Previous studies have mostly focused on sporadic case reports and simulated high-altitude hypoxia experiments. This aim of this study was to explore the proteomic and morphological changes of ocular tissue in mice at real altitude environment.Methods: In this study, mice were flown from Chengdu (elevation: 500 m) to Lhasa (elevation: 3600 m). After exposure for 1day, 3, 6, 10, 20, 30, and 40days, the mice were euthanatized to obtain blood and ocular tissue. Serological tests, ocular pathological examinations, integral ocular proteomics analysis, and Western blot were conducted.Results: We focused on acute phase (1– 3 days) and chronic phase (> 30 days) during high-altitude acclimatization. Serum interleukin-1 was increased at 3 days, while superoxide dismutase, interleukin-6, and tumor necrosis factor-α showed no statistical changes. H&E staining demonstrated that the cornea was edematous at 3 days and exhibited slower proliferation at 30 days. The choroid showed a consistently significant thickening, while there existed no noticeable changes in retinal thickness. Overall, 4073 proteins were identified, among which 71 and 119 proteins were detected to have significant difference at 3 days and 40 days when compared with the control group. Functional enrichment analysis found the differentiated proteins at 3 days exposure functionally related with response to radiation, dephosphorylation, negative regulation of cell adhesion, and erythrocyte homeostasis. Moreover, the differential profiles of the proteins at 40 days exposure exhibited changes of regulation of complement activation, regulation of protein activation cascade, regulation of humoral immune response, second-messenger-mediated signaling, regulation of leukocyte activation, and cellular iron homeostasis. Interestingly, we found the ocular proteins with lactylation modification were increased along high-altitude adaptation.Conclusion: This is the first work reporting the ocular proteomic and morphological changes at real high-altitude environment. This study was designed to explore the association between the preoperative low T3 syndrome and cardiac surgery-associated acute kidney injury (CSA-AKI).Methods: This was a retrospective single-center study. Data on 784 patients undergoing elective coronary artery bypass grafting (CABG) or valve surgery were collected from January 2016 to July 2019. AKI was defined according to Kidney Disease: Improving Global Outcomes guidelines. The effect of preoperative low T3 syndrome (fT3 < 3.5pmol/L) on the risk of the postoperative AKI was analyzed in a logistic regression model.Results: There were 171 (21.8%) patients developing AKI. Preoperative T3 and FT3 levels were lower in patients with AKI than in those without AKI (P < 0.001). The incidence of postoperative AKI was higher in patients with low T3 syndrome than in those without (31.0% vs 19.8%; P = 0.003). Multivariate logistic regression analysis showed that low T3 syndrome was an independent risk factor for CSA-AKI patients (OR = 1.609, 95% CI: 1.033– 2.504; P = 0.035), after adjusting for confounding factors, such as age, albumin, and uric acid. Subgroup analyses showed that preoperative low T3 syndrome also increased incidence of CSA-AKI in those with high risk factors, such as age ≧60 yrs (OR: 1.891, 95% CI: 1.183– 3.022, P = 0.008), hypertension (OR: 2.104, 95% CI: 1.218– 3.3.635, P = 0.008), and hyperuricemia (OR: 2.052, 95% CI: 1.037– 4.06, P = 0.039).Conclusion: Low T3 syndrome independently increases the risk of CSA-AKI. Patients with low T3 syndrome should be considered at higher risk and be evaluated before cardiac surgery.Keywords: acute kidney injury, cardiac surgery, low triiodothyronine syndrome, risk factor

Abstract

The synthesis and twin polymerization (TP) of Si(OCH2py)4 (3a, py=2-cC5H4N; 3b, py=3-cC5H4N; 3c, py=4-cC5H4N) is discussed. The solid state structures of 3b, c were confirmed by single-crystal X-ray crystallography showing non-conventional H-bonding, forming 2D chains (3b) or 3D networks (3c). Thermally induced TP of 3a–c and their simultaneous polymerization with 2,2‘-spiro-bi[4H-1,3,2-benzodioxasiline] (4) is described. The resulting hybrid materials were characterized by 1H, 13C{1H}, and 29Si{1H} CP MAS NMR spectroscopy confirming the transformation of the SiOCH2 moieties into CH2 groups enabling the formation of the respective polymers. These results were supported by HAADF-STEM studies, displaying micro-structuring. Nitrogen-containing porous carbon materials C_1–C_3 show surface areas of 1300 and 1700 m2g-1, large pore volumes between 0.6–1.2 cm3g-1, and nitrogen contents of up to 3.1 at-%. X-ray photoemission spectroscopy reveal that pyrrolic, pyridine, and pyridone nitrogen atoms are present. If equimolar amounts of 3a–c and 4 are simultaneously polymerized in the presence of [Pd(OAc)2] (5), then the Pd nanoparticle-decorated material Pd@C_3 (900 m2g-1) was obtained, which showed k values of -0.083 and -0.066 min-1 in the reduction of methylene blue and methyl orange, proving the accessibility of the Pd NPs.

Abstract

The synthesis and characterization of twin monomers [Ti(OCH2-2-MeO-C6H4)4(HOCH2-2-MeO-C6H4)]2 (3) and [Al(OCH2-2-MeO-C6H4)3]4 (5) by reacting HOCH2-2-MeO-C6H4 (1) with Ti(OiPr)4 (2), or 1 with AlMe3 (4) is discussed. Single crystal X-ray structure analysis of 3 shows a dimeric structure with two alkoxide ligands bridging the titanium ions, while the others are terminal bonded. The respective phenolic resin / metal oxide hybrid materials HM_Ti and HM_Al were obtained in moderate (HM_Ti) to excellent (HM_Al) yields using typical base-catalyzed twin polymerization conditions (230 °C, 2 h). Nuclear magnetic resonance and infrared spectroscopy as well as scanning electron microscopy and scanning transmission electron microscopy combined with energ-dispersive X-ray spectroscopy proved the formation of inorganic–organic hybrid materials consisting of resin and MxOy materials (HM_Ti, TiO2; HM_Al, Al2O3) containing interpenetrating phase nano-domains with sizes of < 5 nm, as is charcteristic for twin polymerization processes. Oxidation of HM_Ti and HM_Al yielded the respective oxide materials Ox_Ti (TiO2) and Ox_Al (Al2O3), which possess low surface areas of ABET = 53 m2/g and 76 m2/g, respectively.

Abstract

Background: Contemporary management of patients with synchronous colorectal cancer and liver metastases is complex. The aim of this project was to provide a practical framework for care of patients with synchronous colorectal cancer and liver metastases, with a focus on terminology, diagnosis, and management. Methods: This project was a multiorganizational, multidisciplinary consensus. The consensus group produced statements which focused on terminology, diagnosis, and management. Statements were refined during an online Delphi process, and those with 70 per cent agreement or above were reviewed at a final meeting. Iterations of the report were shared by electronic mail to arrive at a final agreed document comprising 12 key statements. Results: Synchronous liver metastases are those detected at the time of presentation of the primary tumour. The term 'early metachronous metastases' applies to those absent at presentation but detected within 12 months of diagnosis of the primary tumour, the term 'late metachronous metastases' applies to those detected after 12 months. 'Disappearing metastases' applies to lesions that are no longer detectable on MRI after systemic chemotherapy. Guidance was provided on the recommended composition of tumour boards, and clinical assessment in emergency and elective settings. The consensus focused on treatment pathways, including systemic chemotherapy, synchronous surgery, and the staged approach with either colorectal or liver-directed surgery as first step. Management of pulmonary metastases and the role of minimally invasive surgery was discussed. Conclusion: The recommendations of this contemporary consensus provide information of practical value to clinicians managing patients with synchronous colorectal cancer and liver metastases.
