10. Seasonal association between viral causes of hospitalised acute lower respiratory infections and meteorological factors in China: a retrospective study

Abstract

Acute lower respiratory infections caused by respiratory viruses are common and persistent infectious diseases worldwide and in China, which have pronounced seasonal patterns. Meteorological factors have important roles in the seasonality of some major viruses. Our aim was to identify the dominant meteorological factors and to model their effects on common respiratory viruses in different regions of China. We analysed monthly virus data on patients from 81 sentinel hospitals in 22 provinces in mainland China from 2009 to 2013. The geographical detector method was used to quantify the explanatory power of each meteorological factor, individually and interacting in pairs. 28369 hospitalised patients with ALRI were tested, 10387 were positive for at least one virus, including RSV, influenza virus, PIV, ADV, hBoV, hCoV and hMPV. RSV and influenza virus had annual peaks in the north and biannual peaks in the south. PIV and hBoV had higher positive rates in the spring summer months. hMPV had an annual peak in winter spring, especially in the north. ADV and hCoV exhibited no clear annual seasonality. Temperature, atmospheric pressure, vapour pressure, and rainfall had most explanatory power on most respiratory viruses in each region. Relative humidity was only dominant in the north, but had no significant explanatory power for most viruses in the south. Hours of sunlight had significant explanatory power for RSV and influenza virus in the north, and for most viruses in the south. Wind speed was the only factor with significant explanatory power for human coronavirus in the south. Abstract

After the first wave of SARS-CoV-2 infections in spring 2020, Europe experienced a resurgence of the virus starting in late summer 2020 that was deadlier and more difficult to contain1. Relaxed intervention measures and summer travel have been implicated as drivers of the second wave2. Here we build a phylogeographical model to evaluate how newly introduced lineages, as opposed to the rekindling of persistent lineages, contributed to the resurgence of COVID-19 in Europe. We inform this model using genomic, mobility and epidemiological data from 10 European countries and estimate that in many countries more than half of the lineages circulating in late summer resulted from new introductions since 15 June 2020. The success in onward transmission of newly introduced lineages was negatively associated with the local incidence of COVID-19 during this period. The pervasive spread of variants in summer 2020 highlights the threat of viral dissemination when restrictions are lifted, and this needs to be carefully considered in strategies to control the current spread of variants that are more transmissible and/or evade immunity. Our findings indicate that more effective and coordinated measures are required to contain the spread through cross-border travel even as vaccination is reducing disease burden.

Abstract

Compared with full-bridge modular multilevel converter (MMC), the hybrid MMC composed of half bridge sub-module and full bridge sub-module has the low cost and high DC fault ride-through capability. The DC fault transient analysis is the basis of device selection and proportioning design of hybrid MMC. In order to analyze the transient characteristics of hybrid MMC after DC side fault, the equivalent fault models of DC pole-pole short circuit during the non-blocking DC fault ride-through in rated operation and step-down operation are established. The dynamic switching process of each bridge arm sub-module of hybrid MMC during DC fault ride-through is analyzed. The transient process before the switching of the non-blocking DC fault ride-through control strategy is equivalent to the uncontrollable sub-module capacitor discharge process, while the transient process after switching the control strategy is equivalent to the current limiting process with the initial energy storage of the inductor and the reverse voltage source. The analytical calculation method of the short-circuit current on the DC side of hybrid MMC is also given in this paper. Finally, the effectiveness of the model and calculation method proposed in this paper is verified by the PSCAD/EMTDC simulation platform, which can provide a reference for the optimal design of hybrid MMC.

Abstract

Spatial lifecourse epidemiology is an interdisciplinary field that utilizes advanced spatial, location-based, and artificial intelligence technologies to investigate the long-term effects of environmental, behavioural, psychosocial, and biological factors on health-related states and events and the underlying mechanisms. With the growing number of studies reporting findings from this field and the critical need for public health and policy decisions to be based on the strongest science possible, transparency and clarity in reporting in spatial lifecourse epidemiologic studies is essential. A task force supported by the International Initiative on Spatial Lifecourse Epidemiology (ISLE) identified a need for guidance in this area and developed a Spatial Lifecourse Epidemiology Reporting Standards (ISLE-ReSt) Statement. The aim is to provide a checklist of recommendations to improve and make more consistent reporting of spatial lifecourse epidemiologic studies. The STrengthening the Reporting of Observational Studies in Epidemiology (STROBE) Statement for cohort studies was identified as an appropriate starting point to provide initial items to consider for inclusion. Reporting standards for spatial data and methods were then integrated to form a single comprehensive checklist of reporting recommendations. The strength of our approach has been our international and multidisciplinary team of content experts and contributors who represent a wide range of relevant scientific conventions, and our adherence to international norms for the development of reporting guidelines. As spatial, location-based, and artificial intelligence technologies used in spatial lifecourse epidemiology continue to evolve at a rapid pace, it will be necessary to revisit and adapt the ISLE-ReSt at least every 2-3 years from its release.

Abstract

The global population at risk from mosquito-borne diseases-including dengue, yellow fever, chikungunya and Zika-is expanding in concert with changes in the distribution of two key vectors: Aedes aegypti and Aedes albopictus. The distribution of these species is largely driven by both human movement and the presence of suitable climate. Using statistical mapping techniques, we show that human movement patterns explain the spread of both species in Europe and the United States following their introduction. We find that the spread of Ae. aegypti is characterized by long distance importations, while Ae. albopictus has expanded more along the fringes of its distribution. We describe these processes and predict the future distributions of both species in response to accelerating urbanization, connectivity and climate change. Global surveillance and control efforts that aim to mitigate the spread of chikungunya, dengue, yellow fever and Zika viruses must consider the so far unabated spread of these mosquitos. Our maps and predictions offer an opportunity to strategically target surveillance and control programmes and thereby augment efforts to reduce arbovirus burden in human populations globally.

Abstract

With the rapid increase of Chinese international travel, labour service and business, the pressure of imported mosquito-borne infectious diseases into China is also on the rise, and a better understanding of this phenomenon is required to enable appropriate public health actions. The main purpose of this thesis is to examine the spatiotemporal patterns, driving factors and seasonal risk of the importation and onward transmission for two mosquito-borne diseases, malaria and dengue, in China. Multidisciplinary approaches have been used and a variety of datasets has been integrated, including disease incidence, air travel, environment, and international investment. The analyses presented demonstrate that the spatiotemporal patterns of malaria and dengue have changed significantly in mainland China over the last decade, with an expanded geographic range and increasing incidence of imported infections. However, the geographic extent of autochthonous malaria in China has shrunk significantly due to malaria control and elimination activities, while the incidence of locally transmitted dengue has increased dramatically since 1990. The importation of mosquito-borne diseases is driven by transmission rates in origin countries, increasing travel, as well as investment patterns and economic ties. Taking dengue as an example, the seasonally changing risk and geographic expansion of mosquito-borne disease spread from Southeast Asia into China has been quantified, with emerging routes highlighted. These findings have important public health significance in formulating strategies and technical guidance for mitigating infections in travellers, improving the capacity of screening, diagnosis and treatment, reducing the risk of morbidity, and strengthening surveillance and investigation capabilities, so as to enable timely interruption of the seasonal onward transmission of imported mosquito-borne diseases.
