Background. Fertility preservation (FP) prior to gonadotoxic cancer treatment is underutilized in the United States (US) due to limited access to care. Objective. We aimed to determine the utilization of a regional FP program in an underserved region in the Southeast US. Methods. We performed an institutional review board-approved retrospective cohort study of patients enrolled in the FP program at an academic tertiary medical center from 2014 to 2018. Results. Sixty-nine reproductive age patients were enrolled in the FP program, including 29 men and 40 women between 20 and 49 years of age. Cancer diagnoses among the enrolled patients included hematologic (n = 34), breast (n = 14), testicular (n = 11), reproductive tract (n = 7), and gastrointestinal (n = 3) malignancies. Patients with reproductive tract malignancies were found to have approximately 9.7 times greater odds of proceeding with FP than patients with hematologic malignancies (P Conclusion. Utilization of a regional FP program should be considered to increase access to care to this critical resource for patients desiring future family building. Patients with hematologic malignancy may face unique barriers to FP.

Conventional cytogenetic analysis of products of conception (POC) is of limited utility because of failed cultures, as well as microbial and maternal cell contamination (MCC). Optical genome mapping (OGM) is an emerging technology that has the potential to replace conventional cytogenetic methods. The use of OGM precludes the requirement for culturing (and related microbial contamination). However, a high percentage of MCC impedes a definitive diagnosis, which can be addressed by an additional pre-analytical quality control step that includes histological assessment of H&E stained slides from formalin-fixed paraffin embedded (FFPE) tissue with macro-dissection for chorionic villi to enrich fetal tissue component for single nucleotide polymorphism microarray (SNPM) analysis. To improve the diagnostic yield, an integrated workflow was devised that included MCC characterization of POC tissue, followed by OGM for MCC-negative cases or SNPM with histological assessment for MCC-positive cases. A result was obtained in 93% (29/31) of cases with a diagnostic yield of 45.1% (14/31) with the proposed workflow, compared to 9.6% (3/31) and 6.4% (2/31) with routine workflow, respectively. The integrated workflow with these technologies demonstrates the clinical utility and higher diagnostic yield in evaluating POC specimens.

Background Anxiety, anger, and sadness are related to elevated ambulatory blood pressure (ABP), yet it is unclear whether each emotion exerts unique effects. Moreover, an understanding of who might be most susceptible to the negative effects of these emotions is limited, with the trait tendency to experience them or one’s race as potential moderators. Purpose The study examined the potential for differential effects of momentary anxiety, anger, and sadness on ABP. The study assessed whether a trait tendency to experience these negative emotions and/or race (Black vs. non-Black) would moderate these relationships. Methods Participants (n = 153) completed trait anxiety, anger, and depressive symptoms measures at baseline. ABP was collected over two 24-hour periods 3–4 months apart. Momentary measures of anxiety, anger, and sadness were assessed via ecological momentary assessment (EMA) after each ABP reading. Results Momentary anxiety consistently predicted diastolic blood pressure but not systolic blood pressure. Momentary anger and sadness did not predict blood pressure (BP). Conditional effects were found with momentary anxiety and anger predicting elevated BP in those individuals with trait anxiety/anger at its mean. Trait anxiety and depression consistently predicted heightened BP in Black participants. Trait anger did not moderate the relationships between negative emotions and ABP. Conclusions Findings suggest that momentary anxiety and anger should be given attention as potential risk factors for hypertension and highlight the unique perspective of EMA methods. Black participants who were more anxious and depressed experienced heightened BP, with anxiety and depression providing possible intervention targets in improving racial disparities in cardiovascular health.

Ovarian hyperstimulation syndrome (OHSS) is a feared complication of controlled ovarian stimulation (COS) and can be associated with significant morbidity and mortality. Risk factors for OHSS include a history of OHSS, young age, low body mass index (BMI), polycystic ovary syndrome, elevated serum levels of anti-Müllerian hormone (AMH), large number of recruited follicles, elevated serum levels of estradiol, and higher gonadotropin doses during COS. However, OHSS may develop in patients with minimal risk factors. We present the case of a patient with minimal risk factors who developed severe late-onset OHSS in early pregnancy with liver dysfunction requiring hospitalization. After hospital discharge, her pregnancy resulted in a term live birth. ObjectiveProgestin therapy is the recommended fertility-sparing management of atypical endometrial hyperplasia or early-stage endometrial cancer in reproductive-aged women. Our objective was to evaluate disease relapse after progestin and metformin versus progestin therapy alone in patients with endometrial hyperplasia or cancer. Our secondary outcomes were disease remission, clinical pregnancy and live birth rate.MethodsA systematic review of the literature was conducted (MEDLINE, Web of Science, Cochrane Library, CINAHL, LILACS, clinicaltrials.gov) from inception to April 2021. Studies of reproductive-aged women with atypical endometrial hyperplasia or early endometrial cancer who received progestin and metformin or progestin alone for fertility-sparing management, were included in the review. Early endometrial cancer was defined as grade 1, stage 1 disease. Exclusion criteria included women with higher grade endometrial cancer and when conservative management was not for fertility-sparing purposes. Data are presented as odds ratios (ORs) and 95% confidence intervals (CIs) with fixed or random effects meta-analysis. Quality scoring was based on the Newcastle-Ottawa and Jadad scales.ResultsIn total, 271 reports were identified and six studies met the inclusion criteria. These studies included 621 women; 241 (38.8%) patients received combined therapy and 380 (61.2%) received progestin therapy alone. Relapse rates were lower for progestin and metformin than for progestin therapy alone (pooled OR 0.46, 95% CI 0.24 to 0.91, p=0.03). The remission rates were not different (pooled OR 1.35, 95% CI 0.91 to 2.00, p=0.14). Women who received progestin and metformin achieved pregnancy and live birth rates similar to those who received progestin therapy only (pooled OR 1.01, 95% CI 0.44 to 2.35, p=0.98; pooled OR 0.46, 95% CI 0.21 to 1.03, p=0.06).ConclusionFor reproductive-aged women with atypical endometrial hyperplasia or early endometrial cancer, progestin and metformin therapy compared with progestin therapy alone is associated with lower relapse rates, and similar remission, clinical pregnancy and live birth rates.PROSPERO registration numberCRD42020179069.disease remission

Objective This study examined whether recalling a social stress that evoked lonely feelings produces greater affective and blood pressure responses than recalling non-social stress or a relaxing event. Methods Young adults (n = 114) underwent one of three inductions: recalling a social stressor in which one felt lonely, recalling a non-social non-lonely stressor, and relaxation. Negative affect was assessed during baseline, induction, and recovery. Blood pressure was assessed continuously using a finger cuff. Results Both the lonely and non-lonely stress conditions reported increased negative affect following the induction, with neither condition having fully recovered to baseline levels by the end of the study. For blood pressure, during recovery, blood pressure increased linearly for the lonely stress condition, but remained steady for the other conditions. Conclusions Recalling a social stress evoking lonely feelings may be more harmful for cardiovascular health than recalling non-social stressors. Results extend work demonstrating that in vivo stressors involving social evaluation elicit larger stress responses compared to stressors without this component.

Introduction Bedding can aid in maintaining a comfortable thermal state in the sleep environment. Research is needed to bring scientific rigor to document these benefits and their potential for promoting better sleep. This study examined effects of a comforter designed for warm sleepers used at home. Methods Healthy adults (96% female, ages 23-74) who reported sleeping hot, experiencing night sweats, or having hot flashes during the night participated in a 6-week field study, using a pre-post intervention design. Intervention consisted of using a comforter designed for warm sleepers, made of 300 thread count viscose from bamboo fiber with Tencel/polyester fill. During the 3-week baseline period, participants used their regular bedding. During the 3-week intervention period, they used the comforter without a top sheet to have direct contact with the comforter. Sleep was measured objectively using SleepScore Max every night and by daily and pre-post self-report. Multilevel regression and paired t-tests were used to test for statistical significance. Results There were over 1,000 nights of tracked sleep across all participants. In the full sample (n=31), self-reported sleep (e.g., perceived sleep quality, feeling well-rested) but not objectively-measured sleep showed improvements. Therefore, and given that this type of product may provide the most benefit for sleepers in midlife or older, additional analyses were conducted for the subgroup of participants who were 45 years old and above (n=15). Results indicated increased time in bed (+11 minutes, p=.017), total sleep time (+12 minutes, p=.010), and deep sleep (+6 minutes, p=.005) during the intervention compared to baseline. Improvements also were observed in SleepScore, a measure of overall sleep quality (p=.002), and BodyScore, a measure of deep sleep (p=.002). Conclusion Self-reported but not objectively-measured sleep improved among healthy adults using a comforter designed for warm sleepers. However, looking specifically at those aged 45 and older, objectively-measured time in bed, total sleep time, deep sleep, and overall sleep quality all increased significantly when using the comforter compared to baseline. Introduction The myriad of anecdotal claims about the benefits of different pillow types may complicate consumers’ ability to find a pillow that can support better sleep. Services that provide consumers with personalized recommendations may address this problem. This study examined the sleep of research participants using pillows selected during an in-store pillow fitting process compared to using their original pillows. Methods Healthy adults (71% female, ages 21-59) who reported interest in a new pillow participated in a 9-week field study using a pre-post intervention design. The intervention consisted of being guided through the Mattress Firm in-store pillow fitting process to be paired with a pillow that provided the best fit and support based on body type and sleeping position preferences. During the 4-week baseline period, participants used their regular pillow. Once they were fit for their new pillow, participants slept with that pillow for 5 weeks, including a 1-week adjustment period. Sleep was measured objectively using SleepScore Max every night and by daily and pre-post self-report. Multilevel regression and paired t-tests and were used to test for statistical significance. Results There were 722 nights of tracked sleep across participants (n=17). Objective sleep measurements showed nightly improvement in SleepScore, a measure of objective sleep quality (p=.003), longer time in bed (+13 minutes, p=.034),longer sleep duration (+16 minutes, p=.010), a lower proportion of the night awake after falling asleep (p=.009), and better sleep maintenance (p=.009). In addition, participants got more deep sleep (+5.6 minutes, p=.017) and showed improved BodyScore, a measure of deep sleep (p=.031). Self-reported measures showed improvements in pillow comfort and overall comfort in bed; perceived ability to sleep through the night; perceived reduction in number of awakenings and amount of time awake at night; better overall sleep quality; and feeling more well-rested in the morning when using the fitted pillows compared to baseline (all ps Conclusion Objective and self-reported sleep improved among healthy adults using a pillow selected by an expert, in-person pillow fitting process. Introduction This study examined if a diffused fragrance used at home before bedtime would contribute to sleep improvement in a sample of healthy females. Existing evidence regarding the sleep-promoting properties of fragrances often has been anecdotal or based on clinical research, thereby limiting the generalizability and ecological validity of results. Methods 26 women with self-reported interest in air care to support healthy sleep participated in a 9-week field study. A within-subjects, counterbalanced intervention design was implemented, comparing 3 weeks of nightly product use to 3 weeks without using the product after a baseline period. Intervention consisted of the use of a fragrance diffuser and fragrance by Reckitt’s Scientific Platform Fragrance Research for at least an hour at the participants’ preferred settings in the room where they spent the most time before going to bed. Sleep was measured objectively with SleepScore Max every night. Self-report data were collected at bedtime, in the morning, and after each measurement period. Multilevel regression and paired t-tests were used to test for statistical significance. Results Across all participants there were 835 nights of tracked sleep. Participants (100% female, age 21 to 55, average 36 years old) showed improvement in both objective and perceived sleep during the intervention. Participants got more deep sleep, spent a greater proportion of the night in deep sleep, and had an improved BodyScore, a measure of deep sleep (ps Conclusion Using the fragrance diffuser before bed may contribute to improvement of many aspects of sleep within this study population of females without underlying sleep conditions. Purpose Geographic disparities for assisted reproductive technology (ART) continue to exist. Travel cost and time off work may create additional barriers for patients living remotely. Implementing telehealth can alleviate these barriers by reducing office visits. The aim of this study was to evaluate patient satisfaction with telehealth during ART. Methods This was a cross-sectional survey and retrospective cohort study. Patients living remotely who underwent ART utilizing telehealth between 2015 and 2018 at a single institution were selected for the telehealth group. The non-telehealth control group included randomly selected patients who underwent IVF at the same institution between 2015 and 2018. Demographic variables and treatment outcomes were obtained for both groups. A patient satisfaction questionnaire was distributed to telehealth patients. Statistical analysis using χ2 test was performed to compare ART outcomes between both groups. Results Ninety-seven control and 97 telehealth patients were included. For telehealth patients, the mean number of office visits and distance traveled was 2.9 (± 0.8 SD) and 143.1 miles (± 49.2 SD) respectively. 58.8% of patients completed the survey. 44/57 participants had an oocyte retrieval and 42/44 underwent embryo transfer. For those who completed the survey, the clinical pregnancy rate was 31/44 and the live birth rate was 25/44. There was no difference in treatment outcomes between telehealth compared to controls. 73% of patients were highly satisfied with telehealth. Conclusions Telehealth can improve access to ART in underserved areas and results in high patient satisfaction. Reproductive health providers could consider telehealth as a safe and efficacious tool to ameliorate geographic disparities.

Background Premature ovarian failure is a relatively common condition that affects 1–3% of adult women. Premature ovarian failure occurs when there is loss of ovarian function in women younger than 40 years of age. The causes are mostly iatrogenic or idiopathic. Amenorrhea and infertility are the most important clinical manifestations. So far, no therapeutic intervention has been proved effective in restoring fertility in patients with premature ovarian failure. Attempts to stimulate ovarian function through hormone manipulation typically prove unsuccessful, and patients usually resort to egg donation to achieve pregnancy. In our preclinical work, intraovarian administration of human bone marrow–derived mesenchymal stem cells was able to restore ovarian hormone production, reactivate folliculogenesis, and reverse infertility in a chemotherapy-induced ovarian failure mouse model. Case presentation We present two cases of Caucasian women with premature ovarian failure who resumed ovarian estrogen production and menses 7 months following autologous bone marrow–derived mesenchymal stem cell injections into the ovary. This pilot clinical study is registered with ClinicalTrials.gov (identifier NCT02696889). In this report, we present data from our first two cases that have completed study procedures so far. The bone marrow–derived mesenchymal stem cells were harvested from the bone marrow of the iliac crest of the patients with premature ovarian failure and nucleated cells concentrated and enriched in bone marrow–derived mesenchymal stem cells intraoperatively, and then injected into the patient’s right ovary via laparoscopy. Autologous bone marrow stem cell engraftment into the ovary resulted in several improvements in the treated patients with premature ovarian failure. In measurements by transvaginal ultrasound, there were increases of approximately 50% in volume of the treated ovaries in comparison with the contralateral control ovaries that persisted to the end of the study (1 year). Serum levels of estrogen increased by approximately 150% compared with the preoperative levels. Each of the two patients had an episode of menses, and also both of them reported marked improvement of their menopausal symptoms that also persisted to the end of the study (1 year). The bone marrow–derived mesenchymal stem cell implantation procedure was very well tolerated with no reported adverse events. Conclusions Our study reveals promising improvement of premature ovarian failure–related clinical manifestations in two patients after intraovarian autologous bone marrow–derived mesenchymal stem cells engraftment. These early observations call for additional assessment and further development of intraovarian bone marrow–derived mesenchymal stem cell injection for possible treatment of patients with premature ovarian failure.

Introduction With greater than one-tenth of outpatient gynecology visits attributed to chronic pelvic pain, research into etiology and management of the spectra of chronic pain disorders is very important. One such etiology, pelvic congestion syndrome (PCS), is a disease in which pelvic varicosities are present as a result of dysfunctional venous valves, retrograde blood flow, venous hypertension, and dilation. The presence of these varicosities, which exist in up to 10% of women between 18 and 76 years of age, can be detected by various imaging techniques including ultrasound, contrasted computed tomography, and magnetic resonance imaging. Various treatment methodologies have been described for the management of this chronic pain disorder, including medical, endovascular, and surgical therapies. With advancements in imaging, and the opportunity for endovascular intervention via interventional radiology, minimally invasive left ovarian vein embolization is at the forefront of therapeutic efforts for this nefarious condition. Methods We present here our clinical experience of 9 patients with PCS who underwent left ovarian vein embolization using a standardized protocol. Results Subjective pain relief was noted in 8 of 9 patients; 7 of the 9 patients reported complete resolution of pain at the 4-week follow-up, and associated symptoms similarly resolved. Conclusions Any thorough evaluation of a patient with chronic pelvic pain should include an evaluation for PCS when the diagnosis is not immediately available. Increased physician awareness of PCS is imperative for the future. With these astounding results, we recommend an excellent option for treatment of patients with left ovarian vein embolization.

Background Despite the rapid increase in the size of the geriatric population, no current published literature is available based on the effects of viewing a documentary covering medical and psychosocial issues concerning older adults influencing young people's empathy and ageism. The aim of the current study was to test whether participants who viewed an original documentary about older adults experiencing physical pain would report lower ageism and higher empathy scores when compared to participants who watched a neutral documentary. Method Seventy-seven students (ages 18-29 years) were randomized to either the experimental (pain documentary) or the control (neutral documentary) conditions and given pre- and post-test measures of empathy and ageism. Results The results of a series of Profile Analyses (Multivariate Mixed ANOVAs) showed a significant interaction (Wilk's λ=0.933, F(1,75)=5.389, p=0.023, partial η2=0.067) between treatment and time (pre- vs. post-viewing the film) for the empathy measure that was confirmed by follow-up t-tests. The latter showed a significant increase in empathy scores for only the experimental group, t(37)=-2.999, p=0.005. However, contrary to the original prediction, this same treatment by time effect was not observed for ageism (Wilk's λ=0.994, F(1,75)=0.482, p=0.490, partial η2=0.006), as the experimental participants did not significantly reduce their ageism scores, t(38)=0.725, p=0.473. The results of these analyses, as well as those obtained by using the subscales of each questionnaire, have been discussed. Conclusions The findings of this preliminary study indicate that showing a pain-based, anti-bias documentary feature film has the potential to significantly improve empathy towards older adults in university students.

Uterine fibroids (UFs) are benign smooth muscle neoplasms affecting up to 70% of reproductive age women. Treatment of symptomatic UFs places a significant economic burden on the US health-care system. Several specific genetic abnormalities have been described as etiologic factors of UFs, suggesting that a low DNA damage repair capacity may be involved in the formation of UF. In this study, we used human fibroid and adjacent myometrial tissues, as well as an in vitro cell culture model, to evaluate the expression of MutS homolog 2 (MSH2), which encodes a protein belongs to the mismatch repair system. In addition, we deciphered the mechanism by which polycomb repressive complex 2 protein, EZH2, deregulates MSH2 in UFs. The RNA expression analysis demonstrated the deregulation of MSH2 expression in UF tissues in comparison to its adjacent myometrium. Notably, protein levels of MSH2 were upregulated in 90% of fibroid tissues (9 of 10) as compared to matched adjacent myometrial tissues. Human fibroid primary cells treated with 3-deazaneplanocin A (DZNep), chemical inhibitor of EZH2, exhibited a significant increase in MSH2 expression (P < .05). Overexpression of EZH2 using an adenoviral vector approach significantly downregulated the expression of MSH2 (P < .05). Chromatin immunoprecipitation assay demonstrated that enrichment of H3K27me3 in promoter regions of MSH2 was significantly decreased in DZNep-treated fibroid cells as compared to vehicle control. These data suggest that EZH2-H3K27me3 regulatory mechanism dynamically changes the expression levels of DNA mismatch repair gene MSH2, through epigenetic mark H3K27me3. MSH2 may be considered as a marker for early detection of UFs.

Introduction Retained surgical objects can impact quality of life as it relates to chronic pelvic pain and reproductive health. Case report A 39-year-old G1P0010 with chronic pelvic pain and infertility attributed to pelvic adhesions and bilateral hydrosalpinx caused by an 18-cm surgical drain left 10 years prior. Conclusions Our case highlights the significance of thoughtful perioperative care as well as special considerations when approaching patients with pelvic pain and infertility with histories of abdominopelvic surgery.

Androgen excess (AE) is a key feature of polycystic ovary syndrome (PCOS) and results in, or contributes to, the clinical phenotype of these patients. Although AE will contribute to the ovulatory and menstrual dysfunction of these patients, the most recognizable sign of AE includes hirsutism, acne, and androgenic alopecia or female pattern hair loss (FPHL). Evaluation includes not only scoring facial and body terminal hair growth using the modified Ferriman–Gallwey method but also recording and possibly scoring acne and alopecia. Moreover, assessment of biochemical hyperandrogenism is necessary, particularly in patients with unclear or absent hirsutism, and will include assessing total and free testosterone (T), and possibly dehydroepiandrosterone sulfate (DHEAS) and androstenedione, although these latter contribute limitedly to the diagnosis. Assessment of T requires use of the highest quality assays available, generally radioimmunoassays with extraction and chromatography or mass spectrometry preceded by liquid or gas chromatography. Management of clinical hyperandrogenism involves primarily either androgen suppression, with a hormonal combination contraceptive, or androgen blockade, as with an androgen receptor blocker or a 5α-reductase inhibitor, or a combination of the two. Medical treatment should be combined with cosmetic treatment including topical eflornithine hydrochloride and short-term (shaving, chemical depilation, plucking, threading, waxing, and bleaching) and long-term (electrolysis, laser therapy, and intense pulse light therapy) cosmetic treatments. Generally, acne responds to therapy relatively rapidly, whereas hirsutism is slower to respond, with improvements observed as early as 3 months, but routinely only after 6 or 8 months of therapy. Finally, FPHL is the slowest to respond to therapy, if it will at all, and it may take 12 to 18 months of therapy for an observable response.

Polycystic ovary syndrome (PCOS) is a highly prevalent disorder effecting reproductive-aged women worldwide. This article addresses the evolution of the criteria used to diagnosis PCOS; reviews recent advances in the phenotypic approach, specifically in the context of the extended Rotterdam criteria; discusses limitations of the current criteria used to diagnosis, particularly when studying adolescents and women in the peri- and postmenopause; and describes significant strides made in understanding the epidemiology of PCOS. This review recognizes that although there is a high prevalence of PCOS, there is increased variability when using Rotterdam 2003 criteria, owing to limitations in population sampling and approaches used to define PCOS phenotypes. Last, we discuss the distribution of PCOS phenotypes, their morbidity, and the role that referral bias plays in the epidemiology of this syndrome.

Fertility preservation has become a standard of care in reproductive-age oncology patients. However, research has demonstrated that the knowledge of the provider and referral practice patterns remain suboptimal. Fertility preservation should be discussed with oncology patients of reproductive age, with the full knowledge of the individual. A combined medical and surgical approach may further ensure their reproductive successes in the future.A 38-year-old, nulliparous woman with colorectal cancer desired fertility preservation prior to cancer therapy. She underwent a laparoscopic ovarian transposition using a novel percutaneous suturing technique after emergent ovarian stimulation, followed by oocyte retrieval and cryopreservation. One year after chemotherapy and pelvic radiation the patient resumed regular menstrual cycles.A combined approach using emergent oocyte cryopreservation and a novel laparoscopic ovarian transposition is an optimal fertility preservation strategy in women with colorectal cancer. Fertility preservation success is dependent upon a multidisciplinary approach of well-informed medical teams consisting of an oncologist, a surgeon, a radiation oncologist, and a reproductive specialist.

To assess and quantify the relationship between polycystic ovaries (PCOs) and ovarian hyperstimulation syndrome (OHSS).Published studies, any language, identified through MEDLINE, EMBASE, BIOSIS, Web of Science, and bibliographies. Studies were selected if exposure (ultrasound findings of PCO before treatment) and outcome (OHSS) could be abstracted to two-by-two tables.Ten studies, meeting inclusion and exclusion criteria, were analyzed. When PCO were present, the combined odds ratio for OHSS was 6.8 (95% confidence interval 4.9-9.6).There is a significant and consistent relationship between PCO and OHSS. When PCO are present before treatment with assisted reproduction, deliberate policies to moderate treatment appear justified.

Primary ovarian pregnancy is a rare type of ectopic pregnancy, particularly following in vitro fertilization. Although there have been a few reported cases of primary ovarian pregnancy following in vitro fertilization embryo transfer, we believe this is the first report involving donor embryo transfer. A high index of clinical suspicion, abnormal human chorionic gonadotropin levels, and early ultrasound evaluation may aid timely diagnosis and appropriate management. This report provides a reminder to practitioners to advise patients undergoing embryo transfer of the primary ovarian pregnancy risk. Information is provided herein regarding the diagnosis and management of primary ovarian pregnancy in women treated with in vitro fertilization. We review the criteria for early diagnosis and treatment options.

The proteinuria of preeclampsia is associated with glomerular endotheliosis, and a role for damaged and lost podocytes (glomerular epithelial cells) in the development of proteinuria has received support from studies of patients with glomerular diseases. This study asked whether urinary excretion of viable podocytes, quantified by the expression of podocin and other podocyte-specific proteins, is observed in pregnant women with clinically confirmed preeclampsia. In addition, levels of angiogenic factors implicated in the pathogenesis of preeclampsia were estimated. They included soluble receptor for vascular endothelial growth factor, also called fms-like tyrosine kinase receptor-1 (sFlt-1); placental growth factor (PIGF); and serum soluble endoglin. The cross-sectional study recruited 33 pregnant women with preeclampsia, 11 with HELLP (hemolysis, elevated liver enzymes, low platelets) syndrome, and 23 normotensive pregnant women. All of the 15 preeclamptic women evaluated and also those with HELLP syndrome, but not normotensive control subjects, had podocin-positive cells in their urine. Podocyturia was 100% sensitive and 100% specific for the diagnosis of preeclampsia, and the degree of proteinuria correlated positively with podocyturia. The positive predictive value of urinary podocyte excretion for the presence of preeclampsia was greater than that of sFlt-1, PIGF, or endoglin. Nevertheless, serum levels of sFlt-1 were significantly higher in women with preeclampsia or HELLP syndrome than in normotensive control women, and the same was the case for serum levels of soluble endoglin. Women with preeclampsia or HELLP syndrome had lower serum levels of free PlGF than did normotensive pregnant women. Urine levels of PlGF did not differ from those in normal women. Podocyturia is a very sensitive and specific marker for preeclampsia, and it may contribute to the development of proteinuria in affected women. Further studies may clarify the effects of dysregulated vascular endothelial growth factor on mechanisms regulating podocyte attachment.
