Abstract

The effect of food deprivation on hypothalamic neuropetide Y and corticotropin-releasing factor gene expression in the Djungarian hamster was investigated. The study cloned cDNA from the animals to examine the ob gene expression. Results indicated that neuroendocrine consequences of food deprivation in the hamsters were determined by some factor aside from body mass such as the size of adipose tissue reserves. Furthermore, food-deprived hamsters experienced reduced plasma insulin concentrations.

Abstract

The Djungarian hamster is able to adjust to seasons of low food availability and shorter photoperiods by altering its food intake and maintaining lower body weight. The adjustment mechanism for feeding may be mediated by neuropeptide Y, which is found in the hypothalamus. Hamsters that were given only small amounts of food and exposed to short photoperiods were found to increase the production of neuropeptide Y as well as mRNA coding for the protein precursor to neuropeptide Y.

Abstract

The ability of the cholecystokinin (CCK) receptor antagonists, MK-329 and L-365,260, to selectively inhibit 125 I-Bolton-Hunter-CCK8 binding to ligated rat vagus nerve in vitro was examined at concentrations ranging from 10 −10 M to 10 −6 M. Both antagonists inhibited binding to CCK binding sites accumulating proximal to ligatures on the cervical vagus. Incubation of nerve sections in the presence of both antagonists produced an additive effect, indicating that both CCK-A and CCK-B binding sites are transported towards the periphery. In contrast, CCK binding sites accumulating distal to the ligature possessed the pharmacological characteristics of the CCK-B receptor sub-type only.

9. THE BENEFITS OF INPATIENT DIABETES CARE: IMPROVING QUALITY OF CARE AND THE BOTTOM LINE

Abstract

Objective: To analyze the impact of a hospital-wide inpatient diabetes management program on quality of care, length of stay, and cost. Methods: A retrospective review was conducted for diabetes identification and treatment in a large tertiary care hospital in Oklahoma City, Oklahoma. Cultural change was accomplished by educating the healthcare professionals and medical staff. Systems were implemented to better identify patients with diabetes and to focus on quality improvement. Protocols were developed and implemented as tools for improving care. Results were monitored, and quality of care was assessed by examining lengths of stay and financial outcomes. Results: Hospital-wide training, earlier identification of patients with diabetes, and the implementation of inpatient protocols had positive results on both quality of care and the hospital's bottom line. More patients with diabetes were identified earlier in their stays, care was managed effectively to reduce blood glucose levels, lengths of stay were reduced, and coding was improved to more accurately reflect the complexity of care provided. Conclusion: By increasing diabetes awareness hospital-wide and implementing effective identification and management systems, hospitals are well positioned to provide higher-quality diabetes care, which also translates into a positive impact on the bottom line.
