39 746 COVID-19 infections and 860 COVID-19-related hospitalisations. Except during the first wave of the pandemic, living in immigrant-dominated (adjusted HR 0.98; 95% CI 0.94 to 1.01) or mixed (adjusted HR 1.02; 95% CI 0.99 to 1.05) neighbourhoods was not associated with COVID-19 infection, but living in these areas was associated with an increased risk of having COVID-19-related hospitalisation throughout the study period. Immigrant HCWs, regardless of their neighbourhood of residence, had approximately 2-fold higher risk of being hospitalised for COVID-19 than non-immigrant HCWs living in Swedish-dominated neighbourhoods.Conclusions Among HCWs in Sweden, neighbourhood immigrant density constituted a risk factor for COVID-19-related hospitalisation. However, immigrant HCWs had an elevated risk of COVID-19-related hospitalisation regardless of where they lived.

Introduction Understanding smokers’ purchasing patterns can aid in customizing tobacco control initiatives aimed at reducing the tobacco smoking prevalence. Therefore, this study identified cigarette purchase behavior among Vietnamese male smokers and associated demographic and consumption factors. Methods We analyzed a secondary dataset of male current tobacco smokers (n=3983) who participated in the Vietnam Global Adult Tobacco Survey in 2015. We applied the latent class analysis (LCA) to identify the classes of purchase behavior among cigarette smokers (n=1241). Multinomial logistic regression was performed to identify demographics (education level, ethnicity, partnership status, and household socioeconomic status) and cigarette consumption variables (smoking years and heavy smoking status) related to purchase behavior classes. The results are reported as an adjusted relative risk ratio (ARRR). Results The LCA identified four cigarette purchase behaviors classes: Class 1 (priceinsensitive and purchased international brand: 44.4%), Class 2 (price-sensitive and purchased domestic brand: 27.6%), Class 3 (price-sensitive and purchased cigarettes in a street vendor: 18.6%), and Class 4: price-sensitive and purchased loose/carton cigarette: 9.4%). The poorer economic groups were more likely to belong to the three price-sensitive classes. Heavy smokers and those who had smoked for a longer period were more likely to belong to Class 3 (ARRR=2.33; 95% CI: 1.51–3.58 and ARRR=1.02; 95% CI: 1.001–1.05, respectively) and Class 4 (ARRR=2.94; 95% CI: 1.71–5.06 and ARRR=1.05; 95% CI: 1.02–1.08, respectively). Conclusions Varied purchasing behaviors among male cigarette smokers, influenced by divergent price sensitivities and economic backgrounds, underscore the need for comprehensive tobacco control. Future efforts should include targeted policy interventions, behavior modification, and reshaping social norms.

The Convention on the Rights of Persons with Disabilities recognises the value of knowledge derived from lived experience with disability as a vital part of the co-production of policy affecting disabled people. Furthermore, Article 11 of the Convention specifically highlights the need to safeguard the rights of disabled people during times of crisis. This paper draws on interviews with representatives of Icelandic disabled people’s organisations conducted from 2021 to 2023. Informed by the human rights approach and feminist standpoint theory, it explores critical issues raised by representatives of disabled people regarding the capacity of Icelandic DPOs to effectively participate in policymaking during the pandemic. The findings demonstrate the value of the lived experience of disability, which enabled DPOs to promptly advocate for effective solutions. However, collaboration and consultation were limited, and DPOs were not involved in important decision-making, suggesting their role as knowledgeable stakeholders remains undervalued.

This article critically examines the application of an innovative project aimed at developing a mechanism for people with intellectual disabilities to provide input to the Icelandic government’s report on its implementation of the Convention on the Rights of Persons With Disabilities (CRPD). The project was undertaken to comply with the CRPD’s obligation to ensure the participation of disabled people in the review process and to respond to the recognized need for changes to consultation processes to accommodate the needs of people with intellectual disabilities. The project was successful in producing its intended outcome, to facilitate meaningful input by people with intellectual disabilities to the national review process. However, the research reveals that effective use of the outcome report by the authorities, which had both funded the project and praised its work, was lacking. These findings draw attention to the need to address unspoken norms and biases, and to take assertive steps to institutionalize a more structured and transparent process of co‐creation to ensure that the voices of marginalized groups are in fact heard and effectively taken into account in outcome processes. The research this article draws on is qualitative, comprised of data gathered through document analysis, as well as in‐depth interviews with representatives of disabled people’s organizations and the authorities.

Monoclonal gammopathy of undetermined significance (MGUS) is an asymptomatic precursor condition that precedes multiple myeloma and related disorders but has also been associated with other medical conditions. Since systematic screening is not recommended, MGUS is typically diagnosed due to underlying diseases and most cases are not diagnosed. Most previous studies on MGUS disease associations have been based on clinical cohorts, possibly resulting in selection bias. Here we estimate this selection bias by comparing clinically diagnosed and screened individuals with MGUS with regards to demographics, laboratory features, and comorbidities. A total of 75,422 participants in the Iceland Screens, Treats, or Prevents Multiple Myeloma (iStopMM) study were screened for MGUS by serum protein electrophoresis, immunofixation and free light chain assay (clinicaltrials gov. Identifier: NCT03327597). We identified 3,352 individuals with MGUS, whereof 240 had previously been clinically diagnosed (clinical MGUS), and crosslinked our data with large, nationwide registries for information on comorbidities. Those with clinical MGUS were more likely to have at least one comorbidity (odds ratio=2.24; 95% confidence interval: 1.30-4.19), and on average had more comorbidities than the screened MGUS group (3.23 vs. 2.36, mean difference 0.68; 95% confidence interval: 0.46-0.90). They were also more likely to have rheumatological disease, neurological disease, chronic kidney disease, liver disease, heart failure, or endocrine disorders. These findings indicate that individuals with clinical MGUS have more comorbidities than the general MGUS population and that previous studies have been affected by significant selection bias. Our findings highlight the importance of screening data when studying biological and epidemiological implications of MGUS.

There are astonishingly numerous and profound influences of the Pre-Socratics – especially Herakleitos and Zenon – on Russian literature between realism and the avant-garde of the 1920s. The focus here is on the concepts of Herakleitos’ “panta rhei” and his pre-dialectical thinking in polarities. From there, a bridge can be built to Leo Tolstoy’s narrative technique of the “stream of consciousness” and his speculations on time and history in the context of his novel “War and Peace.” The Russian novelist was particularly fascinated by Zenon’s time paradox (Achilles and the Tortoise). Furthermore, this contribution is concerned with Herakleitos’ model of circulations and dualities in the mytho-poetics of Russian Symbolism around 1900 (Viacheslav Ivanov, Andrey Bely, Konstantin Balmont) and, above all, with Russian poetry of the absurd (Daniil Kharms, Aleksander Vvedenskii) and the concepts of nothingness, of infinity in the context on this side of the categories of space and time (“cisfinite poetry”), and with the spirit of the time paradox of Zenon.

Abstract Background Depression and anxiety disorder contribute to a significant part of the disease burden among men, yet many men refrain from seeking care or receive insufficient care when they do seek it. This is plausibly associated with poorer mental well-being, but there is a lack of population-based research. This study investigated 1) if men who had refrained from seeking mental healthcare at any time in life had poorer mental well-being than those who sought care, 2) if those who had sought care but perceived it as insufficient had poorer mental well-being than those who had perceived care as sufficient, and 3) if these differences persisted after 1 year. Methods This longitudinal study used questionnaire data from a population-based sample of 1240 men, aged 19–64 years, in Sweden. Having refrained from seeking mental healthcare, or perceiving the care as insufficient, at any time in life, was assessed in a questionnaire, 2008. Current mental well-being was assessed in 2008 and 2009 using mean scores on the WHO (Ten) Well-being Index. Lower scores indicate poorer mental well-being. Group differences were calculated using t-tests and multivariable linear regression analysis. Results Of the men who had perceived a need for mental healthcare, 37% had refrained from seeking such care. They had lower mental well-being scores in 2008, compared to those who sought care. Of those seeking care, 29% had perceived it as insufficient. They had lower mental well-being scores in 2008, compared to those who perceived the care as sufficient, but this was not statistically significant when controlling for potential confounders. There were no differences in mental well-being scores based on care-seeking or perceived care-sufficiency in 2009. Conclusions This population-based study indicates that men who have previously refrained from seeking mental healthcare, or perceived the care as insufficient, have poorer mental well-being. However, the lack of differences at the one-year follow-up contradicts these results. The results highlight the need for larger longitudinal studies, measuring care-seeking within a more specified time frame. This should be combined with efforts to increase men’s mental healthcare-seeking and to provide mental healthcare that is perceived as sufficient.

During the coronavirus disease 2019 (COVID-19) pandemic, Sweden emphasized voluntary guidelines over mandates. We exploited a rapid change and reversal of the Public Health Agency of Sweden's COVID-19 testing guidelines for vaccinated and recently infected individuals as a quasi-experiment to examine sociodemographic differences in the response to changes in pandemic guidelines. We analyzed daily polymerase chain reaction tests from 1 October 2021 to 15 December 2021, for vaccinated or recently infected adults (≥20 years; n  = 1 596 321) from three Swedish regions (Stockholm, Örebro, and Dalarna). Using interrupted time series analysis, we estimated abrupt changes in testing rates at the two dates when the guidelines were changed (1 November and 22 November). Stratified analysis and meta-regression were employed to explore sociodemographic differences in the strength of the response to the guideline changes. Testing rates declined substantially when guideline against testing of vaccinated and recently infected individuals came into effect on 1 November [testing rate ratio: 0.50 (95% confidence interval, CI 0.41, 0.61)], and increased again from these lowered levels by a similar amount upon its reversal on 22 November [testing rate ratio: 2.19 (95% CI: 1.69, 2.85)]. Being Sweden-born, having higher household income, or higher education, were all associated with a stronger adherent response to the guideline changes. Adjusting for stratum-specific baseline testing rates and test-positivity did not influence the results. Our findings suggest that the population was responsive to the rapid changes in testing guidelines, but with clear sociodemographic differences in the strength of the response. The concentration of SARS-CoV-2 changes during an individual’s infection, and mutations accumulate as viruses are transmitted between people. Here, the authors use data from Iceland to demonstrate how this information can be exploited at the population-level to determine the phase of the epidemic.

Abstract Background Despite the high prevalence and severe consequences for health and wellbeing, epidemiological research of neglected emotional needs during childhood is scarce and little is known about its relation to parental socioeconomic position (SEP). This study investigates the prevalence of family violence and parental unavailability in childhood and its association with parental SEP and parental psychological problems in four strata of young Swedish women examined 1990, 1995, 2000, and 2013. Method The sample comprised 976 women (mean age 22, range 20–25) living in Sweden. Secular trends for family violence, parental rejection and unavailability were analyzed using logistic regression as a function of year of examination. The associations with parental SEP and parental psychological problems were assessed using logistic regression with results in terms of odds ratios (OR) and 95% confidence intervals. Results Gendered patterns were observed in the associations between parental psychological problems and family violence and parental unavailability. Maternal psychological problems were associated with maternal rejection OR 6.8 (3.5–13.0), maternal lack of time OR 2.4 (1.2–5.0), and paternal rejection OR 1.9 (1.1–3.5). Paternal psychological problems were associated with paternal rejection OR 4.0 (2.1–7.7), paternal lack of time OR 4.9 (2.3–10.6), and experiencing family violence OR 4.9 (2.1–11.6). Low and medium parental SEP were associated with experience of family violence in childhood OR 3.1 (CI 1.1–8.5) and OR 3.4 (1.7–6.9), respectively. No changes between 1990 and 2013 were observed for the prevalence of any of the outcomes. Conclusions A stable prevalence of family violence and parental unavailability was reported by young women examined between 1990 and 2013. Lower socioeconomic position was associated with family violence while the association with parental unavailability was non-significant. Gendered patterns were observed in the association between parental psychological problems and family violence, where paternal but not maternal psychological problems were associated with family violence. Further, maternal psychological problems were associated with paternal rejection while paternal psychological problems were not associated with maternal rejection. Gendered patterns of parental unavailability need further studies.
