Background and purpose: Little is known on the use of metal artifact reduction sequence (MARS) MRI to diagnose osteonecrosis of the femoral head (ONFH) after fixation of femoral neck fractures (FNF) with conventional metal implants present. We compared MARS MRI with radiography in diagnosing ONFH. Secondarily, we determined whether signs of ONFH on MARS MRI correlate with patient-reported outcomes (PROs) via Oxford Hip Score (OHS) and pain (VAS).Patients and methods: 30 adults under 60 years treated with internal fixation after FNF were prospectively included (2015–2018) at 2 hospitals. They were followed up with radiography and PROs at 4, 12, and 24 months and MARS MRI at 4 and 12 months. OHS < 34 or VAS pain > 20 was considered significant.Results: At 12 months, 14 patients had a pathological MRI. 3 of 14 had ONFH on radiographs at 12 months, increasing to 5 at 24 months, and 4 had unfavorable PROs. 2 of 5 patients with ONFH signs on both MRI and radiography had unfavorable PROs. 1 of 10 patients with normal MRI and radiography had unfavorable 2-year PROs. 4 patients had inconsistent MRI results, of which 1 developed ONFH. 1 patient dropped out.Conclusion: Information from a pathological MRI was not useful, as a majority remained free from symptoms and ONFH signs on radiographs. Furthermore, PROs did not correlate with imaging results. MARS MRI findings must be better understood before being taken into clinical practice. However, a normal MARS MRI seems to be a good prognostic finding.

In recruitment, promotion, admission, and other forms of wealth and power apportion, an evaluator typically ranks a set of candidates in terms of their competence. If the evaluator is prejudiced, the resulting ranking will misrepresent the candidates’ actual rankings. This constitutes not only a moral and a practical problem, but also an epistemological one, which begs the question of what we should do—epistemologically—to mitigate it. In a recent paper, Jönsson and Sjödahl in [Episteme 14(4):499–517, 2017], argue that the epistemic problem can be fruitfully addressed by way of a novel statistical method that changes the products of biased behaviour, i.e. the rankings themselves, rather than the biased persons. Jönsson and Sjödahl’s pioneering proposal is a both a welcome addition to the literature on implicit bias, due the problems with existing implicit bias interventions [see e.g. Lai et al. in J Exp Psychol Gen 143:1765–1785; J Exp Psychol Gen 145(8):1001–1016, 2014; 2016; Forscher et al. in J Person Soc Psychol 117(3):522–559, 2019] but also to the literature on prejudice more generally, where many proposed prejudice-reduction strategies enjoy less than adequate empirical support [Paluck and Green in Ann Rev Psychol 60(1):339–367, 2009]. Their proposal, however, needs supplementation in two ways: the circumstances that must hold in order for it to work needs to be refined, and their claim that it works as intended in these circumstances needs to be validated. We argue that four of Jönsson and Sjödahl’s method’s presumed presuppositions can be weakened, but needs to be supplemented by two additional assumptions, overlooked by Jönsson and Sjödahl. Moreover, we demonstrate that the method does work as intended by way of a statistical simulation.

Background: Reliable, widely accessible and affordable biomarkers for predicting Alzheimer’s disease (AD) brain pathology status are a necessity to aid development of prevention strategies in cognitively healthy at-risk older adults, at the right timepoint. Measurements of the key neuropathological hallmark beta-amyloid (Aβ) by PET neuroimaging or cerebrospinal fluid measures reflect its accumulation in the brain, yet recent methodological advancements now enable blood-based measures reflecting cerebral amyloid burden. Objectives: The current study validated the capacity of plasma Aβ42/Aβ40 measured using six different assays to predict amyloid positivity in a subgroup of cognitively unimpaired (CU) participants in the ADNI study and assessed its ability to discriminate CU from AD cases. We also explored economic viability of using two different plasma amyloid assays for pre-screening in AD prevention trials and as routine clinical diagnostic tool, versus amyloid PET alone. Design: A cross-sectional analysis of plasma and brain amyloid data, including comparative cost analysis of the plasma biomarkers in relation to brain amyloid PET. Setting: Alzheimer’s Disease Neuroimaging Initiative (ADNI). Participants: ADNI participants consisting of 115 CU, mild cognitive impairment and AD cases who had plasma Aβ42/Aβ40 measured with six platforms. Measurements: Plasma Aβ42/Aβ40 was measured via six different platforms: three immunoassays (Roche, Quanterix and ADx Neurosciences) and three mass spectrometry (MS) based assays (WashU, Shimadzu and Gothenburg). Aβ-PET imaging was conducted within three months of plasma sampling using [18F]florbetapir. Results: There was a weak to moderate correlation of plasma Aβ42/Aβ40 ratio between platforms. The MS-based WashU test had the highest capacity to discriminate between CU and AD (area under the curve, AUC = 0.734, 95% CI: 0.613-0.854; P = 0.008). Within the CU group, the WashU plasma amyloid test had the best discriminative capacity to distinguish Aβ+ from Aβ– (AUC = 0.753, 95% CI: 0.601-0.905; P = 0.003) closely followed by the immunoassay from Roche (AUC = 0.737, 95% CI: 0.597-0.877; P = 0.006). The exploratory economic analyses showed that the use of Roche or WashU plasma amyloid assay as a pre-screening tool prior to Aβ-PET scans for clinical trial recruitment significantly reduced total screening cost (saving up to $5882 per recruited patient) expected in an AD prevention trial. Conclusions: With few available treatment strategies, dementia prevention is a global priority. CU individuals at risk for AD are the target population for dementia prevention but have been poorly studied. Our findings confirming diagnostic value of ultrasensitive immunoassays and high-performance immunoprecipitation coupled with MS for measurement of plasma Aβ42/Aβ40 to detect PET amyloid positivity in CU participants allude to potential clinical utility of this biomarker. Plasma Aβ42/Aβ40 could be optimal for pre-selecting at-risk candidates for more invasive and expensive investigations across AD prevention clinical trials and clinical care for a rapidly ageing population.

This constitutes not only a moral and a practical problem, but also an epistemological one, which begs the question of what we should do – epistemologically – to mitigate it. The article is an attempt to begin to answer this question. I first explore the presuppositions that must obtain for individual interventions to likely yield positive epistemological effects in ranking situations. I then compare these with the corresponding presuppositions of a novel, ‘post hoc’ approach to deprejudicing due to Jönsson and Sjödahl (Episteme 14(4):499–517, 2017), which does not attempt to change evaluators but attempts to increase the veracity of the rankings they produce after the fact (but before the rankings give rise to discriminatory effects) using statistical methods. With these two sets of presuppositions in place, I describe the limitations imposed by each presupposition on its intervention, compare presuppositions across the two kinds of interventions, and conclude that the two kinds of interventions importantly complement each other by having fairly disjoint, but non–conflicting, presuppositions. The post hoc intervention can thus complement an individual intervention (and vice versa) in situations where both are applicable (by adding further increases in veracity), but also by applying to situations where that intervention is not applicable (and thereby increase veracity in situations beyond the reach of that intervention).

Undersökningen motiveras av studier av meritvärdering inom andra områden som påvisar att just sammanvägningen av meriter tenderar att vara fördomsfull. För att konkretisera diskussionen så utgår texten från regler och praxis vid antagning till forskarutbildning vid Humanistiska och teologiska fakulteterna vid Lunds universitet.

The violation of baryon number, B , is an essential ingredient for the preferential creation of matter over antimatter needed to account for the observed baryon asymmetry in the Universe. However, such a process has yet to be experimentally observed. The HIBEAM/NNBAR program is a proposed two-stage experiment at the European Spallation Source to search for baryon number violation. The program will include high-sensitivity searches for processes that violate baryon number by one or two units: free neutron–antineutron oscillation ( n → n ̄ ) via mixing, neutron–antineutron oscillation via regeneration from a sterile neutron state ( n → [ n ′ , n ̄ ′ ] → n ̄ ), and neutron disappearance (n → n′); the effective Δ B = 0 process of neutron regeneration ( n → [ n ′ , n ̄ ′ ] → n ) is also possible. The program can be used to discover and characterize mixing in the neutron, antineutron and sterile neutron sectors. The experiment addresses topical open questions such as the origins of baryogenesis and the nature of dark matter, and is sensitive to scales of new physics substantially in excess of those available at colliders. A goal of the program is to open a discovery window to neutron conversion probabilities (sensitivities) by up to three orders of magnitude compared with previous searches. The opportunity to make such a leap in sensitivity tests should not be squandered. The experiment pulls together a diverse international team of physicists from the particle (collider and low energy) and nuclear physics communities, while also including specialists in neutronics and magnetics.

Animal welfare is an important aspect of organic broiler (OB) production which involves e.g. up to twice the rearing period compared with conventional production and feedstuffs without prophylactic additives or synthetic amino acids. As market demand for organic broiler meat increases, the formerly modest OB production in Sweden is now rapidly increasing and research is needed to support this development. It is essential to use genotypes that adapt well to the rearing environment, in order to achieve satisfactory animal welfare. Health problems related to fast growth rate are a common animal welfare issue in modern broilers and this applies to systems with long rearing periods. In this study, the Welfare Quality® assessment protocol for poultry was used for assessment of lameness, contact dermatitis, cleanliness, thermal comfort, litter quality and the human-animal relationship in two modern broiler genotypes; the fast-growing Ross 308 (R) and the slower-growing Rowan Ranger (RR). In total, 645 day-old chicks (328 R and 317 RR) were reared until 10 weeks of age, which is the rearing period formerly used in organic production in Sweden, and fed a low-protein (L), high-protein (H) or mussel-meal (M) diet containing 14.5%, 17.0% or 15.6% crude protein, respectively. Broiler welfare was assessed on three occasions (at weeks 2, 6 and 9). The results showed rapid deterioration in welfare for fast-growing broilers when kept beyond 6 weeks. Mortality rate and incidence of lameness and contact dermatitis increased and litter quality, thermal comfort and plumage cleanliness decreased. Indications of poor welfare were also observed in the slower-growing hybrid, but to a lesser extent and later during rearing. Diet type only had minor effects on bird welfare, although R birds grew faster on the M diet. Thus the slower-growing RR hybrid is preferable to the fast-growing broiler type in production systems with a long rearing period. However, the RR growth rate can be regarded as moderate and, to avoid health problems related to fast growth rate, hybrids that grow even more slowly should be considered for OB production systems with a long rearing period.

Perching is important for broiler welfare, however, hampered by body weight. In a mixed flock, resting behaviour and location were compared between three broiler hybrids (Rowan Ranger, Hubbard CYJA...

In spite of our good intentions and explicit egalitarian convictions, we habitually disfavor the underprivileged. The rapidly growing literature on implicit bias – unconscious, automatic tendencies to associate negative traits with members of particular social groups – points towards explanations of this dissonance, although rarely towards generalizable solutions. In a recent paper, Jennifer Saul (2013) draws attention to the alarming epistemological problems that implicit bias carries with it; since our judgments about each other are likely influenced by implicit bias, we have good reason to doubt their veracity. In this paper we explore a novel way to come to terms with the epistemological problem as it manifests itself in ranking situations, i.e. how we can know that the way in which we have ranked a group of people for a certain position, reflects their actual competence (or the best estimate of their competence given the evidence). On our approach, rather than attempting to make people less biased, we suggest that biased behavior can sometimes be corrected after the fact. In particular, the veracity of rankings can sometimes be improved by modifying the rankings directly. We investigate three methods that modify biased rankings, and argue that the last of these solves the epistemological problem that we are concerned with.

When estimated volatilities are not in perfect agreement with reality, delta-hedged option portfolios will incur a non-zero profit-and-loss over time. However, there is a surprisingly simple formula for the resulting hedge error, which has been known since the late 1990s. We call this The Fundamental Theorem of Derivative Trading. This paper is a survey with twists on that result. We prove a more general version of it and discuss various extensions and applications, from incorporating a multi-dimensional jump framework to deriving the Dupire–Gyongy–Derman–Kani formula. We also consider its practical consequences, both in simulation experiments and on empirical data, thus demonstrating the benefits of hedging with implied volatility.

This study compared behavioural time budgets, presence of comfort behaviours and social behaviours in two different broiler genotypes (the fast-growing Ross 308 (R) and the slower-growing Rowan Ranger (RR)) fed organic diets with high (17.0% crude protein (CP)) or low (14.5% CP) protein content during a 10-week rearing period. 429-day-old chicks (218 R and 211 RR, respectively) were included in the study and behaviour was recorded at 2, 6 and 9 weeks of age. The results showed no effect of diet treatments but that R broilers were less active and sat, ate and drank more frequently than RR broilers, which stood and perched more frequently. However, both hybrids showed decreasing activity and foraging behaviour with increasing age, while time spent eating and sleeping was approximately similar over the entire rearing period.

Genetic evaluations of sport performance typically consider competition records of ranking points in each competition, accumulated lifetime points or annual earnings. Repeated observations have the advantage of allowing for adjustment of effects associated with each competition such as rider experience, judge and competing horses, but also demands more computer capacity than single-trait records, which could prohibit multiple-trait evaluations. The aim of the study was to compare CPU times, estimated breeding values (EBVs), reliabilities and model prediction abilities when modelling repeated competition ranking points (run A), mean ranking points (runs B and C), mean ranking points precorrected for effects associated with each competition (run D) and accumulated lifetime points (run E) for Danish Warmblood horses. CPU times for run A were 632–776 times (show jumping) and 59–96 times (dressage) as high as for runs B–E. EBVs of run D were perfectly correlated (1.00) with those of run A. Reliabilities were highest in runs E and A. Best model prediction ability and least bias were found in run C (dressage) and run E (show jumping), but the best choice in each discipline was not preferable for the other. Run D was the second best in both disciplines (D), and is expected to increase in performance over time as omission of a relatively large amount of historic data becomes less important.

There has been much interest in group judgment and the so-called ‘wisdom of crowds’. In many real world contexts, members of groups not only share a dependence on external sources of information, but they also communicate with one another, thus introducing correlations among their responses that can diminish collective accuracy. This has long been known, but it has—to date—not been examined to what extent different kinds of communication networks may give rise to systematically different effects on accuracy. We argue that equations that relate group accuracy, individual accuracy, and group diversity (see Hogarth, 1978; Page, 2007) are useful theoretical tools for understanding group performance in the context of research on group structure. In particular, these equations may serve to identify the kind of group structures that improve individual accuracy without thereby excessively diminishing diversity so that the net positive effect is an improvement even on the level of collective accuracy. Two experiments are reported where two structures (the complete network and a small world network) are investigated from this perspective. It is demonstrated that the more constrained network (the small world network) outperforms the network with a free flow of information.

This paper raises a principled objection against the idea that Bayesian confirmation theory can be used to explain the conjunction fallacy. The paper demonstrates that confirmation-based explanations are limited in scope and can only be applied to cases of the fallacy of a certain restricted kind. In particular; confirmation-based explanations cannot account for the inverse conjunction fallacy, a more recently discovered form of the conjunction fallacy. Once the problem has been set out, the paper explores four different ways for the confirmation theorist to come to terms with the problem, and argues that none of them are successful.

Process-reliabilist analyses of justification and knowledge face the generality problem. Recent discussion of this problem turns on certain untested empirical assumptions that this paper investigates. Three experiments are reported: two are free-naming studies that support the existence of a basic level in the previously unexplored domain of names for belief-forming processes; the third demonstrates that reliability judgments for the basic-level belief-forming process types are very strongly correlated with the corresponding justification and knowledge judgments. I argue that these results lend support to process-reliabilism; I then go on to explore several different ways to make use of them in solutions to the generality problem.

This paper has two goals. First, it aims to investigate the empirical assumptions of a recent proposal due to Olsson (forthcoming), according to which the generality problem for process-reliabilism can be approached by recruiting patterns and models from the basic-level research in cognitive psychology. Second, the paper attempts to generalize findings in the basic-level literature pertaining to concrete nouns to the abstract verbs that denote belief-forming processes. I will demonstrate that verbs for belief-forming processes exhibit the kind of linguistic convergence that is characteristic of basic-level words, although these words are not neatly taxonomically organized or associated with large feature sets. Next, I will evaluate and partially validate Olsson's proposal in light of these findings. Organic poultry meat production in Sweden is characterized by diets based on organic and locally produced feedstuffs, roughage allowance, a long rearing period (>10 weeks), outdoor access for the birds, and ban of synthetic amino acids and prophylactic use of antibiotics and anthelmintic. Until recently, the main hybrids used in organic broiler chicken production in Sweden were the same as used in conventional broiler chicken production. These birds are bred to reach market weight at 5 to 6 weeks on a high-protein quality nonorganic diet. In this study, organic high- and low- protein diets were fed to fast- and slower-growing hybrid broiler chickens with 5 pens per genotype × diet treatment and 21 to 22 broiler chickens per pen. Weight gain, feed conversion, health, and mortality were recorded during a 71-day rearing period. The fast-growing hybrid birds grew faster (55.4 ± 0.7 versus 38.3 ± 0.5 g/day, P = 0.001) and had a better feed conversion ratio (2.6 ± 0.1 versus 2.9 ± 0.1 kg feed/kg body weight, P = 0.001) than the slower-growing hybrid birds. The fast-growing hybrid birds grew slower when fed the high-protein compared to the low-protein diet (55.3 ± 0.5 versus 57.8 ± 1.4 g/day, P = 0.050), while the slower- growing hybrid birds grew faster when fed the high-protein compared to the low-protein diet (39.4 ± 0.7 and 37.2 ± 0.4 g/day, P = 0.038). A higher proportion of fast- compared to slower-growing hybrid birds had sticky droppings at 1 week of age (18.8 ± 1.6 versus 3.8 ± 1.6%, P = 0.001) and were culled because of leg weakness during the 10-week rearing period (10.0 ± 2.0 versus 3.3 ± 2.0%, P = 0.031), indicating poorer welfare among fast-growing hybrid birds when reared for 10 weeks on organic diets.

The results of a measurement of the proton structure function F_2(x,Q^2)and the virtual photon-proton cross section are reported for momentum transfers squared Q^2 between 0.35 GeV^2 and 3.5 GeV^2 and for Bjorken-x values down to 6 10^{-6} using data collected by the HERA experiment H1 in 1995. The data represent an increase in kinematic reach to lower x and Q^2 values of about a factor of 5 compared to previous H1 measurements. Including measurements from fixed target experiments the rise of F_2 with decreasing x is found to be less steep for the lowest Q^2 values measured. We reply to Christoph Jager's criticism of the conditional probability solution (CPS) to the value problem for reliabilism due to Goldman and Olsson (2009). We argue that while Jager raises some legitimate concerns about the compatibility of CPS with externalist epistemology, his objections do not in the end reduce the plausibility of that solution.

Summary Reasons for performing study: Disturbances in skeletal development, primarily osteochondrosis (OC) and palmar/plantar osseous fragments (POF), have been commonly reported as problems in young horses. However, there are few reports of such findings for epidemiological analyses or breeding purposes. Objectives: To evaluate equine hospital data as a possible source of information for genetic evaluations by estimating prevalence and heritability of OC in the stifle, hock and fetlock joints and of POF in the fetlock. Methods: Data on Swedish Warmblood (SWB) horses were obtained from a large equine hospital in south Sweden. Prevalences were based on radiographic examinations of 879 screened horses, mainly evaluated as part of a prepurchase examination and 3639 horses with a reported history of orthopaedic problems. For the heritability study the 2 data sources were pooled and 3199 examined horses with pedigree information were considered for the linear animal model analyses. Results: The overall prevalence of OC was 13% (stifle 9%, hock 6% and dorsal osseous fragments in fetlock [DOF] 10%) and POF 10%. The overall heritability of OC was 0.05 on the visible binomial scale. The corresponding heritabilities for OC in the stifle were 0.03, hock 0.08, DOF 0.10 and POF 0.13. These values correspond to heritabilities of 0.09–0.38 on the underlying quantitative scale. Conclusions and potential relevance: Obtained prevalences and heritabilities were in accordance with other studies, supporting the hypothesis that data regularly obtained from equine hospitals may be a valuable source in studies of inherited disorders such as OC and POF. There is a need for more standardised documentation of diagnoses and consistent recording of identity of examined horses using passports or breed databases. Compilation of results from major clinics is desired in order to cover most progenies of stallions used in a region or nation.

The first limiting nutrients in typical laying hen diets are the sulphur-containing amino acids and, in particular, methionine. To fulfil the birds' recommended requirement, conventional diets are supplemented with synthetic methionine. As this is not allowed in organic production it becomes very important to have access to alternative high-quality protein feed ingredients. An experiment was performed to evaluate the possibility to compose a diet with 100% organically approved feed ingredients using mussel meal as a major source of methionine. The experiment included 678 Lohman Selected Leghorn (LSL) and 678 Hyline White, W-98, layers during 20 to 72 weeks of age. There were 12 aviary pens with 113 birds in each. The birds were fed one of the two experimental diets containing either 3.5% or 7% dried mussel meat meal or a commercial organic diet from a Swedish feed manufacturer for comparison. Production and mortality were recorded daily per group, and egg weight was recorded once weekly. At 33, 55 and 70 weeks, 10 eggs from each treatment group were collected and analysed for internal egg quality. Diets had no significant effect on laying percentage, egg mass, feed intake, feed conversion ratio, mortality, bird live weight or proportion misplaced, cracked or dirty eggs. Egg quality, that is, shell deformation, shell breaking strength, albumen height, shell percentage and proportion of blood and meat spots were also unaffected. There was a significant difference in egg yolk pigmentation, that is, the egg yolk was more coloured when feeding 7% mussel meal compared with the other diets. Hyline hens had lower feed intake and laying percentage, and higher egg weight, but lower egg mass production than LSL birds. The age of the birds influenced all egg quality traits except for meat and blood spots. The dry matter of the excreta was significantly lower for both genotypes fed the 7% mussel meal diet. These results indicate that mussels may be a high-quality protein source and may replace fishmeal in organic diets for layers.

Das Verhalten des Farbstoffes konnte visuell verfolgt werden. Der ubertritt erfolgte hauptsachlich in der kaudalen Darmhalfte und sistierte ein Tag nach der Geburt. Auch bei alteren Ferkeln konnte zwar eine Aufnahme von Farbstoff festgestellt werden, dieser wurde jedoch in der Darmschleimhaut festgehalten, d. h. ein eigentlicher Transfer blieb aus. Es wird angenommen, das die Makromolekulabsorption in zwei Schritten vor sich geht, wobei die Molekule vorerst durch Pinozytose in die Zellen aufgenommen und dann durch die Zelle in die Lymphgefase transferiert werden. Aunque tambien en lechones mayores se pudo comprobar la incorporacion del colorante, este era retenido en la mucosa intestinal, es decir, no se efectuaba un traspaso verdadero. Se supone que la absorcion intestinal de macromoleculas es un mecanismo de dos pasos; por lo pronto, las moleculas pasan por pinocitosis a las celulas y luego son transferidas a traves de las celulas a los vasos linfaticos.

The inclusive production of D*(+/-)(2010) mesons in deep-inelastic e(+/-)p scattering is measured in the kinematic region of photon virtuality 100 1 5 GeV The data were collected by the H1 experiment during the period from 2004 to 2007 and correspond to an integrated luminosity of 351 pb(-1) The charm contribution. Summary Blue mussels (Mytilus edulis) are a potential protein source in poultry feeds because of their high methionine and cystein content. However, mussels may temporarily be contaminated with the toxin, okadaic acid. The aim of this small scale study was to evaluate the effects on animal health and morphology of the digestive tract of laying hens fed three different diets; a commercial feed and diets containing 15% normal or 15% toxic mussel meal, i.e. a concentration of 198.6 μg toxin per kg feed. Twelve laying hens were divided into six groups and fed the experimental diets for 8 weeks. Animal health, production performance and egg quality were recorded. At sacrifice, tissue samples were prepared for histological evaluation using light microscopy. Okadaic acid did not have any adverse effects on animal health or production parameters and no histological changes indicating disturbances in the digestive tract was observed. Internal egg quality was improved as eggs from hens fed either of the two mussel meal diets had an increased yolk colour. These results show that mussel meal functions as a novel protein source for laying hens and those toxic mussels at this level may be included in the feed without negative effects on parameters evaluated in this study.

The degree of myocardial cell necroses in pigs exposed to stress induced by a myorelaxant (succinylcholine) for about 12 min. was investigated. The β-adrenoceptor blocker propranolol was given in varying doses prior to the induced stress. The degree of necroses in these pigs was evaluated according to a point scale, ranging from 0 to 6 points, and compared with the changes found in untreated controls following stress. The blood levels of noradrenaline and adrenaline were assayed before, during, and immediately after the period of stress. No protective effect of propranolol on the cardiac lesions was found when the drug was administered intravenously in a single dose of either 1 mg or 3 mg per kg. During the period of stress the blood levels of noradrenaline were high, indicating a high degree of activity in the sympatho-adrenal system. Propranolol in a single dose of 10 mg per kg intravenously reduced the number and size of heart lesions, while the blood levels of the catecholamines were lower under stress than after administration of 1 and 3 mg propranolol. After oral treatment with propranolol, 120 mg three times a day for 6 or 7 days, an almost complete absence of heart cell necrosis after the stress was obtained. The activity of the sympatho-adrenal system was elevated, as reflected by the high blood levels of catecholamines. The results indicate that stress-induced heart cell necrosis is mediated via the activity of the sympatho-adrenal system. The difference between a single and repeated doses of propranolol in protecting the heart suggests a complex ation of the adrenergic β-receptor blocker.

Esophageal atresia (EA) is a rare malformation, which requires surgical treatment. Survival rates today reach 95%, but EA remains a significant cause of chronic morbidity with increased risk of psychosocial problems and impaired health-related quality of life (HRQOL). No study of coping strategies of children with EA has been reported in the literature to date, but increased knowledge could lead to improved outcomes and better HRQOL.Standardized focus groups with children with EA and their parents were conducted to identify issues related to health care needs and HRQOL, with group members relating their coping experiences. Identified coping statements were content analysed using a card sorting procedure and descriptive statistics.Thirty families (18 children 8-17 years; 32 parents of children with EA 2-17 years) participated in 10 focus groups. A total of 590 coping statements were recorded. Nine coping strategies were identified: problem solving (n = 116), avoidance (n = 95), recognizing responsibility (n = 71), confronting (n = 70), seeking social support (n = 63), positive reappraisal (n = 58), emotional expression (n = 46), acceptance (n = 40) and distancing (n = 31). Nine situational contexts were identified: nutritional intake (n = 227), communication of one's health condition (n = 78), self-perception when experiencing troublesome symptoms (n = 59), appearance of body or scar(s) (n = 57), physical activities like sport and play (n = 43), sleep (n = 34), hospital care (n = 33), stigmatization and social exclusion (n = 30) and medication intake (n = 29).Focus group methodology contributed to an increased understanding of disease-specific coping processes among children and adolescence with EA. Findings illustrate that they use several coping strategies, some of which they seem to adopt at early age and use in disease-related contexts of physical, social and emotional character. Such coping may influence health and HRQOL in children with EA. In view of the importance of establishing good coping strategies early in life, health care professionals should integrate coping aspects into care management. Future studies are warranted.

If people believe that some property is true of all members of a class such as sofas, then they should also believe that the same property is true of all members of a conjunctively defined subset of that class such as uncomfortable handmade sofas. A series of experiments demonstrated a failure to observe this constraint, leading to what is termed the inverse conjunction fallacy. Not only did people often express a belief in the more general statement but not in the more specific, but also when they accepted both beliefs, they were inclined to give greater confidence to the more general. It is argued that this effect underlies a number of other demonstrations of fallacious reasoning, particularly in category-based induction. Alternative accounts of the phenomenon are evaluated, and it is concluded that the effect is best interpreted in terms of intensional reasoning [Tversky, A., & Kahneman, D. (1983). Patients who survive a first stroke are often left with permanent disabilities, and have significant needs for rehabilitation and long-term care. Antihypertensive treatment reduces the risk of cardiovascular events such as stroke. The purpose of this study was to investigate the cost-effectiveness of candesartan-based antihypertensive treatment for the prevention of nonfatal stroke. The cost-effectiveness analysis was based on data from Study on COgnition and Prognosis in the Elderly (SCOPE), where patients were randomly assigned to receive the angiotensin receptor blocker candesartan or placebo, with open-label active antihypertensive treatment added as needed. The analysis was carried out using a Markov model, which combined clinical and resource utilization data from SCOPE with Swedish retail prices for drugs and unit costs for in-patient stays, and outpatient visits. The cost per patient was 1949 EUR in the candesartan group and 1578 EUR in the control group. The largest share of the cost was attributed to antihypertensive treatment in the candesartan group and to the long-term cost of stroke in the control group. Candesartan-based antihypertensive treatment was associated with 0.0289 additional quality-adjusted life-years (QALYs) per patient and an incremental cost per QALY gained of approximately 13,000 EUR. Sensitivity analyses showed that these results were fairly stable. In conclusion, the cost per QALY gained with candesartan-based antihypertensive treatment lies within the range of society's willingness to pay for health gains. The results indicate that candesartan-based antihypertensive treatment is cost-effective for the prevention of nonfatal stroke.

In the past, treatment options for ankylosing spondylitis (AS) have been limited, and the introduction of new treatments such as infliximab will have a noticeable impact on health-care budgets. The objective of this study was therefore to assess the current burden of the disease and estimate the cost-effectiveness of infliximab treatments.A cross-sectional retrospective observational study of resource consumption and utility related to disease severity was performed in patients who had participated in a population survey between 1992 and 1994 at the University of Bath and patients regularly followed at the Royal National Hospital for Rheumatic Diseases in Bath for up to 9 years. Mean costs and utility were estimated using a regression model including age, gender, disease duration, disease activity and functional status, and disease development was expressed as annual progression of functional disability. Cost-effectiveness of infliximab was modelled using a 3-month placebo-controlled clinical trial with open 1-yr extension in 70 patients, over a total time frame of 2 yr. In the model, costs and utility controlled for disease severity and age from the observational study were assigned to individual patients. The effect of long-term treatment was evaluated in a hypothetical model over 30 yr.Fifty-seven per cent of patients answered the questionnaires. The mean age was 57 (s.d. 11.2) yr, 74% were male and mean disease duration was 30.2 (11.7) yr. Mean total costs were estimated at pound 6765 (s.d. pound 166). Indirect costs represented 57.9% and non-medical costs such as investments and informal care accounted for 16.5% of total costs. Mean utility was 0.67 (0.21). In the main model, mean costs for untreated patients are estimated at pound 25,128. For the infliximab group, mean costs (excluding treatment) are estimated at pound 17,240, a reduction of 31%. Thus, part of the treatment cost was offset by savings in other resources ( pound 7888), leaving an incremental cost of pound 6214. Treatment increased the number of quality-adjusted live years (QALYs) by 0.175 QALYs, leading to a cost per QALY gained of pound 35,400 for the first year of treatment. When treatment is assumed to continue for the full 2 yr, the cost per QALY is pound 32,800. When infliximab infusions are given every 8 weeks instead of every 6 weeks, the cost per QALY is reduced to pound 17,300. In the long-term model, the cost per QALY is estimated at pound 9600.Non-medical costs and production losses dominate costs in AS, and economic evaluation must therefore adopt a societal perspective. The cost of treatment with infliximab is partly offset by reductions in the cost of the disease and patients' quality of life is increased, leading to a cost per QALY gained in the vicinity of pound 30,000 to pound 40,000 in the short term, but potentially below pound 10,000 in the long term.

Influenza disease is a major cause of morbidity and mortality in the elderly and at-risk adults. Despite the international recommendations for vaccinating those at-risk populations, many countries still have not adopted this immunization program. The objective of the present study is to evaluate the long-term cost-effectiveness of influenza immunization in the elderly and at-risk adult populations, in the case of funding this vaccination program by health care payers. Because vaccination programs have long-term effects, modelling methods are particularly adapted to economic evaluations of immunization programs. Thus, by using the health care payers' perspective, a Markov decision model has been designed for influenza vaccination program. Markov states and Markov cycle length have been defined according to disease epidemiology. The costs and effectiveness of vaccinating elderly and at-risk adults are compared with the costs and effectiveness of not vaccinating those populations. Epidemiological data that may be considered as international will be collected through publications, whereas countries' specific data will be collected locally through a retrospective study. A case study will be presented for an Argentinean Managed Care Organization (MCO). Calculation method will be the Monte Carlo simulations' one and cost-effectiveness ratios will be presented as cost per Life Year Gained and cost per case avoided.
