Abstract

Digital Education Initiatives and Timely Solutions (DIG-IT) is an Erasmus+ project collaboration between university academics, clinical educators, industry partners, healthcare professionals, and technology experts over 5 European Union (EU) states. The objectives are to create digital educational capacity for academic faculty and mobile learning opportunities for continuing professional healthcare education. The first deliverable of this project was the innovative European Union Digital Education Quality Standards Framework and Toolkit or EU-DEF. This framework can be used to consider all principle and secondary aspects when designing, delivering, and evaluating digital education resources. In this article we explore the framework efficacy in collaboratively developing two open access, mobile healthcare apps, ECG Interpretation for Nurses and Nurse Leadership and Management. We describe the teamwork needed and steps taken during this process, and demonstrate the adaptability, usability, and value of this tool for other online development projects. The EU-DEF is an evidence-based, comprehensive framework that can be used as a starting point for individuals or teams when developing digital educational projects. Considering all variables helps focus the expertise required to produce robust end-products.

Abstract

OBJECTIVE: The purpose of this study is to validate the 13-item version of the Work-Related Stress Questionnaire (WRSQ) on a representative sample of Italian public health residents. MATERIALS AND METHODS: The WRSQ was administered as part of the Public Health Residents' Anonymous Survey in Italy (PHRASI) from 14 June to 26 July 2022. Collinearity between each questionnaire item was assessed with Kendall's t statistic. The latent factors identified associating similar items based on the authors' observations were workplace, job demand, support and unpleasant workplace. Goodness-of-fit was assessed with confirmatory factor analysis calculating: Comparative Fit Index (CFI), Tucker-Lewis Index (TLI), Root Mean Square Error of Approximation (RMSEA), Standardized Root-Mean-Residual (SRMR). Cronbach's alpha (a) and Omega McDonald (ω) were calculated to evaluate the reliability of the questionnaire. R 4.2.2 was used to perform the analyses. RESULTS: 379 Public Health residents (58% women) responded to the questionnaire. No significant collinearity was found between the items (t range -0.31 to 0.49). Confirmatory Factor Analysis s howed C FI = 0 .975, T LI = 0 .967, R MSEA = 0.041 and SRMR = 0.051. Internal consistency of the WRSQ Total Score was a = 0.80 and ω = 0.85. CONCLUSIONS: Although validated in a sectorial subpopulation of healthcare workers, the WRSQ proved to be an excellent tool for assessing work-related stress. Unpleasant workplace latent factors showed lower factor loading and internal consistency than others. This could be due to the fact that topics investigated with unpleasant workplace items (e.g., abuse and discrimination) are experienced on a less regular basis than the ones assessed through the other items. Abstract

Introduction: Italy is one of the high-income countries hit hardest by Covid-19. During the first months of the pandemic, Italian healthcare workers were praised by media and the public for their efforts to face the emergency, although with limited knowledge and resources. However, healthcare workers soon had to face new challenges at a time when the national health system was working hard to recover. This study focuses on this difficult period to assess the impact of the COVID-19 pandemic on the mental health of Italian healthcare workers. Materials and Methods: Healthcare workers from all Italian regions [n = 5,502] completed an online questionnaire during the reopening phase after the first wave lockdown. We assessed a set of individual-level factors (e.g., stigma and violence against HCWs) and a set of workplace-level factors (e.g., trust in the workplace capacity to handle COVID-19) that were especially relevant in this context. The primary outcomes assessed were score ≥15 on the Patient Health Questionnaire-9 and score ≥4 on the General Health Questionnaire-12, indicators of clinically significant depressive symptoms and psychological distress, respectively. Logistic regression analyses were performed on depressive symptoms and psychological distress for each individual- and workplace-level factor adjusting for gender, age, and profession. Results: Clinically significant depressive symptoms were observed in 7.5% and psychological distress in 37.9% of HCWs. 30.5% of healthcare workers reported having felt stigmatized or discriminated, while 5.7% reported having experienced violence. Feeling stigmatized or discriminated and experiencing violence due to being a healthcare worker were strongly associated with clinically significant depressive symptoms [OR 2.98, 95%CI 2.36–3.77 and OR 4.72 95%CI 3.41–6.54] and psychological distress [OR 2.30, 95%CI 2.01–2.64 and OR 2.85 95%CI 2.16–3.75]. Abstract

Background. Work-related stress represents a major health problem within most work environments and its rates of incidence are increasing as time goes by. Work-related stress is both detrimental for workers’ psychophysical health and costly to societies. Besides the direct medical costs that represent only a fraction of the total cost, data is either lacking or unreliable to properly estimate the economic impact of occupational stress. Methods. This paper reviews international studies and organisational reports in order to highlight occupational stress economic impact on an international level. Results. The study shows that work-related stress imposes an undeniable financial burden on worldwide societies. Findings show estimations that vary from € 54 million up to € 280 billion depending on the country. Results suggest that productivity loss due to absenteeism and presenteeism has a greater economic impact compared to the medical expenses. Discussion. Generally speaking, the costs of work-related stress, also according to Europe, impact both on employers and welfare systems, and from the point of view of prevention we should work hard to improve stress-management strategies. Conclusions. The discussion around this matter should be emphasized in order to properly address occupational stress and make implications for job redesign under the perspective that promoting and protecting workers’ mental well-being will benefit the individual and allow cost containment benefitting the collective as well.
