Background: Interleukin-6 signal blockade showed preliminary beneficial effects in treating inflammatory response against SARS-CoV-2 leading to severe respiratory distress. Herein we describe the outcomes of off-label intravenous use of Sarilumab in severe SARS-CoV-2-related pneumonia. Methods: 53 patients with SARS-CoV-2 severe pneumonia received intravenous Sarilumab; pulmonary function improvement or Intensive Care Unit (ICU) admission rate in medical wards, live discharge rate in ICU treated patients and safety profile were recorded. Sarilumab 400 mg was administered intravenously on day 1, with eventual additional infusion based on clinical judgement, and patients were followed for at least 14 days, unless previously discharged or dead. Findings: Of the 53 SARS-CoV-2pos patients receiving Sarilumab, 39(73·6%) were treated in medical wards [66·7% with a single infusion; median PaO2/FiO2:146(IQR:120–212)] while 14(26·4%) in ICU [92·6% with a second infusion; median PaO2/FiO2: 112(IQR:100–141.5)].Within the medical wards, 7(17·9%) required ICU admission, 4 of whom were re-admitted to the ward within 5–8 days. At 19 days median follow-up, 89·7% of medical inpatients significantly improved (46·1% after 24 h, 61·5% after 3 days), 70·6% were discharged from the hospital and 85·7% no longer needed oxygen therapy. Within patients receiving Sarilumab in ICU, 64·2% were discharged from ICU to the ward and 35·8% were still alive at the last follow-up. Overall mortality rate was 5·7%. Interpretation: IL-6R inhibition appears to be a potential treatment strategy for severe SARS-CoV-2 pneumonia and intravenous Sarilumab seems a promising treatment approach showing, in the short term, an important clinical outcome and good safety.

A audiência de custódia foi implantada no Brasil em 2015, por iniciativa do Conselhob Nacional de Justiça. No entanto, apenas recentemente, a partir da Lei nº 13.964/2019, ela foi incorporada ao Código de Processo Penal. O presente artigo tem como objetivo retratar o contexto em que esse instrumento foi inserido no ordenamento jurídico brasileiro, bem como seus objetivos e procedimento. Apresentam-se as alterações trazidas pela denominada Lei Anticrime questionando, especialmente, a nova redação prevista no artigo 287 do Código de Processo Penal e a possibilidade de facultar a realização da audiência para alguns tipos de prisão, bem como a (in)constitucionalidade do § 2º do artigo 310 do mesmo diploma, e à impossibilidade de liberdade provisória para alguns casos.

Objective: Glucocorticoids (GC) are associated with side effects in giant cell arteritis (GCA). Immunosuppressive therapies (ITs) have given conflicting results in GCA, regarding GC sparing effect. Primary endpoint is to evaluate whether very early introduction of ITs in GCA minimize the rate of GC-induced adverse events, in terms of infections, new onset systemic arterial hypertension, GC-induced diabetes and osteoporotic fractures. Methods: A multicenter retrospective case-control study included 165 patients. One group included 114 patients who were treated with at least one IT given at diagnosis or within 3 months from the start of GC. A second group included 51 GCA who received only GC or an IT more than 3 months later. Results: The most frequently used ITs were: methotrexate (138 patients), cyclophosphamide (48 patients) and tocilizumab (27 patients). No difference was observed as concerns the follow-up time between groups [48.5 (IQR 26–72) vs 40 (IQR 24–69), p = 0.3)]. The first group showed a significantly lower incidence of steroid-induced diabetes (8/114, 7% vs 12/51, 23.5%; p = 0.003) and no differences for the rate of infections (p = 0.64). 7. Multidisciplinary Evaluation of Interstitial Lung Diseases: New Opportunities Linked to Rheumatologist Involvement

Multidisciplinary team (MDT) discussion is the gold standard in the management of interstitial lung disease (ILD). The rheumatologist is not routinely involved in MDT, even if up to 20% of ILD are related to systemic autoimmune rheumatic diseases (SARD). The study aims to assess the agreement and its variation over time between rheumatologists and pulmonologists in the screening of SARD and between rheumatologists and an MDT extended to rheumatologists (eMDT) in evaluating the progression of SARD. We computed the agreement between the pulmonologist and rheumatologist in the identification of red flags for SARDs of 81 ILD cases and between the rheumatologist alone and eMDT in the confirmation of 70 suspected SARD-ILD progressions. The agreement between rheumatologists and pulmonologists was moderate for the detection of autoimmunity test positivity (κ = 0.475, p < 0.001) and family history of SARD (κ = 0.491, p < 0.001) and fair for the identification of extrapulmonary symptoms (κ = 0.225, p = 0.064) or routine laboratory abnormalities consistent with SARD. The average agreement between the rheumatologist and eMDT in the identification of ILD progression was moderate (κ = 0.436, p < 0.001). The class of agreement improved from the first to the third semester. The average agreement with the rheumatologist ranged from fair to moderate, suggesting that a shared evaluation of SARD-ILD in eMDT could improve the diagnostic work-up and the evaluation of ILD progression.

BACKGROUND/OBJECTIVES: Several scoring systems have been specifically developed for risk stratification in COVID-19 patients. DESIGN: We compared, in a cohort of confirmed COVID-19 older patients, three specifically developed scores with a previously established early warning score. Main endpoint was all causes in-hospital death. SETTING: This is a single-center, retrospective observational study, conducted in the Emergency Department (ED) of an urban teaching hospital, referral center for COVID-19. PARTICIPANTS: We reviewed the clinical records of the confirmed COVID-19 patients aged 60 years or more consecutively admitted to our ED over a 6-week period (March 1st to April 15th, 2020). A total of 210 patients, aged between 60 and 98 years were included in the study cohort. MEASUREMENTS: International Severe Acute Respiratory Infection Consortium Clinical Characterization Protocol-Coronavirus Clinical Characterization Consortium (ISARIC-4C) score, COVID-GRAM Critical Illness Risk Score (COVID-GRAM), quick COVID-19 Severity Index (qCSI), National Early Warning Score (NEWS). RESULTS: Median age was 74 (67-82) and 133 (63.3%) were males. Globally, 42 patients (20.0%) deceased. All the score evaluated showed a fairly good predictive value with respect to in-hospital death. The ISARIC-4C score had the highest area under ROC curve (AUROC) 0.799 (0.738-0.851), followed by the COVID-GRAM 0.785 (0.723-0.838), NEWS 0.764 (0.700-0.819), and qCSI 0.749 (0.685-0.806). However, these differences were not statistical significant. CONCLUSION: Among the evaluated scores, the ISARIC-4C and the COVID-GRAM, calculated at ED admission, had the best performance, although the qCSI had similar efficacy by evaluating only three items. However, the NEWS, already widely validated in clinical practice, had a similar performance and could be appropriate for older patients with COVID-19.

Alzheimer’s disease (AD) is a type of dementia whose cause is incompletely defined. Copper (Cu) involvement in AD etiology was confirmed by a meta-analysis on about 6000 participants, showing that Cu levels were decreased in AD brain specimens, while Cu and non-bound ceruloplasmin Cu (non-Cp Cu) levels were increased in serum/plasma samples. Non-Cp Cu was advocated as a stratification add-on biomarker of a Cu subtype of AD (CuAD subtype). To further circumstantiate this concept, we evaluated non-Cp Cu reliability in classifying subtypes of AD based on the characterization of the cognitive profile. The stratification of the AD patients into normal AD (non-Cp Cu ≤ 1.6 µmol/L) and CuAD (non-Cp Cu > 1.6 µmol/L) showed a significant difference in executive function outcomes, even though patients did not differ in disease duration and severity. Among the Cu-AD patients, a 76-year-old woman showed significantly abnormal levels in the Cu panel and underwent whole exome sequencing. The CuAD patient was detected with possessing the homozygous (c.1486T > C; p.(Ter496Argext*19) stop-loss variant in the RGS7 gene (MIM*602517), which encodes for Regulator of G Protein Signaling 7. Non-Cp Cu as an add-on test in the AD diagnostic pathway can provide relevant information about the underlying pathological processes in subtypes of AD and suggest specific therapeutic options.

10. Sudden winter iloprost withdrawal in scleroderma patients during COVID-19 pandemic

Intravenous iloprost is currently recommended in the treatment of Raynaud's phenomenon (RP) refractory to oral therapy and of digital ulcers (DUs) related to systemic sclerosis (SSc). In real-life practice there is a huge heterogeneity about the Iloprost regimens used.A survey was carried out on SSc patients that interrupted Iloprost infusion to compare acral vascular symptoms just before Iloprost withdrawal and just after the missed infusion. Severity, and frequency of RP, new DUs onset or aggravation of those pre-existing were reported. Last available capillaroscopic images were also evaluated.The analysis includes 50 patients. After iloprost withdrawal, 11 patients reported a RP worsening because of enhanced intensity (p = 0.007). Only 8 patients of them also complained of an increased frequency (p = 0.07). None of the patients experienced digital ulcers for the first-time during quarantine. Among the 27 patients with a history of digital ulcers, 9 reported worsening and 7 recurrence of DUs. Overall, 17 patients (34.0 %) complained of a worsening of SSc vascular acral manifestations, namely RP or DUs. Reduced capillary density was associated with RP worsening, in particular, each unit increase of capillary density corresponds to an average 44 % decrease in the odds of RP worsening (OR 0.56, CI 95 % 0.36-0.97, p = 0.037). As for RP worsening, the aggravation of DU was associated with a lower capillary density.Low capillary density can predict a worsening of both RP and DUs in controlled quarantine conditions within a month after iloprost discontinuation in SSc patients.

In 1997, it was discovered that maternal plasma contains Cell-Free Fetal DNA (cffDNA). cffDNA has been investigated as a source of DNA for non-invasive prenatal testing for fetal pathologies, as well as for non-invasive paternity testing. While the advent of Next Generation Sequencing (NGS) led to the routine use of Non-Invasive Prenatal Screening (NIPT or NIPS), few data are available regarding the reliability and reproducibility of Non-Invasive Prenatal Paternity Testing (NIPPT or NIPAT). Here, we present a non-invasive prenatal paternity test (NIPAT) analyzing 861 Single Nucleotide Variants (SNV) from cffDNA through NGS technology. The test, validated on more than 900 meiosis samples, generated log(CPI)(Combined Paternity Index) values for designated fathers ranging from +34 to +85, whereas log(CPI) values calculated for unrelated individuals were below −150. This study suggests that NIPAT can be used with high accuracy in real cases.

Introduction The novel coronavirus SARS-Cov-2 can infect the respiratory tract causing a spectrum of disease varying from mild to fatal pneumonia, and known as COVID-19. Ongoing clinical research is assessing the potential for long-term respiratory sequelae in these patients. We assessed the respiratory function in a cohort of patients after recovering from SARS-Cov-2 infection, stratified according to PaO2/FiO2 (p/F) values. Method Approximately one month after hospital discharge, 86 COVID-19 patients underwent physical examination, arterial blood gas (ABG) analysis, pulmonary function tests (PFTs), and six-minute walk test (6MWT). Patients were also asked to quantify the severity of dyspnoea and cough before, during, and after hospitalization using a visual analogic scale (VAS). Seventy-six subjects with ABG during hospitalization were stratified in three groups according to their worst p/F values: above 300 (n = 38), between 200 and 300 (n = 30) and below 200 (n = 20). Results On PFTs, lung volumes were overall preserved yet, mean percent predicted residual volume was slightly reduced (74.8 ± 18.1%). Percent predicted diffusing capacity for carbon monoxide (DLCO) was also mildly reduced (77.2 ± 16.5%). Patients reported residual breathlessness at the time of the visit (VAS 19.8, p Conclusion Approximately one month after hospital discharge, patients with COVID-19 can have residual respiratory impairment, including lower exercise tolerance. The extent of this impairment seems to correlate with the severity of respiratory failure during hospitalization.

Atrophic rhinitis is characterized by progressive nasal mucosal atrophy, nasal crusting, fetor, and enlargement of the nasal space with paradoxical nasal congestion. Among diseases responsible of secondary atrophic rhinitis, infective- and noninfective-granulomatous and autoimmune diseases may be advocated, even if they represent less than 1% of etiologies. This group of pathologies should be considered especially in patients without medical history of nasal surgery or radiation and in presence of systemic symptoms, such as low-grade fever, weight loss, and arthralgias. The most frequent autoimmune diseases resulting in atrophic rhinitis are granulomatosis with polyangiitis, sarcoidosis, and mucous membrane pemphigoid. Among granulomatous infections, tuberculosis and syphilis are the leading reported causes of this group of rhinosinusitis. In this chapter, we review the main autoimmune and granulomatous causes of atrophic rhinitis focusing on ENT and systemic clinical aspects and systemic therapies.

To study the long-term psychological effects of Covid-19 disease, we recruited 61 patients older than 60 years of age and administered the Kessler questionnaire K10 to assess psychological distress and classify them according to mental health risk groups. Patients' affective temperaments were assessed with the 39-item form of the Temperament Evaluation of Memphis, Pisa, Paris, and San Diego (TEMPS-A-39) and emotional dysregulation with the Difficulties in Emotion Regulation Scale (DERS). Patients were divided in two samples according to their scores on the K10, i.e., a high likelihood of psychological distress group (N = 18) and a low likelihood of psychological distress group (N = 43). The two groups differed on their gender composition, in that more women (N = 11) were in the former and more men in the latter (N = 29) (χ2 = 4.28; p = 0.039). The high likelihood of psychological distress group scored higher on the Cyclothymic (3.39 ± 3.45 vs. 0.93 ± 1.08, p < 0.001) and the Depressive (2.28 ± 2.82 vs. 0.65 ± 1.09, p = 0.01) affective temperaments of the TEMPS and on the lack of Impulse control (12.67 ± 4.04 vs. 9.63 ± 3.14, p = 0.003) and lack of Clarity (15.00 ± 5.56 vs. 9.85 ± 4.67, p = 0.004) scales of the DERS. Our results show that having had Covid-19 may be related with high likelihood for psychological distress in advanced-age people and this may in turn be associated with impaired emotional regulation and higher scores on depressive and cyclothymic temperaments.

Background Marfan Syndrome (MFS) is a chronic, life-threatening, autosomal dominant connective tissue disorder caused by mutations in the FBN1 gene, coding for fibrillin-1. All organ systems may be affected, but particularly the cardiovascular system, eyes, and skeleton. Mortality generally results from cardiovascular complications, mainly aortic dissection. Currently, the diagnosis of MFS is based on the revised Ghent nosology. Molecular analysis of the FBN1 gene reduces diagnostic uncertainty in patients with suspected MFS or MFS-related disorders (MFS-RD). To date, more than 2700 FBN1 mutations are known. Methods Using Next Generation Sequencing (NGS) followed by Multiplex Ligation-dependent Probe Amplification on NGS-negative samples, we screened FBN1 gene on 124 unrelated patients (101 MFS fulfilling revised Ghent criteria, 20 suspected MFS, 3 MFS-RD) enrolled from 2008 to 2018 at the Multidisciplinary Marfan Clinic, Tor Vergata Hospital, Rome. Results An FBN1 variant was identified in 107/124 (86.3%) patients, including 48 novel variants (46 pathogenic/likely pathogenic, 2 VUS). A pathogenic/likely pathogenic variant was detected in 90/101 (89.1%) MFS patients. Our approach allowed early diagnosis for 10 young patients (age 3–19 years) with suspected MFS. Conclusions This study broadens the mutation spectrum of FBN1, providing a full update of the molecular basis of MFS in Italy.

A realidade exige que se (re)pensem os meios de controle sobre agentes policiais e as formas de coibi??o das pr?ticas violentas. ? neste ?mbito que se insere a presente pesquisa, que tem como objeto central a audi?ncia de cust?dia, implementada no Brasil em 2015 pelo Conselho Nacional de Justi?a. A sua finalidade ? proporcionar um maior controle do trabalho policial atrav?s da apresenta??o imediata da pessoa presa ou detida a uma autoridade judicial, de forma que o seu advento poderia contribuir com o trabalho de fiscaliza??o, representando um novo canal de den?ncias aos ?rg?os de controle. A partir do substrato te?rico que vem se formando sobre a audi?ncia de cust?dia, chega-se ? pergunta-problema que norteia a pesquisa: quais s?o os reflexos da implanta??o das audi?ncias de cust?dia no controle das pr?ticas de viol?ncia policial? Analisa-se, portanto, de que forma o advento das audi?ncias de cust?dia influencia o trabalho dos ?rg?os de controle, em especial do Minist?rio P?blico, quanto ? fiscaliza??o e coibi??o da viol?ncia. Para tanto, al?m da pesquisa te?rica realizada, apresenta-se um estudo de caso da Comarca de Porto Alegre/RS, mediante pesquisa etnogr?fica, aliando a observa??o direta das audi?ncias a entrevistas e conversas informais com os atores jur?dicos nelas envolvidos, al?m do levantamento de dados quantitativos. O trabalho, desenvolvido no Programa de P?s- Gradua??o em Ci?ncias Criminais da PUCRS, insere-se na ?rea de concentra??o Sistema Penal e Viol?ncia e na linha de pesquisa Sistemas Jur?dico-Penais Contempor?neos, embora dialogue tamb?m com a linha Viol?ncia, Crime e Seguran?a P?blica, em abordagem transdisciplinar. Para al?m do importante estudo te?rico do novo instituto, pretende-se colaborar com um debate cr?tico que proporcione a implanta??o efetiva da audi?ncia de cust?dia, acompanhada por uma mudan?a de paradigma em dire??o a um processo penal humanizado. Police violence is one of the faces of the systematic violation of human rights in Brazil and has been denounced by international organizations, having been the subject of a recent condemnation of the Brazilian State in the Inter-American Court of Human Rights (Favela Nova Bras?lia v. Brazil Case). Reality demands that one (re) thinks the means of control over police agents and the ways of restraining violent practices. This is the scope of this research, which has as its central object the bail hearing, implemented in Brazil in 2015 by the National Council of Justice. Its purpose is to provide greater control of police work through the immediate presentation of the person arrested or detained to a judicial authority, so that its advent could contribute to the inspection work, representing a new channel of complaints to the control bodies. From the theoretical substrate that has been formed on the bail hearing, we arrive at the problem question that guides the investigation: what are the reflexes of the implantation of bail hearings in the control of police violence practices? It is analyzed, therefore, how the advent of bail hearings influences the work of the control organs, especially of the Public Prosecutor's Office, regarding the control and restraining of violence. In addition to the theoretical research carried out, a case study of Porto Alegre/RS is presented, through ethnographic research, combining the direct observation of the hearing with interviews and informal conversations with legal actors involved in them, as well as the gathering of quantitative data. The work, developed in the Postgraduate Program in Criminal Sciences of PUCRS, is part of the Criminal System and Violence concentration and in the line of research on Contemporary Criminal-Legal Systems, although it also deals with Violence, Crime and Public Security, in a transdisciplinary approach. In addition to the important theoretical study of the new institute, it is intended to collaborate with a critical debate that provides for the effective implementation of the bail hearing, accompanied by a paradigm shift toward a humanized criminal process. Rhabdoid tumors are aggressive malignancies that show loss-of-function mutations of SMARCB1 gene, a member of the SWI/SNF chromatin-remodeling complex controlling gene transcription. One-third of patients affected by rhabdoid tumor harbor a germ-line mutation of SMARCB1 defining a rhabdoid tumor predisposition syndrome. The occurrence of a second somatic mutation determines the development of neoplasia in a two-hit model. Most germ-line mutations occur de novo, and few cases of recurrence in a sibship have been described. Here we report on a new Italian family with recurrence of SMARCB1 germ-line deletion in two siblings due to gonadal mosaicism. The deletion was identified in the 9-month-old proband with malignant rhabdoid tumor of the right kidney and disseminated metastases. Testing of both parents confirmed the de novo origin of the mutation, but recurrence was then detected prenatally in a new pregnancy. This is the sixth family with malignant rhabdoid tumor predisposition syndrome with the recurrence of the same germ-line SMARCB1 mutation in the sibship but not in healthy parents, suggesting that gonadal mosaicism is a less rare event than supposed. The clinical outcome in our patient confirms previous data of poorer outcome in patients with rhabdoid tumor predisposition syndrome.

- Abstract
O artigo busca contextualizar as possiveis formas de abordar os crimes sexuais cometidos durante a ditadura civil-militar iniciada no Brasil em 1964. Considerando a importância do reconhecimento destes crimes como parte da violencia estatal e institucional praticada, propoe-se a sua complementacao a partir de uma perspectiva de genero. Desde os aportes da criminologia feminista, o artigo questiona a universalizacao dos mecanismos utilizados para investigar os crimes cometidos durante o periodo ditatorial, chama a atencao para a possibilidade (inaceitavel, porem possivel) de revitimizacao a partir da inadequacao desses metodos e convoca ao desvelamento do posicionamento ideologico do Estado nas violencias sexual e de genero. O objetivo do artigo, assim, e questionar a necessaria, porem insuficiente perspectiva tradicional de tratamento dos crimes sexuais cometidos durante o periodo, reafirmando seu aspecto de violencia institucional, de responsabilidade do Estado, mas considerando a perspectiva de genero, a fim de possibilitar a superacao de vulnerabilidades historicas.
