Background: Eradication of oral Helicobacter pylori (H. pylori) not only reduces the infection rate from the transmission route but also improves the success rate of intragastric eradication. MAXPOWER Biological Bacteriostatic Liquid, developed in our previous work, is a composite biological preparation with strong antibacterial ability and unique antibacterial mechanism. The present study evaluated the efficacy of the MAXPOWER biocontrol solution on H. pylori and its success rate in eradicating oral H. pylori in clinical patients. Methods: Live-dead cell staining and hemolysis test were used to evaluate the cellular safety of MAXPOWER biocontrol solution; plate spreading, live-dead bacterial staining, and scanning electron microscopy methods were used to evaluate its antimicrobial effect against H. pylori. Transcriptomics was used to analyze the changes in H. pylori genes before and after treatment. After seven days of gavage treatment, H&E staining and mice feces were collected for 16SrDNA sequencing to evaluate the animals' safety. Oral H. pylori-positive patients were randomized to be given a placebo and MAXPOWER Bio-Bacteriostatic Liquid gargle for seven days to evaluate the effect on oral H. pylori eradication. Results: In vitro tests demonstrated that this product has excellent biocompatibility and hemocompatibility and can effectively eradicate oral H. pylori. In vivo tests further showed that it has good biosafety and virtually no adverse effect on intestinal microflora. Transcriptomics analysis revealed that it kills H. pylori cells mainly by disrupting their cell membranes and metabolism. Additionally, the results of randomized controlled trials on humans disclosed that the oral H. pylori eradication rates achieved by MAXPOWER Biological Antibacterial Liquid were 71.4% and 78.9% according to the intention-to-treat and the per-protocol analysis, respectively. Conclusion: MAXPOWER Biological Antibacterial Liquid is both safe and efficacious in the eradication of oral H. pylori. Background: Educational initiatives on Helicobacter pylori (H. pylori) constitute a highly effective approach for preventing its infection and establishing standardized protocols for its eradication. ChatGPT, a large language model, is a potentially patient‐friendly online tool capable of providing health‐related knowledge. This study aims to assess the accuracy and repeatability of ChatGPT in responding to questions related to H. pylori. Materials and Methods: Twenty‐one common questions about H. pylori were collected and categorized into four domains: basic knowledge, diagnosis, treatment, and prevention. ChatGPT was utilized to individually answer the aforementioned 21 questions. Its responses were independently assessed by two experts on H. pylori. Questions with divergent ratings were resolved by a third reviewer. Cohen's kappa coefficient was calculated to assess the consistency between the scores of the two reviewers. Results: The responses of ChatGPT on H. pylori‐related questions were generally satisfactory, with 61.9% marked as "completely correct" and 33.33% as "correct but inadequate." The repeatability of the responses of ChatGPT to H. pylori‐related questions was 95.23%. Among the responses, those related to prevention (comprehensive: 75%) had the best response, followed by those on treatment (comprehensive: 66.7%), basic knowledge (comprehensive: 60%), and diagnosis (comprehensive: 50%). In the "treatment" domain, 16.6% of the ChatGPT responses were categorized as "mixed with correct or incorrect/outdated data." However, ChatGPT still lacks relevant knowledge regarding H. pylori resistance and the use of sensitive antibiotics. Conclusions: ChatGPT can provide correct answers to the majority of H. pylori‐related queries. It exhibited good reproducibility and delivered responses that were easily comprehensible to patients. Further enhancement of real‐time information updates and correction of inaccurate information will make ChatGPT an essential auxiliary tool for providing accurate H. pylori‐related health information to patients. The optimal number of examined lymph nodes (ELNs) for gastric signet ring cell carcinoma recommended by National Comprehensive Cancer Network guidelines remains unclear. This study aimed to determine the optimal number of ELNs and investigate its prognostic significance. In this study, we included 1723 patients diagnosed with gastric signet ring cell carcinoma in the Surveillance, Epidemiology, and End Results database. X-tile software was used to calculate the cutoff value of ELNs, and the optimal number of ELNs was found to be 32 for adequate nodal staging. In addition, we performed propensity score matching (PSM) analysis to compare the 1-, 3-, and 5-year survival rates; 1-, 3-, and 5-year survival rates for total examined lymph nodes (ELNs < 32 vs. ELNs ≥ 32) were 71.7% vs. 80.1% (p = 0.008), 41.8% vs. 51.2% (p = 0.009), and 27% vs. 30.2% (p = 0.032), respectively. Furthermore, a predictive model based on 32 ELNs was developed and displayed as a nomogram. The model showed good predictive ability performance, and machine learning validated the importance of the optimal number of ELNs in predicting prognosis. [ABSTRACT FROM AUTHOR]

Background: Peptic ulcer bleeding remains a typical medical emergency with significant morbidity and mortality. Peptic ulcer rebleeding often occurs within three days after emergent endoscopic hemostasis. Our study aims to develop a nomogram to predict rebleeding within three days after emergent endoscopic hemostasis for high-risk peptic ulcer bleeding.Methods: We retrospectively reviewed the data of 386 patients with bleeding ulcers and high-risk stigmata who underwent emergent endoscopic hemostasis between March 2014 and October 2018. The least absolute shrinkage and selection operator method was used to identify predictors. The model was displayed as a nomogram. Internal validation was carried out using bootstrapping. The model was evaluated using the calibration plot, decision-curve analyses, and clinical impact curve.Results: Overall, 386 patients meeting the inclusion criteria were enrolled, with 48 patients developed rebleeding within three days after initial endoscopic hemostasis. Predictors contained in the nomogram included albumin, prothrombin time, shock, haematemesis/melena and Forrest classification. The model showed good discrimination and good calibration with a C-index of 0.854 (C-index: 0.830 via bootstrapping validation). Decision-curve analyses and clinical impact curve also demonstrated that it was clinically valuable.Conclusion: This study presents a nomogram that incorporates clinical, laboratory, and endoscopic features, effectively predicting rebleeding within three days after emergent endoscopic hemostasis and identifying high-risk rebleeding patients with peptic ulcer bleeding. Background: Endoscopic submucosal dissection (ESD) and endoscopic submucosal excavation (ESE) have been widely used and have gradually become the main endoscopic treatment for gastrointestinal mucosal and submucosal lesions. Whether antibiotics are necessary for fever after gastric ESD and ESE remain unclear. The aim of this study was to analyse the value of using antibiotics in patients without perforation after ESD or ESE with fever.Methods: In this retrospective study, patients with fever without perforation after ESD or ESE from January 2014 to January 2019 were included and divided into 2 groups: the antibiotic group and the non-antibiotic group. Fever and hospitalization time were compared between the 2 groups after propensity score matching.Results: Overall, 253 patients meeting the inclusion criteria were enrolled in the present study, with 186 patients in the non-antibiotic group and 67 patients in the antibiotic group before matching, 55 patients in the non-antibiotic group and 55 patients in the antibiotic group after matching with all baseline characteristics balanced (p > 0.05). The duration of fever was not significantly different between the 2 groups (p = 0.12). However, the median hospitalization stay in the antibiotic group was longer than that in the non-antibiotic group (8 vs 7, p = 0.007).Conclusions: Antibiotics may be unnecessary for fever in patients without perforation and without serious co-morbidities after gastric ESD or ESE. • Microscopic spectroscopy imaging is explored for needle reflectance measurement. • This method measures bidirectional reflectance and is easy to be implemented. • Needle azimuth and specular reflection affect correlations to chlorophyll content. • The method can eliminate specular reflection from reflectance of conifer needles. Leaf reflectance is widely used to retrieve leaf chlorophyll content (Cab) and parameterize canopy radiative transfer models. Measurements of broadleaf reflectance are typically made by using integrating sphere devices, but the approach is generally limited in conifer needle measurements due to the narrow needle coverage relative to the sample port of the integrating sphere. In this study, we proposed a method to measure the bidirectional reflectance factor (BRF) of needles by integrating a hyperspectral imaging spectrometer and an optical microscope. Pure needle pixels can be easily extracted from hyperspectral images after microscope magnification. The method was first validated by the narrow strips of broad leaves as proxies of needles, resulting in the difference between the measured BRF of strips and the BRF of whole broad leaves in the visible bands (400–700 nm) and in the near-infrared bands (700–1000 nm) being 0.0043 and 0.019, respectively. The method was also indirectly verified by tracking the variation in the natural needle BRF with its Cab, resulting in the change in BRF being consistent with the change in Cab. In addition, linear relationships between the needle Cab and the classic vegetation indices (i.e. , the red-edge simple ratio and normalized difference vegetation index) calculated from the BRF were found. Apart from the above, we found that (1) the BRF of the abaxial side has a discrepancy with that of the adaxial side of needles, (2) the BRF of a needle varies with its azimuth, hence affecting the correlation between vegetation indices and Cab, and (3) filtering the specular reflection on the microsurfaces of the needle could enhance the correlation between the vegetation indices and needle Cab. The method is expected to accurately measure the BRF of needles and hence to improve the applications of needle optical properties in plant sciences, ecology, and remote sensing fields. The clumping effect is the main issue causing the heterogeneity in vegetation canopies and the underestimation of leaf area index (LAI) obtained using indirect measurement methods. Significant efforts have been exerted to correct for the clumping effect and derive the true LAI. Recent research has shown that the fractal dimension (FD) is directly related to the clumping effect of foliage, yet practical methods are needed to calculate field estimates. Considering that widely used LAI applications such as digital hemispherical photography (DHP), tracing radiation and architecture of canopies (TRAC), and digital cover photography (DCP) estimate LAI with one-dimensional (1D) gap probability and gap size data, we propose a method to correct for the clumping effect using 1D FD. Resulting formulae describing the relationship between LAI, CI, and 1D FD were based on the box-counting method (BCM) and a binomial distribution model. Sixty-four simulated scenes including four RAdiation transfer Model Intercomparison (RAMI) actual canopies and field measurements from nine plots (four orchard plots and five coniferous forest plots) were used to validate the novel method. Results showed good agreement with reference LAI values for simulated scenes (R2 = 0.96 and RMSE = 0.35). The 1DFD method generated higher LAI estimates compared with the LAI measured using TRAC at the four orchard plots especially at high canopy closure, while its results were more consistent with LAI obtained by litter collection than those of comparable methods at coniferous forest plots (bias from −13.5% to 9.9% for DCP images, from −3.0% to 19.7% for DHP images, and from −3.8% to 17.0% for TRAC transects). Our validation efforts indicate that the method proposed herein corrects for the clumping effect of vegetated canopies more effectively with DCP images, DHP images, and TRAC measurement when compared with traditional indirect optical methods. The 1DFD method is expected to improve indirect measurement accuracy of LAI. • One-dimensional fractal dimension (1D FD) is explored for LAI field measurement. • A formula between 1D FD, LAI, and CI (clumping index) is rigorously deduced. • LAI and CI can be estimated using the formula, 1D FD, and gap fraction. • The new method corrects for clumping effect more properly than classical methods. • The novel method is suitable for many widely used LAI-measurement instruments.
