OBJETIVO: Traçar o perfil clínico e demográfico de uma amostra de médicos em tratamento por dependência química, avaliar comorbidades psiquiátricas e conseqüências associadas ao consumo. MÉTODOS: Foram coletados dados de 198 médicos em tratamento ambulatorial por uso nocivo e dependência química, através de questionário elaborado pelos autores. RESULTADOS: A maioria de indivíduos foi do sexo masculino (87,8%), casados (60,1%), com idade média de 39,4 anos (desvio padrão=10,7 anos). Sessenta e seis por cento já tinham sido internados por causa do uso de álcool e/ou drogas. Setenta e nove por cento possuía residência médica e as especialidades mais envolvidas foram: clínica médica, anestesiologia e cirurgia. Comorbidade psiquiátrica foi diagnosticada em 27,7% (Eixo I do DSM-IV)¹ e em 6% (Eixo II do DSM-IV)¹. Quanto às substâncias consumidas, o mais freqüente foi uso associado de álcool e drogas (36,8%), seguido por uso isolado de álcool (34,3%) e uso isolado de drogas (28,3%). Observou-se o intervalo de 3,7 anos em média entre a identificação do uso problemático de substâncias e a procura de tratamento. Quanto à busca por tratamento, 30,3% o fizeram voluntariamente. Quanto aos problemas sociais e legais observou-se: desemprego no ano anterior em quase 1/3 da amostra; problemas no casamento ou separação (52%), envolvimento em acidentes automobilísticos (42%), problemas jurídicos (19%), problemas profissionais (84,8%) e 8,5% tiveram problemas junto aos Conselhos Regionais de Medicina. CONCLUSÃO: Os autores recomendam medidas assistenciais e preventivas para o problema. The misuse of alcohol and drugs among physicians is a common cause of malpractice, absenteeism and complaints to the Medical Councils. This problem demands more attention, because it entails risks to the population and to the physicians themselves. AIMS: To describe the clinical and demographic profile of a sample of physicians in treatment for alcohol and drug dependence also to evaluate psychiatric comorbidity and consumption-related consequences. METHODS: Data was collected from a sample of 198 physicians attending outpatient treatment by a questionnaire specifically designed for this study. RESULTS: Most of the subjects were men (87.8%), married (60.1%), with a mean age of 39.4 years (S.D. =10.7). Sixty-six per cent had already been in inpatient treatment for alcohol and drug misuse. Sixty-nine per cent were specialists practicing mainly: internal medicine, anaesthesiology and surgery. Psychiatric comorbidity was diagnosed in 27.7% for DSM-IV Axis I and in 6% for DSM-V Axis II. With regard to drugs the most frequent pattern was use of alcohol and drugs (36.8%), followed by exclusive use of alcohol (34.3%) and exclusive use of drugs (28.3%). It was observed that the mean interval between the identification of misuse of substances and the seeking of treatment was of 3.7 years. Thirty per cent tried to achieve treatment by themselves. The social and legal problems observed were: unemployment in the previous year (1/3 of the sample), marital problems and divorce (52%), car accidents (42%), legal problems (19%). 84.8% presented professional problems and 8.5% had problems within the Medical Councils. CONCLUSION: The researchers suggest supportive and preventive attitudes for this matter.

CONTEXT AND OBJECTIVE: Nurses are one of the largest groups of healthcare professionals sharing in patient care responsibilities, including caring for those who use and abuse psychoactive substances. The objective was to evaluate the theoretical-practical knowledge acquired by nurses in undergraduate and postgraduate studies and their perceptions about alcohol users. DESIGN AND SETTING: Quantitative, descriptive survey at Universidade Federal de São Paulo - Escola Paulista de Medicina and Hospital São Paulo. METHODS: The sample included nurses, students and nursing teachers. The survey included questions about sociodemographic characteristics; a nurses' attitudes and beliefs scale; and a questionnaire to identify formal nursing education on the use of alcohol and its consequences. RESULTS: 59.7% out of 319 volunteers were nurses, 22.7% were nursing teachers and 17.6% were nursing students. 70% of the participants had received little or no information on physical, family and social problems related to alcohol use; 87% had received little or no information on high risk related to specific segments of the population; 95% had received little or no information on nursing procedures for alcohol-abuse patients. CONCLUSION: Formal education regarding the use of alcohol and its consequences is limited, especially with regard to offering adequate care and management for patients who have problems with or are addicted to alcohol. CONTEXTO E OBJETIVO: Os enfermeiros constituem um dos maiores grupos de profissionais de saúde que compartilham a responsabilidade pela assistência de enfermagem prestada aos pacientes de modo geral, incluindo-se os usuários de substâncias psicoativas. O objetivo foi avaliar os conteúdos teóricos e práticos adquiridos na graduação e pós-graduação pelos enfermeiros e suas percepções sobre os alcoolistas. TIPO DE ESTUDO E LOCAL: Trata-se de um estudo quantitativo, descritivo, realizado na Universidade Federal de São Paulo - Escola Paulista de Medicina (Unifesp-EPM) e Hospital São Paulo. METODOS: A amostra foi composta por enfermeiros, estudantes e docentes de enfermagem. O instrumento para a coleta de dados foi composto por informações sócio-demográficas; Escala de Atitudes e Crenças dos Enfermeiros e um questionário para identificar os conteúdos sobre o uso do álcool e conseqüências durante a formação educacional. RESULTADOS: 86% (n = 319) sujeitos participaram do estudo (59,7% enfermeiros assistenciais, 22,7% docentes de enfermagem e 17,6% estudantes de enfermagem). 70% dos participantes receberam poucas ou nenhuma informação a respeito dos problemas orgânicos, familiares e sociais relacionados ao álcool; 87% receberam pouca ou nenhuma informação sobre as populações de risco ou específicas e 95% receberam pouca ou nenhuma informação referente às intervenções de enfermagem aos pacientes alcoolistas. CONCLUSÃO: A educação formal sobre o uso do álcool e suas conseqüências apresenta limitações, principalmente no âmbito da assistência, em oferecer cuidados adequados e nos manejos dos pacientes com problemas ou dependentes de álcool.

Objetivo: Exploramos os padrões de internação e as características clínicas e individuais de uma grande coorte de pacientes submetidos à internação psiquiátrica involuntária no Brasil (n=64.685). Métodos: Foram coletados dados do Ministério Público do Estado de São Paulo sobre todas as internações psiquiátricas involuntárias reportadas na cidade de São Paulo entre janeiro de 2003 e fevereiro de 2020. A taxa anual de internações psiquiátricas involuntárias foi calculada e foram produzidas estatísticas descritivas de suas características. Resultados: As internações psiquiátricas involuntárias aumentaram de 5,8 para 25,5 por 100.000 habitantes, com um aumento de oito vezes no primeiro período de 10 anos (2003-2013). A maioria das internações ocorreu em instituições públicas (86,6%), envolveu transtornos psicóticos no diagnóstico primário (26,1%), envolveu mais de um diagnóstico (83,7%) e durou menos de 7 dias (52,4%). A maioria dos pacientes tinha entre 18 e 39 anos e era solteira, sendo a reinternação relativamente comum (13%). Apesar de o motivo da internação estar ausente em muitas das notificações (44%), o risco de dano a si ou a terceiros foi o mais comum (68,5%). Conclusão: Esta é uma das maiores coortes de registros de internação psiquiátrica involuntária já explorada. Os achados se baseiam em evidências internacionais existentes sobre internações psiquiátricas involuntárias e mostram tendências recentes nas taxas de internação na maior cidade do Brasil. Objective: We explored hospitalization patterns and the clinical and individual characteristics of a large cohort of patients who underwent involuntary psychiatric hospitalization in Brazil (n=64,685). Methods: Data were collected from the District Attorney’s Office of the State of São Paulo (Ministério Público do Estado de São Paulo) on all involuntary psychiatric hospitalizations in the city of São Paulo between January 2003 and February 2020. The annual involuntary psychiatric hospitalization rate was calculated and descriptive statistics of the characteristics were produced. Results: Involuntary psychiatric hospitalizations increased from 5.8 to 25.5 per 100,000 population, with an eight­folder increase in the first 10­year period (2003­2013). The majority of admissions were to public institutions (86.6%), involved a psychotic disorder in the primary diagnosis (26.1%), involved more than one diagnosis (83.7%), and lasted less than 7 days (52.4%). The majority of the patients were aged 18 to 39 years and were single, and readmission was relatively common (13%). Although the reason for admission was missing in many reports (44%), the risk of harm to self or others was the most common (68.5%). Conclusion: This is one of the largest cohorts of involuntary psychiatric hospitalization records ever explored. These findings build upon existing international evidence about involuntary psychiatric hospitalizations and show recent trends in admission rates in the largest city in Brazil.
