Background: The Resection And Partial Liver Transplantation with Delayed total hepatectomy (RAPID) procedure involves left hepatectomy with orthotopic implantation of a left lobe and right portal vein ligation. This technique induces volumetric graft increase, allowing for a right completion hepatectomy within 15 days. Notably, there is a lack of data on the hemodynamics of small-for-size grafts exposed to portal overflow without triggering small-for-size syndrome. Methods: A prospective single-center protocol included 8 living donors and 8 RAPID noncirrhotic recipients. Comprehensive clinical and biological data were collected, accompanied by intraoperative arterial and portal flow and pressure measurements. Early kinetic growth rate (eKGR%) and graft function were assessed using computed tomography and 99Tc-mebrofenin scintigraphy on postoperative days 7 and 14. Findings were compared with retrospective data from 13 left living donor liver transplantation (LDLT) recipients. Results: The median Graft-body weight ratio was 0.41% (interquartile range: 0.34--0.49), markedly lower than in LDLT. However, there was no significant difference in eKGR between RAPID and LDLT grafts. Sequential analysis revealed variable eKGR per day: 10.6% (7.8--13.2) in the first week and 7.6% (6--9.1) in the second week posttransplantation. Indexed portal flow (indexed portal vein flow) was significantly higher in RAPID compared with left LDLT (P = 0.01). No hemodynamic parameters were found to correlate with regeneration speed. We modulated portal flow in 2 out of 8 cases. Conclusions: This study presents the first report of hemodynamic and volumetric data for the RAPID technique. Despite initial graft volumes falling below conventional LDLT recommendations, the study highlights acceptable clinical outcomes. [ABSTRACT FROM AUTHOR]

Metabolic dysfunction-associated steatohepatitis (MASH) confers a risk for cardiovascular diseases in patients. Animal models may help exploring the mechanisms linking liver and heart diseases. Hence, we explored the cardiac phenotype in two MASH mouse models: foz/foz mice fed a high-fat diet (HFD) for 24 or 60 weeks and C57BL/6J mice fed a high-fat-, high-cholesterol-, and high-fructose diet for 60 weeks. Angiotensin II (AngII) was used as an additional cardiovascular stressor for 4 weeks in 10 weeks HFD-fed foz/foz mice. Foz/foz mice with fibrosing MASH developed cardiac hypertrophy with adverse cardiac remodelling not seen in WT similarly fed the HFD. AngII caused hypertension and up-regulated the expression of genes contributing to pathological cardiac hypertrophy (Nppa, Myh7) more severely so in foz/foz mice than in controls. After 60 weeks of HFD, while liver disease had progressed to burn-out non steatotic MASH with hepatocellular carcinoma in 50% of the animals, the cardiomyopathy did not. In an independent model (C57BL/6J mice fed a fat-, cholesterol- and fructose-rich diet), moderate fibrosing MASH is associated with cardiac fibrosis and dysregulation of genes involved in pathological remodelling (Col1a1, Col3a1, Vim, Myh6, Slc2a1). Thus, animals with MASH present consistent adverse structural changes in the heart with no patent alteration of cardiac function even when stressed with exogenous AngII. Liver disease, and likely not overfeeding or aging alone, is associated with this cardiac phenotype. Over the past 20 years, basic research has robustly demonstrated the regulatory role of bile acids in physiological processes, primarily by the discovery and the study of their specific receptors and the understanding of the pathways they modulate. The dysregulation of the bile acid pool and the perturbation of bile acid signaling have been implicated in the pathophysiology of various clinical conditions, including cardiometabolic, cholestatic and inflammatory diseases. Consequently, bile acids have emerged as promising therapeutic targets, with compounds to modulate bile acid metabolism and signaling being actively investigated in pre-clinical and clinical settings. Despite these advancements, much remains to be understood before bile acid metabolism and bile acid-regulated pathways can be effectively manipulated for health protection. As a step forward in this direction, Clinical Science has curated a themed collection on the new discoveries in bile acids, gut microbiota and host interactions in health and diseases. This collection emphasizes mechanistic research papers, connecting basic science to disease mechanisms, aiming to enhance the understanding of the role of bile acids in physiological and pathophysiological conditions. This paper briefly presents the West African Monsoon (WAM) Modeling and Evaluation Project (WAMME) and evaluates WAMME general circulation models’ (GCM) performances in simulating variability of WAM precipitation, surface temperature, and major circulation features at seasonal and intraseasonal scales in the first WAMME experiment. The analyses indicate that models with specified sea surface temperature generally have reasonable simulations of the pattern of spatial distribution of WAM seasonal mean precipitation and surface temperature as well as the averaged zonal wind in latitude-height cross-section and low level circulation. But there are large differences among models in simulating spatial correlation, intensity, and variance of precipitation compared with observations. Furthermore, the majority of models fail to produce proper intensities of the African Easterly Jet (AEJ) and the tropical easterly jet. AMMA Land Surface Model Intercomparison Project (ALMIP) data are used to analyze the association between simulated surface processes and the WAM and to investigate the WAM mechanism. It has been identified that the spatial distributions of surface sensible heat flux, surface temperature, and moisture convergence are closely associated with the simulated spatial distribution of precipitation; while surface latent heat flux is closely associated with the AEJ and contributes to divergence in AEJ simulation. Common empirical orthogonal functions (CEOF) analysis is applied to characterize the WAM precipitation evolution and has identified a major WAM precipitation mode and two temperature modes (Sahara mode and Sahel mode). Results indicate that the WAMME models produce reasonable temporal evolutions of major CEOF modes but have deficiencies/uncertainties in producing variances explained by major modes. Furthermore, the CEOF analysis shows that WAM precipitation evolution is closely related to the enhanced Sahara mode and the weakened Sahel mode, supporting the evidence revealed in the analysis using ALMIP data. An analysis of variability of CEOF modes suggests that the Sahara mode leads the WAM evolution, and divergence in simulating this mode contributes to discrepancies in the precipitation simulation.
