4. Diagnostic Performance of the CBCL-Attention Problem Scale as a Screening Measure in a Sample of Brazilian Children with ADHD

Abstract

Objective: To evaluate the diagnostic performance of the Attention Problem Scale of the Child Behavior Checklist (CBCL-APS) for the screening of Attention-Deficit/Hyperactivity Disorder (ADHD) in a sample of Brazilian children and adolescents. Methods: The CBCL-APS was given to 763 children and adolescents. Child psychiatrists using DSM-IV criteria confirmed the clinical diagnoses. Diagnostic performance was evaluated through Receiver-Operating Characteristic (ROC) curves. Results: Only moderate areas under the curve (AUC) were found for the general sample (AUC = 0.79; 95%CI = 0.76-0.82), and for the subsample of referred patients (AUC = 0.78; 95%CI = 0.74-0.82). The subsample of patients with ADHD of the combined type presented the largest AUC (AUC = 0.85; 95%CI = 0.82-0.88). Conclusion: Our findings concur with previous studies of different cultures demonstrating adequate diagnostic performance of the CBCL-APS for the screening of ADHD, especially of the combined type. Abstract

Background: Swimming is a leisure activity with great potential for promoting health and development. This articleexamines the proportion of children and adolescents in Germany who are unable to swim, taking into account age, gender, socioeconomic status (SES) and migration background.Methods: Data were obtained from the first follow-up of the German Health Interview and Examination Survey for Children and Adolescents (KiGGS Wave 1), conducted by the Robert Koch Institute (RKI) from 2009 to 2012. Information regarding the swimming ability of 5- to 17-year-olds (n=9,750) was collected bytelephone interviews. The SES index is a composite measure of parentseducation, occupational status and income. Migration background refers to country of birth and nationality of both parents and child.Results: In total, 14.5% of 5- to 17-year-olds in Germany were unable to swim. At pre- and elementary school age, prevalence was considerably higher, in adolescence much lower. Those who wereable to swim acquired the ability at just over 6 years of age on average. Among children of primary school age, fewer boys than girlscould swim. Girls also learnedto swim 4 months earlier on average. Children and adolescents from low-SES families were more likely to be unable to swim than their peers with a high SES (OR=5.95; 95% CI=3.74-9.47). A two-sided migration background (both parents, or the child and one parent, immigrated) was also associated with an elevated odds of being unable to swim (OR=2.39; 95% CI=1.63-3.50).Conclusions: The KiGGS data show that a substantial proportion of children and adolescents in Germany are unable to swim. Initiatives promoting swimming ability should focus on socially disadvantaged children and adolescents and those with a two-sided migration background.KEY WORDS: Swimming Ability, Drowning, Socioeconomic Status, Migration Background, Health Inequalities

Abstract

BACKGROUND: In epidemiologic studies, standardised measurement of socio-demographic and employment-related factors is becoming increasingly important, as variables such as gender, age, education or employment status are factors influencing health and disease risks. AIMS: The article gives an overview of the scientific background and assessment of socio-demographic factors in the German National Cohort Study. In addition, the distribution of individual characteristics in the cohort as well as relationships with health-related measures are presented by way of example. MATERIAL AND METHODS: The analysis is based on the data of the first half of the baseline survey (n = 101,724). On this basis, we present the distribution of key socio-demographic characteristics and analyse relationships with exemplary selected health indicators (body mass index, self-reported health) to assess the validity of socio-demographic data measurements. RESULTS: On average, study participants were 52.0 years old (SD = 12.4). Of the participants, 53.6% were women, 54.3% had high education, 60.1% were married and 72% were employed while 3.4% were unemployed. Well-established correlations between socio-demographic factors and health could be reproduced with the German National Cohort data. For example, low education, old age and unemployment were associated with an increased prevalence of obesity and poor self-reported health. DISCUSSION: The German National Cohort provides a comprehensive measurement of socio-demographic characteristics. Combined with a wide range of health data and the longitudinal measurements available in the future, this opens up new opportunities for health science and social epidemiological research in Germany.
