Abstract

According to two remarkable theorems of Nyman and B\'aez-Duarte, the Riemann hypothesis is equivalent to a simply-stated criterion concerning least-squares approximation. In carrying out computations related to this criterion, we have observed a curious phenomenon: for no apparent reason, at least the first billion entries of a certain infinite triangular matrix associated to the Riemann zeta function are all positive. In this article we describe the background leading to this observation, and make a conjecture.

Abstract

Objectives: The Prospective Randomized On-X Mechanical Prosthesis Versus St Jude Medical Mechanical Prosthesis Evaluation (PROSE) trial purpose was to investigate whether a current-generation mechanical prosthesis (On-X; On-X Life Technologies/Artivion Inc) reduced the incidence of thromboembolic-related complications compared with a previous-generation mechanical prosthesis (St Jude Medical Mechanical Prosthesis; Abbott/St Jude Medical). This second report documents the valve-related complications by individual prostheses and by Western and Developing populations. Methods: The PROSE trial study was conducted in 28 worldwide centers and incorporated 855 subjects randomized between 2003 and 2016. The study enrollment was discontinued on August 31, 2016. The study protocol, and analyses of 10 demographic variables and 24 risk factors were published in detail in 2021. Results: The total patient population (N = 855) included patients receiving an On-X valve (n = 462) and a St Jude Medical valve (n = 393). The overall freedom evaluation showed no differences at 5 years between the prostheses for thromboembolism or for valve thrombosis. There were also no differences in mortality. There were several differences between Developing and Western populations. The freedom relations at 5 years for mortality favored Western over Developing populations. Valve thrombosis was differentiated by position and site: aortic < mitral (P = .007) and Western < Developing (P = .005). In the mitral position there were no cases in Western populations, whereas there were 8 in Developing populations (P = .217). Conclusions: The On-X valve and St Jude Medical valve performed equally well in the study with no differences found. The only differentiation occurred with valve thrombosis in the mitral position more than the aortic position and occurring in Developing more than Western populations. Abstract

Objectives: The PROSE trial purpose is to investigate whether the incidence of thromboembolic—related complications is reduced with a current generation mechanical prosthesis (On-X Life Technologies/CryoLife Inc.—On-X) compared with a previous generation mechanical prosthesis (St Jude Medical—SJM). The primary purpose of the initial report is to document the preoperative demographics, and the preoperative and operative risk factors by individual prosthesis and by Western and Developing populations. Methods: The PROSE study was conducted in 28 worldwide centres and incorporated 855 subjects randomized between 2003 and 2016. The study enrollment was discontinued on August 31, 2016. The preoperative demographics incorporated age, gender, functional class, etiology, prosthetic degeneration, primary rhythm, primary valve lesion, weight, height, BSA and BMI. The preoperative and operative evaluation incorporated 24 risk factors. Results: The total patient population (855) incorporated On-X population (462) and the St Jude Medical population (393). There was no significant difference of any of the preoperative demographics between the On-X and SJM groups. The preoperative and operative risk factors evaluation showed there was no significant difference between the On-X and St Jude Medical populations. The preoperative and operative risk factors by valve position (aortic and mitral) also documented no differentiation. The dominant preoperative demographics of the Western world population were older age, male gender, sinus rhythm, aortic stenosis, congenital aortic lesion, and mitral regurgitation. The dominant demographics of the Developing world population were rheumatic etiology, atrial fibrillation, aortic regurgitation, mixed aortic lesions, mitral stenosis and mixed mitral lesions. The Developing world group had only one significant risk factor, congestive heart failure. The majority of the preoperative and operative risk factors were significant in the Western world

A study was conducted to prepare oxetanone 1233A, a novel category of natural products, from 3-methylglutaric anhydride. 3-methylglutaric anhydride was synthesized under Heathcock protocol while optical rotations were carried out at 20 degrees C. Abstract

alpha,beta-Unsaturated esters or nitroalkenes were reacted with oxazoline N-oxide to produce the corresponding adducts. The high regio- and stereoselectivities of the adducts were found to be a function of the substitution pattern on the dipolarophile. Inversion of regioselectivity resulted from the switching from a 1,2-disubstituted to a 1,1-disubstituted alpha,beta-unsaturated ester.

Condensation of the corresponding orthoesters having (+)-3-(hydroxyamino)borneol results in the formation of a series of camphor-derived oxazoline N-oxides. These unstable dipole oxides are used without any purification in several (2+3)asymmetric cyclocondensations. The reactions involved with the oxides exhibit diastereo- and regioselectivities.

Abstract

In order to limit the hemodilution effect during cardiopulmonary bypass (CPB) in low weight animal patients, blood is often used as a component of the prime solution. This study was undertaken to evaluate the effects of the addition of blood to the prime solution on the hemodynamic and respiratory parameters during and following mitral valve replacement in dogs. Ten dogs were randomly assigned to receive either a hemic (HP), 75% blood component, or a nonhemic prime (NP) solution. The hemodilution was 5 +/- 4% and 25 +/- 10% for the HP and NP groups, respectively. Cardiopulmonary measurements were taken 20 minutes before initiating CPB, during CPB, and 20 min after termination. The hematocrit level, the hemoglobin concentration, and the arterial oxygen content were significantly lower in the NP group during and following CPB. However, the systemic oxygen transport index was not significantly different between the NP group (355 +/- 87 mL/min/m2) and the HP group (546 +/- 155 mL/min/m2) following CPB. Our study indicates that, in normal dogs undergoing hemodilution from a nonhemic prime solution, the cardiovascular function is able to maintain the systemic oxygen transport in the period immediately following mitral valve replacement., Afin de limiter l'hémodilution pendant la circulation extracorporelle (CPB) chez les animaux de faible poids, du sang peut être ajouté dans la solution d’amorçage. L'objectif de cette étude consistait à évaluer les effets de l'ajout de sang dans la solution d’amorçage sur les paramètres hémodynamiques et respiratoires pendant et après la réalisation d'un remplacement valvulaire mitral chez le chien. Chez 10 chiens subissant un remplacement valvulaire mitral, le circuit extracorporel était amorcé de façon aléatoire soit avec une solution contenant 75 % de sang (HP) soit avec une solution cristalloïde dépourvue de sang (NP). L'hémodilution résultante était respectivement de 5 ± 4 % et de 25 ± 10 % pour les groupes HP et NP. Les mesures cardiorespiratoires étaient effectuées 20 minutes avant l’établissement de la CPB, pendant la CPB et 20 minutes après l’arrêt de la CPB. L'hématocrite, la concentration en hémoglobine et le contenu en oxygène du sang artériel était significativement plus bas dans le groupe NP que dans le groupe HP pendant et après la CPB. Cependant, l'index du transport systémique d’oxygène n’était pas significativement différent entre le groupe NP (355 ± 87 mL/min/m2) et le groupe HP (546 ± 155 mL/min/m2) après la CPB. Notre étude indique que la fonction cardiovasculaire est capable de maintenir le transport systémique d’oxygène immédiatement après un remplacement valvulaire mitral chez des chiens normaux malgré l'hémodilution associée a un amorçage dépourvu de sang.

38. Optimal time-window duration for computing time / frequency representations of normal phonocardiograms in dogs

Abstract

The optimal duration of the time-window used to compute the time-frequency representation (spectrogram) of the phonocardiogram was studied in four dogs by using intracardiac and thoracic measurements of the PCG. The power and cross-spectrograms of the intracardiac and thoracic PCGs were computed using a fast Fourier transform algorithm and a sine-cosine window with 10 per cent decaying functions. A coherence spectrogram was also computed for each dog to study the linear relationship between the two signals and determine the optimal time-window duration. Results show that the optimal range of the time-window duration is between 16 and 32 ms. A time-window shorter than 16ms spreads out low-frequency components into the higher frequencies and generates a spectrographic representation with poor frequency resolution (=62·5 Hz). A window larger than 32 ms increases the frequency resolution but smears the spectrographic representation of the signal in the time domain and thus cannot correctly reflect the time-varying properties of the signal. In both cases, the amplitude of the coherence function computed between the left ventricular and the thoracic phonocardiograms is overestimated.

Abstract

This study examined the association between patient-reported anxiety and post-cardiac surgery mortality and major morbidity. Frailty ABC'S was a prospective multicenter cohort study of elderly patients undergoing cardiac surgery (coronary artery bypass surgery and/or valve repair or replacement) at 4 tertiary care hospitals between 2008 and 2009. Patients were evaluated a mean of 2 days preoperatively with the Hospital Anxiety and Depression Scale (HADS), a validated questionnaire assessing depression and anxiety in hospitalized patients. The primary predictor variable was high levels of anxiety, defined by HADS score ≥11. The main outcome measure was all-cause mortality or major morbidity (stroke, renal failure, prolonged ventilation, deep sternal wound infection, or reoperation) occurring during the index hospitalization. Multivariable logistic regression examined the association between high preoperative anxiety and all-cause mortality/major morbidity, adjusting for Society of Thoracic Surgeons (STS) predicted risk, age, gender, and depression symptoms. A total of 148 patients (mean age 75.8 ± 4.4 years; 34% women) completed the HADS-A. High levels of preoperative anxiety were present in 7% of patients. There were no differences in type of surgery and STS predicted risk across preoperative levels of anxiety. After adjusting for Society of Thoracic Surgeons predicted risk, age, gender, and symptoms of depression, preoperative anxiety remained independently predictive of postoperative mortality or major morbidity (OR 5.1; 95% CI 1.3, 20.2; p=0.02). In conclusion, although high levels of anxiety were present in a minority of patients anticipating cardiac surgery, this conferred a strong and independent heightened risk of mortality or major morbidity.
