Abstract

Summary: Background: While vaccines have established utility against COVID-19, phase 3 efficacy studies have generally not comprehensively evaluated protection provided by previous infection or hybrid immunity (previous infection plus vaccination). Individual patient data from US government-supported harmonized vaccine trials provide an unprecedented sample population to address this issue. We characterized the protective efficacy of previous SARS-CoV-2 infection and hybrid immunity against COVID-19 early in the pandemic over three-to six-month follow-up and compared with vaccine-associated protection. Methods: In this post-hoc cross-protocol analysis of the Moderna, AstraZeneca, Janssen, and Novavax COVID-19 vaccine clinical trials, we allocated participants into four groups based on previous-infection status at enrolment and treatment: no previous infection/placebo; previous infection/placebo; no previous infection/vaccine; and previous infection/vaccine. The main outcome was RT-PCR-confirmed COVID-19 >7–15 days (per original protocols) after final study injection. We calculated crude and adjusted efficacy measures. Findings: Previous infection/placebo participants had a 92% decreased risk of future COVID-19 compared to no previous infection/placebo participants (overall hazard ratio [HR] ratio: 0.08; 95% CI: 0.05–0.13). Among single-dose Janssen participants, hybrid immunity conferred greater protection than vaccine alone (HR: 0.03; 95% CI: 0.01–0.10). Too few infections were observed to draw statistical inferences comparing hybrid immunity to vaccine alone for other trials. Vaccination, previous infection, and hybrid immunity all provided near-complete protection against severe disease. Interpretation: Previous infection, any hybrid immunity, and two-dose vaccination all provided substantial protection against symptomatic and severe COVID-19 through the early Delta period. Thus, as a surrogate for natural infection, vaccination remains the safest approach to protection. Funding: National Institutes of Health.

Abstract

BackgroundPraziquantel represents the frontline chemotherapy used to treat schistosomiasis, a neglected tropical disease (NTD) caused by infection with macro-parasitic blood fluke schistosomes. While this drug is safe, its inability to kill all schistosome lifecycle stages within the human host often requires repeat treatments. This limitation, amongst others, has led to the search for novel anti-schistosome replacement or combinatorial chemotherapies. Here, we describe a repositioning strategy to assess the anthelmintic activity of epigenetic probes/inhibitors obtained from the Structural Genomics Consortium.Methodology/principle findingsThirty-seven epigenetic probes/inhibitors targeting histone readers, writers and erasers were initially screened against Schistosoma mansoni schistosomula using the high-throughput Roboworm platform. At 10 μM, 14 of these 37 compounds (38%) negatively affected schistosomula motility and phenotype after 72 hours of continuous co-incubation. Subsequent dose-response titrations against schistosomula and adult worms revealed epigenetic probes targeting one reader (NVS-CECR2-1), one writer (LLY-507 and BAY-598) and one eraser (GSK-J4) to be particularly active. As LLY-507/BAY-598 (SMYD2 histone methyltransferase inhibitors) and GSK-J4 (a JMJD3 histone demethylase inhibitor) regulate an epigenetic process (protein methylation) known to be critical for schistosome development, further characterisation of these compounds/putative targets was performed. RNA interference (RNAi) of one putative LLY-507/BAY-598 S. mansoni target (Smp_000700) in adult worms replicated the compound-mediated motility and egg production defects. Furthermore, H3K36me2, a known product catalysed by SMYD2 activity, was also reduced by LLY-507 (25%), BAY-598 (23%) and siSmp_000700 (15%) treatment of adult worms. Oviposition and packaging of vitelline cells into in vitro laid eggs was also significantly affected by GSK-J4 (putative cell permeable prodrug inhibitor of Smp_034000), but not by the related structural analogue GSK-J1 (cell impermeable inhibitor).Conclusion/significanceCollectively, these results provide further support for the development of next-generation drugs targeting schistosome epigenetic pathway components. In particular, the progression of histone methylation/demethylation modulators presents a tractable strategy for anti-schistosomal control.

Abstract

STRUCTURED Summary Background Registered nurses perform numerous functions critical to the success of antimicrobial stewardship but only 63% of pre-registration nursing programmes include any teaching about stewardship. Updated nursing standards highlight nurses require antimicrobial stewardship knowledge and skills. Aim To explore the delivery of key antimicrobial stewardship competencies within updated pre-registration nursing programmes. Method A cross-sectional survey design. Data were collected between March and June 2021. Findings Lecturers from 35 UK universities responsible for teaching antimicrobial stewardship participated. The provision of antimicrobial stewardship teaching and learning was inconsistent across programmes with competencies in infection prevention and control, patient centred care, and interprofessional collaborative practice taking precedent over those pertaining to the use, management, and monitoring of antimicrobials. On-line learning and teaching surrounding hand hygiene, personal protective equipment, and immunisation theory was reported to have increased during the pandemic. Only a small number of respondents reported that students shared taught learning with other healthcare professional groups. Conclusion There is a need to ensure consistency in antimicrobial stewardship across programmes, and greater knowledge pertaining to the use, management and monitoring of antimicrobials should be included. Programmes need to adopt teaching strategies and methods that allow nurses to develop interprofessional skill in order to practice collaboratively.

Abstract

Intestinal helminths are extremely widespread and highly prevalent infections of humans, particularly in rural and poor urban areas of low and middle-income countries. These parasites have chronic and often insidious effects on human health and child development including abdominal problems, anaemia, stunting and wasting. Certain animals play a fundamental role in the transmission of many intestinal helminths to humans. However, the contribution of zoonotic transmission to the overall burden of human intestinal helminth infection and the relative importance of different animal reservoirs remains incomplete. Moreover, control programmes and transmission models for intestinal helminths often do not consider the role of zoonotic reservoirs of infection. Such reservoirs will become increasingly important as control is scaled up and there is a move towards interruption and even elimination of parasite transmission. With a focus on southeast Asia, and the Philippines in particular, this review summarises the major zoonotic intestinal helminths, risk factors for infection and highlights knowledge gaps related to their epidemiology and transmission. Various methodologies are discussed, including parasite genomics, mathematical modelling and socio-economic analysis, that could be employed to improve understanding of intestinal helminth spread, reservoir attribution and the burden associated with infection, as well as assess effectiveness of interventions. For sustainable control and ultimately elimination of intestinal helminths, there is a need to move beyond scheduled mass deworming and to consider animal and environmental reservoirs. A One Health approach to control of intestinal helminths is proposed, integrating interventions targeting humans, animals and the environment, including improved access to water, hygiene and sanitation. This will require coordination and collaboration across different sectors to achieve best health outcomes for all.

Abstract

This paper provides a description and analysis of cranial remains of a rhynchosaur from the Upper Triassic Evangeline Member of the Wolfville Formation of Nova Scotia, Canada. This material, primarily comprising jaw elements, represents the first definitive record of Late Triassic rhynchosaurs from eastern North America. All cranial bones can be assigned to Hyperodapedontinae. The dentition of the Nova Scotian hyperodapedontine differs from those of Hyperodapedon and Isalorhynchus. The maxilla apparently did not have more than two lateral rows and two medial rows of teeth. The teeth of the lateral row closest to the single groove dividing the maxillary tooth plate are more than twice as large as those of the medial row closest to the groove. The lateral and medial tooth-bearing portions of the maxilla become increasingly crest-like during ontogeny. The dentary lacks a lingual row of teeth. Based on this combination of features, the hyperodapedontine material from the Evangeline Member is assigned to a new taxon, Oryctorhynchus bairdi, gen. et sp. nov., which is the sister species to an unnamed hyperodapedontine taxon from Wyoming. This clade was recovered as the sister taxon to Hyperodapedon spp. in the phylogenetic analysis. The late Carnian or earliest Norian tetrapod assemblage from the Evangeline Member represents a mixture of faunal elements from a wide paleolatitudinal range, suggesting a more cosmopolitan distribution for continental tetrapods during the early Late Triassic than previously assumed.

The current study investigated the impact of affordable robotic pet ownership on agitation, quality of life, and depression in older adults with dementia. Data are reported for five adults with dementia receiving day services at an all-inclusive care center for older adults (one male; mean age = 82.8 years). Data were collected three times to obtain baseline (Time 1 to Time 2) and post-intervention (Time 2 to Time 3) scores. Self-reported quality of life and depression symptoms improved post-intervention, but not during baseline. Caregiver-reported agitation did not change consistently across participants during baseline or post-intervention. Simple ownership of affordable robotic pets may be a feasible option to improve quality of life and affective experiences in individuals with dementia. Further work with larger samples should examine whether a combination of ownership and structured activity would produce changes more perceptible to caregivers. Abstract

Teucrium yemense (Defl) (T. yemense), locally known as Reehal Fatima, is a medicinal plant commonly grown in Saudi Arabia and Yemen. Phytochemical investigation of the aerial parts of T. yemense yielded six new neoclerodane diterpenoids, namely fatimanol A-E (1, 2, 3, 5, 6) and fatimanone (4), and the known teulepicephin (7). As both the Teucrium genus and the related Lamiaceae family have previously been widely reported to possess anthelmintic and antimicrobial activities, the structural and biological characterisation of the seven diterpenoids was pursued. The structures of the new compounds were elucidated from their 2D NMR and MS profiles, and by comparison to related compounds. The structure of fatimanol D (5) was confirmed by X-ray crystallographic analysis. The new structures contribute to the breadth of knowledge of secondary metabolites in this genus.

Employers have justifiable legal concerns about on-the-job computer usage by employees. Monitoring of employee computer usage by employers is a method to address these concerns. Federal laws, such as the Electronic Communications Privacy Act, and state laws provide some guidelines on the appropriate monitoring of employee computer usage. Consent to inspect computer usage by an employee may provide additional protection for employers against employee complaints of invasion of privacy. Off-duty employee electronic activity may be addressed by employer-created policies regarding social networking and/or through training programs. No cases have been reported that impose liability on employers for searching employees' computers or email. J Pharm Technol 2010;26:9-13.

Abstract

Dicynodont fossils were first collected in the Luangwa Basin, Zambia, in the 1920s, but limited detailed study and taxonomic uncertainty have obscured their biostratigraphic utility and their implications for topics such as dicynodont biogeography and the effects of the end-Permian extinction. Here we present a comprehensive taxonomic revision of the dicynodonts of the Luangwa Basin, taking into account specimens in all major museum collections and new material collected by our team in 2009. We recognize 14 dicynodont species from the Upper Permian Upper Madumabisa Mudstone: Pristerodon mackayi, Endothiodon sp., Diictodon feliceps, Compsodon helmoedi, Emydops sp., Dicynodontoides cf. D. nowacki, a new tusked cistecephalid, cf. Katumbia parringtoni, Kitchinganomodon crassus, Oudenodon bainii, Odontocyclops whaitsi, Dicynodon huenei, Syops vanhoepeni, and a new lystrosaurid. Previous reports of Lystrosaurus in the basin appear to be in error. In addition, we found no significant partitioning of dicynodont taxa in the northern and southern parts of the basin, despite substantial differences in preservation, indicating the presence of a single faunal assemblage in the Upper Permian. The Madumabisa dicynodont assemblage is best correlated with the Cistecephalus Assemblage Zone of South Africa. The shared presence of Dicynodon huenei and possibly Katumbia in the Luangwa Basin and the Ruhuhu Basin of Tanzania suggests that the Tanzanian Usili Formation also can be correlated with the Cistecephalus zone. Interestingly, the Madumabisa assemblage from Zambia is more similar to the coeval assemblage from South Africa, despite its closer geographic proximity to Tanzania. The Karoo and Ruhuhu basins also include more endemic species in the Permian than the Luangwa Basin. The Middle Triassic Ntawere Formation preserves four dicynodont species (Kannemeyeria lophorhinus, “Kannemeyeria” latirostris, Zambiasaurus submersus, Sangusaurus edentatus), which occur at two stratigraphic levels. The lower Ntawere assemblage resembles that of the Omingonde Formation of Namibia in the presence of Kannemeyeria lophorhinus and potentially Dolichuranus (if “K.” latirostris represents this taxon). The upper Ntawere assemblage shares the genus Sangusaurus with that of the Manda beds of Tanzania and includes the endemic Zambiasaurus. Comparisons of these assemblages to the Omingonde and Manda suggest that both are best correlated with the Cynognathus C subzone. When combined with data on other tetrapod taxa, our revised dicynodont assemblages contribute to an emerging picture of broad faunal similarity in southern and eastern Africa during the Late Permian, and increasing differentiation between the South African and other Karoo basins following the end-Permian extinction.

Abstract

This study examines Problem-focused Coping and Emotion-focused Coping, as measured by the Revised Ways of Coping Checklist, in relation to Dysphoria, Anxiety, Anger, Social Support, and course grades in psychology. Women university students ( N = 245) were assessed two times six weeks apart. Problem-focused Coping was significantly negatively correlated with scores on the Center for Epidemiological Studies–Depression scale and with scores on the Self-rating Anxiety Scale at both time periods. Emotion-focused Coping was significantly positively correlated with scores on these scales and with scores on the State-Trait Anger scale at both times. Emotion-focused Coping was significantly negatively correlated with scores on the Social Support Questionnaire at both times and significantly negatively correlated with grades.

Abstract

The discovery of a Middle to Late Triassic (∼225 to 230 million years old) terrestrial vertebrate fauna from Madagascar is reported. This fauna documents a temporal interval not well represented by continental vertebrate assemblages elsewhere in the world. It contains two new prosauropod dinosaurs, representing some of the earliest dinosaur occurrences known globally. This assemblage provides information about the poorly understood transition to the dinosaur-dominated faunas of the latest Triassic.

Abstract

Peroxides present in non-ionic surfactants used to stabilize certain recombinant protein formulations (e.g. polysorbate 80) can result in the oxidative degradation of proteins. In this study, the ability of various pharmaceutically acceptable antioxidants to prevent the oxidative degradation of two therapeutic proteins, recombinant human Ciliary Neurotrophic Factor (rhCNTF) and recombinant human Nerve Growth Factor (rhNGF), caused by alkyl hydroperoxides and hydrogen peroxide was studied. For rhCNTF, the rank order of effectiveness of the antioxidants tested was: thiols (cysteine, glutathione, thioglycerol)thioethers (methionine). Other parenterally acceptable antioxidants (ascorbic acid, propyl gallate and sodium bisulfite) destabilized the protein. The thiol antioxidants (cysteine and glutathione) were also the most effective antioxidants for rhNGF; however, in contrast to rhCNTF, ascorbic acid did not destabilize rhNGF. The rank order of effective antioxidants for rhNGF was: thiols (cysteine, glutathione)thioethers (methionine)ascorbic acid.

Abstract

The present study examined the link between social support and suicidal ideation among 305 university students (90 men and 215 women) whose ages ranged from 18 to 24 years. Social support was significantly correlated with suicidal ideation ( r = –.38), but social support did not contribute to the variation in suicidal ideation scores beyond the joint contribution of scores on the Beck Depression Inventory and scores on the Hopelessness Scale. A stronger relationship between social support and suicidal ideation might be found if investigators assessed different aspects of social support and if they examined the interaction of social support and level of reported stress.

Abstract

A liposome system was developed which demonstrates suitability as an intravenous drug carrier for a lipophilic drug compound (RS-93522, a dihydropyridine CA2+ channel blocker). An aqueous phospholipid suspension was employed as a nontoxic solubilizing vehicle for this drug. The liposome formulation, composed of a 3% mixture of dioleoylphosphatidylcholine and dioleoylphosphatidylglycerol, produced a physically and chemically stable preparation which solubilized the lipophilic drug compound at a concentration 500 times above its intrinsic aqueous solubility. Characterizing the liposome–drug system by gel filtration chromatography showed that the drug comigrated with the lipid constituents of the liposome. Further in vitro studies established that the liposome–RS-93522 formulation allowed for rapid and complete transfer of the drug from the liposome to bind with albumin when added to human serum. In vivo studies with rats were performed in which the pharmacokinetics of the liposomal–RS-93522 system were compared to those of a cosolvent-solubilized RS-93522 solution. This study showed that the pharmacokinetic profiles of the two solutions were identical. All the evidence indicates that a liposome formulation of this type does not alter the distribution of the drug in serum and is, therefore, not likely to affect the intrinsic pharmacological or toxicological parameters of the drug relative to the conventional solvent/excipient-containing formulation. This liposome system demonstrates utility as a biocompatible, nontoxic drug delivery vehicle.

Claims that young infants fail to react in a social manner to one another and that toys preempt attention to peers were assessed by comparing the interactions observed between infant peers when they met in the presence of toys versus in their absence. 44 pairs of unacquainted infants (either 10--12 or 22--24 months of age) came with their mothers to an unfamiliar room. Without toys available in the room, infants of both ages more often contacted one another, smiled at and gestured to one another, and duplicated each other's actions. With toys, they showed and exchanged toys and spent more time synchronously manipulating similar play material. The results document that infants as young as 10 months of age are responsive to the person and behavior of an unfamiliar peer and that they are no less responsive than older infants to the social versus nonsocial aspects of a novel setting.

Abstract

Medical students in a course that included instruction in patient interviewing participated in an experiment devised to alert them to sources of bias which might influence their judgements and management of patients. Students were randomly assigned to one of four groups and exposed to either a videotaped or audiotaped interview of the same patient presented as either normal weight or overweight. Questionnaire responses of students in the two audio groups indicated no detectable differences in the sound tracks of the overweight and normal weight versions of the interview, and these groups were combined for subsequent analysis. A discriminant analysis indicated that students exposed to the overweight video version formed impressions and assessed patient treatment and outcome differently from those exposed to either the video normal or audio versions of the interview. Implications of these findings for medical education are discussed, and suggestions are made for incorporating such sensitization experiments in the medical curriculum.

Abstract

In the pea straight growth bioassay, both benazolin and indole-3-acetic acid (auxin) solutions promoted the elongation of etiolated pea (Pisum sativumL. ‘Alaska’) epicotyls. Maximal elongation occurred at auxin concentrations ranging from 10-6to 10-4M; comparable elongation occurred at a benazolin concentration of 10-4M. Results of this bioassay indicate that benazolin functions as an auxin, but is much less active than naturally occurring indole-3-acetic acid.

Abstract

Objective: Sexually transmitted infections (STIs), including HIV, are a key contributor to psychological and physical morbidity across the United States (US). African American (AA) women are disproportionately impacted by STIs, particularly in the Deep South of the US. Strong patient-provider communication can help to increase client understanding of STI prevention and treatment options. This study aimed to explore factors influencing effective patient-provider relationships and communication around STIs (including HIV) and sexual health broadly, which will ultimately inform the refinement of a patient-provider communication tool for discussing PrEP as an HIV prevention method., Methods: As part of a larger study focused on PrEP implementation, in-depth, semi-structured qualitative interviews were conducted among cisgender AA women who are PrEP-eligible, both with and without PrEP experience, as well as providers from three federally qualified health centers (FQHCs) and HIV service agencies in Alabama. Data were analyzed using content analysis., Results: A total of 41 participants enrolled, including 21 clients (n = 6 PrEP experienced; n = 15 PrEP naïve) and 20 providers. Qualitative data were organized across the following domains: meaning of health and sexual health, factors influencing the client-provider relationship, and factors influencing sexual health discussions among clients and providers. Key factors influencing client-provider relationships were organized at client, provider, and clinic levels. Factors impacting sexual health discussions between clients and providers were organized at client, provider, and structural levels., Conclusion: These interviews identified key determinants of effective client-provider communication pertaining to sexual health among cisgender AA women in the US South who were receiving care at FQHCs and HIV service organizations. Objective: African American (AA) women in the U.S. South experience significant HIV incidence, and efforts to support antiretroviral pre-exposure prophylaxis (PrEP) uptake and maintenance among this group have been insufficient. This study aimed to explore perceptions, attitudes, and implementation preferences surrounding PrEP use for AA women in the U.S. South., Methods and Measures: The study team conducted qualitative interviews with AA cisgender women clients ( n  = 21) and their providers ( n  = 20) in Federally Qualified Health Centers and HIV clinics in Alabama. The research team employed directed qualitative content analysis to analyze interview data., Results: Five themes emerged: a) inconsistent access to PrEP and PrEP knowledge, b) need for improving low PrEP awareness, c) managing hesitancy to prescribe or use PrEP, d) perceived HIV vulnerability and inherent stigma, and e) normalizing PrEP as part of routine sexual healthcare to increase uptake and maintenance. Interviews revealed an openness towards PrEP as an HIV prevention strategy for AA, cisgender women in Alabama., Conclusion: Improving PrEP uptake and maintenance among AA women in the U.S. South must go beyond increasing awareness to improving PrEP access and trust through visibility of AA women's PrEP use and incorporating PrEP education and services into routine sexual healthcare.

Abstract

Introduction: African American women (AA), particularly those living in the Southeastern USA, experience disproportionately high rates of HIV infection. Pre-exposure prophylaxis (PrEP) is a highly effective HIV prevention tool that may circumvent barriers to traditional HIV prevention tools, such as condom use; however, very little is known about how to improve PrEP access and uptake among AA women who may benefit from PrEP use. This project aims to understand how to increase PrEP access among AA women in the rural Southern USA, which may ultimately affect HIV incidence in this population., Methods and Analysis: The goal of the current study is to systematically adapt a patient-provider communication tool to increase PrEP uptake among AA women receiving care at a federally qualified health centre in Alabama. We will use an iterative implementation process, by assessing the feasibility, acceptability and preliminary impact of the tool on PrEP uptake, using a pilot preintervention/postintervention design (N=125). We will evaluate women's reasons for declining a referral to a PrEP provider, reasons for incomplete referrals, reasons for not initiating PrEP after a successful referral and ongoing PrEP use at 3 and 12 months after PrEP initiation among our sample. The proposed work will significantly contribute to our understanding of factors impacting PrEP uptake and use among AA women, particularly in underserved areas in the Deep South that are heavily impacted by the HIV epidemic and experience worse HIV-related health outcomes relative to other areas in the USA., Ethics and Dissemination: This protocol has been approved by the Institutional Review Board (IRB) at University of Alabama at Birmingham (Birmingham, AL; protocol 300004276).
