17. Syphilitic polyradiculopathy in an HIVpositive man

Abstract

We report a case of syphilitic lumbosacral polyradiculopathy in an HIV-positive, 22-year-old bisexual man with a recent history of secondary syphilis treated with intramuscular penicillin. He presented with rapidly progressive pain and weakness, and muscle wasting in the legs. CSF was under increased pressure and showed a marked pleocytosis (1,130 cells/mm3), hypoglycorrhachia (19 mg/dl), and very elevated protein (1,000 mg/dl). Serum and CSF VDRL serologies were positive. In the legs, nerve conduction studies and needle EMG were consistent with an asymmetric lumbosacral polyradiculopathy with active denervation. His clinical state, CSF, and electrophysiologic studies all improved promptly and markedly after intravenous penicillin. This report documents an uncharacteristically aggressive case of neurosyphilis accompanied by marked changes in the CSF in an HIV-positive patient. While the immunologic effects of HIV and syphilis in combination are not yet fully understood, the cellular immunity defect associated with HIV may alter the natural history of syphilis in patients with concomitant infection, producing unusually aggressive forms or atypical presentations of neurosyphilis.

21. Conditions Associated With Huntington's Disease at Death: A Case-Control Study

Abstract

• To identify conditions associated with reduced survival in patients with Huntington's disease, we studied all 1978 US death certificates on which Huntington's disease (331.0 in the International Classification of Diseases, eighth revision) was listed. For each of the 495 cases identified, two control deaths were matched by age, race, sex, county, and year of death. Pneumonia, choking, nutritional deficiencies, and chronic skin ulcers were increased in cases relative to controls.

24. Thoracic Disk Herniation Associated With Chiropractic Spinal Manipulation

Abstract

TO THE EDITOR. —Chiropractic spinal manipulation has been associated with numerous neurologic complications, including brain-stem infarction,1 cervical spinal cord injury,2 and cauda equina syndrome.3-5 We describe a case of thoracic intervertebral disk herniation with resultant myelopathy following spinal manipulation. REPORT OF A CASE. —This previously healthy 55-year-old man visited a chiropractor with a complaint of low back pain. Aggressive rotatory and hyperextensional manipulation of the entire spine was performed, immediately following which he noted the onset of numbness and paresthesias in both feet. His sensory complaints progressed and, five days later, he presented with bandlike pain in the upper thoracic region and increased numbness to approximately the level of the umbilicus. Physical examination showed mild left leg weakness and a wide-based, unsteady gait. Deep-tendon reflexes in the lower extremities were brisk and symmetric. Babinski's sign was present on the left side and equivocal on the right side.

25. Treatable Causes of Meningomyeloradiculitis in Individuals Infected With Human Immunodeficiency Virus

Abstract

TO THE EDITOR. —Woolsey and colleagues1 reported an interesting case of mycobacterial meningomyeloradiculitis in a man infected with human immunodeficiency virus. Since submission of their report, several other treatable causes of this syndrome in individuals infected with the human immunodeficiency virus have been reported, including syphilis2,3 and leptomeningeal malignancy.4 Treatable causes of other myelopathic syndromes have also been reported.5,6Patients infected with human immunodeficiency virus who develop meningomyeloradiculitis have presented a remarkably uniform and distinct clinical profile.1-4,7-9 Generally, asymmetric leg weakness develops over several weeks to months, often associated with burning or aching pain in the legs and buttocks. There is frequently early sphincter involvement. Examination reveals flaccid paraparesis, areflexia in the affected extremities, and diffuse or patchy hypesthesias in the legs and saddle area. Babinski's sign is present in some cases. Involvement may progress through contiguous segments to thoracic or even cervical levels. Myelograms

33. Screening for Cushing's Syndrome

Abstract

To the Editor.—In the report by Hiramatsu et al1 describing a method of screening and monitoring patients with Cushing's syndrome on the basis of circumference measurements, the authors state that "a simple evaluation technique for central obesity has not been reported previously." However, several methods of measuring central or android obesity, similar to their central obesity index (COI), have been published.2-4 For example, Hartz and colleagues2 used the ratio of waist girth to hip girth (WHR) as an index of central obesity in their study of body fat distribution and diabetes in 15,532 women. This study by Hartz et al and other studies of body fat distribution and disease,3,5 suggested that an altered fat distribution may contribute to diabetes, whereas in Cushing's syndrome, fat distribution seems to be influenced by the disease.In addition, the suggestion by Hiramatsu et al that their COI be used

Abstract

To the Editor.—The findings reported by O'Sullivan in The Journal (1982; 248;949) that body weight in patients with normal gestational glucose tolerance test results is not related to diabetes have a serious flaw. With only 66 obese subjects in the normal control group, the author had only a 12% chance to detect a relationship between relative weight and diabetes. Therefore, it is not surprising that no relationship was found. This low chance of detection is based on the assumption that obese patients have a relative risk of 1.82 for diabetes. This assumption was obtained using the women who had previous transient gestational glucose intolerance. The incidence of diabetes in the obese women in this group (46.7%) was divided by the incidence of diabetes in the nonobese group (25.6%). Therefore, if obesity confers a risk of 1.82 for diabetes, there is a 88% chance that this risk level (or lower)
