BACKGROUND: The understanding of bile acid (BA) and unsaturated fatty acid (UFA) profiles, as well as their dysregulation, remains elusive in individuals with type 2 diabetes mellitus (T2DM) coexisting with non-alcoholic fatty liver disease (NAFLD). Investigating these metabolites could offer valuable insights into the pathophy-siology of NAFLD in T2DM. AIM: To identify potential metabolite biomarkers capable of distinguishing between NAFLD and T2DM. METHODS: A training model was developed involving 399 participants, comprising 113 healthy controls (HCs), 134 individuals with T2DM without NAFLD, and 152 individuals with T2DM and NAFLD. External validation encompassed 172 participants. NAFLD patients were divided based on liver fibrosis scores. The analytical approach employed univariate testing, orthogonal partial least squares-discriminant analysis, logistic regression, receiver operating characteristic curve analysis, and decision curve analysis to pinpoint and assess the diagnostic value of serum biomarkers. RESULTS: Compared to HCs, both T2DM and NAFLD groups exhibited diminished levels of specific BAs. In UFAs, particular acids exhibited a positive correlation with NAFLD risk in T2DM, while the ω-6:ω-3 UFA ratio demonstrated a negative correlation. Levels of α-linolenic acid and γ-linolenic acid were linked to significant liver fibrosis in NAFLD. The validation cohort substantiated the predictive efficacy of these biomarkers for assessing NAFLD risk in T2DM patients. CONCLUSION: This study underscores the connection between altered BA and UFA profiles and the presence of NAFLD in individuals with T2DM, proposing their potential as biomarkers in the pathogenesis of NAFLD.

Oral administration of resveratrol is able to ameliorate the progression of diabetic nephropathy (DN); however, its mechanisms of action remain unclear. Recent evidence suggested that the gut microbiota is involved in the metabolism therapeutics. In the current study, we sought to determine whether the anti-DN effects of resveratrol are mediated through modulation of the gut microbiota using the genetic db/db mouse model of DN. We demonstrate that resveratrol treatment of db/db mice relieves a series of clinical indicators of DN. We then show that resveratrol improves intestinal barrier function and ameliorates intestinal permeability and inflammation. The composition of the gut microbiome was significantly altered in db/db mice compared to control db/m mice. Dysbiosis in db/db mice characterized by low abundance levels of Bacteroides, Alistipes, Rikenella, Odoribacter, Parabacteroides, and Alloprevotella genera were reversed by resveratrol treatment, suggesting a potential role for the microbiome in DN progression. Furthermore, fecal microbiota transplantation, derived from healthy resveratrol-treated db/m mice, was sufficient to antagonize the renal dysfunction, rebalance the gut microbiome and improve intestinal permeability and inflammation in recipient db/db mice. These results indicate that resveratrol-mediated changes in the gut microbiome may play an important role in the mechanism of action of resveratrol, which provides supporting evidence for the gut-kidney axis in DN.

Objective To evaluate the clinical efficacy and safety of bosentan and sildenafil in the treatment of the severe pulmonary artery hypertension with congenital heart disease. Methods All patients with severe pulmonary hypertension as well as congenital heart disease in our hospital from January 2009 to February 2012 were selected as the object, and ten patients were enrolled, of which, 3 cases were male, 7 cases were female, the average age was 43.04±19.74 years old. They were all treated with bosentan and sildenafil for 6 months. The main end points were WHO classifications of pulmonary hypertension, and the second end points were hemodynamic indexes such as pulmonary artery pressure. Results After treatment for 6 months, there were significant improvements in the main end points including WHO classifications of pulmonary hypertension; there improvements of the second end points such as pulmonary artery pressure and cardiac output; and then there were notable improvement of physical properties for instance red blood cell and uric acid; difference of which had statistical significance. But there were no significant changes in aminotransferase and three tricuspid regurgitation velocity of other physical properties than those of before treatment. No severe side effects and complications were observed. Conclusion The results reveald the efficacy and safety of oral administration of combining bosentan and sildenafil in the severe pulmonary artery hypertension with congenital heart disease. Objective To observe the curative effects of atorvastation on the pulse pressure and arterial elasticity of hypertension with atrial fibrillation. Methods 70 patients ( pulse pressure ≥ = 60 mmHg) were randomly divided into the control group and the observation group. The observation group (40 cases) was treated with atorvastatin combined with antihypertensive therapy. Before treatment and 24 weeks after drugs, the patient's blood pressure, arterial elasticity index C1 and small artery elasticity index C2 were detected. The improved situation of pulse pressure and arterial flexibility in the two groups were compared. Results Compared with the control group, pulse pressure was dramatically reduced in the observation group after 24 weeks of treatment, and the differences were statistically significant (P < 0.05). C2 of the observation group was obviously higher than that of the control group (P < 0.05), while no significant change was observed in arterial elasticity index C1 (P > 0.05) . In the control group, there were no significant changes in C1 and C2 (P > 0.05) . Conclusion Atorvastatin can significantly improve the arterial elasticity, reduce the pulse pressure of hypertensive patients with atrial fibrillation. [ABSTRACT FROM AUTHOR]

Objective To observe the clinical efficacy of bosentan in treatment of severe pulmonary hypertension related to congenital heart disease (CHD-PAH). Methods 5 patients with severe CHD-PAH patients received bosentan therapy, then pulmonary artery pressure, pulmonary vascular resistance (PVR), 6min walk test, right ventricular end-systolic diameter (RVSD) changes were observed and statistically analyzed after six months medication. Results Pulmonary arterial systolic pressure (sPAP) was significantly decreased from (96 ± 11) mmHg to (86 ± 10) mmHg, P<0.01. pulmonary arterial diastolic pressure (dPAP) was significantly decreased from (56 ± 10) mmHg to (46 ± 9) mmHg (P<0.01), pulmonary arterial mean pressure (mPAP) was significantly decreased from (73 ± 11) mmHg to (59 ±10) mmHg (P < 0.05), pulmonary vascular resistance was significantly decreased from (17.8 ± 1.9) Wood to (13.1 ± 1.7) Wood (P<0.01) 6min walk test was improved from (136 ± 40) m to (198 ± 55) m, P<0.01. right ventricular end-systolic diameter significantly decreased from (40 ± 5) mm to (36 ± 6) mm after 6 months therapy (P<0.05) . Conclusion Bosentan can decrease pulmonary arterial systolic pressure, improve exercise tolerance, improve right ventricular function in patients with severe CHD-PAH. [ABSTRACT FROM AUTHOR]

Objective To study the effect of implanted stent or selected operation in patients with acute occlusion of right coronary artery during the emergency coronary arteriongraphy. Methods Forty-three patients with acute occlusion of right coronary artery who underwent the emergency coronary arteriongraphy were divided into two groups: the implanted stent group (n = 23) during the emergency coronary arteriongraphy and the selected operation group (n = 20) during the emergency coronary arteriongraphy. Then we observed the general data, the coronary artery pathological changes, preoperative and postoperative thrombus, the blood flow, remained stenosis and the prognosis. Results The implanted stent group was similar to the control group in general condition, but implanted stent group was significantly different from the control group in the near, intermediate and distal coronary artery pathological changes (P < 0.05). The control group was significantly different from the implanted stent group in the thrombus and slowly blood flow (P < 0.05). After three months, we reviewed coronary arteriongraphy and selected operation. The implanted stent group was significantly different from the control in the remained stenosis and slowly blood flow (P < 0.05). But the implanted stent group was similar to the control group in the thrombus and new proceeding cardiovascular events. Conclusion When thrombus appears in acute occlusion of right coronary artery, we can implant stent during the emergency coronary arteriongraphy if the thrombus is little, or we can select operation after PTCA on the contrary. Objective To discusses the efficacy and safety of patent ductus arteriosus sealing for low body weight children. Methods From January 2008 to June 2012 in our hospital, 32 cases (male 14 and female 18) of PDA babies < 8 kg weight were given transcatheter closure treatment. The enrolled PDA babies aged 6 month-18 months, average (11 ± 5.5 months), weighted (7.2 ± 0.6) kg, aortic imaging showed the narrowest diameter of PDA was 1 ~ 8 mm (4.6 ± 1.7) mm, descending aorta imaging was performed immediately and 30 min after transcatheter closure treatment. Echocardiography and X-ray radiography examination were performed 1d, 1, 3, 6 months after treatment. Results 30 cases got successful patent ductus arteriosus sealing, all the successful patients were treated with domestic closer, and 1 case was complicated with the descending aorta stenosis and ventricular septal defect, and was transferred to surgerical treatment. 1 case of PDA gave up patent ductus arteriosus sealing as descending aorta stenosis caused by the closer. 2 cases were found a trace-residual shunt immediately after patent ductus arteriosus sealing. No serious complications were found in all patients during the follow-up period. Conclusions Application of domestic closer in treatment of PDA infants < 8 kg is a safe, effective and simple intervention treatment method with little trauma and fast recovery. In the detection process of atmospheric laser absorption spectroscopy in open space, the transmitted beam is inevitably affected by atmospheric turbulence, resulting in superimposed fluctuation noise in the received optical signal. First, the correction method of atmospheric turbulence is theoretically analyzed. In order to reduce the error influence factors and the error transfer coefficient, a new method of spectral data processing based on co-frequency and dual-wave has been proposed. By modifying scintillation noise and background noise, the influence of atmospheric turbulence noise in open space is reduced. An atmospheric detection system in open space based on co-frequency and dual-wave has been established. The experimental results show that the maximum fluctuation of the spectral signal processed by the method of spectrum data processing based on the co-frequency and dual-wave is reduced from 12.854% to 4.635%, and the single-intensity absorbance is fitted by Voigt with its correlation coefficient of 0.9525. The mean of the standard deviation of the algorithm is 0.1370, while the mean value of the standard deviation of the existing algorithm in a short time is 0.6928. And, through the comparative experiment, the standard deviation of the existing data processing techniques of two-wavelength differential absorption is 0.2974, while the standard deviation of the method of spectrum data processing based on the co-frequency and dual-wave is 0.1038. It can be concluded that the co-frequency and dual-wave method can effectively reduce the influence of atmospheric turbulence noise and laser flashing to improve the stability of concentration measurement, which has practical engineering value.

Objective: To explore the risk factors of patent ductus arteriosus (PDA) patients with thrombocytopenia after PDA interventional occlusion., Methods: Thrombocytopenia occurred in 14 out of 350 patients underwent PDA occlusion. Age, gender, body weight, PDA size, occluder size, mean pulmonary arterial pressure, the dose of heparin, the manufacturer of occluder, residual shunt after operation were analyzed. The recovery time of different grades of thrombocytopenia was observed., Results: Multivariate logistic regression showed that the PDA size (OR = 2.238, P < 0.05), the dose of heparin (OR = 3.247, P < 0.05), residual shunt after operation (OR = 1.912, P < 0.01) were the independent risk factors of thrombocytopenia after PDA occlusion. The recovery time of mild thrombocytopenia was (7 ± 2) days without treatment. The recovery time of moderate thrombocytopenia was (12 ± 4) days with glucocorticoids treatment. The recovery time of severe thrombocytopenia was (21 ± 7) days with platelet transfusion., Conclusions: The occluder size, dose of heparin, residual shunt are the independent risk factors of thrombocytopenia after PDA interventional occlusion. Recover time of thrombocytopenia after PDA interventional occlusion is closely related to the severity of thrombocytopenia.
