Background Short-term pain relief can be achieved by local anaesthetic infiltration of port sites at the end of laparoscopic surgery. This study aimed to assess feasibility of performing an RCT to evaluate short-term postoperative analgesia after laparoscopic surgery in Nigeria using two local anaesthetics for port-site infiltration versus saline placebo. Methods This was a placebo-controlled, patient- and outcome assessor-blinded, external feasibility RCT. Patients undergoing elective laparoscopic cholecystectomy for symptomatic ultrasound-proven gallstones were randomized into three groups: lidocaine with adrenaline (epinephrine), bupivacaine or saline control. The feasibility of recruitment, compliance with randomized treatment allocation, and completion of pain and nausea outcome measures were evaluated. Pain was assessed at 2, 6, 12 and 24 h after surgery using a 0-10-point numerical rating scale (NRS) and a four-point verbal rating scale. Nausea was assessed using NRS at the same time points. Clinical outcomes were assessed only in patients who received the correct randomized treatment allocation. Results Of 79 patients screened for eligibility, 69 were consented and randomized (23 per group). Overall, compliance with randomized treatment allocation was achieved in 64 patients (93 per cent). All pain and nausea assessments were completed in these 64 patients. On the NRS, most patients had moderate to severe pain at 2 h (39 of 64, 61 per cent), which gradually reduced. Only six patients (9 per cent) had moderate to severe pain at 24 h. Conclusion Recruitment, compliance with the randomized allocation, and completion of pain outcome measures were satisfactory. Macroalgae are of increasing interest for high-value biotechnological applications, but some seaweeds, such as coralline red algae, cannot be grown in cultivation cost-effectively. Wild harvesting of seaweeds, particularly of those that are ecosystem engineers, must be demonstrably sustainable: here we address the topic of resource sustainability in the context of harvesting Corallina officinalis in Ireland for bioceramics. C. officinalis provides habitat for a diverse macrofaunal community and the effects of harvesting C. officinalis on the associated fauna must be included in any assessment of harvesting sustainability. Corallina intertidal turfs subject to experimental harvesting were confirmed, using DNA barcoding with cox1, to comprise only C. officinalis and not the pseudocryptic species C. caespitosa, despite the wide range of morphologies, and they had high genetic diversity. Harvesting of C. officinalis was carried out at experimental sites by two techniques (hand cutting and pulling) to test the recovery of the primary resource and the associated macroinvertebrate assemblage. Harvesting the alga by both methods encouraged regrowth: cut and pulled plots had a much higher growth rate than unharvested turfs, regaining their original length within 4-6 months of harvesting, suggesting that turfs of this species may grow to a predetermined length. The structure, richness and evenness of the invertebrate assemblage were not significantly affected by harvesting C. officinalis by cutting or pulling, though some organisms within the community showed a response to harvesting. The pattern of recovery of the sediment, an important component of the C. officinalis habitat, was consistent with the shorter (harvested) turf trapping more sediment than longer natural turfs. As many of the organisms associated with the habitat use the sediment for food or building materials, this may have ameliorated the effects of harvesting on the community. A period of a year between harvests is recommended to allow the C. officinalis biomass to return to baseline levels and unharvested fallow areas should be included in a harvesting plan to allow macroinvertebrates to re-colonise the harvested turf.

Background and aims There is untapped potential for public transport-related physical activity to make an important contribution to total physical activity and hence improve health. This study aimed to determine the willingness of public transport users to walk further than necessary to a stop, and the characteristics of those willing to do so. Methods This study used data from three waves (2016, 2017, 2018) of the Metro Tasmania Customer Satisfaction Survey (CSS), an annual cross-sectional survey of bus passengers in Tasmania, Australia. Linear and logistic regression modelled associations between socio-demographic and transport behaviour characteristics with willingness to travel further to usual bus stop if frequency was improved (yes/no), and if willing, the extra distance (metres) prepared to travel. Analyses were restricted to those reporting walking to their bus stop (n = 1402). Results Half of 1402 bus passengers were prepared to walk further (average 521 m) to their usual bus stop if frequency was improved. A range of characteristics were positively (residential location) and negatively (age, female sex, retired/on pension, no regular motor vehicle access, intermittent public transport use, higher public transport-related physical activity) associated with willingness to walk further to the bus stop. Distance willing to walk varied by a range of demographic characteristics (sex, household income, age, employment status, bus use, public transport-related physical activity). Only age was consistently associated across all three surveys: younger bus passengers were more willing to walk further and recorded greater distances willing to walk. Conclusions The characteristics identified provide insights into which population groups may be more amenable to strategies to increase public transport-related physical activity, and which groups may require further investigation to identify reasons for lack of willingness to walk further to bus stops. Improving bus service frequency may increase public transport-related physical activity and confer health benefits.

Nine new polycyclic aromatic BN-1,2-azaborine analogues containing the N-BOH moiety were synthesized using a convenient two-step, one-pot procedure. Characterization of the prepared compounds show the luminescence wavelength and the quantum yields of the azaborines were tunable by controlling the power and location of the donor and acceptor substituents on the chromophore. UV-visible spectroscopy and density functional theory (DFT) computations revealed that the addition of electron-donating moieties to the isoindolinone hemisphere raised the energy of the HOMO, resulting in the reduction of the HOMO–LUMO gap. The addition of an electron-accepting moiety to the isoindolinone hemisphere and an electron-donating group to the boronic acid hemisphere decreased the HOMO–LUMO gap considerably, leading to emission properties from partial intramolecular charge transfer (ICT) states. The combined effect of an acceptor on the isoindolinone side and a donor on the boronic acid side (strong acceptor–π-donor) gave the most red-shifted absorption. The polycyclic aromatic BN-1,2-azaborines emitted strong fluorescence in solution and in the solid-state with the largest red-shifted emission at 640 nm and a Stokes shift of Δλ = 218 nm, or Δν = 8070 cm−1.

Our knowledge of non-native algae in benthic estuarine habitats is relatively poor, especially compared to algal introductions along open shores or on floating structures. Gracilaria vermiculophylla is a widespread macroalgal invader in the temperate estuaries of the Northern Hemisphere, and, here, we expand its documented range within northeastern Ireland and England. Established populations occur within two inlets in the border counties, Carlingford Lough (Counties Louth and Down) and Dundrum Bay (County Down), but G. vermiculophylla is absent from open coasts between these sites. Repeated surveys in Dundrum Bay showed variable abundances, with an increase in biomass between 2013 and 2016. Three populations were discovered in England, where this species had not previously been identified: Christchurch Harbour (Dorset), Brownsea Island in Poole Harbour (Dorset), and Kingsbridge Estuary (Devon). The Irish and English thalli belong to the most common, invasive cox1 haplotype 6. Using a combination of morphological observations and 10 microsatellite loci, we found that the population at Carlingford Lough included both reproductive haploid gametophytes and diploid tetrasporophytes and genetic signatures of sexual reproduction, but the populations at Christchurch and Brownsea displayed signatures of partial clonality. Genetic diversity was higher along the south coast of England as compared to the Irish population, consistent with patterns of diversity previously described for the European coasts. Finally, we also note the occurrence of a putative G. vermiculophylla population in Wales at Porthmadog, Gwynedd. As the sites in which we have now documented G. vermiculophylla in the British Isles also host shellfish aquaculture activities, our study is further evidence for the role of aquaculture in the spread of invasive species.

We present extensive datasets for a class of intermediate-luminosity optical transients known as "luminous red novae" (LRNe). They show double-peaked light curves, with an initial rapid luminosity rise to a blue peak (at -13 to -15 mag), which is followed by a longer-duration red peak that sometimes is attenuated, resembling a plateau. The progenitors of three of them (NGC4490-2011OT1, M101-2015OT1, and SNhunt248), likely relatively massive blue to yellow stars, were also observed in a pre-eruptive stage when their luminosity was slowly increasing. Early spectra obtained during the first peak show a blue continuum with superposed prominent narrow Balmer lines, with P Cygni profiles. Lines of Fe II are also clearly observed, mostly in emission. During the second peak, the spectral continuum becomes much redder, Halpha is barely detected, and a forest of narrow metal lines is observed in absorption. Very late-time spectra (~6 months after blue peak) show an extremely red spectral continuum, peaking in the infrared (IR) domain. Halpha is detected in pure emission at such late phases, along with broad absorption bands due to molecular overtones (such as TiO, VO). We discuss a few alternative scenarios for LRNe. Although major instabilities of single massive stars cannot be definitely ruled out, we favour a common envelope ejection in a close binary system, with possibly a final coalescence of the two stars. We present the one-year long observing campaign of SN 2012A which exploded in the nearby (9.8 Mpc) irregular galaxy NGC 3239. The photometric evolution is that of a normal type IIP supernova. The absolute maximum magnitude, with MB = -16.23 +- 0.16 mag. SN2012A reached a peak luminosity of about 2X10**42 erg/s, which is brighter than those of other SNe with a similar 56Ni mass. The latter was estimated from the luminosity in the exponential tail of the light curve and found to be M(56Ni) = 0.011 +-0.004 Msun. The spectral evolution of SN 2012A is also typical of SN IIP, from the early spectra dominated by a blue continuum and very broad (~10**4 km/s) Balmer lines, to the late-photospheric spectra characterized by prominent P-Cygni features of metal lines (Fe II, Sc II, Ba II, Ti II, Ca II, Na ID). The photospheric velocity is moderately low, ~3X10**3 km/s at 50 days, for the low optical depth metal lines. The nebular spectrum obtained 394 days after the shock breakout shows the typical features of SNe IIP and the strength of the [O I] doublet suggests a progenitor of intermediate mass, similar to SN 2004et (~15 Msun). A candidate progenitor for SN 2012A has been identified in deep, pre-explosion K'-band Gemini North (NIRI) images, and found to be consistent with a star with a bolometric magnitude -7.08+-0.36 (log L/Lsun = 4.73 +- 0.14$ dex). The magnitude of the recovered progenitor in archival images points toward a moderate-mass 10.5 (-2/+4.5) Msun star as the precursor of SN 2012A. The explosion parameters and progenitor mass were also estimated by means of a hydrodynamical model, fitting the bolometric light curve, the velocity and the temperature evolution. We found a best fit for a kinetic energy of 0.48 foe, an initial radius of 1.8X10**13 cm and ejecta mass of 12.5 Msun., 25 pages, 17 figures. Revised version (minor revision) submitted to MNRAS 2013

Six new heteroaromatic polycyclic azaborine chromophores were designed, synthesized, and investigated as easily tunable high-luminescent organic materials. The impact of the nitrogen-boron-hydroxy (N-BOH) unit in the azaborines was investigated by comparison with their N-carbonyl analogs. Insertion of the N-B(OH)-C unit into heteroaromatic polycyclic compounds resulted in strong visible absorption and sharp fluorescence with efficient quantum yields. The solid-state fluorescence of the heteroaromatic polycyclic compounds displayed a large Stokes shift compared to being in solution. The large Stokes shifts observed offset the self-quench effect in the solid state.

We present an extensive set of photometric and spectroscopic data for SN 2009jf, a nearby Type Ib supernova, spanning from ~20 days before B-band maximum to one year after maximum. We show that SN 2009jf is a slow evolving and energetic stripped-envelope SN and is likely from a massive progenitor (25-30 solar masses). The large progenitor's mass allows us to explain the complete hydrogen plus helium stripping without invoking the presence of a binary companion. The supernova occurred close to a young cluster, in a crowded environment with ongoing star-formation. The specroscopic similarity with the He-poor Type Ic SN 2007gr suggests a common progenitor for some supernovae Ib and Ic. The nebular spectra of SN 2009jf are consistent with an asymmetric explosion, with an off-center dense core. We also find evidence that He-rich Ib supernovae have a rise time longer than other stripped-envelope supernovae, however confirmation of this result and further observations are needed.

Jellyfish are increasingly topical within studies of marine food webs. Stable isotope analysis represents a valuable technique to unravel the complex trophic role of these long-overlooked species. In other taxa, sample preservation has been shown to alter the isotopic values of species under consideration, potentially leading to misinterpretation of trophic ecology. To identify potential preservation effects in jellyfish, we collected Aurelia aurita from Strangford Lough (54(o)22'44.73aEuro(3)N, 5(o)32'53.44aEuro(3)W) during May 2009 and processed them using three different methods prior to isotopic analysis (unpreserved, frozen and preserved in ethanol). A distinct preservation effect was found on delta N-15 values: furthermore, preservation also influenced the positive allometric relationship between individual size and delta N-15 values. Conversely, delta C-13 values remained consistent between the three preservation methods, conflicting with previous findings for other invertebrate, fish and mammalian species. These findings have implications for incorporation of jellyfish into marine food webs and remote sampling regimes where preservation of samples is unavoidable.

In an effort to curb monetary and environmental costs of food waste disposal, colleges and universities are developing composting programs. Incorporating additional on-site wastes could improve composting efficiency and provide cost savings. A living laboratory team of faculty and students with expertise in biology, engineering, and economics investigated non-food organic wastes as co-compostable supplementary feedstocks with dining hall residues. The interdisciplinary learning environment inspired students to develop a specialised sampling method using nylon mesh bags embedded in large bins to monitor biotic and abiotic composting responses. Bark, leaves, and paper were equally effective as co-compostable wastes. Economic analysis showed that incorporating non-food organic feedstocks into food waste composting could save $17,500 USD per dining hall per year when balanced with waste disposal or recycling costs for these materials. This project shows how a living laboratory approach can address local waste management issues with sustainable economic practices by examining alternative disposal methods of common organic wastes.

We present ultraviolet, optical and near-infrared data of the Type Ibn supernovae (SNe) 2010al and 2011hw. SN 2010al reaches an absolute magnitude at peak of M(R) = -18.86 +- 0.21. Its early light curve shows similarities with normal SNe Ib, with a rise to maximum slower than most SNe Ibn. The spectra are dominated by a blue continuum at early stages, with narrow P-Cygni He I lines indicating the presence of a slow-moving, He-rich circumstellar medium. At later epochs the spectra well match those of the prototypical SN Ibn 2006jc, although the broader lines suggest that a significant amount of He was still present in the stellar envelope at the time of the explosion. SN 2011hw is somewhat different. It was discovered after the first maximum, but the light curve shows a double-peak. The absolute magnitude at discovery is similar to that of the second peak (M(R) = -18.59 +- 0.25), and slightly fainter than the average of SNe Ibn. Though the spectra of SN 2011hw are similar to those of SN 2006jc, coronal lines and narrow Balmer lines are cleary detected. This indicates substantial interaction of the SN ejecta with He-rich, but not H-free, circumstellar material. The spectra of SN 2011hw suggest that it is a transitional SN Ibn/IIn event similar to SN 2005la. While for SN 2010al the spectro-photometric evolution favours a H-deprived Wolf-Rayet progenitor (of WN-type), we agree with the conclusion of Smith et al. (2012) that the precursor of SN 2011hw was likely in transition from a luminous blue variable to an early Wolf-Rayet (Ofpe/WN9) stage., 23 pages, 11 figures, 6 tables. Accepted by MNRAS

This paper quantifies cost savings to correctional systems and to society resulting from implementation of the Transcendental Meditation (TM)1 program. Based on results from studies at several maximum-security prisons, including Folsom and San Quentin in California and Walpole in Massachusetts, up to 56% fewer inmates are convicted of new crimes when they are released after beginning the TM program. Furthermore, practitioners of this program are returned to prison up to 45% less often for parole violations or any other reason. These and other effects described in this paper allow the TM program to lower costs. Savings are projected to arise from: (a) reduced recidivism, (b) reduced medical expenses, (c) a more manageable prison environment, (d) improved quality of life for staff, and (e) a reduced crime rate for society. Conservatively, the overall ratio of program cost to total savings is estimated at 1 to 10, with 46% of savings accruing to the correctional system and 54% to the general public....

In response to numerous external pressures, business schools today are struggling to enact and evaluate educational outcomes to help establish both validity and accountability. This article describes the process used by a management department to create an outcomes document that serves as a student guide for skills development. Student portfolios are now a required part of the curriculum to measure the attainment of knowledge, skills, and abilities. The article concludes by discussing the ramifications of these processes (for curriculum, students, and faculty) as well as articulating the lessons learned from this process, which may serve to guide other institutions.

The authors develop theory for predicting the distribution of decision making between the corporate and business-unit levels of management for a subset of information systems (IS) resources referred to as systems development. Drawing on literature from the fields of MIS, strategic management, and organization theory, they first determine how potentially influential context factors are likely to affect the locus of the lead decision-making role from a multiple-contingencies perspective. Then they theorize how conflicting corporate and business-unit contingencies are likely to be resolved. They present a set of six propositions that predict a centralized, decentralized, or compromise design solution for a given business unit on the basis of (1) business-level strategy, (2) whether or not information technology (IT) plays a strategic role for the business unit, (3) the degree of line managers' IT knowledge at the business-unit level, and (4) the level at which opportunities for IT-related synergies across business units are being pursued at the corporate level.

We present optical and near-infrared (NIR) photometry and spectroscopy of the Type IIb supernova (SN) 2011dh for the first 100 days. We complement our extensive dataset with SWIFT ultra-violet (UV) and Spitzer mid-infrared (MIR) data to build a UV to MIR bolometric lightcurve using both photometric and spectroscopic data. Hydrodynamical modelling of the SN based on this bolometric lightcurve have been presented in Bersten (2012). We find that the absorption minimum for the hydrogen lines is never seen below ~11000 km/s but approaches this value as the lines get weaker. This suggests that the interface between the helium core and hydrogen rich envelope is located near this velocity in agreement with the Bersten et al. (2012) He4R270 ejecta model. Spectral modelling of the hydrogen lines using this ejecta model supports the conclusion and we find a hydrogen mass of 0.01-0.04 solar masses to be consistent with the observed spectral evolution. We estimate that the photosphere reaches the helium core at 5-7 days whereas the helium lines appear between ~10 and ~15 days, close to the photosphere and then move outward in velocity until ~40 days. This suggests that increasing non-thermal excitation due to decreasing optical depth for the gamma-rays is driving the early evolution of these lines. We also provide and discuss pre- and post-explosion observations of the SN site which shows a reduction by 75 percent in flux at the position of the yellow supergiant coincident with SN 2011dh. The B, V and r band decline rates of 0.0073, 0.0090 and 0.0053 mag/day respectively are consistent with the remaining flux being emitted by the SN. Hence we find that the star was indeed the progenitor of SN 2011dh as previously suggested by Maund et al. The mission statement is recognized as a vital component to achieving long-term success for organizations. What is the appropriate process for developing the mission statement? The bottom-up mission process is described and advocated for the growing number of professional service organizations comprised of opinionated and highly trained individuals. A case study based on an educational organization is detailed and a flowchart is developed. Guidelines for a bottom-up mission process for professional service organizations are developed for those organizations attempting to create their own bottom-up mission.

We report extensive observational data for five of the lowest redshift Super-Luminous Type Ic Supernovae (SL-SNe Ic) discovered to date, namely PTF10hgi, SN2011ke, PTF11rks, SN2011kf and SN2012il. Photometric imaging of the transients at +50 to +230 days after peak combined with host galaxy subtraction reveals a luminous tail phase for four of these SL-SNe. A high resolution, optical and near infrared spectrum from xshooter provides detection of a broad He I $\lambda$10830 emission line in the spectrum (+50d) of SN2012il, revealing that at least some SL-SNe Ic are not completely helium free. At first sight, the tail luminosity decline rates that we measure are consistent with the radioactive decay of \co, and would require 1-4M of \ni to produce the luminosity. These \ni masses cannot be made consistent with the short diffusion times at peak, and indeed are insufficient to power the peak luminosity. We instead favour energy deposition by newborn magnetars as the power source for these objects. A semi-analytical diffusion model with energy input from the spin-down of a magnetar reproduces the extensive lightcurve data well. The model predictions of ejecta velocities and temperatures which are required are in reasonable agreement with those determined from our observations. We derive magnetar energies of $0.4\lesssim E$($10^{51}$erg) $\lesssim6.9$ and ejecta masses of $2.3\lesssim M_{ej}$(\M) $\lesssim 8.6$. We present observations of the Type Ic supernova (SN Ic) 2011bm spanning a period of about one year. The data establish that SN 2011bm is a spectroscopically normal SN Ic with moderately low ejecta velocities and with a very slow spectroscopic and photometric evolution (more than twice as slow as SN 1998bw). The Pan-STARRS1 retrospective detection shows that the rise time from explosion to peak was 40 days in the R band. Through an analysis of the light curve and the spectral sequence, we estimate a kinetic energy of 7-17 foe and a total ejected mass of 7-17 Mo, 5-10 Mo of which is oxygen and 0.6-0.7 Mo is 56Ni. The physical parameters obtained for SN 2011bm suggest that its progenitor was a massive star of initial mass 30-50 Mo. The profile of the forbidden oxygen lines in the nebular spectra show no evidence of a bi-polar geometry in the ejected material., 3 Figures - 2 Tables - Accepted for publication in ApJL

A pilot study was undertaken to determine the accuracy of computed tomography (CT) staging in identifying patients with high-risk colon cancers who would be considered as candidates for a neoadjuvant therapy trial (FOxTROT) and those at low risk (T1/T2) who would be excluded.Participating radiologists from 19 centres attended workshops for standardization of image interpretation according to previously defined prognostic criteria: good prognosis tumours, including, T1/T2; intermediate prognosis, T35 mm tumour invasion beyond the muscularis propria (MP); and poor prognosis tumours, including T3 with tumour extension ≥ 5 mm beyond the MP or T4. The CT findings were compared with histopathology as the reference standard.Of 94 patients with radiological and pathological data, 71% were categorized by CT as having a poor prognosis. The sensitivity and specificity of CT in identifying these tumours were 87% (95% CI, 74-94) and 49% (95% CI, 33-65). Sensitivity and specificity for tumour infiltration beyond the MP (T3/T4 vs T1/T2) were 95% (95% CI, 87-98) and 50% (95% CI, 22-77), respectively. Including all CT-staged T3 and T4 patients in the trial would have increased the proportion eligible for entry to 89% (n = 84) without affecting the false-positive rate of 7%. Some 20% of T3/T4 patients would have been ineligible for FOxTROT because of synchronous metastases.In a multicentre setting, CT scanning identified high-risk (T3/4) colon cancers with minimal overstaging of T1/T2 tumours, thus establishing the feasibility of radiologically guided neoadjuvant chemotherapy.

We present the detection of the progenitor of the Type II SN 2011dh in archival pre-explosion Hubble Space Telescope images. Using post-explosion Adaptive Optics imaging with Gemini NIRI+ALTAIR, the position of the SN in the pre-explosion images was determined to within 23mas. The progenitor object was found to be consistent with a F8 supergiant star (log L/L_{\odot}=4.92+/-0.20 and T_{eff}=6000+/-280K). Through comparison with stellar evolution tracks, this corresponds to a single star at the end of core C-burning with an initial mass of M_{ZAMS}=13+/-3M_{\odot}. The possibility of the progenitor source being a cluster is rejected, on the basis of: 1) the source is not spatially extended; 2) the absence of excess H\alpha\, emission; and 3) the poor fit to synthetic cluster SEDs. It is unclear if a binary companion is contributing to the observed SED, although given the excellent correspondence of the observed photometry to a single star SED we suggest the companion does not contribute significantly. Early photometric and spectroscopic observations show fast evolution similar to the transitional Type IIb SN 2008ax, and suggest that a large amount of the progenitor's hydrogen envelope was removed before explosion., Comment: Submitted to ApJ Letters - 6 pages, 5 figures

Recent searches by unbiased, wide-field surveys have uncovered a group of extremely luminous optical transients. The initial discoveries of SN 2005ap by the Texas Supernova Search and SCP-06F6 in a deep Hubble pencil beam survey were followed by the Palomar Transient Factory confirmation of host redshifts for other similar transients. The transients share the common properties of high optical luminosities (peak magnitudes ~ -21 to -23), blue colors, and a lack of H or He spectral features. The physical mechanism that produces the luminosity is uncertain, with suggestions ranging from jet-driven explosion to pulsational pair-instability. Here we report the most detailed photometric and spectral coverage of an ultra-bright transient (SN 2010gx) detected in the Pan-STARRS 1 sky survey. In common with other transients in this family, early-time spectra show a blue continuum, and prominent broad absorption lines of O II. However, about 25d after discovery, the spectra developed type Ic supernova features, showing the characteristic broad Fe II and Si II absorption lines. Detailed, post-maximum follow-up may show that all SN 2005ap and SCP-06F6 type transients are linked to supernovae Ic. This poses problems in understanding the physics of the explosions: there is no indication from late-time photometry that the luminosity is powered by 56Ni, the broad lightcurves suggest very large ejected masses, and the slow spectral evolution is quite different from typical Ic timescales. The nature of the progenitor stars and the origin of the luminosity are intriguing and open questions., 16 pages, 4 figures, 1 table. Microbial cells, and ultimately the Earth's biosphere, function within a narrow range of physicochemical conditions. For the majority of ecosystems, productivity is cold-limited, and it is microbes that represent the failure point. This study was carried out to determine if naturally occurring solutes can extend the temperature windows for activity of microorganisms. We found that substances known to disorder cellular macromolecules (chaotropes) did expand microbial growth windows, fungi preferentially accumulated chaotropic metabolites at low temperature, and chemical activities of solutes determined microbial survival at extremes of temperature as well as pressure. This information can enhance the precision of models used to predict if extraterrestrial and other hostile environments are able to support life; furthermore, chaotropes may be used to extend the growth windows for key microbes, such as saprotrophs, in cold ecosystems and manmade biomes.

The null hypothesis is that active labor is a more important factor with regard to both timing and progression of periventricular-intraventricular hemorrhage than is route of delivery. Infants delivered by cesarean section after entering the active phase of labor will behave in a manner similar to that of previously studied infants delivered vaginally as to when periventricular-intraventricular hemorrhage occurs and frequency of progression.The 106 infants of 85 women delivered by cesarean section were the subjects of this study. Forty-six infants were in the no-labor group, 33 in the latent-phase labor group, and 27 in the active-phase labor group. Head ultrasonographic examinations were performed at delivery, at 1, 6, 12, and 24 hours, and then daily for the first 7 days of life. Continuous variables were compared by one-way analysis of variance among those infants with no hemorrhage or with periventricular-intraventricular hemorrhage. Categoric variables were compared by chi 2 analysis and Fisher's exact test when appropriate. A p value of less than 0.05 was considered significant.There was no difference in the frequency of early hemorrhage (less than or equal to 1 hour of age), late hemorrhage (greater than 1 hour of age), or overall periventricular-intraventricular hemorrhage in the infants not in labor, in latent-phase labor, or in active-phase labor at the time of cesarean section. However, the frequency of grade 3 or 4 hemorrhage and the progression of hemorrhage were significantly higher in the infants whose mothers had an active phase of labor compared with infants whose mothers had no labor or did not progress beyond the latent phase. Infants who had early periventricular-intraventricular hemorrhage (less than or equal to 1 hour of age) also had a higher frequency of progression of hemorrhage.Cesarean section before the active phase of labor does not change the overall frequency of hemorrhage but results in a lower frequency of progression to grade 3 or 4 hemorrhage. We do not feel that these data support performing more cesarean sections for preterm delivery as a method of preventing progression of periventricular-intraventricular hemorrhage in the preterm infant.

Although international joint ventures have been widely studied for many years, very little is known about such alliances as used by high technology firms based in the United States. Therefore, this study surveys 9 U.S. high technology firms regarding 14 of their international joint ventures. The rationale for these alliances, the control of key resources, and the successfulness of high tech international joint ventures are explored. Based on this preliminary study, it appears that these operations, if established with a trustworthy partner, can be successful while requiring only minimal transaction costs and minimal control efforts.

While avid accumulation of gallium-67 citrate and technetium-99m methylene diphosphonate (MDP) occurs initially in most cases of primary Ewing sarcoma, uptake after therapy is less well defined. Thirty patients with Ewing sarcoma who underwent Ga-67 and bone scintigraphy at diagnosis, at completion of therapy, and at relapse from 1978 to 1988 were evaluated. All 30 patients showed less primary site Ga-67 activity following therapy. Twenty-three of 28 patients who underwent corresponding bone scintigraphy showed less uptake, but residual activity was usually more intense than with Ga-67. Avid reaccumulation of Ga-67 occurred in four of five patients with primary site relapse, while patients who underwent bone scintigraphy showed less change. It was concluded that a greater decrease in Ga-67 than in Tc-99m MDP uptake often occurs in patients successfully treated for primary Ewing sarcoma. Information obtained at Ga-67 scintigraphy is most likely to be helpful if results of bone scintigraphy remain abnormal or if occult relapse is suspected.

Dipyridamole TI-201 imaging is an ideal alternative to exercise TI-201 scintigraphy in patients who are unwilling or unable to perform maximum exercise stress. The use of intravenous dipyridamole, alone or in combination with exercise, has not been approved for clinical practice by the Food and Drug Administration. Once approval is granted, the test will become a widely used and important component of the cardiac work-up. The indications, methodology, side effects, and utility of dipyridamole cardiac imaging in the clinical setting are discussed and a variety of examples presented.59 references.

The association between periventricular-intraventricular hemorrhage (PV-IVH) and frequent handling resulting from various neonatal intensive care procedures and routine interventions was evaluated in a prospective clinical study. Inborn premature babies with birth weight less than or equal to 1500 gm (n = 156) who did not have PV-IVH or who had grade 1 PV-IVH at less than or equal to 1 hour were randomly assigned to the reduced manipulation protocol (n = 62) or to standard care (n = 94). A bedside microcomputer-based data acquisition system was used to monitor the duration of rest or the number of interventions per day. Infants assigned to receive reduced manipulation spent a significantly higher percentage of time each day at rest than did those who received standard manipulation (p less than 0.006). However, the incidence of grades 2 to 4 PV-IVH did not differ significantly (30% in the study vs 37% in the standard manipulation group). When we analyzed the effect of manipulation in relation to risk of PV-IVH, while taking into account other perinatal variables, standard manipulation was not associated with increased risk of grades 2 to 4 PV-IVH. However, low birth weight, maternal smoking, general anesthesia, early grade 1 PV-IVH, low hematocrit, lowest arterial oxygen pressure within the first 6 hours of life, and large base deficit at 6 hours of age all increased the relative risk of grades 2 to 4 PV-IVH.

Hypophosphatasia, a rare heritable form of rickets/osteomalacia, is characterized by deficient activity of the tissue nonspecific (liver/bone/kidney) isoenzyme of alkaline phosphatase (ALP). Signs may be present prenatally or not until late adult life. Although the infantile form of hypophosphatasia has usually been categorized as an autosomal recessive (AR) disorder, several studies suggest that childhood cases are the consequence of either AR or autosomal dominant (AD) inheritance and adult cases are primarily AD. Eastman and Bixler (J Craniofac Genet Dev Biol 3:213-234, 1983) propose that all cases of hypophosphatasia may reflect AD inheritance with 85% penetrance and homozygous lethality. We report on 3 patients with hypophosphatasia in a black family, first manifested clinically during infancy, where the pattern of inheritance for each is consistent with AR transmission. Two were brothers who died from the disorder. The other patient, a cousin, presented with classic stigmata of hypophosphatasia during infancy, but is now age 5 1/2 years and has had a much milder clinical course. Although consanguinity is absent, the maternal grandmothers are sibs as are the maternal grandfathers and the paternal grandmothers. The family history is otherwise negative for skeletal or dental disease. Laboratory and radiographic results are consistent with heterozygosity in each parent. Fibroblast ALP activity is less than 1% normal in all 3 patients with no complementation observed in heterokaryon analysis. Accordingly, the genetic defects appear to be identical in all 3 patients. Our findings show that infantile hypophosphatasia may be inherited as an AR condition where there is variable expressivity and that homozygosity or compound heterozygosity, as may be the case in this family, is not necessarily lethal.

Pain associated with advanced cancer is multifaceted and complex, and is influenced by physiological, psychological, social, and spiritual phenomena. Suffering may be identified in patients when pain is associated with impending loss, increased dependency, and an altered understanding of one's existential purpose. Comprehensive pain management aims to address problematic symptoms in order to improve comfort, peace of mind, and quality of life. Music therapy is a treatment modality of great diversity that can offer a range of benefits to patients with advanced cancer pain and symptoms of suffering. Music therapists perform comprehensive assessments that include reviews of social, cultural, and medical history; current medical status; and the ways in which emotions are affecting the pain. A variety of music therapy techniques may be used, including vocal techniques, listening, and instrumental techniques. These techniques provide opportunities for exploration of the feelings and issues compounding the pain experience. Case examples are presented to demonstrate the "lifting", "transporting", and "bringing of peace" qualities of music that offer patients moments of release, reflection, and renewal.

Aggressive cardiac revascularization requires recognition of stunned and hibernating myocardium, and cost considerations may well govern the technique used. Dynamic low-dose (1 mCi) [123I]iodophenylpentadecanoic acid (IPPA) metabolic imaging is a potential alternative to PET using either 18FDG or 15O-water. Resting IPPA images were obtained from patients with severe ischemic cardiomyopathy, and transmural myocardial biopsies were obtained during coronary bypass surgery to confirm viability. Thirty-nine of 43 (91%) biopsies confirmed the results of the IPPA images with a sensitivity for viability of 33/36 (92%) and a specificity of 6/7 (86%). Postoperatively, wall motion improved in 80% of IPPA-viable, dysfunctional segments. Furthermore, when compared to reinjection thallium (SPECT-TI) scans after myocardial infarction, IPPA-SPECT-TI concordance occurred in 27/35 (77%) (K = 0.536, p = 0.0003). Similar to PET, IPPA demonstrated more viability than SPECT-TI, 26/35 (74%) versus 18/35 (51%) (p = 0.047). Metabolic IPPA cardiac viability imaging is a safe, inexpensive technique that may be a useful alternative to PET.

Through the use of verbal and nonverbal techniques, a social worker and music therapist have combined their fields into an integral therapeutic modality to provide patients with cancer and their children opportunities to experience intimacy in a time of crisis. Skilled verbal interventions and the sensitive application of the expressive and less threatening medium of music create a relaxed environment where families and patients may explore deeply and express freely.

Clearance of the radiopharmaceutical Tc-99m DTPA estimated from blood samples with no urine collection can provide a reliable estimate of glomerular filtration rate (GFR) in adults, but has not been well studied in children. The disposition of Tc-99m DTPA was determined in 17 children with cancer, and the influence of binding and study design on the estimates for serum clearance were evaluated. Nine blood samples were obtained over 6 hours in each patient, and serum was assayed for total and free Tc-99m. Free Tc-99m DTPA was determined by ultrafiltration. Estimates of clearance derived from a two-compartment model for ultrafiltrable Tc-99m DTPA were determined from all nine measured concentrations, and these results served as a reference value for GFR in each subject. Total Tc-99m DTPA concentrations also were best described by a two-compartment model, but the median total clearance (35 ml/min) was significantly (p0.01) lower than the ultrafiltrate clearance (58 ml/min). The effect of a simplified sampling schedule was assessed from clearance estimates based on a 3-point subset of the ultrafiltrable data. The median clearance of 69 ml/minute was significantly higher (p0.01) than the reference GFR. However with a correction factor included to account for the positive bias arising from the limited sampling schedule, the reference estimates for GFR were well predicted (r2 = 0.99) with no significant bias. Ultrafiltrable Tc-99m DTPA serum clearance using a limited sampling schedule can provide a consistent and clinically feasible estimate of glomerular filtration rate in children, but binding in serum and study design are potentially important confounding factors.
