Reconstruction of vermillion defects of the lower lip requires careful consideration of functional and aesthetic aspects. Traditionally, various local flap methods involving tissue advancement from the corner of the mouth, lateral chin, and medial cheek have been commonly employed to fill lower lip defects. However, these approaches have inherent limitations, which include technical complexity, disruption of the orbicularis oris muscle, lip tightening, microstomia, and visible scarring. To overcome these limitations, we employed a free myomucosal composite graft from the lower lip to reconstruct small to medium vermilion defects. Our technique is based on a simple and reproducible surgical approach that facilitates natural volume rearrangement of tissues. Moreover, this method enables precise inset and tension‐free repair, prevents lip tightening, and offers excellent aesthetic outcomes with no vertical scarring and appropriate color matching with surrounding tissues. Although many good techniques have been described, they often have disadvantages such as necrosis and extensive suture lines. In our approach, we aim to minimize the suture line and avoid tissue necrosis for medium-sized lip defects (30–80 %). This is a surgical technique report from a single center. After tumor resection, we made a bilateral 15 mm horizontal skin and mucosa incision from the angles of the lip to the lateral sides. The mucosa and skin were dissected from the underlying muscle, and the muscle was cut approximately 15 mm on each side. The lip defect was then closed and sutured in four layers. Finally, the released mucosa was sutured to the corner of the incised skin. We followed the patients for 36 months and found that their speech intelligibility, sensation, mobility, and aesthetic satisfaction were preserved. The scars were also less pronounced compared to flaps, and there were no signs of edema or drooling. In conclusion, our technique offers many advantages for moderate defects of lower lip tumors. By avoiding the use of flaps, we eliminate the complications associated with flap surgery while achieving aesthetically satisfactory results. However, further evaluation by other surgeons is necessary to fully examine the technique's benefits.

Low-dose-rate (LDR) and high-dose-rate (HDR) interstitial brachytherapy are known to be effective in the treatment of lip carcinomas. The aim of this study was to retrospectively compare oncologic and toxicity outcomes between the two techniques. From 2007 to 2018, patients at the Institut de cancérologie de Lorraine (France) who received exclusive or adjuvant interstitial brachytherapy for lip squamous carcinomas were studied. Two groups were defined: the LDR/PDR group, including patients treated with iridium-192 wires, or pulsed-dose rate technique, and the high-dose-rate group, with patients treated by high-dose-rate technique. The dose ranged between 50 Gy and 65 Gy (depending on previous surgery) for low-dose-/pulsed-dose rate treatments, and 39 Gy for high-dose-rate (twice a day). Early, late toxicity events and oncologic control were reported. Among the 61 patients whose data were analyzed retrospectively, 36 received the low-dose-/pulsed-dose rate treatment (59%) and 25 the high-dose-rate brachytherapy (41%). The median follow-up time was 44 months. At 36 months, the local control rates were 96.3% for LDR/PDR group and 100% for HDR (P = 0.180). The regional control rates were 85.9% and 92% without any difference according to the two groups (P = 0.179). The specific overall survival rate was 95.5% with no difference between groups. There were more grade 2 or higher mucositis in the HDR group than in LDR/PDR group (40% versus 16.7%, P = 0.042). One case of grade 3 mucositis was recorded in each group. No grade 3 late complications were recorded. High-dose-rate brachytherapy reduced the length of hospitalization by 2 days (P < 0.001). Cranial base invasion involves the cranial nerves, causing various neurological symptoms and seriously affecting patients' quality of life. We encountered a 64-year-old woman with recurrent lip cancer with skull base invasion treated with nivolumab followed by cetuximab and paclitaxel (Cmab+PTX) combination chemotherapy. The therapy showed promising efficacy based on FoundationOne® CDx and achieved complete response (CR). The patient discovered an ulcer on the left side of her lip and visited our department in August 201X. She was diagnosed with left-sided lip cancer (squamous cell carcinoma, T1N0M0) and received external-beam radiation (70 Gy). However, primary-site recurrence was noted in December 201X, and she underwent tumor resection. In April 201X+ 2, she underwent neck dissection for cervical lymph node metastasis and postoperative radiation chemotherapy (60 Gy, two courses of CDDP). In February 201X+ 3, local recurrence due to tumor invasion into the middle skull base and ocular symptoms were noted. Therefore, eight nivolumab courses were administered; however, the primary tumor increased, and an oncogene panel test was performed. Based on EGFR amplification results, Cmab+PTX was administered. Recurrent metastasis is frequently associated with poor prognoses, and cytotoxic chemotherapy after immune checkpoint inhibitor therapy is effective in managing cancer recurrence and metastasis, as in the case of our patient. Lip and oral cavity cancer is a severe and growing problem, ranked 16th for both incidence and mortality worldwide. These malignancies are mainly treated with surgery, which can cause a wide range of sequelae. Despite ultrasound-guided microwave ablation (MWA) being widely used, there is no report concerning its application in lip cancer. This study presents a case of ultrasound-guided MWA in a 97-year-old man with squamous cell carcinoma (SCC) and lymph node metastases under the lower lip. The patient was unsuitable for surgery due to his older age. At the most recent 1-year follow-up after treatment, the patient remained in good condition with no symptoms of dysphagia or slurred speech, and the cosmetic results were excellent. Based on clinical evaluation and radiographic imaging, there was no evidence of metastasis or recurrence. Ultrasound-guided MWA could be a promising option for the management of lip cancer. There is no standardized technique for lip reconstruction because the structure and functions of the lip are diverse. We developed a new lip reconstructive approach using a bilateral oblique mucosal V-Y advancement flap. We present the case of a 76-year-old woman with severe dementia, referred to our institute for a tumor on her lower lip. She was diagnosed with lip squamous cell carcinoma (cT2N0M0). The tumor measured 25 × 20 mm. It was resected with a surgical safety margin of 6 mm. Bilateral triangular flaps were fashioned obliquely on the rear lateral side of the defect extending from the labial to the buccal mucosa and used to repair the defect. The operation time was 66 min. She was discharged on the fourth postoperative day without any complications. Speech and food intake functions have been preserved and she has been followed up for 26 months with no recurrence. The lip closing and color match has been adequate despite a slight thinning of the lip. This technique offered a major benefit of short operation and hospitalization time because it was a simple, less-invasive, one-step procedure. Simple Summary: High IMP3 expression is correlated with poorer prognosis in many tumour entities. To date, there have been no data on IMP3 expression and clinical outcome in high-risk localisations (lip, ear) of squamous cell carcinoma of the skin. These are almost twice as likely to metastasise compared to other sites. In this study, the tumour marker IMP3 showed clear correlations with aggressiveness features (lymph node metastases, local recurrences, and progression-free survival). The identification of these more-aggressive tumours could influence therapy and diagnostics (radicality of neck dissection, follow-up intervals, staging). The analysis method presented here is efficient and could be easily incorporated into a clinical workflow and used for prospective testing. Background: High IMP3 expression is correlated with a worse outcome. Until now, there have been no data about IMP3 expression and clinical outcome for high-risk localisation of squamous cell carcinoma of the skin (cSCC). Methods: One-hundred twenty-two patients with cSCC of the lip and ear were included, and IMP3 expression in the tumours was immunohistochemically assessed in different evaluation approaches. Subsequently, subgroups were analysed in a matched pair approach and correlated with clinical pathologic parameters. In the following, different IMP3 analysis methods were tested for clinical suitability. Results: We found a significant correlation between IMP3 expression and risk for lymph node metastasis, local relapse, and progression-free survival. Conclusions: On basis of our data, we suggest a prognostic benefit cutoff value for high (>50%) and low (<50%) IMP3 expression. Thus, IMP3 expression has a high scientific potential for further studies and could potentially be used as a prognostic marker in diagnostic and therapeutic decision-making. [ABSTRACT FROM AUTHOR]

Background Squamous cell carcinoma (SCC) is the most common malignancy on the lower lip. Surgical excision, the standard treatment for SCC, requires full-thickness excision. However, no consensus exists about the appropriate surgical margin. Therefore, we investigated the appropriate surgical margin and excision technique by analyzing 23 years of surgical experience with lower-lip SCC.

Background: Cancer ranks as a leading cause of death and a major barrier to increasing life expectancy in every country of the world. The incidence and demographic data of cancer are important for cancer prevention. Lip, oral cavity, and pharyngeal cancers (LOCP) has multiple risk factors and a varied geographic distribution. Objectives: This study aims to describe and compare the epidemiological and clinical profile of LOCP cancers. Methodology: Results: A total of 441 LOCP cases were recorded, of which most cases belonged to the age group of 51-70 years and male gender. The most common subtype among LOCP cancers is mouth (27%), followed by the tongue (25%). Histo-pathologically, squamous cell carcinoma is the most common type contributing to 425 (96.5%) of cases. Nearly 267 patients (60.1%) were smokers, and 206 patients (46.7%) used alcohol whereas 266 patients (60.3%) used Smokeless tobacco products. Majority of the patients underwent a combination therapy of surgery and radiotherapy (69.6%). Conclusion: The increased incidence of LOCP cancer and its subtypes associated with smoking and tobacco points to the pivotal role of these risk factors in the pathogenesis of LOCP cancers. Hence better awareness and stringent measures are the need of the hour to decrease the ongoing burden of these cancer types.

Lip carcinoma is one of the most frequent conditions affecting the general population. It is among the ten most common neoplasms, but despite advances in research and therapy, its prognosis has not improved in a significant way in the past few years, making it a challenge in the medical research field and in surgical treatment. This study was conducted with the aim of evaluating the available reconstructive surgical options for the treatment of lip carcinomas in order to define which could be the most appropriate technique to achieve satisfying aesthetic and functional outcomes considering hospital resources in the COVID-19 era. Seventeen patients were included in this retrospective study, which took place between January 2019 and April 2021. There were two groups: seven patients who underwent a radial forearm free flap and ten who underwent locoregional flaps. The statistical analysis was performed to evaluate four different endpoints. Surgical length, ICU stay, and hospitalization time were minor for locoregional flaps. There was no statistically significant difference between the two groups when considering post-operative complications. Locoregional flaps have a more aesthetically pleasing result, but from a functional point of view, the results can be superimposable. Both techniques are associated with adequate speech, mouth opening, sealing, and symmetry. Given the impact of the COVID-19 pandemic on the healthcare system, locoregional flaps have been proven to be a good surgical option in the reconstruction of lip defects both in terms of aesthetics and functional outcome.

Low-dose-rate brachytherapy (LDR-BT) is a well-established treatment for lip cancer. High-dose-rate (HDR)-BT is a promising alternative to LDR-BT, but data are limited. In this context, we retrospectively evaluated treatment outcomes in a series of patients who underwent HDR-BT for lip carcinoma between 2003 and 2021. A total of 32 patients were included in this study, with a median age of 73.5 years (range, 61 – 88). The indications for HDR-BT were as follows: primary treatment (n = 17), adjuvant treatment (n = 3), and recurrent disease after surgery (n = 12). The prescribed dose was 18 fractions of 3 Gy administered twice daily. At a median followup of 45 months (range, 12 -232), the 5-year local recurrence-free interval was 96.9% (95% CI: 90.9–100%), the disease-free interval was 85% (95% CI: 70.9–99.1), and 5-year overall survival was 64.7% (95% CI: 44.7–84.8). Eleven patients died, all on age related comorbidities. Acute toxicity manifested as G1 dry desquamation in 6 patients (18.8%), G2 erythema in 10 patients (31.2%) and G3 confluent moist desquamation in 16 patients (50%). Late complications included G1 fibrosis (100% of cases). G1 and G2 depigmentation was observed in 8 (25%) and 6 (18%) patients, G1 telangiectasia occurred in 5 patients (16%). These data support the use of HDR-BT for lip cancer. The dose and fractionation schedule used in this study (18 fractions x 3 Gy twice daily) seems to be effective and safe. [ABSTRACT FROM AUTHOR]

Methods We reviewed 44 patients with lower-lip SCC who underwent surgery from November 1997 to October 2020. Frozen biopsy was performed with an appropriate margin on the left and right sides of the lesion, and the margin below the lesion was the skin above the sulcus boundary. If the frozen biopsy result was positive, an additional session was performed to secure a negative margin. Full-thickness excision was performed until the final negative margin. In each patient, the total number of sessions performed, final surgical margin, and recurrence were analyzed. Results Forty-one cases ended in the first session, 2 ended in the second session, and 1 ended in the third session. The final surgical margins (left and right; n = 88) were 5 mm (66%), 7 mm (9%), 8 mm (2.3%), 10 mm (20.4%), and 15 mm (2.3%). During an average follow-up of 67.4 months (range, 12–227 months), recurrence occurred in one patient. Conclusion The final surgical margin was 5 mm in 66% (58/88) of the cases, and 97.7% (86/88) were within 10 mm. Therefore, we set the first frozen biopsy margin to 5 mm, and we suggest that a 5-mm additional excision is appropriate when frozen biopsy results are positive. [ABSTRACT FROM AUTHOR]

While certain occupations, such as agriculture and fishery, have been associated with an increased risk of lip cancer, the occupational risk profile of lip cancer and its change over time remain poorly known. This study aims to evaluate the incidence of lip cancer across different occupations in the Nordic countries. The Nordic Occupational Cancer Study (NOCCA) covers 14.9 million people and includes 45 years of cancer incidence data, from 1961 to 2005, linked to occupational categories for all the five Nordic populations. Standardized incidence ratios (SIRs) with 95% confidence intervals (CIs) were used to quantify the risk of lip cancer across occupational categories relative to the entire national populations. There were a total of 14,477 male and 3008 female lip cancer patients identified during follow up. The highest SIRs were observed among male fishermen (SIR 2.26, 95% CI: 2.04–2.50), gardeners (SIR 1.60, 95% CI: 1.48–1.72), and farmers (SIR 1.60, 95% CI: 1.55–1.66). A significantly reduced risk of lip cancer (SIR < 0.50) was observed among male physicians, teachers, religious workers, artistic workers, journalists, administrators, printers, waiters, and hairdressers. Among women, no occupations were associated with an increased risk of lip cancer. The incidence of lip cancer varies widely between outdoor and indoor occupations. Occupations involving outdoor activity and exposure to sunlight show the most elevated SIRs. Certain outdoor occupations, such as agriculture and fishery, have been associated with an increased risk of lip cancer. However, the occupational risk profile of lip cancer and its change over time remain poorly known. This study highlights the excess risk of lip cancer among men with outdoor occupations and further corroborates previous studies. Background: Squamous cell carcinoma of the lip (LSCC) and oral cavity can be life-threatening if not diagnosed early. Precancerous lesions like actinic cheilitis (AC), can transform into LSCC. Laminin is a fundamental component for basement membrane (BM) and its integrity may prevent neoplastic invasion. Therefore, laminin immunostaining of BM may be useful in identifying early invasion in actinic cheilitis and thus in the differential diagnosis between AC and invasive LSCC or high-grade epithelial dysplasia (ED). Materials and methods: Biopsies from 46 patients with oral lesions were histologically analyzed and immunohistochemically stained for laminin-1. Results: AC was diagnosed in 34 patients and LSCC in 12 patients, including 3 patients with AC and concomitant high-grade ED/in situ carcinoma. Laminin-1 immunostaining revealed intense and linear expression of the BM in AC with low-grade ED. Loss of laminin expression was observed in LSCC. Intracellular laminin expression in parabasal cells was noted in AC with high-grade ED/in situ carcinoma. Conclusion: Laminin immunostaining could be useful in identifying AC cases suspected of early invasion. It could also contribute to the histopathological differential diagnosis between AC with low- and high-grade ED and between AC and invasive LSCC. The findings of this study provide new insights into the mechanism involved in the progression process of AC into LSCC, encouraging preclinical studies that may document the stochastic role of laminin in this process. Background: Lower lip reconstruction aims to maintain facial subunit function and aesthetics. We present a minor modification of the McGregor flap technique, in which depressor anguli oris (DAO) muscle is separately elevated as a chimeric flap together to investigate the outcomes for reconstructing lower lip defects using various functional parameters. Methods: The study included patients who underwent surgical repair of lower lip defects using a McGregor fan flap with or without DAO muscle chimeric flap between January 2018 and May 2021. The patients were divided into our modified technique with DAO chimeric flap (study group) and conventional McGregor (control group). The primary outcomes were functional evaluation of oral competency, speech intelligibility, and tactile sensation of reconstructed lip outcomes. The functional satisfaction and Patient and Observer Scar Assessment Scale (POSAS) were analyzed. Results: A total of 20 patients were enrolled (10 patients for each group). Excellent oral competence was present in 12 patients, favorably significant in patients with DAO chimeric flap (n = 8/10, p = 0.018). All patients had intelligible speech with insignificant differences between the two groups. No patients demonstrated differences to light touch from baseline sensation at 1 year. Patients in the study group reported more lip function satisfaction (p = 0.049). The POSAS score indicates satisfaction with the appearance of all patients. Conclusion: McGregor fan technique combined with DAO muscle chimeric flap offers a good choice by maintaining sensory and motor functions. Data collected included patients' demographics, lip involved, treatment given, and the treatment outcomes data were analyzed using IBM SPSS version 25. Results: Of the 47 cases in the study, 31 (66.0%) were male and 16 (34.0%) were female and the majority of them were aged over 50 years. The range was 43-76 years with a mean of ± standard deviation of 55.06 ± 8.9. There were no statistically significant differences in either age or gender. The lower lip was involved in 41 (87.2%), whereas the upper lip was involved in 6 (12.8%) cases. Squamous cell carcinoma (SCC) constituted the majority at 42 (89.4%), which were mostly well differentiated. Of the 6 cases of the lower lip, 4 cases were basal cell carcinoma, 1 case was SCC, and the remaining 1 case was sebaceous carcinoma. A total of 38 (80.9%) patients were surgically treated, and the remaining 9 (19.1%), were referred to the oncology department. Of the various options available in the literature for lip reconstruction after tumor excision, nasolabial flap 17 (44.7%) was the most frequently used in our cases, and the outcome of all the surgeries was found satisfactory. Conclusion: Lip cancer is a relatively common cancer in the orofacial region, and the majority is SCC. All the cases of basal cell carcinoma occur in the upper lip. Background: The majority of oral cancers, which make up 25% to 30% of all malignancies in the oral area, are lip cancers (LCs). As far as we are aware, there haven’t been any thorough epidemiologic research on LC. This study’s objective is to look at the trend of tumor prevalence across the whole lip area in Iranians between 2004 and 2016 (Tehran, Iran).Methods: This retrospective cross-sectional study, which examined tumor location, age, gender distribution, histological tumor type, metastasis, clinical diagnosis, and recurrence of the lesion, was carried out on the pathology records of patients admitted to the Department of Pathology at the Cancer Institutes of Imam Khomeini hospital and Tajrish hospital between 2004 and 2016. The data was analyzed using the chi-square test and Fisher’s exact test.Results: Out of a total of 237392 biopsy reports filed with the Department of Pathology, 512 instances (0.21%, 92% malignant, and 8% premalignant) involving 385 (75.2%) males and 127 (24.8%) females were discovered to be lip-related. A statistically significant difference between sex and the site of the lesion was found using the chi-square test (P = 0.032). Lower lip and squamous cell carcinomas (SCC) were the most typical locations and types, respectively. Recurrence and metastasis were noted in 7.8% and 25.84% of cases, respectively, with males more likely than women to experience these outcomes. Moreover, there was a significant (P = 0.025) correlation between gender and histological tumor type.Conclusion: The chance of developing LC was greater in men over the age of 54. The most typical location for SCC involvement is the lower lip. We discovered that women had an increased frequency of upper lip lesions. These disparities might be the result of gender-specific professional and behavioral differences. Hence, as possible contributors to the occurrence of LCs, variables including occupational and behavioral variations and public knowledge of sun exposure should be carefully evaluated.

Tobacco use is associated with diseases worldwide, including cancer. This is one of the major public health problems globally, causing more than 19 million new cases in 2020. Lip and oral cavity cancer (LOCC) is neoplastic growth in the tongue, gums, and lips. The objective of this ecological study was to quantify the strength of the association between incidence and mortality of LOCC, with tobacco use and with the Human Development Index (HDI). Incidence and mortality data on LOCC were obtained for 172 countries in 2020, from the Global Cancer Observatory (GLOBOCAN). The prevalence of tobacco smoking and chewing was obtained from reports conducted in 2019. The inequality in human development was estimated using the HDI from the United Nations Development Program, Human Development Report (2019). Statistically significant correlations were observed between the incidence of LOCC and tobacco smoking and chewing prevalence, except for negative correlations between the prevalence of tobacco smoking LOCC mortality in women, just as in the case of the HDI. No statistically significant differences were found between the prevalence of tobacco chewing only and the incidence of LOCC overall and by sex. A higher LOCC incidence overall and by sex was associated with higher HDI. In conclusion, the present study found positive correlations for various HDI socioeconomic indicators and tobacco use with the incidence and mortality of LOCC, but also a few inverse correlations. Objectives: Obtaining robust evidence about the local mortality levels, trends and impact of oral cavity/base of tongue cancers and lip cancer, especially for women, is imperative in the fight against cancer. This descriptive retrospective ecological time‐series study explored trends in oral cavity/base of tongue cancers and lip cancer mortality rates for women in Brazil from 1980 to 2018, by geographic region and anatomical location. Methods: The crude and age‐adjusted annual mortality rates were obtained by sex, anatomical location and macro‐regions of Brazil. The number of deaths from oral cavity/base of tongue cancers and lip cancers in Brazil was based on official population counts and estimates. The annual percentage change was calculated based on age‐adjusted rates. Data set were analysed using the Joinpoint Regression program. Results: A total of 81,918 individuals died of oral cavity/base of tongue cancers and lip cancer between 1980 and 2018 in Brazil. The age‐adjusted mortality rate for women was 0.47 and 0.57 per 100,000 in 1980 and 2018, respectively. The cumulative female mortality rates standardized by age were 0.01/100,000 for lip cancer and 0.5/100,000 for oral cavity and base of tongue cancers. A decrease in deaths related to oral cavity and base of tongue cancers was identified in the 1980s; however, over the last two decades, there has been an increase in the number of deaths of women with cancer at the base of tongue and neighbouring areas and on the floor of mouth. Importantly, Brazilian regions showed wide variability in trends of oral cavity, base of tongue and lip cancers rate and, in 2018, the regions with the highest rates were the Southeast, South and Northeast for both sexes and specifically for women. The North region showed the greatest recent significant upward trend. Conclusions: During the last 38 years, Brazil has shown a significant increase in the trend of the mortality rate due to oral cavity/base of tongue and lip cancers in women. Preventive strategies with control of risk factors should be strongly emphasized in order to improve the survival rates of individuals with oral cavity/base of tongue and lip cancers. Simple Summary: The presence of tumor-infiltrating lymphocytes (TILs) in the tumor milieu has been linked to better patient prognosis and response to chemo-radiotherapy in head and neck carcinoma, among other malignancies. We studied 60 cases of squamous cell carcinoma of the lip to assess the presence of TILs and their subpopulations and tertiary lymphoid structures (TLSs) in the invading front and inner stroma of the tumor. Thereafter, we investigated the impact of histopathological parameters and prevailing conditions of the tumor microenvironment (TME) in TIL and TLS density. Lip SCCs appeared as more immunogenic compared to other HNCs. Poor lymphocytic infiltration was related to larger tumors, higher invasive ability, dense presence of activated stroma fibroblasts and expression of hypoxia markers. High angiogenic activity was linked with high CD4+, FOXP3+, and low CD8+ TIL density, as well as high CD68+ macrophage presence. LDH5 expression was linked with high CD4+ and FOXP3+ TIL density. Purpose – The purpose of this study was to assess the knowledge of head and neck cancers (HNCs), their risk factors, signs and symptoms among the general public of Karachi, Pakistan. Design/methodology/approach – Initially, 503 individuals were approached for data collection, out of which 404 fit the inclusion criteria (response rate = 80.03%). Nonprobability convenience sampling was utilized to select participants who fell under the age group of 15–60 years, barring the fields of Bachelor of Medicine and Bachelors of Surgery (MBBS) and Bachelors of Dentistry (BDS). Knowledge was evaluated by a 9-item questionnaire, the scores of which yielded the following interpretations: No (0), low (1–3), moderate (4–6) and high (7–9) knowledge. Until now, there have been no data about IMP3 expression and clinical outcome for high-risk localisation of squamous cell carcinoma of the skin (cSCC). Methods: One-hundred twenty-two patients with cSCC of the lip and ear were included, and IMP3 expression in the tumours was immunohistochemically assessed in different evaluation approaches. Subsequently, subgroups were analysed in a matched pair approach and correlated with clinical pathologic parameters. In the following, different IMP3 analysis methods were tested for clinical suitability. Although surgical resection is the mainstay of treatment for lip cancer, reconstruction of the tissue defect after lip cancer resection must take into consideration not only lip function, but also the patient's appearance. However, there is no standard recommendation regarding the reconstruction method. We describe here a case in which we achieved good functional and esthetic results using a tongue flap and V-Y advancement flap for reconstruction of the tissue defect resulting from resection of lower lip cancer. Objective: This study aimed to assess the functional and esthetic outcomes of a chimeric innervated buccinator myomucosal-submental island flap (BMM-SIF) for large composite lower lip reconstruction., Methods: This retrospective study included five patients who underwent lower lip tumor resection and BMM-SIF reconstruction at the Hospital of Stomatology, Sun Yat-sen University, between August 2021 and February 2023. Lip function was evaluated using water leakage, cheek puffing tests, and superficial electromyography. Lip appearance was observed using photographs and evaluated through subjective interviews. Donor-site conditions, including facial symmetry and mouth opening, were monitored., Results: All the BMM-SIFs survived. Drooling was the main complication observed shortly after surgery. The water leakage test showed complete oral competence for liquid holding in the 7th month; however, moderate air leakage was present in two patients. Electromyography revealed myoelectric signals from the innervated buccinator at the recipient site. Facial expression and food intake were typically managed. The shape and projection of the vermilion were harmonious and satisfactory for each patient. Head and neck reconstruction serves two purposes, namely to restore function and aesthetics. The priority goals for lip reconstruction should be determined before surgery. In cases of large lip defects, maintaining the goal makes lip reconstruction difficult. This is a case report of a 66 year old man who came to ORL-HNS OPD, Dr. Soetomo Hospital, Surabaya with the main complaint of ulcers on the upper lip since one year ago accompanied by pain. A mass biopsy result on the right vestibule in Malang on October 11, 2016 showed adenosquamous cell carcinoma. The patient planned to have a wide excision and reconstruction of the lip defect. Wide excision and lip reconstruction was performed on 15 September 2016. Total resection of the upper lip was performed up to the vestibule of the nose. The results showed that squamous cell carcinoma was poorly differentiated. The evaluation after myocutaneous flap surgery was well combined, there were no signs of inflammation or infection. Patients can open their mouths, eat, drink and speak without complaint. The patient then received regular chemoradiotherapy two weeks after surgery.

The aim of this study was to establish whether PD-L1, PD-1, and markers of the tumor microenvironment (CD4, CD8, FOXP3) could have a prognostic value in squamous cell carcinoma of the lip (LSCC). In patients with histologically proven LSCC, tumor specimens were stained using immunohistochemistry (for PD-1, PD-L1, CD4, CD8, and FOXP3) on paraffin-embedded tissues. Patients with (N+) and without (N−) nodal metastasis were stratified and matched to each other according to prognostically relevant clinicopathological parameters. 58 patients (29 N+ and 29 N−) were included. PD-L1 expression was positive (>1%) in 56.1% (n = 33) of all LSCC cases, but its expression did not differ significantly between metastasis groups (65.5% in N+ versus 48.3% in N−; p = 0.144). Nodal disseminated LSCC showed a tendency for higher PD-L1 expression. None of the analyzed markers showed significant correlation with the risk for nodal disease, or revealed significant prognostic value. Due to their significant expression, PD-L1 and PD-1 are potential targets for checkpoint inhibitor therapy in LSCC. Their expression should be analyzed in advanced and metastasized LSCC cases. Introduction: Lip cancer is the most common type of cancer in the oral cavity. It occurs in the lower lip at a rate of 90%–95%, and among lower lip tumors, squamous cell carcinoma (SCC) is the most common type. Objective: In this study, we investigated the influence of the depth of invasion on the decision for an elective lymph node dissection in cases with early-stage (T1–T2 N0 M0) SCC of the lower lip. Methods: Thirty-two patients that were surgically treated were retrospectively reviewed. Results: Three patients had metastatic lymph node, and their mean depth of invasion was 10.60 mm. Lymph node metastasis significantly increased in tumors with a depth of invasion larger than 5 mm (P = 0.033). Conclusion: The rate of cervical lymph node metastasis increases as the depth of invasion increases.

Background: The role of viral agents in the development of head and neck cancers has remained controversial. While markers of viral origin have been isolated from oral cancer tissues, a causative relationship has yet to be shown. The aim of this study was to evaluate the relationship between head and neck cancers and Herpes simplex virus, one of the most common viral infections of the oral orifice., Methods: Here, we conducted a retrospective analysis of two age- and gender-matched cohorts extracted from the real-world database TriNetX on March 10th, 2023, each consisting of 249,272 patients with and without Herpes simplex infections (ICD-10: B00). The diagnoses C00-C14 were analyzed, and risk analysis and Kaplan-Meier survival statics were computed., Results: The strongest association was found for lip cancer (ICD-10: C00) with a hazard ratio [HR (CI 95% low-high)] of 3.08 (1.77-5.35). A significant association with HR of 1.17 (1.02-1.34) was found for the entire group of head and neck cancers. Objectives: The rate of occult metastasis in lip cancer is poorly studied. Management of the regional nodal basin in lip cancer is thus controversial. This study sought to understand the true rate of micrometastasis in lip cancer., Materials and Methods: Systematic review and meta-analysis was conducted of English language studies reporting lip cancer sentinel node biopsy results. Studies were obtained from the PubMed database between the years 2000 and 2023 using the search terms "sentinel node biopsy" and "squamous cell carcinoma." Random effect and fixed effect meta-analyses were performed., Results: Thirteen studies met inclusion criteria. Low heterogeneity was noted among the studies, as indicated by the I 2 inconsistency test (I 2  = 0%). The rate of occult metastasis ranged between 0 and 33% (mean 9%). A total of 189 lip sentinel node biopsies had been performed. Of these, 21 revealed occult nodal metastasis (11.1%, 95% CI 7.36%-16.44%). One step, generalized linear mixed modeling revealed the true rate of occult nodal metastasis to be 10% (95% CI (0.0504, 0.1746), p < 0.0001)., Conclusion: The rate of occult metastasis in lip cancer approaches the threshold for elective management of the regional nodal basin. Objectives: This study aimed to analyze the expression of miR-181b, miR-21, miR-31, and miR-345 in actinic cheilitis with and without epithelial dysplasia and lower lip squamous cell carcinomas, and to verify if the deregulated expression of these miRNAs would be indicative of malignant transformation. Materials and methods: The sample was selected from formalin-fixed paraffin-embedded tissues of 19 actinic cheilitis without epithelial dysplasia, 32 actinic cheilitis with epithelial dysplasia, 42 lower lip squamous cell carcinomas, and 10 nonaltered oral mucosa of the lip. The microRNA (miR, miRNA) expression was quantified by real-time RT-PCR and the expression of the selected miRNAs among the groups of actinic cheilitis and lower lip cancer was compared by chi-square. Results: A higher expression of miR-181b, miR-31, and miR-345 was found in actinic cheilitis without epithelial dysplasia in comparison to that in actinic cheilitis with epithelial dysplasia and with lower lip cancer. There were no differences in miR-21 expression between actinic cheilitis and lower lip cancer. Hierarchical clustering analysis showed a tendency for a downregulation of miR-181b, miR-21, miR-31, and miR-345 in most patients with lower lip cancers. Conclusions: The upregulation of miR-181b, miR-31, and miR-345 expression in actinic cheilitis without epithelial dysplasia and the decrease in the expression of these miRNAs in actinic cheilitis with epithelial dysplasia and in lower lip cancer are potential biomarkers of malignant progression. Background: Squamous cell carcinoma (SCC) is one of the most common neoplasms affecting the oral cavity and the face. Its more differentiated forms may be associated with human papilloma virus (HPV) infection. Case report: In this paper, we report the case of an 86-year-old patient with a well-differentiated SCC of the lower lip associated with HPV treated with surgery with a non-complete histological resolution. Imiquimod 5% cream was applied on the surgical scar once a day for two weeks and then once a week. Two years after SCC removal, no relapse has occurred. Conclusions: Topical imiquimod may be a safe and effective treatment after surgery in SCC of the oral area to reduce the risk of relapses. [ABSTRACT FROM AUTHOR]

Abstract Background Actinic cheilitis (AC) is a variant of actinic keratosis which is known to be a premalignant condition that could develop into squamous cell carcinoma (SCC). Epimyoepithelial carcinoma (EC) is a very rare salivary gland (SG) neoplasm that has classical biphasic histologic findings of small tubules and glandular lumina surrounded by clear myoepithelial cells. Case presentation We report a very rare case of AC occurring on the lower lip of a 70-year-old woman, which is developing to the EC later. Conclusions Diverse appearances of AC include edematous reddish in the acute stage and grey-whitish or dried hyperkeratotic wrinkled lesions in the chronic stage for several months or even years. Accurate treatment of AC in its initial stage could be recommended to avoid further malignant transformation; proper management of clinically suspicious AC is suggested.

Objective: Robot-assisted neck dissection requires a larger wound, is expensive and requires specialised equipment which is not easily available. We have developed an inexpensive minimally invasive neck dissection (MIND) procedure using simple endoscopic instruments in the past. This study was conducted to evaluate the safety, efficacy and reproducibility of the technique. Materials and Methods: From January 2013 to December 2016, we performed MIND on 45 patients with oral cancer using the standard endoscopic equipment. CO2 gas insufflation was used to create the working space. Intra-operative data, post-operative data and pathological characteristics were evaluated and overall survival (OS) and disease-free survival (DFS) Kaplan–Meier curves were compared using the Log-Rank test. Results: Median operative time was 130 (80–190) min with a mean blood loss of 63 (20–150) ml. Major intra-operative complications were not observed. The median number of nodes retrieved was 14 (range: 7–38). Three patients with a positive lymph node were advised to undergo adjuvant radiotherapy. After consultation, 12 out of 13 tongue cancer patients with a tumour depth >3 mm underwent adjuvant radiotherapy. Mean follow-up period was 31.5 (95% confidence interval [CI] 27.9–35.1) months and 27.8 (95% CI 23.6–32.1) months for OS and DFS, respectively. Four (8.9%) deaths and 8 (17.8%) recurrences were observed. The 3-year OS and DFS was 91.1% and 82.2%, respectively. Conclusion: MIND is aesthetically better than conventional procedures for oral cancer patients due to its safety, efficacy and reproducibility at any centre using the standard laparoscopic equipment.

Recent European and US studies reported increased risks of skin cancers associated with hydrochlorothiazide (HCTZ) treatment. Our study aimed to determine the risk of lip cancer and malignant melanoma among Australians prescribed HCTZ. We conducted a case‐control study nested within a population of veterans residing in New South Wales in 2004‐2015, using Australian Department of Veterans' Affairs data linked with cancer registrations, hospitalisation and prescription dispensings. Among DVA healthcare card holders 65 years and older, we identified incident cases of squamous cell carcinoma of the lip and of cutaneous melanoma, each matched with up to 20 controls through risk‐set sampling. We estimated odds ratios (ORs) associating HCTZ use with each cancer using conditional logistic regression, adjusting for predefined confounders. For lip cancer (45 cases), ever‐use of HCTZ yielded an OR of 2.6 (95% CI: 1.4‐5.0) and high HCTZ use (≥25 000 mg) an OR of 4.7 (95% CI: 1.6‐13.7). For cutaneous melanoma (659 cases), ever‐use of HCTZ resulted in an OR of 1.2 (95% CI 1.0‐1.5) and high HCTZ use in an OR of 1.2 (95% CI: 0.8‐1.8). Our findings align with risk estimates from previous studies and provide further evidence that HCTZ's photosensitising properties may promote carcinogenesis in sun‐exposed tissues. Lip cancer is a common malignancy of the oral cavity as it accounts for 25% of them and contributes - 12% to all tumors of the head and neck region. The most frequent lip carcinomas are squamous cell carcinoma (SCC), basal cell carcinoma (BCC), and basosquamous carcinoma (BSC). The aim of this study was to describe clinical characteristics of tumors, surgical technique, and outcomes after lip cancer surgery performed at the University Clinical Center Niš. We conducted a single institution retrospective study including patients who consecutively underwent surgery for carcinoma of the lip at the Clinic of Plastic and Reconstructive Surgery, Clinical Center Niš, in the 5-year period. A total of 32 patients with lip cancer were included in the study. Nineteen (59%) patients were male and 13 (41%) were female. There were 20 cases of SCC, 11 with BCC and one with BSC. The mean patient age was 73.44 (SD 9.95) at the time of primary examination. Tumors were excised with a minimum surgical margin of 5 mm. Surgical treatment depended on the size of the tumor and its localization. Different surgical techniques were used for reconstructions of the lip defects after tumor removal. Thirty one percent of patients had postoperative complications., There was no recurrence of tumors or tumor related deaths during the follow-up of patients. In patients who had a wider resection of tissue and a more complex reconstruction technique performed, the possibility of occurrence of early postoperative complications is greater. OBJECTIVES: The aim of this study is to investigate the role of neck dissection and wait-see policy for neck treatment on prognosis of patients with early stage lip cancer.METHODS: The study included 20 patients ( 5 females, 15 males; mean age 62.64 years; range 38 to 80 years) who underwent surgery for primary site and neck between March 2007 and April 2015 because of T1-2N0 lower lip cancer. Data were reviewed retrospectively on clinical features, localization of the lesion, regional lymph node status, tumor staging, results of surgical treatment and pathological examination. Tumor was excised with 1 cm surgical margin in all cases. The tissue defect was reconstructed by primary closure or varied reconstrction techniqcs. All T2N0 stage patients underwent neck dissection. If the tumor was located in the middle of the lip, bilateral neck dissection, otherwise unilateral neck dissection was performed. RESULTS: Histopathologic diagnosis was squamous cell carcinoma in all cases. The mean follow-up period was 45.82 months. There were 15 patients with clinically diagnosed T1N0 tumors and 5 patients with clinically diagnosed T2N0 tumors. For T1N0 tumors, 3 suprahyoid, 2 bilateral supraomohyoid neck dissections were performed. Supraomohyoid neck dissection was performed in bilaterally in 3 T2N0 tumors, and suprahyoid neck dissection was performed in remaing 2 tumors. CONCLUSION: Four years survival rates without illness were significantly higher in cases with early stage lip cancer. Our results present that neck dissection may not be necessary in patients with T1N0 lower lip cancers.

Adenoid cystic carcinoma is a relatively rare epithelial tumor of the major and minor salivary glands that makes up less than 1% of head and neck neoplasms. The typical clinical and pathological findings of this neoplasm include slow growth, perineural invasion, multiple local recurrences, and distant metastasis. Cancer is uncontrollable growth of cells that invade and cause damage to surrounding tissues. Oral cancer is defined as a growth of sore in the mouth that does not go away. Carcinoma includes cancers of the lips, cheeks, tongue, floor of the mouth, laborious and tender roof of the mouth, sinuses and tubular cavity (throat), and it should be life threatening if not diagnosed and handled early. The main aim of the study is to assess the knowledge and awareness of oral cancer among college students. A survey based questionnaire was done to study the knowledge about oral cancer among college students. A total questionnaire of 15 questions were collected by google form app. SPSS statistical analysis was done. A total of 100 participants participated in this survey of age group 18-23 years. The results were collected and data were analysed. The result shows that 84% of participants were aware of recent advances in treatment of oral cancer. Although the incidence of lower lip cancer is not high in Japan, its treatment requires an approach that considers both esthetics and function. When surgical resection is required, the method used for reconstruction varies depending on the affected part. Despite various studies proposing different types of algorithms, no single method is considered the best. If the loss of half or more of the lip is predicted, a free flap may need to be considered, depending on the case. Here, we report a case involving a 78-year-old edentulous woman with lower lip cancer whose resection area involved approximately 70% of the red and white portions of the lower lip. Fortunately, no resection was required at the commissure. We accordingly performed reconstruction with a double Abbe flap in accordance with a detailed treatment plan.
