Differential use of resources among bird species has been examined extensively in diet and nesting sites, but few studies have assessed this regarding avian nest materials. We assessed the structure and composition of nests in a group of co‐existing passerine shrubland birds at a site in Massachusetts, USA. We found, measured, collected, and dissected nests, and then weighed nest materials in morphological groups (e.g., bark, twigs, feathers) to determine if our seven focal species were using different nest materials. Among species, we compared proportional material masses in complete nests, and also separately in the exterior, structural part of the nest and the interior, cup lining. We found that the proportional masses of all 17 material types that we examined in nests differed among species. The compositions of nests among all seven bird species were distinct in multivariate ordination space and only a few pairs of species had substantial niche overlap. Proportional masses of materials within discrete sections (exterior and interior) also varied among species. Although some differences in nest composition could be partially explained by bird species size, nest materials differed even within the three larger bodied species and within four smaller bodied species. Our study builds upon previous studies that have shown species‐specificity in avian nest composition and supports the notion that birds using the same environment have distinct niches in relation to the materials placed in their nests. Niche partitioning due to interspecific competition could partially explain our findings, as certain materials are limited as resources, and searching for suitable nest materials is energetically costly. Additionally, other factors, such as partitioned nest sites, may have led to differential nest material use. As the coronavirus disease 2019 (COVID-19) pandemic progresses to an endemic phase, a greater number of patients with a history of COVID-19 will undergo surgery. Major adverse cardiovascular and cerebrovascular events (MACE) are the primary contributors to postoperative morbidity and mortality; however, studies assessing the relationship between a previous severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and postoperative MACE outcomes are limited. Here, we analyzed retrospective data from 457,804 patients within the N3C Data Enclave, the largest national, multi-institutional data set on COVID-19 in the United States. However, 7.4% of patients had a history of COVID-19 before surgery. When comorbidities, age, race, and risk of surgery were controlled, patients with preoperative COVID-19 had an increased risk for 30-day postoperative MACE. MACE risk was influenced by an interplay between COVID-19 disease severity and time between surgery and infection; in those with mild disease, MACE risk was not increased even among those undergoing surgery within 4 wk following infection. In those with moderate disease, risk for postoperative MACE was mitigated 8 wk after infection, whereas patients with severe disease continued to have elevated postoperative MACE risk even after waiting for 8 wk. Being fully vaccinated decreased the risk for postoperative MACE in both patients with no history of COVID-19 and in those with breakthrough COVID-19 infection. Together, our results suggest that a thorough assessment of the severity, vaccination status, and timing of SARS-CoV-2 infection must be a mandatory part of perioperative stratification. NEW & NOTEWORTHY With an increasing proportion of patients undergoing surgery with a prior history of COVID-19, it is crucial to understand the impact of SARS-CoV-2 infection on postoperative cardiovascular/cerebrovascular risk. Our work assesses a large, national, multi-institutional cohort of patients to highlight that COVID-19 infection increases risk for postoperative major adverse cardiovascular and cerebrovascular events (MACE). MACE risk is influenced by an interplay between disease severity and time between infection and surgery, and full vaccination reduces the risk for 30-day postoperative MACE. Background: Obstructive sleep apnoea increases risk of respiratory depression with administration of sedatives, narcotics or anxiolytics. To reduce adverse events during hospital admission, we implemented STOP-BANG screening to prompt respiratory monitoring for inpatients receiving these medications. This study reports on protocol development, implementation and an initial analysis over 5 years to evaluate implementation success and outcomes., Intervention: The STOP-BANG measure was embedded in the nurse navigator at admission. If the score was ≥3 and sedatives, narcotics and/or anxiolytics were ordered, the provider was prompted to monitor patients with continuous pulse oximetry and/or capnography., Methods: We assessed the impact of the intervention using a retrospective pre-post design. Preprotocol data from all adult inpatients over a 2.5-year period, and postprotocol data from all adult inpatients from over a 5-year period, were extracted from the electronic health record. Outcomes included use of monitoring; adverse events during hospitalisation were included to evaluate the effects of the intervention: mortality, rate of rapid response team events, reversal and/or rescue, intensive care unit admission and orders for positive airway pressure equipment., Results: The combined preprotocol and postprotocol sample included 254 121 patients. After protocol implementation, overall mortality for patients receiving sedatives, narcotics or anxiolytics decreased slightly from 2.1% to 1.9% (p<0.001). In the postprotocol cohort only (n=193 744), monitored patients had a higher probability of experiencing all adverse events. Among monitored patients, mortality was lowest in the high-risk group (STOP-BANG≥5)., Discussion: Triaging by STOP-BANG coupled with monitoring appeared to be helpful for patients at highest risk of obstructive sleep apnoea. Standardization and accreditation of fellowship training have been considered in the field of neuroanesthesiology. A prior survey of members of the Society for Neuroscience in Anesthesiology and Critical Care (SNACC) suggested strong support for accreditation and standardization. In response, SNACC created a Task Force that developed curricular guidelines for neuroanesthesiology fellowship training programs. These guidelines represent a first step toward standards for neuroanesthesiology training and will be useful if accreditation is pursued in the future.
