BACKGROUND: Tuberculosis (TB) prevalence is high among socially marginalized citizens in Denmark, and management of latent TB infection (LTBI) may be part of preventing new cases. Patients with LTBI are offered either preventive treatment (TPT) or follow-up chest x-rays, but knowledge about the long-term outcome in terms of active TB is sparse.METHODS: We performed a retrospective cohort study investigating the long-term outcomes for socially marginalized citizens who were diagnosed with LTBI or who had a positive interferon-gamma release assay (IGRA) but were lost to follow-up. Information on TB examinations, diagnostics, and treatment along with data on death were gathered from medical records from the date of positive IGRA to February 1, 2021.RESULTS: We identified 119 patients with LTBI, 18 of which (15.1%) were diagnosed with TB during the follow-up period (mean, 4.5 years). TPT was completed by 36.1% and the TB incidence rate ratio of those completing TPT to those who did not was 0.78 (confidence interval, 0.25-2.17; P =.6). Of the patients with TB, 16 of 18 achieved treatment success.CONCLUSION: High rates of TB development are found among socially marginalized citizens with LTBI. Overall incidence of TB was not significantly reduced by administration of TPT, although TB did not develop in the first 2 years following TPT.

The aim of this study was to evaluate the frequency of TB and the frequency of latent TB infection (LTBI), in clinical practice, for juvenile idiopathic arthritis (JIA) patients from high and low risk of TB incidence endemic countries. Methods: This is an international, multicenter, cross-sectional, observational study of data collection from Brazil and Registry of Portugal at REUMA.PT. The inclusion criteria were patients with Juvenile Idiopathic Arthritis (JIA) with age ≤ 18 years who underwent screening for Mycobacterium tuberculosis infection [tuberculin skin test (TST) and/or interferon gamma release assay (IGRA)]. Chest X-rays and history of exposure to TB were also assessed. Results: 292 JIA patients were included; mean age 14.3 years, mean disease duration 7.5 years, 194 patients (66.4%) performed only TST, 14 (4.8%) only IGRA and 84 (28.8%) both. The frequency of LTBI (10.6%) and TB was similar between the two countries. The reasons for TB screening were different; in Brazil it was performed more often at JIA onset while in Portugal it was performed when starting Disease Modified Anti-Rheumatic Drugs (DMARD) treatment (p < 0.001). Isoniazid therapy was prescribed in 40 (13.7%) patients (31 with LTBI and 9 with epidemiologic risks and/or due to contact with sick people). Only three patients (1%) developed active TB. Conclusion: We found nearly 10% of patients with LTBI, a small percentage of patients with treatment due to epidemiologic risks and only 1% with active TB. Distinct reasons and screening methods for LTBI were observed between the two countries.

The risk of tuberculosis (TB) is variable among individuals with latentMycobacterium tuberculosisinfection (LTBI), but validated estimates of personalized risk are lacking. In pooled data from 18 systematically identified cohort studies from 20 countries, including 80,468 individuals tested for LTBI, 5-year cumulative incident TB risk among people with untreated LTBI was 15.6% (95% confidence interval (CI), 8.0-29.2%) among child contacts, 4.8% (95% CI, 3.0-7.7%) among adult contacts, 5.0% (95% CI, 1.6-14.5%) among migrants and 4.8% (95% CI, 1.5-14.3%) among immunocompromised groups. We confirmed highly variable estimates within risk groups, necessitating an individualized approach to risk stratification. Therefore, we developed a personalized risk predictor for incident TB (PERISKOPE-TB) that combines a quantitative measure of T cell sensitization and clinical covariates. Internal-external cross-validation of the model demonstrated a random effects meta-analysis C-statistic of 0.88 (95% CI, 0.82-0.93) for incident TB. In decision curve analysis, the model demonstrated clinical utility for targeting preventative treatment, compared to treating all, or no, people with LTBI. We challenge the current crude approach to TB risk estimation among people with LTBI in favor of our evidence-based and patient-centered method, in settings aiming for pre-elimination worldwide. The risk of developing active tuberculosis (TB) in individuals with latent TB infection is highly variable within and among different risk groups. A personalized risk predictor was developed to better target preventative treatment to individuals at greatest risk, supporting evidence-based clinical decision-making for latent TB.

Early, accurate diagnosis of tuberculosis is one of the major pillars of the control of the disease. The purpose of this consensus statement is to provide health professionals with the most current, useful evidence for the diagnosis of tuberculosis in Brazil. To that end, the Tuberculosis Committee of the Brazilian Thoracic Association brought together 14 members of the Association with recognized expertise in tuberculosis in Brazil to compose the statement. A nonsystematic review of the following topics was carried out: clinical diagnosis, bacteriological diagnosis, radiological diagnosis, histopathological diagnosis, diagnosis of tuberculosis in children, and diagnosis of latent tuberculosis infection. RESUMO O diagnóstico precoce e adequado da tuberculose é um dos pilares mais importantes no controle da doença. A proposta deste consenso brasileiro é apresentar aos profissionais da área de saúde um documento com as evidências mais atuais e úteis para o diagnóstico da tuberculose. Para tanto, a Comissão de Tuberculose da Sociedade Brasileira de Pneumologia e Tisiologia reuniu 14 membros da Sociedade com reconhecida experiência em tuberculose no Brasil. Foi realizada uma revisão não sistemática dos seguintes tópicos: diagnóstico clínico, diagnóstico bacteriológico, diagnóstico radiológico, diagnóstico histopatológico, diagnóstico da tuberculose na criança e diagnóstico da tuberculose latente.

Tuberculosis (TB) remains a public health emergency worldwide. WHO recommends low-incidence countries to increase awareness of latent tuberculosis infection (LTBI) as part of a national preventive strategy. This review focuses on the burden of TB in Denmark, which might be reduced, if more focus was paid on diagnosing LTBI in relevant patients, and treatment to prevent future active TB cases was initiated. LTBI testing should be considered in persons at risk of TB disease, who will benefit from LTBI treatment, e.g. recently infected persons or persons having undergone organ transplantation or having comorbidity such as HIV, dialyses, and immunosuppression.

Over the last decade, molecular biology techniques for identifying mycobacteria in pulmonary secretions have become more and more sensitive and rapid, even in smear negative samples. Nuclear amplification techniques also allow the rapid detection of resistance to first or second line anti-tuberculous drugs. The sensitivity of these techniques for non respiratory samples is yet to be determined. The diagnosis of latent tuberculous infection (LTBI) has also increased in sensitivity, specificity and positive predictive value through the use of interferon-γ release assays (IGRAs), which are tending to replace the tuberculin skin test, except for children aged under five. These tests, however, do have limitations which are important for the clinician; especially their inability to distinguish active from latent tuberculosis and their inability, in most circumstances, to exclude a diagnosis of active TB.

Rationale: In the absence of active tuberculosis, a positive tuberculin skin test (TST) or interferon-gamma release assay (IGRA) result defines latent infection with Mycobacterium tuberculosis, although test results may vary depending on immunodeficiency. Objectives: This study compared the performance of TST and IGRAs in five different groups of immunocompromised patients, and evaluated their ability to identify those at risk for development of tuberculosis. Methods: Immunocompromised patients with HIV infection, chronic renal failure, rheumatoid arthritis, solid-organ or stem-cell transplantation, and healthy control subjects were evaluated head-to-head by the TST, QuantiFERON-TB-Gold in-tube test (ELISA), and T-SPOT. TB test (enzyme-linked immunospot) at 17 centers in 11 European countries. Development of tuberculosis was assessed during follow-up. Measurements and Main Results: Frequencies of positive test results varied from 8.7 to 15.9% in HIV infection (n = 768), 25.3 to 30.6% in chronic renal failure (n = 270), 25.0% to 37.2% in rheumatoid arthritis (n = 199), 9.0 to 20.0% in solid-organ transplant recipients (n = 197), 0% to 5.8% in stem-cell transplant recipients (n = 103), and 11.2 to 15.2% in immunocompetent control subjects (n = 211). Eleven patients (10 with HIV infection and one solid-organ transplant recipient) developed tuberculosis during a median follow-up of 1.8 (interquartile range, 0.2-3.0) years. Six of the 11 patients had a negative or indeterminate test result in all three tests at the time of screening. Tuberculosis incidence was generally low, but higher in HIV-infected individuals with a positive TST (3.25 cases per 100 person-years) than with a positive ELISA (1.31 cases per 100 person-years) or enzyme-linked immunospot result (1.78 cases per 100 person-years). No cases of tuberculosis occurred in patients who received preventive chemotherapy. Conclusions: Among immunocompromised patients evaluated in this study, progression toward tuberculosis was highest in HIV-infected individuals and was poorly predicted by TST or IGRAs.

BACKGROUND: Interferon gamma release assays (IGRAs) are the first new diagnostic tests for latent tuberculosis (TB) infection (LTBI) since the century-old tuberculin skin test (TST). They are cell-mediated immune-based blood tests that have revolutionised LTBI diagnosis and are increasingly recommended by national guidelines.OBJECTIVES: With the rapid expansion of the IGRA evidence-base in recent years, the limitations of IGRA and uncertainties in clinical interpretation of IGRA results have increasingly come into focus. In LTBI diagnosis these include: prognostic power of IGRAs relative to TST for quantifying risk of progression to active disease, false-negative rates in immunocompromised patients, the clinical meaning of IGRA reversion and the significance of the size of IGRA response. Furthermore, the role of IGRAs in the diagnostic work-up of active TB is unclear, and there is little evidence supporting use of the tests in anti-TB treatment monitoring.METHODOLOGICAL APPROACH: On-going large prospective longitudinal clinical endpoint cohort studies of active and latent TB will tackle some of the uncertainties regarding IGRAs. Here we discuss clinical practice and guidance in light of the current uncertainties, based on existing evidence.CONCLUSIONS AND IMPACT: Current and planned clinical research will fill the gaps in the evidence-base, narrowing the areas of uncertainty and informing future policy. Translational research into next-generation IGRAs and new T cell-based diagnostic platforms will likely overcome the limitations of current IGRAs in the near future.

La tuberculosis continúa siendo un problema enorme en salud pública. Se ha estimado que anualmente en todo el mundo 9 millones de personas contraen esta enfermedad y que 2 millones de muertes se deben a la tuberculosis. La exposición al M. tuberculosis causa la enfermedad en cerca del 10% de la población expuesta. En el 90% restante de los expuestos la respuesta inmune inhibe la multiplicación del M. tuberculosis. En una parte de las personas expuestas (~40%) algunos bacilos no son muertos permaneciendo en una condición que no les permite replicarse, generándose así la infección denominada tuberculosis latente. Los bacilos inactivos pueden recuperar su vitalidad (capacidad de replicación) y causar una tuberculosis activa, si la respuesta inmune está alterada. La detección y el tratamiento de la infección tuberculosa latente han sido reconocidos como objetivos estratégicos efectivos en el control de la tuberculosis. En el pasado reciente, el único test disponible para la detección de infección tuberculosa latente era el test cutáneo con tuberculina (PPD). Recientemente se ha desarrollado un ensayo basado en la liberación "in vitro" de interferón-gama (IGRA)por células T. Este ensayo usa antígenos específicos del M. tuberculosis y no es interferido por la vacunación previa con BCG, lo que lo convierte en un test de tamizaje útil en poblaciones vacunadas con BCG. La FDA ha aprobado dos IGRAs comerciales: a) QuantiFERON-TB Gold (QFT, Cellestis) cuya variación simplificada se denomina Quantiferon T in Tube (QFT-G-IT) y b) el T-SPOT TB test (Oxford, Immunotec). Según el Programa Nacional de Control de la Tuberculosis el tratamiento para la infección tuberculosa latente es la administración diaria de isoniacida por 9 meses; este tratamiento proporciona una protección superior al 90% si es administrado regularmente. Otra opción, usada en EE.UU. y Europa es prescribir rifampicina por 4 meses. La identificación y el tratamiento de los portadores de tuberculosis latente es una acción crucial en los esfuerzos para disminuir la incidencia de tuberculosis en países en desarrollo con tasas de incidencia de tuberculosis intermedias o bajas. Tuberculosis continues being a huge public health problem. It has been estimated that worldwide every year about 9 million people adquire the disease and 2 million of deaths are because of tuberculosis. Exposure to Mycobacterium tuberculosis causes active disease in nearly 10% of the people. In the remaining 90% of the exposed population the immune response inhibits M. tuberculosis multiplication. In part of the exposed people (~40%) some bacilli are not killed remaining in a non-replicating condition, generating the so called latent tuberculosis infection. The inactive bacilli can regain their vitality (replication capacity) and cause active tuberculosis if the immune response is disrupted. Targeted screening and treatment for latent tuberculosis infection has been recognized as an effective strategy for tuberculosis control. Until recently, the only test available for the detection of latent tuberculosis infection was the tuberculin (PPD) skin test. Recently it has been developed an in vitro T-cell-based interferon-gamma release assay (IGRA). This assay use antigens specific to M. tuberculosis and is not affected by previous BCG vaccination, making it a useful screening test for latent tuberculosis in BCG-vaccinated populations. FDA has approved two commercial IGRAs: a) QuantiFERON-TB Gold (QFT, Cellestis) which simplified variation is called Quantiferon T in Tube (QFT-G-IT) and b) T-SPOT TB test (Oxford, Immunotec). According to the Chilean National Program of Tuberculosis Control, therapy for latent tuberculosis infection is daily administration of isoniazid for 9 months; administered regularly this therapy gives over 90% of protection. Another option used in USA and Europe is to prescribe rifampicin for 4 months. Identification of latent tuberculosis carriers and their treatment is a pivotal action in the efforts for decreasing tuberculosis incidence in developing countries with low or intermediate rates of tuberculosis incidence.
