Objective: To determine the stability of fusion constructs with unilateral pedicle screw (UPS) or bilateral pedicle screw (BPS) fixation with and without an interbody implant using the lateral lumbar interbody (LLIF) approach., Methods: Standard nondestructive flexibility tests were performed on 13 cadaveric lumbar specimens to assess spinal stability of intact specimens and 5 configurations of posterior and interbody instrumentation. Spinal stability was determined as mean range of motion in flexion-extension, lateral bending, and axial rotation. Nonpaired comparisons were made for these 6 conditions: 1) intact; 2) unilateral interbody via the LLIF approach (LLIF construct); 3) unilateral interbody + unilateral pedicle screws (UPS) via the LLIF approach (LLIF + UPS); 4) unilateral interbody + bilateral pedicle screws (BPS) using the LLIF approach (LLIF+BPS); 5) UPS alone; and 6) BPS alone., Results: UPS and BPS, with and without interbody support, significantly reduced range of motion during the majority of directions of loading. BPS alone provided greater stability than UPS alone and LLIF alone in all directions of motion except axial rotation. With interbody support, there was no significant difference in stability between BPS and UPS across all movement directions., Conclusions: These biomechanical results suggest that fixation in the lumbar spine with an interbody support using an LLIF approach with UPS is a promising alternative to BPS. Although BPS provides greater immediate stability compared with UPS, in the presence of a lateral interbody implant, UPS and BPS provide equivalent stability. Background: Occipitocervical junction instability can lead to serious neurological injury or death. Open surgical fixation is often necessary to provide definitive stabilization. However, long-term results are limited to small case series., Objective: To review the causes of occipitocervical instability, discuss the indications for surgical intervention, and evaluate long-term surgical outcomes after occipitocervical fixation., Methods: The charts of all patients undergoing posterior surgical fixation of the occipitocervical junction by the senior author were retrospectively reviewed. A total of 120 consecutive patients were identified for analysis. Patient demographic characteristics, occipitocervical junction pathology, surgical indications, and clinical and radiographic outcomes are reported., Results: The study population consisted of 64 male and 56 female patients with a mean age of 39.9 years (range, 7 months to 88 years). Trauma was the most common cause of instability, occurring in 56 patients (47%). Ninety patients (75%) were treated with screw/rod constructs; wiring was used in 30 patients (25%). The median number of fixated segments was 5 (O-C4). Structural bone grafts were implanted in all patients (100%). Preoperative neurological deficits were present in 83 patients (69%); 91% of those patients improved with surgery. Mean follow-up was 35.1 ± 27.4 months (range, 0-123 months). Two patients died, and 10 were lost to follow-up before the end of the 6-month follow-up period. Fusion was confirmed in 107 patients (89.1%). Study Design: The authors retrospectively reviewed a consecutive series of 231 patients with anterior lumbar interbody fusion (ALIF)., Objective: To determine the correlations among common medical conditions, demographics, and the natural history of lumbar surgery with outcomes of ALIF., Summary of Background Data: Multiple spinal disorders are treated with ALIF with excellent success rates. Nonetheless, adverse outcomes and complications related to patients' overall demographics, comorbidities, or cigarette smoking have been reported., Methods: The age, sex, body mass index (BMI), comorbidities, history of smoking or previous lumbar surgery, operative parameters, and complications of 231 patients who underwent ALIF were analyzed. Regression analyses of all variables with complications and surgical outcomes based on total Prolo scores were performed. Two models predicting Prolo outcome score were generated. The first model used BMI and sex interaction, whereas the second model used sex, level of surgery, presence of diabetes mellitus, and BMI as variables., Results: At follow-up, the rate of successful fusion was 99%. The overall complication rate was 13.8%, 1.8% of which occurred intraoperatively and 12% during follow-up. The incidence of complications failed to correlate with demographics, comorbidities, smoking, or previous lumbar surgery (P>0.5). ALIF at T12-L4 was the only factor significantly associated with poor patient outcomes (P=0.024). Both models successfully predicted outcome (P=0.05), although the second model did so only for males., Conclusions: Surgical level of ALIF correlated with poor patient outcomes as measured by Prolo functional scale. BMI emerged as a significant predictor of Prolo total score. Both multivariate models also successfully predicted outcomes. Surgical or follow-up complications were not associated with patients' preoperative status.

OBJECTIVE The rule of Spence is inaccurate for assessing integrity of the transverse atlantal ligament (TAL). Because CT is quick and easy to perform at most trauma centers, the authors propose a novel sequence of obtaining 2 CT scans to improve the diagnosis of TAL impairment. The sensitivity of a new CT-based method for diagnosing a TAL injury in a cadaveric model was assessed. METHODS Ten human cadaveric occipitocervical specimens were mounted horizontally in a supine posture with wooden inserts attached to the back of the skull to maintain a neutral or flexed (10°) posture. Specimens were scanned in neutral and flexed postures in a total of 4 conditions (3 conditions in each specimen): 1) intact (n = 10); either 2A) after a simulated Jefferson fracture with an intact TAL (n = 5) or 2B) after a TAL disruption with no Jefferson fracture (n = 5); and 3) after TAL disruption and a simulated Jefferson fracture (n = 10). The atlantodental interval (ADI) and cross-sectional canal area were measured. RESULTS From the neutral to the flexed posture, ADI increased an average of 2.5% in intact spines, 6.25% after a Jefferson fracture without TAL disruption, 34% after a TAL disruption without fracture, and 25% after TAL disruption with fracture. The increase in ADI was significant with both TAL disruption and TAL disruption and fracture (p < 0.005) but not in the other 2 conditions (p > 0.6). Changes in spinal canal area were not significant (p > 0.70). CONCLUSIONS This novel method was more sensitive than the rule of Spence for evaluating the integrity of the TAL on CT and does not increase the risk of further neurological damage.

OBJECTIVE Craniovertebral junction (CVJ) injuries complicated by transverse atlantal ligament (TAL) disruption often require surgical stabilization. Measurements based on the atlantodental interval (ADI), atlas lateral diameter (ALD1), and axis lateral diameter (ALD2) may help clinicians identify TAL disruption. This study used CT scanning to evaluate the reliability of these measurements and other variants in the clinical setting. METHODS Patients with CVJ injuries treated at the authors' institution between 2004 and 2011 were evaluated retrospectively for demographics, mechanism and location of CVJ injury, classification of injury, treatment, and modified Japanese Orthopaedic Association score at the time of injury and follow-up. The integrity of the TAL was evaluated using MRI. The ADI, ALD1, and ALD2 were measured on CT to identify TAL disruption indirectly. RESULTS Among the 125 patients identified, 40 (32%) had atlas fractures, 59 (47.2%) odontoid fractures, 31 (24.8%) axis fractures, and 4 (3.2%) occipital condyle fractures. TAL disruption was documented on MRI in 11 cases (8.8%). The average ADI for TAL injury was 1.8 mm (range 0.9-3.9 mm). Nine (81.8%) of the 11 patients with TAL injury had an ADI of less than 3 mm. In 10 patients (90.9%) with TAL injury, overhang of the C-1 lateral masses on C-2 was less than 7 mm. ADI, ALD1, ALD2, ALD1 - ALD2, and ALD1/ALD2 did not correlate with the integrity of the TAL. CONCLUSIONS No current measurement method using CT, including the ADI, ALD1, and ALD2 or their differences or ratios, consistently indicates the integrity of the TAL. A more reliable CT-based criterion is needed to diagnose TAL disruption when MRI is unavailable.

Object: The effects of obesity on lumbar biomechanics are not fully understood. The aims of this study were to analyze the biomechanical differences between cadaveric L4-5 lumbar spine segments from a large group of nonobese (body mass index [BMI] < 30 kg/m2) and obese (BMI ≥ 30 kg/m2) donors and to determine if there were any radiological differences between spines from nonobese and obese donors using MR imaging., Methods: A total of 168 intact L4-5 spinal segments (87 males and 81 females) were tested using pure-moment loading, simulating flexion-extension, lateral bending, and axial rotation. Axial compression tests were performed on 38 of the specimens. Sex, age, and BMI were analyzed with biomechanical parameters using 1-way ANOVA, Pearson correlation, and multiple regression analyses. MR images were obtained in 12 specimens (8 from obese and 4 from nonobese donors) using a 3-T MR scanner., Results: The segments from the obese male group allowed significantly greater range of motion (ROM) than those from the nonobese male group during axial rotation (p = 0.018), while there was no difference between segments from obese and nonobese females (p = 0.687). There were no differences in ROM between spines from obese and nonobese donors during flexion-extension or lateral bending for either sex. In the nonobese population, the ROM during axial rotation was significantly greater for females than for males (p = 0.009). There was no significant difference between sexes in the obese population (p = 0.892). Axial compressive stiffness was significantly greater for the obese than the nonobese population for both the female-only group and the entire study group (p < 0.01); however, the difference was nonsignificant in the male population (p = 0.304). Correlation analysis confirmed a significant negative correlation between BMI and resistance to deformation during axial compression in the female group (R = -0.65, p = 0.004), with no relationship in the male group (R = 0.03, p = 0.9). There was also a significant negative correlation between ROM during flexion-extension and BMI for the female group (R = -0.38, p = 0.001), with no relationship for the male group (R = 0.06, p = 0.58). Qualitative analysis using MR imaging indicated greater facet degeneration and a greater incidence of disc herniations in the obese group than in the control group., Conclusions: Based on flexibility and compression tests, lumbar spinal segments from obese versus nonobese donors seem to behave differently, biomechanically, during axial rotation and compression. The differences are more pronounced in women. MR imaging suggests that these differences may be due to greater facet degeneration and an increased amount of disc herniation in the spines from obese individuals.

OBJECT The Universal Clamp Spinal Fixation System (UC) is a novel sublaminar connection for the spine that is currently used in conjunction with pedicle screws at the thoracic levels for the correction of scoliosis. This device allows the surgeon to attach rods and incorporate a pedicle screw construction. The flexible composition of the UC should provide flexibility intermediate to the uninstrumented spine and an all-screw construct. This hypothesis was tested in vitro using nondestructive flexibility testing of human cadaveric spine segments. METHODS Six unembalmed human cadaveric thoracic spine segments from T-3 to T-11 were used. The specimens were tested under the following conditions: 1) intact; 2) after bilateral screws were placed at T4-T10 and interconnected with longitudinal rods; 3) after placement of a hybrid construction with screws at T-4, T-7, and T-10 with an interconnecting rod on one side and screws at T-4 and T-10 with the UC at T5-9 on the contralateral side; (4) after bilateral screws were placed at T-4 and T-10 and interconnected with rods and bilateral UC were placed at T5-9; and 5) after bilateral screws at T-4 and T-10 were placed and interconnected with rods and bilateral sublaminar cables were placed at T5-9. Pure moments of 6.0 Nm were applied while optoelectronically recording 3D angular motion. RESULTS Bilateral UC placement and bilateral sublaminar cables both resulted in a significantly greater range of motion than bilateral pedicle screws during lateral bending and axial rotation, but not during flexion or extension. There were no differences in stability between bilateral UC and bilateral cables. The construct with limited screws on one side and UC contralaterally showed comparable stability to bilateral UC and bilateral cables. CONCLUSIONS These results support using the UC as a therapeutic option for spinal stabilization because it allows comparable stability to the sublaminar cables and provides flexibility intermediate to that of the uninstrumented spine and an all-screw construct. Equivalent stability of the hybrid, bilateral UC, and bilateral cable constructs indicates that 6-level UC provides stability comparable to that of a limited (3-point) pedicle screw-rod construct.

Study Design: Nondestructive flexibility tests were performed in vitro, comparing multiple conditions of fixation in a single group of specimens., Objective: To compare the biomechanical behavior of the lumbar spine in the intact condition, after implanting a novel motion stabilizer, and after implanting a rigid fixator., Summary of Background Data: Two specific scenarios that may benefit from dynamic lumbar stabilization are single-level moderate instability, where the stabilizing tissues are relatively incompetent, and juxta-level to fusion, where the last instrumented level requires intermediate stiffness ("topping off") to prevent transfer of high stresses from the stiffer fusion construct to the intact adjacent levels. Both scenarios were evaluated in vitro., Methods: Seven human cadaveric L2-S1 segments were tested (1) intact, (2) after moderate destabilization, (3) after 2-level hybrid posterior fixation, consisting of bilateral dynamic pedicle screws at L4 interconnected with rigid rods to standard pedicle screws at L5 and S1, (4) after 2-level rigid fixation, (5) after 1-level (L4-L5) dynamic fixation, and (6) after 1-level rigid fixation. In each condition, angular range of motion (ROM) and sagittal instantaneous axis of rotation (IAR) were assessed., Results: In 1-level constructs, dynamic hardware allowed 104% of intact ROM, whereas rigid hardware allowed 49% of intact ROM. Relative to the intact, the IAR was shifted significantly farther posterior by rigid 1-level instrumentation than by dynamic 1-level instrumentation. In 2-level constructs, the dynamic level allowed significantly greater ROM than the rigid level in all directions but allowed significantly less ROM than the intact level in all directions except axial rotation., Conclusion: Dynamic instrumentation shifted the IAR less than rigid instrumentation, providing more favorable kinematics. This dynamic stabilizer provided 1-level ROM that was close to intact ROM during all loading modes in vitro. In the topping-off construct, the dynamic segment allowed intermediate ROM to give balanced transitional flexibility., Level of Evidence: N/A.

Over the past 120 years, spinal stabilization has advanced immensely. An updated review highlighting these advancements has not been performed in the past 20 years. The objective of this report is to provide a historical assessment of the decades outlining various key innovators, their techniques, and instrumentation. It is important to provide new generations of surgeons and students with historical evidence of the value of developing new techniques and instrumentation to improve patient care and outcomes.

Introduction: Sacroiliac (SI) joint pain has become a recognized factor in low back pain. The purpose of this study was to investigate the effect of a minimally invasive surgical SI joint fusion procedure on the in vitro biomechanics of the SI joint before and after cyclic loading., Methods: SEVEN CADAVERIC SPECIMENS WERE TESTED UNDER THE FOLLOWING CONDITIONS: intact, posterior ligaments (PL) and pubic symphysis (PS) cut, treated (three implants placed), and after 5,000 cycles of flexion-extension. The range of motion (ROM) in flexion-extension, lateral bending, and axial rotation was determined with an applied 7.5 N · m moment using an optoelectronic system. Results for each ROM were compared using a repeated measures analysis of variance (ANOVA) with a Holm-Šidák post-hoc test., Results: Placement of three fusion devices decreased the flexion-extension ROM. Lateral bending and axial rotation were not significantly altered. All PL/PS cut and post-cyclic ROMs were larger than in the intact condition. The 5,000 cycles of flexion-extension did not lead to a significant increase in any ROMs., Discussion: In the current model, placement of three 7.0 mm iFuse Implants significantly decreased the flexion-extension ROM. Joint ROM was not increased by 5,000 flexion-extension cycles.

Background: Literature about long constructs in the cervicothoracic junction (CTJ) implanted with a minimally invasive surgical technique is practically nonexistent. Our objective is to present a less invasive-minimally invasive (LIS-MIS) surgical technique to stabilize the CTJ., Patients and Methods: A midline cervical short incision was made, three or four level lateral mass screws were inserted bilaterally and rods were placed in a conventional technique (LIS field). Percutaneous screws were placed in the upper thoracic spine, and thoracic rods were threaded subfascially through the pedicle sleeves up to the cervical incision (MIS field). Cervical and thoracic rods were linked with parallel connectors. Two cross-links were used in each case., Results: Two patients (33 and 53 years of age) with instability of the CTJ due to metastases were operated on in this way without attempting bone fusion. Mean duration of surgery was 7.5 hours. No patient required blood transfusion. There were no complications related to surgery or the hardware. Opioid consumption diminished after surgery, and both patients remained ambulatory until decease., Conclusion: This LIS-MIS technique seems feasible to stabilize the CTJ in very selected cases when fusion is not necessary., (Georg Thieme Verlag KG Stuttgart · New York.)

Background: : Endoscopic endonasal approaches to the craniovertebral junction and clivus, which are increasingly performed for ventral skull base pathology, may require disruption of the occipitocondylar joint., Objective: : To study the biomechanical implications at the craniovertebral junction of progressive unilateral condylectomy as would be performed through an endonasal exposure., Methods: : Seven upper cervical human cadaveric specimens (C0-C2) underwent nondestructive biomechanical flexibility testing during flexion-extension, axial rotation, and lateral bending at C0-C1 and C1-C2. Each specimen was tested intact, after an inferior one-third clivectomy, and after stepwise unilateral condylectomy with an anterior approach. Angular range of motion (ROM), lax zone, and stiff zone were determined and compared with the intact state., Results: : At C0-C1, mobility during flexion-extension and axial rotation increased significantly with progressive condylectomy. ROM increased from 14.3 ± 2.7° to 20.4 ± 5.2° during flexion and from 6.7 ± 3.5° to 10.8 ± 3.0° during right axial rotation after 75% condyle resection (P < .01). At C1-C2, condylectomy had less effect, with ROM increasing from 10.7 ± 2.0° to 11.7 ± 2.0° during flexion, 36.9 ± 4.8° to 37.1 ± 5.1° during right axial rotation, and 4.3 ± 1.9° to 4.8 ± 3.3° during right lateral bending (P = NS). Because of marked instability, the 100% condylectomy condition was untestable. Changes in ROM were a result of changes more in the lax zone than in the stiff zone., Conclusion: : Lower-third clivectomy and unilateral anterior condylectomy as would be performed in an endonasal approach cause progressive hypermobility at the craniovertebral junction. On the basis of biomechanical criteria, craniocervical fusion is indicated for patients who undergo > 75% anterior condylectomy.

Study Design: Seven different combinations of posterior screw fixation, with or without interbody support, were compared in vitro using nondestructive flexibility tests., Objective: To study the biomechanical behavior of a new cortical screw (CS) fixation construct relative to the traditional pedicle screw (PS) construct., Summary of Background Data: The CS is an alternative to the PS for posterior fixation of the lumbar spine. The CS trajectory is more sagittally and cranially oriented than the PS, being anchored in the pars interarticularis. Like PS fixation, CS fixation uses interconnecting rods fastened with top-locking connectors. Stability after bilateral CS fixation was compared with stability after bilateral PS fixation in the setting of intact disc and with direct lateral interbody fixation (DLIF) or transforaminal lateral interbody fixation (TLIF) support., Methods: Standard nondestructive flexibility tests were performed in cadaveric lumbar specimens, allowing non-paired comparisons of specific conditions from 28 specimens (4 groups of 7) within a larger experiment of multiple hardware configurations. Condition tested and group from which results originated were as follows: (1) intact (all groups); (2) with L3-L4 bilateral PS-rods (group 1); (3) with bilateral CS-rods (group 2); (4) with DLIF (group 3); (5) with DLIF + CS-rods (group 4); (6) with DLIF + PS-rods (group 3); (7) with TLIF + CS-rods (group 2), and (8) with TLIF + PS-rods (group 2). To assess spinal stability, the mean range of motion, lax zone, and stiff zone at L3-L4 were compared during flexion-extension, lateral bending, and axial rotation., Results: With intact disc, stability was equivalent after PS-rod and CS-rod fixation, except that PS-rod fixation was stiffer during axial rotation. With DLIF support, there was no significant difference in stability between PS-rod and CS-rod fixation. With TLIF support, PS-rod fixation was stiffer than CS-rod fixation during lateral bending., Conclusion: Bilateral CS-rod fixation provided about the same stability in cadaveric specimens as PS-rod fixation regardless of the presence of interbody, TLIF, or DLIF support.

Object: Endoscopic endonasal approaches to the craniovertebral junction (CVJ) and clivus are increasingly performed for ventral skull-base pathology, but the biomechanical implications of these approaches have not been studied. The aim of this study was to investigate the spinal biomechanics of the CVJ after an inferior-third clivectomy and anterior intradural exposure of the foramen magnum as would be performed in an endonasal endoscopic surgical strategy., Methods: Seven upper-cervical human cadaveric specimens (occiput [Oc]-C2) underwent nondestructive biomechanical flexibility testing during flexion-extension, axial rotation, and lateral bending at Oc-C1 and C1-2. Each specimen was tested intact, after an inferior-third clivectomy, and after ligamentous complex dissection simulating a wide intradural exposure using an anterior approach. Angular range of motion (ROM), lax zone, and stiff zone were determined and compared with the intact state., Results: Modest, but statistically significant, hypermobility was observed after inferior-third clivectomy and intradural exposure during flexion-extension and axial rotation at Oc-C1. Angular ROM increased incrementally between 6% and 12% in flexion-extension and axial rotation. These increases were primarily the result of changes in the lax zone. No significant changes were noted at C1-2., Conclusions: Inferior-third clivectomy and an intradural exposure to the ventral CVJ and foramen magnum resulted in hypermobility at Oc-C1 during flexion-extension and axial rotation. Although the results were statistically significant, the modest degree of hypermobility observed compared with other well-characterized CVJ injuries suggests that occipitocervical stabilization may be unnecessary for most patients.

The wide spectrum of symptoms and radiographic findings in patients with Chiari I malformation makes the decision to proceed with intervention controversial. We evaluated symptomatic outcomes using diverse surgical techniques in 104 patients who underwent decompression surgery. The symptoms of most patients improved. Patients with syringomyelia showed less symptomatic improvement; however, syringomyelia was not associated with postoperative symptomatic worsening. Durotomy was performed in 97.1% and arachnoid opening was performed in 60.6% with visualization of the fourth ventricle in 51.9% of patients. Neither arachnoid opening nor fourth ventricle visualization was associated with the clinical outcome. Duraplasty was performed in 94.2% of patients. A Chiari plate was used in 13.4% of patients and was associated with favorable outcomes. Use of postoperative steroids or muscle relaxants was not associated with outcome. Syringomyelia showed a 62.5% improvement rate on postoperative MRI. In conclusion, bony decompression and dural opening are important aspects of Chiari I surgery, with symptomatic improvement observed in most patients., (Copyright © 2012. Published by Elsevier Ltd.)

The field of anatomy, one of the most ancient sciences, first evolved in Egypt. From the Early Dynastic Period (3100 BC) until the time of Galen at the end of the 2nd century ad, Egypt was the center of anatomical knowledge, including neuroanatomy. Knowledge of neuroanatomy first became important so that sacred rituals could be performed by ancient Egyptian embalmers during mummification procedures. Later, neuroanatomy became a science to be studied by wise men at the ancient temple of Memphis. As religious conflicts developed, the study of the human body became restricted. Myths started to replace scientific research, squelching further exploration of the human body until Alexander the Great founded the city of Alexandria. This period witnessed a revolution in the study of anatomy and functional anatomy. Herophilus of Chalcedon, Erasistratus of Chios, Rufus of Ephesus, and Galen of Pergamon were prominent physicians who studied at the medical school of Alexandria and contributed greatly to knowledge about the anatomy of the skull base. After the Royal Library of Alexandria was burned and laws were passed prohibiting human dissections based on religious and cultural factors, knowledge of human skull base anatomy plateaued for almost 1500 years. In this article the authors consider the beginning of this journey, from the earliest descriptions of skull base anatomy to the establishment of basic skull base anatomy in ancient Egypt.

Object: Ossification of the posterior longitudinal ligament (OPLL) is a rare disease that results in progressive myeloradiculopathy related to pathological ossification of the ligament from unknown causes. Although it has long been considered a disease of Asian origin, this disorder is increasingly being recognized in European and North American populations. Herein the authors present demographic, radiographic, and comorbidity data from white patients with diagnosed OPLL as well as the outcomes of surgically treated patients., Methods: Between 1999 and 2010, OPLL was diagnosed in 36 white patients at Barrow Neurological Institute. Patients were divided into 2 groups: a group of 33 patients with cervical OPLL and a group of 3 patients with thoracic or lumbar OPLL. Fifteen of these patients who had received operative treatment were analyzed separately. Imaging analysis focused on signal changes in the spinal cord, mass occupying ratio, signs of dural penetration, spinal levels involved, and subtype of OPLL. Surgical techniques included anterior cervical decompression and fusion with corpectomy, posterior laminectomy with fusion, posterior open-door laminoplasty, and anterior corpectomy combined with posterior laminectomy and fusion. Comorbidities, cigarette smoking, and previous spine surgeries were considered. Neurological function was assessed using a modified Japanese Orthopaedic Association Scale (mJOAS)., Results: A high-intensity signal on T2-weighted MR imaging and a history of cervical spine surgery correlated with worse mJOAS scores. Furthermore, mJOAS scores decreased as the occupying rate of the OPLL mass in the spinal canal increased. On radiographic analysis, the proportion of signs of dural penetration correlated with the OPLL subtype. A high mass occupying ratio of the OPLL was directly associated with the presence of dural penetration and high-intensity signal. In the surgical group, the rate of neurological improvement associated with an anterior approach was 58% compared with 31% for a posterior laminectomy. No complications were associated with any of the 4 types of surgical procedures. In 3 cases, symptoms had worsened at the last follow-up, with only a single case of disease progression. Laminoplasty was the only technique associated with a worse clinical outcome. There were no statistical differences (p > 0.05) between the type of surgical procedure or radiographic presentation and postoperative outcome. There was also no difference between the choice of surgical procedure performed and the number of spinal levels involved with OPLL., Conclusions: Ossification of the posterior longitudinal ligament can no longer be viewed as a disease of the Asian population exclusively. Since OPLL among white populations is being diagnosed more frequently, surgeons must be aware of the most appropriate surgical option. The outcomes of the various surgical treatments among the different populations with OPLL appear similar. Compared with other procedures, however, anterior decompression led to the best neurological outcomes.
