Abstract Background In Nigeria, since 2002, Imatinib mesylate (glivec®) has been available freely to chronic myeloid leukaemia (CML) patients but only at a tertiary health care centre in the southwestern part of the country. Despite this, it is not readily accessible to many patients due to the distance and other challenges including low socioeconomic status and political problems, preventing timely access to specialist care. This study evaluated the effect of the baseline characteristics on the prognostic implication and treatment outcome of CML patients in Nigeria. Method This study retrospectively evaluated the baseline characteristics, clinical presentations and treatment outcomes of 889 CML patients over 18 years (2002–2020). Of these, 576 (65%) patients had complete information with up-to-date BCR::ABL1 records. The purpose of this study is to provide an empirical evidence on the relationship between audit quality and creative accounting strategy evidence from Nigerian Public Listed companies. Using a panel data set of all companies listed on the Nigerian Stock Exchange (NSE) for the period of nine years from 2010-2018. This paper employs 84 companies listed on the board of NSE with 756 firm-year observations. The study used secondary method to retrieve data from the annual reports of the listed companies and Thompson Reuters DataStream. The study employs multiple regression to examine the relationship between audit quality and creative accounting strategy. The results of the study displayed an insignificant negative relationship between audit fee and creative accounting strategy. This shows that changes in audit fee is not in anywhere associated with creative accounting strategy. Audit size is found to be insignificant and positive relationship with creative accounting strategy. This implies that audit size may not limit managers opportunistic behavior toward engaging in creative accounting strategy. However, a significant positive relationship was found between audit independence and creative accounting strategy. This implies that the more independence an audit firm enjoys, the more creative accounting strategy engaged by managers. Finally, our findings suggest that regulators of the Nigeria capital market should reform the audit market so as to ensure that audit firms would carry out the audit assignment in such a way to mitigate and prevent managers on engaging in fraudulent creative accounting strategy.

Background: Transfusion-transmissible infections (TTIs) in blood donors increase the risk of acquisition of these infections in blood and blood products recipients. Sensitive screening methods reduce the risk of transmission of TTIs to blood and products recipients. Objectives: The study sought to determine the prevalence of TTIs among voluntary non-remunerated blood donors (VNRBDs) and examine the need for the use of advanced donor screening methods to ensure blood infection safety.. Materials and Methods: This was a cross-sectional study conducted at the National Blood Transfusion Service (NBTS). Consenting VNRBD aged 18–65 years, were consecutively recruited during routine blood drives organized by the NBTS, between August and October 2016 after completing a structured questionnaire. Samples from donated blood units were screened for Human immunodeficiency virus (HIV), Hepatitis B virus (HBV), Hepatitis C virus (HCV), and syphilis using the fourth-generation ELISA. Results: The data obtained from 865 participants were analyzed. One hundred and eighty-two (21%) participants tested positive for TTIs. The prevalence of HBV, HCV, HIV, and syphilis among the participants was 14.7%, 4.6%, 1.8%, and 1.6%, respectively. Coinfection was found in 8.7% of the study participants. A statistically significant association was established between the sex of the participants and TTIs (χ2= 6.217, P = 0.0013). Conclusion: The prevalence of TTIs among VNRBD was high. Due to its attendant consequences on blood infection safety and implication on donor retention, there is a need for adoption of sensitive screening assays to ensure blood safety.

Background Vitamin D supplementation has been shown to improve hemoglobin levels in patients with sickle cell anemia (SCA). However, very little is known about the prevalence of its deficiency, its role in hemolysis, and its effects on transfusion requirements in SCA, hence this study. Patients and Methods Serum level of vitamin D was determined in 50 SCA patients in steady state, 50 SCA patients in hemolytic crises, and 50 in normal HbAA individuals. All were 18 years or older and were age- and sex-matched. In addition, PCV and hemoglobin (Hb) concentration, absolute reticulocyte count, uric acid, total and conjugated bilirubin, creatinine, lactate dehydrogenase (LDH), and estimated glomerular filtration rate (eGFR) were also assessed in all subjects and controls. Baseline sociodemographic, clinical, and anthropometric data were also recorded. Results The prevalence of vitamin D insufficiency and deficiency was found to be 18% and 79%, respectively. The mean serum levels of vitamin D were 18.84 ± 6.86 ng/mL and 17.58 ± 6.23 ng/mL in the steady-state and the hemolytic crises SCA groups, respectively. Transfusion requirements were found to be higher in SCA patients with lower vitamin D levels. Unlike those in crisis who received one or more units of blood transfusion, 42% of those in the steady state did not receive blood transfusion over the same period of 12 months. Hemoglobin levels were, however, found to predict reduced vitamin D levels. Conclusion Vitamin D deficiency (VDD) is highly prevalent among adults with SCA and those with lower vitamin D levels are more anemic and hence may require more blood transfusions.

Background and Objectives Sickle cell anemia (SCA) and glucose-6-phosphate dehydrogenase (G6PD) deficiency are both hereditary diseases of the red blood cells that cause hemolysis. The impact of the interaction of both conditions on the clinical and laboratory presentations of the affected persons is sparse. This study, therefore, correlated G6PD activity with disease severity in persons with SCA by comparing disease severity in G6PD-deficient SCA persons with those with normal G6PD activity. Methodology This cross-sectional study was conducted in the department of Haematology and Blood Transfusion of the Obafemi Awolowo University Teaching Hospitals Complex, Ile-Ife, Nigeria. G6PD activity, SCA disease severity, and hematological parameters including reticulocyte counts and Heinz body estimation, bilirubin, and aspartate transaminase were estimated in 67 SCA persons. The results were compared between SCA persons with G6PD deficiency and those with normal enzyme activity. Results The prevalence of G6PD deficiency was found to be 23.9%. The G6PD-deficient SCA patients included 4 (25.0%) males and 12 (75.0%) females. G6PD deficiency was significantly higher in females ( P = .047). There was no significant difference in disease severity scores between G6PD-deficient and G6PD-nondeficient SCA patients. However, G6PD-deficient persons reported significantly higher episodes of severe vasoocclusive crisis (VOC) per annum ( P = .048). The hematological and biochemical parameters were similar between G6PD-deficient and G6PD normal SCA persons except that the G6PD-deficient SCA persons have significantly higher reticulocyte response ( P = .001). There was no correlation between disease severity resulting from reduced G6PD activity and Heinz body formation in SCA persons in the steady state. Conclusion G6PD deficiency significantly contributes to recurrent painful vasoocclusive crisis in SCA persons in the steady state.

Background: The risk of acquiring transfusion transmissible viral infections is said to be higher in patients requiring regular blood transfusions such as those with sickle cell disease (SCD). Aim: We determined the seroprevalence of blood transfusion viral markers among our patients with SCD. Subjects and Methods: This a case–control analytical study consisting of 82 confirmed SCD patients on routine follow-up at our facility in steady state and 90 age-matched controls. Demographic and transfusion history were recorded while 5 ml of blood was drawn for hematocrit levels, and serum tested for transfusion transmissible viral markers for hepatitis B, (hepatitis B surface antigen [HBsAg], hepatitis B surface antibody [HBsAb], hepatitis B e antigen [HBeAg], hepatitis B e antibody [HBeAb], hepatitis B core antibody [HBcAb]), hepatitis C virus (HCV) and human immunodeficiency viruses (HIVs) using the rapid test kits. Ethical approval for the study was obtained from the Institutional Review Board and each participant gave informed consent. Data were analyzed using descriptive and inferential statistics. Results: The seroprevalence of HBsAg, HBsAb, and HBeAg in cases was 2 (2.4%), 7 (8.5%), and 0 (0.0%), respectively, whereas it was 7 (8.5%), 11 (13.4%), 6 (7.3%), and 2 (2.4%) for HBeAb, HBcAb, HCV, and HIV antibodies. Compared to the controls, cases had higher prevalence rate of HBeAb (P = 0.005). No significant difference was observed in those with or without low hematocrit (≤18%) or those that received blood transfusion and those that did not (P > 0.05). Conclusion: We conclude that blood transfusion did not significantly increase the seroprevalence of markers of transfusion transmissible viral infection in SCD patients.

The production of yam-derived (Dioscorea rotundata) foodstuffs is mainly performed by small and medium scale processors that employ old traditional methods. This can lead to differences in quality from processor to processor, and from location to location, with consequent safety concerns. As such, the effects of processing and post-processing phases (i.e., storage, transport, etc.) on the safety of some yam-derived foodstuffs—namely chips, flakes, and flour—has been evaluated, with a focus on bacterial and fungal contamination, aflatoxins, pesticides, and heavy metals (Pb, Ni, Cd and Hg). Yams harvested and processed in Nigeria were screened, being that the country is the largest producer of the tuber, with 70–75% of the world production. Results highlighted no presence of pesticides, however, many samples showed high levels of bacterial and fungal contamination, together with heavy metal concentrations above the recommended safety levels. No trend was observed between the items considered; it was noticed, however, that samples purchased from the markets showed higher contamination levels than those freshly produced, especially regarding bacterial and aflatoxins presence. The processing stage was identified as the most critical, especially drying. Nonetheless, post-processing steps such as storage and handling at the point of sale also contributed for chemical contamination, such as aflatoxin and heavy metals. The results suggested that both the processing and post-processing phases have an impact on the safety of yam chips, flakes, and flour.

Objective: We aimed in this paper to investigate some immunohematological characteristics of Nigerian sickle cell anemia (SCA) patients with osteomyelitis.Material and Methods: Thirty SCA patients with osteomyelitis (SO) and 30 SCA patients without osteomyelitis (S) were investigated. Objective. The tyrosine kinase inhibitors have markedly changed the disease course for patients with Ph+ and/or BCR-ABL1+ chronic myeloid leukemia (CML). This study was embarked upon to assess the long-term effects of imatinib therapy on survival in adult Nigerian patients with CML. Methods. All adult patients on imatinib (400–600 mg) seen from July 2003 to December 2010 were assessed. Male/female distribution was 171/101, with a median age of 38 (range, 20–75) years. Overall survival (OS) and progression-free survival (PFS) were determined using the Kaplan-Meier techniques. Results. Of all the 272 patients, 205 were in chronic phase, 54 in accelerated phase, and five in blastic phase, at commencement of imatinib. As at December 2010, 222 were alive. OS at 1 and 5 years was 94% and 63%, while PFS was 89% and 54%, respectively. Similarly, amongst the 205 patients in chronic phase, OS at 1 and 5 years was 97% and 68%, while PFS was 92% and 57%. Conclusion. Imatinib’s place as first-line therapy in the treatment of CML has further been reinforced in our patients, with improved survival and reduced morbidity, comparable with outcomes in other populations.

The screening for cytomegalovirus (CMV) specific antibodies is not routine in our setting, thus the transfusion of blood portends risk for CMV transmission in susceptible individual. We sought to determine the prevalence of IgG and IgM specific antibodies and associated characteristics in blood donors seen at a referral teaching hospital in Nigeria. From February to April 2013, we assessed the levels of CMV specific IgG and IgM antibodies (AccuDiagTM ELISA, DIAGNOSTIC AUTOMATION INC, USA) in blood donors negative for human immunodeficiency virus, hepatitis B and C viruses and syphilis. A structured proforma was used to detail subjects’ biodata and other characteristics including the type and history of prior donations. Ninety six percent of the donors were males while those with secondary education and below (61.3%), unmarried (70%), artisans and unemployed including students (82.5%) were in the majority. The prevalence of CMV IgG and IgM antibodies were 97.4% and 52.6%, while CMV IgG antibody range from 94.7% to 100% in all types of donation. All female donors (3.8%) were positive for IgG and IgM antibodies. There was no significant difference in the prevalence of CMV antibodies in the marital status, the level of education and the different type of donation. The prevalence of CMV antibodies is high (IgG = 97.4%; IgM = 52.6%) in our setting irrespective of donor background or type of donation. Leucodepletion of donor blood for sero-negative and immuno-compromised host is advocated while a government policy on CMV infection and blood transfusion services in Nigeria is highly recommended.

BACKGROUND: In West Africa, envenoming by saw-scaled or carpet vipers (Echis ocellatus) causes great morbidity and mortality, but there is a crisis in supply of effective and affordable antivenom (ISRCTN01257358). METHODS: In a randomised, double-blind, controlled, non-inferiority trial, "EchiTAb Plus-ICP" (ET-Plus) equine antivenom made by Instituto Clodomiro Picado was compared to "EchiTAb G" (ET-G) ovine antivenom made by MicroPharm, which is the standard of care in Nigeria and was developed from the original EchiTAb-Fab introduced in 1998. Both are caprylic acid purified whole IgG antivenoms. ET-G is monospecific for Echis ocellatus antivenom (initial dose 1 vial) and ET-Plus is polyspecific for E. ocellatus, Naja nigricollis and Bitis arietans (initial dose 3 vials). Both had been screened by pre-clinical and preliminary clinical dose-finding and safety studies. Patients who presented with incoagulable blood, indicative of systemic envenoming by E. ocellatus, were recruited in Kaltungo, north-eastern Nigeria. Those eligible and consenting were randomly allocated with equal probability to receive ET-Plus or ET-G. The primary outcome was permanent restoration of blood coagulability 6 hours after the start of treatment, assessed by a simple whole blood clotting test repeated 6, 12, 18, 24 and 48 hr after treatment. Secondary (safety) outcomes were the incidences of anaphylactic, pyrogenic and late serum sickness-type antivenom reactions. FINDINGS: Initial doses permanently restored blood coagulability at 6 hours in 161/194 (83.0%) of ET-Plus and 156/206 (75.7%) of ET-G treated patients (Relative Risk [RR] 1.10 one-sided 95% CI lower limit 1.01; P = 0.05). ET-Plus caused early reactions on more occasions than did ET-G [50/194 (25.8%) and 39/206 (18.9%) respectively RR (1.36 one-sided 95% CI 1.86 upper limit; P = 0.06). These reactions were classified as severe in 21 (10.8%) and 11 (5.3%) of patients, respectively. CONCLUSION: At these doses, ET-Plus was slightly more effective but ET-G was slightly safer. Both are recommended for treating E. ocellatus envenoming in Nigeria. TRIAL REGISTRATION: Current Controlled Trials ISRCTN01257358.

Marine plastic pollution, a major public health issue worldwide, releases toxic persistent organic pollutants (POPs) causing endocrine disruption, cancers, and other adverse effects. However, research on these compounds in the Nigerian marine environment is limited. Using a cross-sectional design and incorporating a laboratory component, this study aimed to assess the health risks associated with marine plastic pollution at Orimedu, Idado, and Eleko along the coastline of Lagos, Nigeria. A total of three coastal water and sediment composite samples were collected over 10 m from the shore into the ocean at each study area. The samples were analyzed for concentration levels of six phthalate esters (PEs) and 23 polychlorinated biphenyls (PCBs) using gas chromatography/mass spectrometry (GC/MS) and gas chromatography-electron capture detector (GC-ECD) after preparation. Health risk assessment was also computed using risk assessment models developed by the United States Environmental Protection Agency (USEPA). Results obtained were compared to the World Health Organization's (WHO) standards for cancer risk (1.0 × 10 -5 ) and non-cancer risk (≤ 1.0). Eleko had the highest concentrations of both PEs (24.89 ± 0.01 mg/L of DIOP) and PCBs (0.06 ± 0.01 of PCB 206-2,2',3,3',4,4'5,5',6-nonachlorobiphenyl) in the water samples, while in the sediment samples, Idado had the highest PE (69.00 ± 0.05 of DIOP) and Orimedu was found with the highest PCB (0.09 ± 0.03 of PCB 180-2,2',3,4,4',5,5'-heptachlorobiphenyl). Most of the PEs and PCBs were above the limits set by the WHO. Further studies are needed to address the health risk assessment of PEs and PCBs, especially in children and adults, in the coastline communities of Lagos., Competing Interests: Declarations. This study is aimed at assessing the effectiveness of hospital's wastewater treatment plant (WWTP) in removing nutrients, pathogenic bacteria, and addressing antibiotic resistance using a case study of a tertiary hospital in Ibadan, Nigeria. During the dry and wet seasons in the month of July and December, respectively, samples were collected, and analyzed using standard guidelines to examine significant physicochemical parameters of the WTTP; to evaluate the removal efficiency of biological oxygen demand (BOD) and chemical oxygen demand (COD), and to examine the prevalence of pathogenic and antibiotic-resistant bacteria. The results of this study showed that during the dry season, certain parameters exceeded acceptable limits, including temperature, total suspended solids (TSS), total dissolved solids (TDS), phosphate, and nitrate. Although there were reductions in BOD (1555 mg/L to 482 mg/L) and COD levels (3160 mg/L to 972 mg/L), they remained above acceptable limits by World Health Organization. In the wet season, the level of COD (20 mg/L) in the effluent was within acceptable limit, while the BOD (160 mg/L) was above the acceptable limit. The WWTP effectively removed nutrients and reduced the microbial load, as evident from the absence of fecal coliforms in the effluent in both seasons. In respect to BOD removal efficiency, the level of purification of wastewater by the WWTP was 69% during the dry season, while the removal efficiency of COD was 83.54% which showed the efficiency of the WWTP at the removal of COD. However, antibiotic resistance was still present. The study concludes that while the WWTP effectively addressed nutrients and microbial load, additional measures such as tertiary treatment methods like chlorination and UV radiation are necessary to tackle antibiotic resistance. This is crucial to prevent the release of antibiotic-resistant bacteria into the environment, safeguarding human health, animals, plants, and overall environmental well-being., (© 2024. The use of reclaimed water for crop irrigation presents a route through which pharmaceuticals enter the agro-environment, raising concerns about their potential inclusion into the food chain and associated health risks. The main objective of this study was to determine the accumulation of six pharmaceuticals (paracetamol, diclofenac and ibuprofen, ciprofloxacin, tetracycline, and sulfamethoxazole) in edible part of jute mallow (JM) (Corchorus olitorius) irrigated with treated hospital wastewater (THWW) and potential health risks associated with the consumption of the contaminated JM. In a greenhouse experiment, JM vegetable was grown in soils irrigated with groundwater and THWW. After 4 weeks of cultivation, the pharmaceutical concentrations in the soil and JM tissues were determined. The uptake and accumulation of the pharmaceuticals in the irrigated JM and the human health risks associated with their consumption were evaluated. Results showed that the THWW-irrigated and groundwater-irrigated soils accumulated all the studied pharmaceuticals except paracetamol and sulfamethoxazole, with the concentrations in the soil before and after irrigation ranging from 0.01 to 0.14 μg g -1 and 0.03 to 1.35 μg g -1 , respectively. In JM leaves, the accumulation was in the order of tetracycline > ciprofloxacin > ibuprofen > diclofenac and tetracycline > ciprofloxacin > diclofenac > ibuprofen under THWW-irrigated and groundwater-irrigated treatments, respectively. Under both treatments, the uptake and accumulation of the studied pharmaceuticals were in the order of roots > stem > leaves. The health risk assessment indicated that the consumption of the studied pharmaceuticals through JM implies some risks to human health and the risks were in the order of tetracycline > diclofenac > ciprofloxacin > ibuprofen > paracetamol > sulfamethoxazole. Endocrine disruptors (EDs) such as bisphenol A (BPA), nonylphenol (NP), octylphenol (OP) and heavy metals in drinking water supply represent a significant threat to human health. In Nigeria, little is known about the presence of EDs in various environmental media. This study was conducted to determine the concentrations of BPA, NP and OP in groundwater samples from selected communities in Ibadan, Nigeria. Water samples were collected from 30 different sites (26 hand-dug wells, 2 boreholes and 2 spring water sources), 15 from each of Ibadan North-West (IbNW) and Ido Local Government Area (LGA). Samples were collected in triplicate from all the sampling points and analysed for BPA, NP, OP and physicochemical parameters (including heavy metals) using a standard procedure. Bisphenol A and octylphenol were not detected in any samples, while NP was detected in spring water and the concentration (0.00279 mg/L) was less than the maximum allowable limit (0.015 mg/L). All (100.0%) boreholes in IbNW and 100.0% of the springs in Ido LGA showed iron concentrations that exceeded the permissible limit. There is a need for public awareness on the health risk of EDs in drinking water supply and appropriate preventive measures to be adopted.

Methylene blue (MB) is an important compound in textile and wood processing industries as well as in medical research for combating malaria parasites. Despite these versatilities, direct contact with human beings results in adverse health challenges, and contamination of water bodies affects aquatic biotas. Hence, it is important to treat MB-contaminated wastewaters before disposal into water bodies. Adsorption, which depends on some parameters, proves to be an easy, cheap, and efficient technique to remove pollutants in wastewater. However, investigating these parameters experimentally is a laborious, expensive, and time-consuming process whose efficiency is limited by the conditions imposed on the experiments. Herein, we developed polynomial multiple linear regression (MLR) and the three other machine learning models to study the interplay of five adsorption parameters (descriptors) and their effects on the removal of methylene blue from water using aluminized activated carbon (Al-AC). The optimized machine learning models, that is random forest (R = 0.9905), support vector regression (R = 0.9946), and multilayer perceptron (R = 0.9993), outperformed the best MLR model (R = 0.9845) by small margins. High statistical R and low error values are not enough to satisfactorily classify a model. Hence, the generalizability of the models was further determined under different experimental conditions, and the order of predictive accuracy of the models was established as ANN > SVR > RF > 2-degree MLR. Aluminum loading, adsorbent dosage, and initial adsorbate concentration are the most important factors affecting MB removal. The removal efficiency, which could reach 99.9% at optimum conditions, does not depend on the temperature thus eliminating the need to install temperature control apparatus for practical setup., (© 2022. Water and wastewater treatment applications stand to benefit immensely from the design and development of new materials based on silica nanoparticles and their derivatives. Nanosilica possesses unique properties, including low toxicity, chemical inertness, and excellent biocompatibility, and can be developed from a variety of sustainable precursor materials. Herein, we provide an account of the recent advances in the synthesis and utilization of nanosilica for wastewater treatment. This review covers key physicochemical aspects of several nanosilica materials and a variety of nanotechnology-enabled wastewater treatment techniques such as adsorption, separation membranes, and antimicrobial applications. It also discusses the prospective design and tuning options for nanosilica production, such as size control, morphological tuning, and surface functionalization. Informative discussions on nanosilica production from agricultural wastes have been offered, with a focus on the synthesis methodologies and pretreatment requirements for biomass precursors. The characterization of the different physicochemical features of nanosilica materials using critical surface analysis methods is discussed. Bio-hybrid nanosilica materials have also been highlighted to emphasize the critical relevance of environmental sustainability in wastewater treatment. To guarantee the thoroughness of the review, insights into nanosilica regeneration and reuse are provided. Overall, it is envisaged that this work's insights and views will inspire unique and efficient nanosilica material design and development with robust properties for water and wastewater treatment applications., (© 2022. Background: One of the world's leading causes of death among females is breast cancer. Oncolytic viruses are promising anticancer therapy that can overcome resistance to current conventional therapies. Measles virus replicates in and destroys malignant cells without affecting healthy cells. The study aimed to evaluate the lives attenuated Measles virus vaccine against Iraqi patient derived breast cancer cells that have functional BRCA1/BRCA2 genes and compare its activity against international breast cancer MCF-7 and CAL-51 cell lines., Methods: The virus was propagated in VERO-hSLAM slam cells. The MTT cytotoxicity assay used to test the virus's ability to kill three human breast cell lines (AMJ13), (MCF-7), and (CAL-51). The cytopathic effect of the measles virus was determined using an H&E stain. Immunocytochemistry assay using specific anti H protein monoclonal antibody for measles virus in the virally infected cells. Finally, apoptosis induction in the infected cells tested using double staining of acridine orange/propidium iodide., Results: The result shown that breast cancer cells are effectively infected and destroyed by live attenuated measles virus vaccine, and it caused a significant cytopathic effect in the infected cell lines after 48-72 h of infection with remarkable effect on AMJ13 cells (IC50 was 3.527 for AMJ13, when it was 5.079 and 9.171 for MCF-7 and CAL-51 respectively). Background: Despite growing concern over occupational exposure to particulate matter (PM) such as grain dust and diesel exhaust, information about the exposure level and health implications among workers in small-scale milling enterprises in developing countries like Nigeria has not been adequately documented. The purpose of this study was to assess the level of exposure to grain dust and diesel exhaust and effect on lung function among grain millers in food markets in Ibadan metropolis, Nigeria., Methods: The study adopted descriptive cross-sectional design with a comparative approach. Sixteen grain milling shops each were randomly selected from two major food markets in Ibadan metropolis for indoor PM 10 and PM 2.5 monitoring. Seventy-two respondents each were proportionately selected from grain millers and shop owners for forced expiratory volume in one second and peak expiratory flow rate tests., Results: The PM 2.5 concentrations for both market locations ranged between 1,269.3 and 651.7 μg/m 3 , while PM 10 concentrations were between 1,048.2 and 818.1 μg/m 3 . The recorded concentrations exceeded the World Health Organization guideline limit of 50 μg/m 3 and 25 μg/m 3 for PM 2.5 and PM 10 , respectively. As compared with control group (2.1 L), significantly lower forced expiratory volume in one second value (1.61 L) was observed among the exposed group ( p  < 0.05). Likewise, significantly lower peak expiratory flow rate value (186.7 L/min) was recorded among the exposed group than the control group (269.51 L/min) ( p  < 0.05)., Conclusion: Exposure to grain dust and diesel exhaust accentuated respiratory disorders with declines in lung functions amongst grain millers. Improved milling practices and engaging cleaner milling facilities should be adopted to minimize exposure and related hazards.

Anaerobic co-digestion of cow manure (CM) and waste milk (WM), produced by sick cows during treatment with antibiotics, was evaluated in two-stage process under thermophilic condition (55 °C) to determine the effect of WM addition on hydrogen (H2) and methane (CH4) production potentials, volatile solids (VS) removal, and energy recovery. Six CM to WM VS ratios of 100:0, 90:10, 70:30, 50:50, 30:70, and 10:90 were examined using 1-L batch digesters. The WM VS ratio of 30 % was found to be the minimum limit for significant increases in specific H2 and CH4 yields, and VS removal as compared to digestion of manure alone (P < 0.05). The highest specific H2 and CH4 yields, VS removal and energy yield were 38.2 mL/g VS, 627.6 mL/g VS, 78.4 % and 25,459.8 kJ/kg VS, respectively, in CM:WM 30:70. Lag phases to H2 and CH4 productions were observed in CM-WM mixtures, increased with increasing the amount of WM in the feedstock and were greater than 72 h in CM:WM 50:50 and 30:70. The digestion system failed in CM:WM 10:90. The results suggest that CM:WM 30:70 was optimum, however, due to limited amount of WM usually generated and long lag phase at this ratio which may make the process uneconomical, CM:WM 70:30 is recommended in practice.

The use of cefazolin to treat mastitic cows leads to cefazolin residues in milk and manure. This is responsible for the high occurrence of cefazolin resistant bacteria (CRB) in waste and the environment. Anaerobic digestion is considered to have the potential to reduce antibiotic-resistant bacteria present in waste that results from concentrated animal feeding operations. Thus, the objective of this study was to investigate the survival of CRB and the digester performance in mesophilic co-digestion of dairy manure and waste milk. The experiment was carried out using three digester compositions: 100% slurry (slurry), 50% slurry + 50% manure (manure mixture) and 50% slurry + 45% manure + 5% waste milk (milk mixture) in batch digesters of 1 l with a working volume of 800 ml in triplicate at 37°C for 34 days. The daily biogas production in each digester, and methane (CH4) and carbon dioxide compositions in the gas were determined. The population densities of total culturable bacteria (TCB) and CRB were determined by plate counts on agar media at day 0, 10, 20 and 34 of digestion. Milk mixture produced the highest (P < 0.05) daily and cumulative total and CH4 gas. The maximum percentage reductions of TCB and CRB in manure and milk mixture was observed at day 20, the values being 96.2%, 96.0% and 99.8% and 99.8% respectively. Final volatile fatty acids (VFA) and pH values of the digesters confirmed the digester stability. Based on the findings, mesophilic anaerobic digestion can be considered a potent method to avoid the dissemination of CRB in nature.

Anaerobic digestion is considered as a promising method to manage animal waste with antibiotic-resistant bacteria. Current research was conducted to investigate the survival of multidrug-resistant bacteria (MDRB) resistant to three groups of antibiotics: (i) cefazolin, neomycin, vancomycin, kanamycin (group 1); (ii) penicillin, oxytetracycline, ampicillin, streptomycin (group 2); and (iii) cefazolin, neomycin, vancomycin, kanamycin, penicillin, oxytetracycline, ampicillin, streptomycin (group 3), in anaerobic digestion of dairy manure and co-digestion of dairy manure and waste milk at 37°C and 55°C for 22 days, respectively. The population densities of three groups of MDRB on peptone, tryptone, yeast and glucose agar plates incubated at 30°C for 7 days before and after digestion showed 100% destruction in both digestates at thermophilic temperature. Overall reduction of more than 90% of three groups of MDRB was observed in mesophilic digestion with no significant differences (P > 0.05) between manure and milk mixture. Co-digestion of dairy manure and waste milk always produced significantly (P < 0.05) higher total gas and methane gas than digestion of manure alone at both temperatures. Gas production in each case was significantly (P < 0.05) higher in thermophilic digestion than in mesophilic digestion. The individual and combined effect of cefazolin (CFZ) and oxytetracycline (OTC) on thermophilic anaerobic digestion (55°C) of dairy manure in batch digesters was investigated. Methane productions from the concentrations tested (30, 60 and 90 mg L(-1)) were compared with no-antibiotic control. CFZ concentrations showed no inhibition (P>0.05) for methane production. The individual OTC and combined CFZ and OTC at concentrations of 30, 60 and 90 mg L(-1) represented 79.1%, 70.3%, 68.6% (P<0.05) and 88.5%, 82.7%, 70.3% (P<0.05) respectively, of the control values. Background: Controversy exists regarding the role of PD1 and its ligand PD-L1 in chronic hepatitis B infection. In some studies, persistent HBV infection has been attributed to high levels of PD-1 and PD-L1 expression on HBV-specific T-cells and antigen-presenting cells (APCs) respectively. Other studies revealed that the up-regulation of PD-1 and PD-L1 during an acute inflammation phase is required to offset increasing positive co-stimulatory signals to avoid severe damage by an over-vigorous immune response., Methods: Fifteen chronic hepatitis B patients, with inflammatory flare episode, were recruited prospectively. Based on serum HBV-DNA, HBsAg load, and ALT values, inflammatory flare episode were divided into initial, climax, decline and regression phase. Blood sample and liver biopsy tissues from each individual were taken in these 4 phases respectively. Circulating and intra-hepatic PD1 and PD-L1 expression levels were monitored throughout the inflammatory flare episode by flow cytometry and immunostaining and these expression levels were related to the HBV-specific T-cell changes, expression of pro-inflammatory cytokines, HBV-DNA replication and HBV antigen load., Results: ]The levels of PD-1 and PD-L1 expressions were significantly up-regulated in the inflammation ascending phase, initial and climax period and in parallel with HBV-specific colon expansion. It showed increasing the level of serum ALT and decreasing the HBV-DNA loads. As the level of inflammation reduced, the circulating and intra-hepatic PD1 and circulating PD-L1 decreased progressively in concordance with serum ALT, HBV-DNA and HBsAg loads decreased except intra-hepatic PD-1 expression. Intra-hepatic PD-L1 expression did not decrease significantly during the regression phase of inflammation compared to that in prior period. The intra-hepatic PD-L1 expression remained relatively on higher level when serum HBV-DNA load and ALT decreased to approximately normal range., Conclusion: The relatively high level of intra-hepatic PD-L1 expression during the inflammatory regression period may contribute to constitute an immunosuppressive microenvironment, which facilitate persistent HBV infection via the inhibition of HBV-specific T cell clonal expansion.

Biohydrogen production from co-digestion of cow manure (M) and waste milk (WM), milk from mastitis cows treated with cefazolin, was evaluated in a 3×5 factorial design. Organic loading of 20, 40 and 60g volatile solid (VS)L(-1) were tested at temperature of 55°C using M:WM (VS/VS) 70:30, 50:50, 30:70, 10:90 and 0:100. Hydrogen production increased with organic loading and M:WM to a maximum of 59.5mLg(-1) VS fed at 40g VSL(-1) in M:WM 70:30. Butyrate was the main volatile fatty acid (VFA) accumulated in M:WM 50:50, 30:70 and 10:90. Overall reduction of more than 90% of cefazolin resistant bacteria was observed in all the treatments. The reduction was higher at 40 and 60 than 20g VSL(-1) (P<0.05).
