Objective To explore the value of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid （Gd-EOB-DTPA） enhanced MRI in evaluating the degree of esophageal varices. Methods Clinical data of 114 patients diagnosed with hepatitis B cirrhosis and Gd-EOB-DTPA enhanced MRI examination through clinical comprehensive diagnosis at Jiangsu Provincical Hospital of Chinese Medicine from September 2021 to June 2022 was retrospective analyzed. The patients were divided into non-varicose vein group, small varicose vein group and large varicose vein group based on the diagnostic threshold of 3 mm esophageal vein width. The signal intensity (SI) of liver, spleen and erector spinae muscles were measured in each group during the plain scan and hepatobiliary phase (20 minutes after enhancement). The relative liver enhancement (RLE), contrast uptake index （CUI）, and liver-to-spleen contrast index (LSI) were calculated. The differences in indicators between each group were compared, and the receiver operating characteristic curve (ROC) were drawn. The area under the curve (AUC) was used to evaluate the diagnostic efficacy of each indicator in diagnosing varicose veins. Results There was a significant difference in RLE, CUI, and LSI between the non-varicose vein group and the large varicose vein group (P＜0.05), while there was no significant difference in RLE, CUI, and LSI between the small varicose vein group, as well as between small varicose vein group and large varicose vein group (P>0.05). The AUC of RLE, CUI and LSI for diagnosing large varicose veins was 0.668 (95%CI: 0.558-0.793), 0.676 (95%CI: 0.567-0.804) and 0.685 (95%CI: 0.550-0.780), respectively. Conclusion Gd-EOB-DTPA enhanced MRI has certain value in evaluating the degree of esophageal varices.

Since December, 2019, Wuhan, China, has experienced an outbreak of coronavirus disease 2019 (COVID-19). We conducted a retrospective study of COVID-19 inpatients in Wuhan Pulmonary Hospital (Wuhan, China) from January 1 to February 29, 2020. The subjects were divided into four groups due to different treatment regimes. We used the Kaplan–Meier method to determine the cumulative rates of in-hospital death and the Cox proportional hazard model to calculate the risk factors and corresponding hazard ratios. A total of 185 patients were included in this study. The median age of the patients was 62 years, including 94 men and 91 women. Kaplan–Meier analysis demonstrated that mortality was higher in older patients, higher in men, and lower in the low-flow oxygen therapy group. Body mass index (BMI) had no influence on mortality, as well as high flow oxygen therapy, Lopinavir–ritonavir (LPV/r) therapy, and the interferon-alpha add LPV/r therapy. Cox proportional hazard regression confirmed that the low flow oxygen therapy was independent protective factor for in-hospital death after adjusting for age, gender, and BMI. In conclusion, the mortality was higher in older patients, higher in men, and lower in the low-flow oxygen therapy group. BMI had no influence on mortality, as well as high flow oxygen therapy, LPV/r therapy, and interferon-alpha add LPV/r therapy.

This paper explores the argumentative strategies employed by Daoism and Buddhism in their debates during the Tang dynasty (618–907). Despite being significant in Chinese intellectual history, these debates, particularly the strategies behind them, remain understudied, and two gaps remain. First, the argumentative strategies of the two teachings are yet to be fully examined. Secondly, few studies have examined why Daoism was consistently defeated in the debates. This paper aims to address the two gaps. The paper first examines the argumentative strategies of the two teachings. Overall, the strategies employed by Buddhists were far superior to those employed by Daoists. Buddhists excelled in exposing contradictions and illogical reasoning in their opponents' arguments. In contrast, the strategies employed by Daoists were often weak, superficial and aimless, primarily serving to undermine Buddhism, whether as a branch of Daoism or a foreign religion. The second part of the paper identifies three reasons for the disparities in argumentative strategies between the two teachings. Firstly, Buddhist doctrines and scriptures underscored the significance of debates, whereas Daoism, both from philosophical and religious perspectives, often overlooked or even discouraged their significance. Secondly, Buddhists actively learned from previous sources and debates, while Daoists displayed limited inclination to do so. Thirdly, Buddhists possessed extensive knowledge of various schools, including Daoism, while Daoists exhibited limited proficiency beyond their own tradition. [ABSTRACT FROM AUTHOR]

为了拓展南极磷虾油的应用范围，采用喷雾干燥法制备南极磷虾油微囊粉。通过对比微囊化效果确定最佳壁材、乳化剂及工艺条件，并对最终产品各项性能指标进行测定。结果表明，制备南极磷虾油微囊粉的最佳条件为：最佳壁材组合为麦芽糊精（75%）与酪蛋白酸钠（2%），最佳乳化剂为单甘酯，单甘酯添加量2.0%，均质压力40 MPa,喷雾干燥进风温度180 ℃、出风温度90 ℃。在最佳条件下制得的南极磷虾油微囊粉表面含油率为0.68%，包埋率达到95%以上，载油量约为20%。南极磷虾油微囊粉为淡红色，具有一定的虾油味道，流动性良好，没有出现结块现象，并且复水后能够快速溶解，没有分层或漂油现象。 In order to expand the application of Antarctic krill oil, Antarctic krill oil microencapsulated powder was prepared by spray drying method. The best wall materials, emulsifier and process conditions were determined by comparing the microencapsulation effect. In addition, the performance indexes of the final product were measured. The results showed that the optimal preparation conditions of Antarctic krill oil microencapsulated powder were obtained as follows: maltodextrin (75%) and sodium caseinate (2%) used as the best wall materials combination, monoglyceride used as the best emulsifier, dosage of emulsifier 2.0%, homogenization pressure 40 MPa, inlet air temperature of spray drier 180 ℃, and outlet air temperature 90 ℃. Under the optimal conditions, the surface oil content of the Antarctic krill oil microencapsulated powder obtained was 0.68%, the embedding rate was over 95% and the oil loading capacity was about 20%. The microencapsulated powder was light red, with a certain smell of krill oil, good flowability, no agglomeration, and could be dissolved quickly after rehydration, without any delamination or oil bleaching phenomenon.

Abstract Background Hypoxia is a major contributor to global kidney diseases. Targeting hypoxia is a promising therapeutic option against both acute kidney injury and chronic kidney disease; however, an effective strategy that can achieve simultaneous targeted kidney hypoxia imaging and therapy has yet to be established. Herein, we fabricated a unique nano-sized hypoxia-sensitive coassembly (Pc/C5A@EVs) via molecular recognition and self-assembly, which is composed of the macrocyclic amphiphile C5A, the commercial dye sulfonated aluminum phthalocyanine (Pc) and mesenchymal stem cell-excreted extracellular vesicles (MSC-EVs). Results In murine models of unilateral or bilateral ischemia/reperfusion injury, MSC-EVs protected the Pc/C5A complex from immune metabolism, prolonged the circulation time of the complex, and specifically led Pc/C5A to hypoxic kidneys via surface integrin receptor α4β1 and αLβ2, where Pc/C5A released the near-infrared fluorescence of Pc and achieved enhanced hypoxia-sensitive imaging. Meanwhile, the coassembly significantly recovered kidney function by attenuating cell apoptosis, inhibiting the progression of renal fibrosis and reducing tubulointerstitial inflammation. Mechanistically, the Pc/C5A coassembly induced M1-to-M2 macrophage transition by inhibiting the HIF-1α expression in hypoxic renal tubular epithelial cells (TECs) and downstream NF-κB signaling pathway to exert their regenerative effects. Conclusion This synergetic nanoscale coassembly with great translational potential provides a novel strategy for precise kidney hypoxia diagnosis and efficient kidney injury treatment. Furthermore, our strategy of coassembling exogenous macrocyclic receptors with endogenous cell-derived membranous structures may offer a functional platform to address multiple clinical needs. Addressing the impact of near-field effects in the Controlled Source Electromagnetic Method(CSEM) has long been a focal point in the realm of geophysical exploration. Therefore, we propose a deep learning-based near-field correction method for controlled-source electromagnetic methods. Initially, diverse datasets for a layered geologic model are generated through forward simulation. Building upon the characteristics of near-field effects, a deep learning network utilizing LSTM-CNN is meticulously constructed. Multiple experiments are executed to scrutinize the network's effectiveness in mitigating near-field effects and its resilience against noise. Following this, the proposed method is applied to actual CSEM data to validate its applicability in practice. The method is subsequently tested on measured CSEM data, confirming its practical efficacy. Results from experiments indicate that, for theoretical data, the LSTM-CNN network-trained data closely aligns with simulated data, showcasing a significant improvement. Moreover, when applied to measured data, the method eradicates false high-resistance anomalies at lower frequencies. In conclusion, this deep learning-based correction method proficiently eliminates the influence of near-field effects in the CSEM, delivering practical application benefits that more accurately reflect the authentic geologic structure. BackgroundCurrently, the SARS-CoV-2 promptly spread across China and around the world. However, there are controversies about whether preexisting chronic kidney disease (CKD) and acute kidney injury complication (AKI) are involved in the COVID-19 pandemic.MeasurementsStudies reported the kidney outcomes in different severity of COVID-19 were included in this study. Standardized mean differences or odds ratios were calculated by employing Review Manager meta-analysis software.ResultsThirty-six trials were included in this systematic review with a total of 6395 COVID-19 patients. The overall effects indicated that preexisting CKD (OR = 3.28), complication of AKI (OR = 11.02), serum creatinine (SMD = 0.68), abnormal serum creatinine (OR = 4.86), blood urea nitrogen (SMD = 1.95), abnormal blood urea nitrogen (OR = 6.53), received continuous renal replacement therapy (CRRT) (OR = 23.63) were significantly increased in severe group than that in nonsevere group. Additionally, the complication of AKI (OR = 13.92) and blood urea nitrogen (SMD = 1.18) were remarkably elevated in the critical group than that in the severe group.ConclusionsCKD and AKI are susceptible to occur in patients with severe COVID-19. CRRT is applied frequently in severe COVID-19 patients than that in nonsevere COVID-19 patients. The risk of AKI is higher in the critical group than that in the severe group.

Abstract Background Stroke-induced immunodeficiency syndrome (SIDS) is regarded as a protective mechanism for secondary inflammatory injury as well as a contributor to infection complications. Although stroke-induced hyperactivation of the sympathetic system is proved to facilitate SIDS, the involved endogenous factors and pathways are largely elusive. In this study, we aim to investigate the function of beta-arrestin-2 (ARRB2) in the sympathetic-mediated SIDS. Methods Splenic ARRB2 expression and the sympathetic system activity were detected after establishing transient models of middle cerebral artery occlusion (MCAO). In addition, a correlation between ARRB2 expression and the sympathetic system activity was analyzed using a linear correlation analysis. Any SIDS reflected in monocyte dysfunction was investigated by measuring inflammatory cytokine secretion and neurological deficit scores and infarct volume were tested to assess neurological outcome. Further, ARRB2 expression in the monocytes was knocked down in vitro by siRNAs. Following the stimulation of noradrenaline and lipopolysaccharide, cytokine secretion and the nuclear factor-κB (NF-κB) pathway were evaluated to gain insight into the mechanisms related to the contribution of ARRB2 to adrenergic-induced monocyte dysfunction. Results Splenic ARRB2 expression was significantly increased after stroke and also showed a significant positive correlation with the sympathetic system activity. Stroke-induced monocyte dysfunction resulted in an increase of the interleukin-10 (IL-10) level as well as a decrease of the interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) levels. Also, blockade of adrenergic-activity significantly reversed these cytokine levels, and blockade of adrenergic-activity improved stroke-induced neurological results. However, the improved neurological results had no significant correlation with ARRB2 expression. Furthermore, the in vitro results showed that the deficiency of ARRB2 dramatically repealed adrenergic-induced monocyte dysfunction and the inhibition of NF-κB signaling phosphorylation activity. Conclusions ARRB2 is implicated in the sympathetic-triggered SIDS, in particular, monocyte dysfunction after stroke. Accordingly, ARRB2 may be a promising therapeutic target for the immunological management of stroke in a clinic.
