The paper offers innovative cooking utensil designs for remote, isolated, and even peri-urban communities at a low price, with high reliability and simple construction. It can alleviate energy poverty and improve food security. This utensil uses only local solar energy directly and allows comfortable indoor cooking. This paper provides the design principles of a solar cooker/frying pan or generic heater, based on a PV panel or a plurality of them, which are directly connected to a plurality of Positive Thermal Coefficient (PTC) resistors to match the power. PTCs are nowadays produced in massive quantities and are widely available at low cost. The proposed device does not require an electronic controller or a battery for its operation. The aim is for family use, although the design can be easily scaled to a larger size or power, maintaining its simplicity. Electric heating inside or attached to the cooking pot, plus the temperature self-limiting effect of PTCs, allows for thermally insulating the cooking pot from its outside using ordinary materials. Insulation enhances energy efficiency during cooking and keeps cooked food warm for a long time. Clean development would receive a significant impulse with its application. A simple mathematical model describes its functioning and states guidelines for adequate design. Music performance anxiety (MPA) is a common damaging phenomenon in musicians' careers. Mindfulness stands as a promising construct to prevent MPA. However, the relationships between mindfulness and MPA are scarcely explored, alongside other relevant attention‐based (e.g., self‐consciousness) or emotion‐based constructs (e.g., negative affect). This study explores the relationships between these constructs. A sample of 151 musicians was assessed to explore the relationships between these constructs. Self‐reports of mindfulness, MPA, negative affect, and self‐consciousness were applied. We implemented network analysis following a general (second‐order) and specific (first‐order) framework. Networks showed dispositional mindfulness as negatively associated with negative affect and MPA in both general and facet levels, while mindfulness in past performances was only negatively associated with negative affect. MPA was positively associated with negative affect and self‐consciousness. Mindfulness displayed light or no associations with self‐consciousness. Therefore, mindfulness seems to be a relevant construct toward MPA. We propose a preliminary model to improve mindfulness research and interventions applied to music performers. Consensual non-monogamy (CNM) is a non-normative relationship that can generally be defined as relationships in which the involved individuals have openly agreed that they can have other sexual and/or affective partners. Despite growing interest in CNM, little research has been conducted regarding characteristics of the involved individuals, as well as comparing CNM individuals and monogamous individuals' characteristics. Expanding this research is important, as CNM constitutes a sizeable minority group, and CNM individuals and relationships are subject to stigma and dehumanization. The present study compared CNM and monogamous individuals in terms of personality (including "dark" personality) and mating orientations (sociosexuality and long-term mating) in young Spanish university students. Main results indicated that CNM individuals did not differ much in personality nor were they prone to higher "dark" personality traits when compared to monogamous individuals. Regarding mating orientations, CNM individuals were prone to higher sociosexuality levels and lower levels of long-term mating. Interestingly, results showed positive correlations between sociosexuality and "dark" personality in monogamous individuals, but not in CNM individuals. In conclusion, young Spanish university CNM individuals seem to be more pro-sex than monogamous people and less committed to the romantic love narrative of long-term mating, but these individuals were not more untrustworthy, self-centered, ruthless, or malevolent. Background: Catatonia is an underdiagnosed psychomotor syndrome that can occur in the context of various mental and somatic diseases. Malignant catatonia is particularly relevant in the context of intensive medical care. Clear recommendations in guidelines are missing. Objective: To present the current state of the diagnosis and treatment of catatonia, especially malignant catatonia. Material and methods: The literature was evaluated with respect to acute catatonic conditions, with a special focus on the differential diagnosis, relevance to intensive medical care and treatment of catatonia. Results: In psychiatric inpatients, catatonic syndromes are relatively frequent with a prevalence between 9% and 17%, and in neurological patients somewhat less frequent with a prevalence of 3.3%. There is a clear recommendation for pharmacological treatment with lorazepam. Additional electroconvulsive therapy (ECT) should be considered as early as possible, especially in cases not responding to benzodiazepines. Response rates to ECT have been shown to be 80–100%. In malignant catatonia, ECT should be performed immediately as an emergency indication. Conclusion: Several factors lead to the underdiagnosis of catatonia. It is problematic that even life-threatening malignant catatonia is often not recognized as such, although there is a mortality of about 50% if untreated. The present study aimed to use a novel method (i.e., network analysis) to examine if the Internet Gaming Disorder Scale–Short-Form (IGDS9-SF) has its unidimensional structure supported across different subgroups, including gender groups, continent groups, and age groups. Participants from five continents (n = 545 in Africa; n = 27,910 in Americas; n = 4640 in Asia; n = 88,835 in Europe; and n = 1633 in Oceania) completed the IGDS9-SF. The IGDS9-SF items showed a positively correlated network with moderate-to-high predictability (mean of R2 = 0.338, SD = 0.068). Moreover, the network analysis results suggested that the IGDS9-SF is very likely to have a one-factor model. This one-factor model was further found to be invariant across gender groups (men vs. women), continent groups (Africa, Americas, Asian, Europe, and Oceania), and age groups (minor aged under 18 years, young adults aged between 18 and 30 years, and adults aged 30 years or above). Objectives: Malignant catatonia (MC) is a rare, yet potentially life-threatening neuropsychiatric condition. Evidence on its therapy is weak, treatment recommendations are scarce and predominantly unprecise. The aim of this study was to compare the effectiveness of different MC treatment approaches regarding outcome and severity of MC. Methods: We conducted systematic searches for MC case reports in biomedical databases and the psychiatric archive of University Hospital Ulm. Treatments were compared considering MC severity and temporal aspects. Results: A total of 117 cases were included. Treatment had a significant influence on outcome: treatment with both benzodiazepines and electroconvulsive therapy (ECT) entailed the most favourable, purely supportive therapy the least favourable outcome. Earlier application of benzodiazepines was significantly associated with a favourable outcome. A classification of MC severity was developed. Patients with severe MC were significantly more often subject to intensive care treatment and had a 78% higher risk of dying than in moderate MC. Conclusions: This is the first study to introduce a severity classification for MC, and the largest to compare outcomes of MC treatments with clear distinction from neuroleptic malignant syndrome (NMS). Linear Fresnel collectors offer great potential for producing solar heat for industrial processes, both on a small and large scale. Thermal oil, pressurized water, or steam are used as heat transfer fluid in most applications, due to their suitable thermal properties. A heat exchanger is required for thermal power delivery to the user unless the heat transfer fluid is directly used as a medium in the industrial process. For those industrial applications using hot air as a process fluid, direct air heating inside linear Fresnel collectors can be an interesting solution aiming at simplification, cost reduction, and environmental and safety benefits. In this experimental study air is heated inside a commercial small-scale Linear Fresnel collector row of 79.2 m2 active surface, demonstrating the viability of direct air heating up to 500 °C at its outlet. A commercial turbocharger pressurizes the air and reduces the blowing power. The purposely designed test campaign carried out in Madrid (Spain) during spring and summer seasons allowed the experimental contrast of a detailed 1D opto-thermal numerical model. Both static and purposely developed dynamic models of the receiver tube have been scrutinized and tested. The models and the experimental information allowed for the accurate characterization of both thermal losses and optical efficiency. • Air is directly heated inside a commercial small-scale linear Fresnel collector. • Air temperature up to 500 °C is reached without receiver tube damage. • A dynamic 1D model is developed and experimentally verified. • The optical collector efficiency is experimentally estimated. • The solar tracking accuracy is characterized indicating specific improvements. Romantic relationships are changing in Western culture. The recent rise of alternative relationship models called consensual non-monogamies (CNMs) has drawn attention toward them. Western cultures largely stereotype and stigmatize CNMs, portraying their practitioners (among other aspects) as psychologically distinct from monogamous practitioners. Likewise, recent scientific literature suggests probable differences for CNMs practitioners, mainly them being more extraverted, agreeable, and open, but also less jealous and dominant toward their romantic partners. However, although CNMs differentiate themselves from cheating monogamous practitioners, potential differences are unclear. The present study compared 372 participants (with 193 CNMs practitioners) from Spanish culture regarding their personality, sexual satisfaction, dominance, and jealousy. On most dimensions, monogamous and CNMs practitioners did not differ significantly from each other. CNMs practitioners showed significantly higher openness and lower conscientiousness, although these differences were small and debatable; thus, these findings should be taken with caution. These differences could not be explained by differences in age, gender, education, marital status, sexual orientation, or other demographic variables. The addition of cheating monogamous mitigated these differences. Overall, this study suggests that monogamous and non-monogamous practitioners are not psychologically different. The anatomy of the archosaurian pelvis and hindlimb has adopted a diversity of successful configurations allowing a wide range of postures during the evolution of the group (e.g., erect, sprawling). For this reason, thorough studies of the structure and function of the pelvic and hindlimb musculature of crocodylians are required and provide the possibility to expand their implications for the evolution of archosaurian locomotion, as well as to identify potential new characters based on muscles and their bony correlates. In this study, we give a detailed description of the pelvic and hindlimb musculature of the South American alligator Caiman yacare, providing comprehensive novel information regarding lower limb and autopodial muscles. Particularly for the pedal muscles, we propose a new classification for the dorsal and ventral muscles of the autopodium based on the organisation of these muscles in successive layers. We have studied the myology in a global background in which we have compared the Caiman yacare musculature with other crocodylians. In this sense, differences in the arrangement of m. flexor tibialis internus 1, m. flexor tibialis externus, m. iliofibularis, mm. puboischiofemorales internii 1 and 2, between Ca. yacare and other crocodylians were found. We also discuss the muscle attachments that have different bony correlates among the crocodylian species and their morphological variation. Most of the correlates did not exhibit great variation among the species compared. The majority of the recognised correlates were identified in the pelvic girdle; additionally, some bony correlates associated with the pedal muscles are highlighted here for the first time. This research provides a wide framework for future studies on comparative anatomy and functional morphology, which could contribute to improving the character definition used in phylogenetic analyses and to understand the patterns of musculoskeletal hindlimb evolution. Gracilisuchus stipanicicorum Romer, 1972 is a basal suchian from the Late Triassic Chañares Formation (Argentina), nested in the recently erected Gracilisuchidae, along with Turfanosuchus dabanensis Young, 1973 and Yonghesuchus sangbiensis Wu et al., 2001 from China. The six known specimens of Gracilisuchus Romer, 1972 preserve most of the skeleton, lacking only most of the shoulder girdle and forelimb. Our latest fieldwork has recovered two specimens that preserve previously unknown elements, including the humerus, radius, and ulna, as well as the femur, presacral vertebrae, and paramedian osteoderms. The femur and osteoderms were histologically sectioned, which has never been done for Gracilisuchidae. The anatomical analysis revealed characters in the new elements that improve our understanding of the anatomy of Gracilisuchus. Bone histology revealed that the specimen CRILAR PV 490 died before reaching somatic maturity and that growth was relatively slow compared to other pseudosuchians. This study increases the knowledge of the anatomy of Gracilisuchus and represents a new step towards a broader understanding of Gracilisuchidae and basal suchian clades. [ABSTRACT FROM AUTHOR]

Introduction: Neuroleptic malignant syndrome (NMS) is a rare, potentially life‐threatening antipsychotic‐associated disorder that requires an efficient and timely therapy. The aim of the study was to compare the effectiveness of different NMS therapies and to analyze its outcome depending on NMS severity. Method: Systematic search for NMS cases in biomedical databases. The focus of the analysis was on therapy with dantrolene, bromocriptine, and electroconvulsive therapy (ECT) when each was compared with symptomatic therapy. Primary outcomes were the survival rate and the duration of treatment. Result: 405 case reports were included. Overall, no statistically significant differences regarding mortality rate or duration of treatment were found between dantrolene, bromocriptine, or ECT compared to supportive care. A subgroup analysis regarding NMS severity showed that the mortality under specific NMS pharmacotherapy (dantrolene, bromocriptine) and under ECT was significantly lower than under purely symptomatic therapy in severe NMS (P = 0.018). The difference was not significant in mild and moderate cases. Discussion: An overall superiority of the specific NMS therapy (dantrolene, bromocriptine, and ECT) was not found in this study. When regarding severity classification, specific therapies were superior but only in severe cases, and ECT showed the lowest mortality rate. In previous case series, an effect on survival or the duration of the disease could only be observed in part for specific therapies, but the evidence available is inconsistent. The results of this study support our hypothesis that NMS treatment with dantrolene, bromocriptine, and ECT is advantageous over purely symptomatic therapy in severe NMS cases. Despite the widespread use of antibiotics, bacterial pneumonias in donors strongly predispose to the fatal syndrome of primary graft dysfunction (PGD) following lung transplantation. We report that bacterial endotoxin persists in human donor lungs after pathogen is cleared with antibiotics and is associated with neutrophil infiltration and PGD. In mouse models, depletion of tissue-resident alveolar macrophages (TRAMs) attenuated neutrophil recruitment in response to endotoxin as shown by compartmental staining and intravital imaging. Bone marrow chimeric mice revealed that neutrophils were recruited by TRAM through activation of TLR4 in a MyD88-dependent manner. Intriguingly, low levels of endotoxin, insufficient to cause donor lung injury, promoted TRAM-dependent production of CXCL2, increased neutrophil recruitment, and led to PGD, which was independent of donor NCMs. Reactive oxygen species (ROS) increased in human donor lungs starting from the warm-ischemia phase and were associated with increased transcription and translocation to the plasma membrane of TLR4 in donor TRAMs. Consistently, scavenging ROS or inhibiting their production to prevent TLR4 transcription/translocation or blockade of TLR4 or coreceptor CD14 on donor TRAMs prevented neutrophil recruitment in response to endotoxin and ameliorated PGD. Our studies demonstrate that residual endotoxin after successful treatment of donor bacterial pneumonia promotes PGD through ischemia/reperfusion-primed donor TRAMs. [ABSTRACT FROM AUTHOR]

The Five Facets Mindfulness Questionnaire (FFMQ) is a popular tool in mindfulness research. However, its psychometric qualities and its replicability have caused controversy. This study carried out a psychometric review and a conceptual replication of the FFMQ latent structure. The review showed that previous validation studies of the FFMQ used nonoptimal methods. In addition, this conceptual replication study tested the structure of the FFMQ using frequentist and Bayesian techniques. The original structure did not provide a good fit with both techniques, while the proposed alternative provided mixed results. We also found systematic fit improvements in both techniques when the Observe facet was excluded and method factors were included. With these findings, we conclude that the conceptual replication of the FFMQ's structure failed. Alternatively, we propose a new provisional FFMQ model with a set of recommendations regarding its application. Background: Volume reductions in brain structures of patients with schizophrenia spectrum disorder (SSD) have repeatedly been found in voxel-based morphometry MRI studies. Hence, an underlying neurodegenerative etiological component of SSD is currently being discussed. In recent years, the imaging method of optical coherence tomography (OCT) has shown its potential in evaluating structural changes in the retina in patients with confirmed neurodegenerative disorders, providing a window into the brain.Methods: Twenty-six patients with schizophrenia or schizoaffective disorder and 23 age- and sex-matched healthy controls were examined with the Heidelberg Spectralis OCT system to derive a single-layer analysis of both retinas. The segmentation of retinal layers was manually corrected to minimize artifacts and software imprecisions.Results: Compared to the control group, SSD patients showed reduced thickness and volume measurements for nearly all retinal layers, and these differences reached significance for macular volume, macular thickness, retinal nerve fiber layer (RNFL) and inner nucleiform layer (INL). Furthermore, a significant correlation between the duration of illness and the total volume of the RNFL was found.Conclusion: Our OCT measurements demonstrate reduced single retinal layer thickness in patients with SSD. In the context of the MRI volume changes, our results provide further evidence that structural changes seen in the brain of patients are also observable in the retina, potentially allowing further insights into the different components of the nervous system that are altered in this highly etiologically complex disorder. The Avoidance and Fusion Questionnaire for Youth-Short Form (AFQ-Y8) was designed to measure psychological inflexibility in children and adolescents. However, it has not yet been validated to the Spanish population. The present study examines the factorial structure, internal consistency, convergent and incremental validity, and measurement invariance between children and adolescents, male and female, and male and female across age of the AFQ-Y8 with 982 students from Spain (children = 459, adolescents = 523). The results confirmed, in both children and adolescents, a unifactorial structure of general psychological inflexibility. AFQ-Y8 was positively related to negative psychological health outcomes, and negatively related to positive psychological health outcomes and acceptance-related skills, such as mindfulness. In addition, psychological inflexibility showed incremental validity in the explanation of negative psychological health over mindfulness. Measurement invariance across age and gender groups was supported, suggesting that the scores in the AFQ-Y8 are comparable between children and adolescents, and between males and females. In conclusion, results show that the Spanish version of the AFQ-Y8 is a reliable and valid measure of psychological inflexibility in children and adolescents. Introduction The neuroleptic malignant syndrome (NMS) is a potentially life-threatening condition associated to the use of antipsychotics. Since it requires rapid and efficient medical care, high-quality treatment guidelines should be available. In this article, we analyzed and compared different international therapy guidelines for the treatment of schizophrenia, in which NMS treatment recommendations might be contained. Methods We performed an Internet-based search for schizophrenia guidelines via the website of the respective medical society. Guidelines in English, French, Italian, and German from countries whose medical care meets high standards were selected for further analysis and comparison of the NMS treatment recommendations (if present), and their underlying evidence. Results The NMS is mentioned in 12 of 14 guidelines. Only 9 report concrete therapy recommendations (benzodiazepines/dantrolene/bromocriptine/amantadine/intensive care and/or electroconvulsive therapy (ECT)), however, with high heterogeneity. Only 5 guidelines included all possible drug therapy options and ECT, but with differing combination strategies, dosages, application forms, and combinability of options. The level of evidence of the different recommendations was estimated as low. Discussion One-third of the selected guidelines do not report any NMS therapy recommendations. Most guidelines mentioning the NMS do not provide therapy recommendations that include all relevant treatment options. The results show a very high heterogeneity, and the recommendations and statements are of low-evidence levels. The lack of knowledge about the NMS and its treatment may delay the onset of therapy, impair the quality of treatment, and lead to a worse outcome or death. [ABSTRACT FROM AUTHOR]

Objective Malignant catatonia (MC) is an extremely rare, life-threatening disorder. It is characterized by catatonic symptoms accompanied by autonomic instability, hyperthermia, and changes in laboratory values. In many cases, MC is not recognized as such. Evidence-based guidelines are essential to ensure quality of treatment, but what do current national and international guidelines recommend? Method Online search for international guidelines from English-, French-, Italian-, and German-speaking countries whose medical care meets high standards addressing the treatment of MC. These were analyzed and compared regarding statements on MC, recommendations, and strength of scientific evidence. Results Fifteen of the identified guidelines were included. Only 5 of 15 comment on the treatment of MC. As for other rare diseases, no detailed recommendations are available. Suggested therapies are limited to benzodiazepines and electroconvulsive therapy. Levels of evidence and grades of recommendation are predominantly low. Conclusion Many international guidelines do not mention MC. It is not possible to derive a clear algorithm for the treatment of MC from most current guidelines. A thorough update of most guidelines appears to be necessary. Introduction: The paradigm shift to a knowledge-based economy has incremented the use of personal information applied to health-related activities, such as biomedical research, innovation, and commercial initiatives. The convergence of science, technology, communication and data technologies has given rise to the application of big data to health; for example through eHealth, human databases and biobanks.Methods: In light of these changes, we enquire about the value of personal data and its appropriate use. In order to illustrate the complex ground on which big data applied to health develops, we analyse the current situation of the European Union and two cases: the Catalan VISC+/PADRIS and the UK Biobank, as perspectives.Discussion and Conclusions: Personal health-related data in the context of the European Union is being increasingly used for big data projects under diverse schemes. There, public and private sectors participate distinctively or jointly, pursuing very different goals which may conflict with individual rights, notably privacy. Given that, this paper advocates for stopping the unjustified accumulation and commercialisation of personal data, protecting the interests of citizens and building appropriate frameworks to govern big data projects for health. A core tool for achieving such goals is to develop consent mechanisms which allow truly informed but adaptable consent, conjugated with the engagement of donors, participants and society. The formation of dimer [(μ-Cl)Rh-(κ3(P,Si,Si)PhP(o-C6H4CH2SiiPr2)(o-C6H4CH2SiiPrnPr))]2 (Rh-3) with an n-propyl group on one of the silicon atoms as a minor product was affected by the reaction of [RhCl(COD)]2 with proligand PhP(o-C6H4CH2SiHiPr2)2, L1. The major product of the reaction was monomeric 14-electron Rh(III) complex [ClRh(κ3(P,Si,Si)PhP(o-C6H4CH2SiiPr2)2)] (Rh-1). Computations revealed that the monomer–dimer equilibrium is shifted toward the monomer with four isopropyl substituents on the two Si atoms of the ligand as in Rh-1; conversely, the dimer is favored with only one n-propyl as in Rh-3, and with less bulky alkyl substituents such as in [ClRh(κ3(P,Si,Si)PhP(o-C6H4CH2SiMe2)2]2 (Rh-2). Computations on the mechanism of formation of Rh-3 directly from [RhCl(COD)]2 are in agreement with the experimental findings and it is found to be less energetic than if stemming from Rh-1. Additionally, a Si–O–Si complex, [μ-Cl-Rh{κ3(P,Si,C)PPh(o-C6H4CH2SiiPrO SiiPr2CH-o-C6H4)}]2, Rh-4, is generated from the reaction of Rh-1 with adventitious water as a result of intramolecular C–H activation. Background: Recent studies on the pathophysiology of major depression (MD) indicate that degenerative and inflammatory processes may play a role. This finding is supported by magnetic resonance imaging (MRI)-based meta-analysis that show volume reductions in circumscribed areas of the brain in patients with MD. Using optical coherence tomography (OCT), retinal changes have been demonstrated in neurodegenerative disorders. In light of this inflammatory/degenerative hypothesis, we tested whether patients with MD exhibit retinal alterations that might correlate with the severity and duration of the disease.Methods: Patients with MD and age- and gender-matched healthy controls were recruited for the measurement of the total volume and thickness of their retina as well as the thicknesses and volumes of five different retinal layers using single-layer-analysis provided by the spectral-domain-OCT.Results: OCT data from 28 patients with MD and 20 healthy controls were available for evaluation. The exploratory intra-individual group comparison of the two eyes showed a small but significant difference in the retinal total volume (right = 8.69mm3; left = 8.72mm3; p = 0.03) only in patients with MD. There were no other significant differences between the patients with MD and the healthy controls with respect to the OCT measurements.Limitations: The small group size as well as the absence of correction for multiple testing due to the exploratory design should be considered as limitations of our study.Conclusion: While retinal total volume differs between the eyes of patients with MD, the comparison of retinal parameters between these patients and age- and gender-matched healthy volunteers did not show any differences. The stable flies, Stomoxys calcitrans (L.) (Diptera: Muscidae), are blood-sucking insects that represent a serious problem not only because of their hematophagy and transmission of pathogens, but also because of their considerable economic impact In animal production. No colony of these Insects- exists in Argentina. The objectives of this work were to establish the first Stomoxys calcitrans colony in this country, and to standardize and report the duration of each stage under controlled laboratory conditions. The adult flies were successfully reared in a growth chamber (28 ± 1 °C and 47 ± 1 %RH, under a 14 h light: 10 h dark photoperiod); whereas the immature stages were kept at 25 ± 2 °C and natural light. The cycle from eggs to adult emergence lasted 16.75 ± 2.9 days. The developmentTime required to reach a new stage was: 2.0 ± 0.8, 6.75 ± 1.3 and 7.75 ± 1.7 days for eggs, larvae and pupae, respectively. Adult flies lived 16.5 ± 1.91 days. The preoviposltion period lasted 5 ± 0.8 days. The larval and pupal survivals were 93.28% and 70.25%, respectively. Gases are sensed by lung cells and can activate specific intracellular signalling pathways, and thus have physiological and pathophysiological effects. Carbon dioxide (CO2), a primary product of oxidative metabolism, can be sensed by eukaryotic cells eliciting specific responses via recently identified signalling pathways. However, the physiological and pathophysiological effects of high CO2 (hypercapnia) on the lungs and specific lung cells, which are the primary site of CO2 elimination, are incompletely understood. In this review, we provide a physiological and mechanistic perspective on the effects of hypercapnia on the lungs and discuss the recent understanding of CO2 modulation of the alveolar epithelial function (lung oedema clearance), epithelial cell repair, innate immunity and airway function. An extension for 1-D transient two-phase flows of the SIMPLE-ESDIRK method, initially developed for incompressible viscous flows by Ijaz is presented. This extension is motivated by the high temporal order of accuracy demanded to cope with fast phase change events. This methodology is suitable for boiling heat exchangers, solar thermal receivers, etc. The methodology of the solution consist in a finite volume staggered grid discretization of the governing equations in which the transient terms are treated with the explicit first stage singly diagonally implicit Runge-Kutta (ESDIRK) method. It is suitable for stiff differential equations, present in instant boiling or condensation processes. It is combined with the semi-implicit pressure linked equations algorithm (SIMPLE) for the calculation of the pressure field. The case of study consists of the numerical reproduction of the Bartolomei upward boiling pipe flow experiment. The steady-state validation of the numerical algorithm is made against these experimental results and well known numerical results for that experiment. In addition, a detailed study reveals the benefits over the first order Euler Backward method when applying 3rd and 4th order schemes, making emphasis in the behaviour when the system is subjected to periodic square wave wall heat function disturbances, concluding that the use of the ESDIRK method in two-phase calculations presents remarkable accuracy and computational advantages. Background: Tracheal suctioning costs are higher with a closed tracheal suction system (CTSS) than with an open system (OTSS), due to the need for complete daily change as recommended by the manufacturer. However, is it necessary to change the closed system daily?Objective: To evaluate the tracheal suctioning costs and incidence of ventilator-associated pneumonia (VAP) using closed system without daily change vs OTSS.Design: Prospective and randomised study.Setting: An Intensive Care Unit in a university hospital.Patients: Patients requiring mechanical ventilation.Interventions: Patients were randomly assigned to CTSS without daily change or OTSS. We used a CTSS that allowed partial or complete change.Measurements and Results: There were no significant differences between both groups of patients (236 with CTSS and 221 with OTSS) in gender, age, diagnosis, APACHE-II score, mortality, number of aspirations per day, percentage of patients who developed VAP (13.9 vs 14.1%) or the number of ventilator-associated pneumonia per 1000 days of mechanical ventilation (14.1 vs 14.6). There were not significant differences in tracheal suctioning costs per patient/day between CTSS vs OTSS (2.3+/-3.7 vs 2.4+/-0.5 Euros; p=0.96); however, when length of mechanical ventilation was lower than 4 days, the cost was higher with CTSS than with OTSS (7.2+/-4.7 vs 1.9+/-0.6 Euros; p<0.001); and when length of mechanical ventilation was higher than 4days, the cost was lower with CTSS than with OTSS (1.6+/-2.8 vs 2.5+/-0.5 Euros; p<0.001).Conclusion: CTSS without daily change is the optimal option for patients needing tracheal suction longer than 4 days. Objective: The aim of this study was to analyze the prevalence of ventilator-associated pneumonia (VAP) using a closed-tracheal suction system vs. an open system.Design: Prospective and randomized study, from October 1, 2002, to December 31, 2003.Setting: A 24-bed medical-surgical intensive care unit in a 650-bed tertiary hospital.Patients: Patients requiring mechanical ventilation for >24 hrs.Interventions: Patients were randomized into two groups; one group was suctioned with the closed-tracheal suctioning system and another group with the open system.Measurements: Throat swabs were taken at admission and twice a week until discharge to classify pneumonia in endogenous and exogenous.Main Results: A total of 443 patients (210 with closed-tracheal suction system and 233 with the open system) were included. There were no significant differences between groups of patients in age, sex, diagnosis groups, mortality, number of aspirations per day, and Acute Physiology and Chronic Health Evaluation II score. No significant differences were found in either the percentage of patients who developed VAP (20.47% vs. 18.02%) or in the number of VAP cases per 1000 mechanical ventilation-days (17.59 vs. 15.84). There were also no differences in the VAP incidence by mechanical ventilation duration. At the same time, we did not find any differences in the incidence of exogenous VAP. Likewise, there were also no differences in the microorganisms responsible for pneumonia. Patient cost per day for the closed suction was more expensive than the open suction system (11.11 US dollars +/- 2.25 US dollars vs. 2.50 US dollars +/- 1.12 US dollars, p < .001).Conclusion: We conclude that in our study, the closed-tracheal suction system did not reduce VAP incidence, even for exogenous pneumonia. El presente análisis bibliométrico forma parte de la tesis doctoral «El Sistema Diseño en la Euroregión del Arco Mediterráneo: prioridades para la caracterización de un modelo estratégico»1. Su propósito es configurar una herramienta con la cual se pueda comprender mejor la bibliografía relacionada con la implementación de las políticas de diseño en regiones o países; este análisis bibliométrico no solo es útil para creadores de políticas, sino también para investigadores de diferentes áreas de conocimiento en diseño. Este estudio comenzó con una búsqueda de términos claves, relacionados con políticas de diseño en Bases de Datos Bibliográficas (BDB) y motores de búsqueda, seguida por la utilización de herramientas y parámetros comparativos y de evaluación de impacto en las publicaciones seleccionadas, finalizando con el análisis de la información por medio del software The Vantage Point. Las políticas de diseño impulsan el desarrollo de los recursos de este campo en una región, lo que puede influir directamente en la dinamización de la economía, pero como campo de estudio carece de suficiente material publicado en BDB para poder realizar un análisis bibliométrico relevante, ya que gran parte del material generado por autores especializados es de carácter divulgativo y no científico (50% de los documentos identificados en este estudio). Además, las publicaciones de diseño carecen de una categoría de indexación específica, por lo que no hay parámetros comparativos homogéneos. This bibliometric analysis is part of the doctoral thesis entitled "The Design System in the Mediterranean Arch Euroregion: Priorities for the Characterization of a Strategic Model»2. Its purpose is to set up a tool with which to better understand the literature pertaining to the implementation of design policies in regions or countries. This bibliometric analysis not only is useful for policy makers, but also for researchers in different areas of knowledge in design. This study began with a search for key terms related to design policies in bibliographic databases (BDB) and search engines, followed by the use of tools and comparative parameters, as well as with the assessment of impact in the selected publications. It ended with the analysis of the information by means of the The Vantage Point software. Design policies drive the development of resources in this field in a region. This can have a direct influence on the revitalization of the economy. Nevertheless, as a field of study, it lacks of enough material published in BDB to be able to make a relevant bibliometric analysis, since great part of the material generated by specialized authors is more informative rather than scientific (50% of the documents identified in this study). Background: Nontuberculous mycobacteria (NTM) are common bacteria in water and especially water supply distribution systems. Some species can cause infections, especially in immunocompromised patients and other risk groups. This study examined the frequency of occurrence of NTM in 135 household potable water samples collected from household water taps in Tenerife Island.Methods: Mycobacteria species were identified by polymerase chain reaction targeting the 16S rRNA and 16S-23S rRNA regions, and by double-reverse hybridization on a dipstick using colloidal gold-bound and membrane-bound probes (Speed-Oligo(®) Mycobacteria). Some species were identified by sequencing the gene that encodes the 16S rRNA region.Results: NTM were present in 47.4% of the samples. Mycobacterium fortuitum was the NTM isolated most frequently (70.3%), followed by Mycobacterium canariasense (6.3%) and Mycobacterium chelonae (6.3%). Other species were isolated at lower percentage frequencies.Conclusion: We isolated and identified the species M. canariasense in water supplies for public consumption. This species has previously been reported only in hospital settings. Background This study explores the safety and efficacy of strontium 90 (Sr-90) brachytherapy as the sole adjuvant therapy for carcinoma in situ (CIS) and squamous cell carcinoma (SCC) of the conjunctiva in a high HIV prevalent area. Methods This is a retrospective case review of patients treated with 60 Gray Sr-90 brachytherapy in four divided doses after resection with a 2 mm margin and histological confirmation. Cryotherapy or alcohol debridement was not performed at the time of excision due to limited resources. Two plaque sizes, 8.5 mm and 18 mm, were used. Results Sixty-nine patients were treated and had a median follow-up of 27 months (range 6-127). Thirtythree (47.8%) were HIV-positive. CIS was present in 40.6% and SCC in 59.4%. The surgical margins were positive in 39 (56.5%). Twenty patients (29.0%) were treated with the 18 mm plaque and 49 (71.0%) with the 8.5 mm plaque. Eight (11.6%) patients developed a recurrence at a median of 5 months (range 2-40). Recurrences only occurred in patients treated with the 8.5 mm plaque (p=0.094). There was no significant effect of HIV status, positive margins or staging on the number of recurrences. Treatment side effects were a dry eye in five patients which was successfully managed with topical lubricants, and induced astigmatism of 1 dioptre of cylinder in one patient. Conclusions Sr-90 brachytherapy is safe and effective in preventing recurrences in ocular surface squamous neoplasia in a high HIV prevalent setting. Background: The Intensification of Job Demands Scale (IDS) is a multidimensional instrument designed to capture the phenomenon of work intensification that characterizes modern Western societies. However, in Spain the instrument has not been validated to assess this phenomenon. The aim of this study was to examine the psychometric properties of the IDS adaptation with a heterogeneous sample of Spanish workers. Method: The sample consisted of 722 Spanish workers (57.8% female, 42.2% male) who completed a survey between January and April 2022. Results: A five-factor correlated model, as in the original IDS, had an optimal fit with 15 items, and displayed invariance across gender, level of education, and organizational tenure. Reliability coefficients were all satisfactory, and scale adaptation showed evidence of convergent, discriminant, and incremental validity. Conclusions: The Spanish adaptation of the IDS is a valid, reliable measure for analyzing the phenomenon of work intensification and intensified job demands in Spain. [ABSTRACT FROM AUTHOR]

The authors quantified the frequency of TO possession in an inpatient psychiatric setting and assessed whether TO use is specific to a personality disorder (PD) diagnosis, focusing on borderline PD (BPD). TO possession was assessed using the Transitional Objects Questionnaire, and PD diagnosis was established using standard DSM-IV clinical interviews. Of the 104 female patients assessed, 57.7% showed TO use; 84% of BPD patients, 71% of BPD-trait patients, 65% of patients with PD traits (other than BPD), and 56% of PD patients (other than BPD) displayed TO use, whereas 30.6% of patients without PD showed TO use. Patients with TOs were significantly younger and had significantly longer hospital stays. The specificity and sensitivity for TO use in the BPD group were 0.506 and 0.84, respectively. Clonal distribution of methicillin-resistant Staphylococcus aureus (MRSA) in hospitals may differ according to the geographic location and time period. Knowledge of MRSA clonal epidemiology in hospital settings involves much more than the study of healthcare-associated MRSA (HA-MRSA) clones. In recent years, investigators have documented the introduction of both community-associated MRSA (CA-MRSA) and livestock-associated MRSA (LA-MRSA) clones, the emergence of clones carrying Staphylococcal cassette chromosome mec (SCC mec) XI, and the genetic diversity among sporadic MRSA isolates. The allocation of certain antibiotypes to dominant MRSA clones in an institution allows their use as phenotypic markers for a preliminary search for new clones, early detection of clonal shift, and as a guide for better empirical therapy, infection control, and treatment within a particular institution. For these reasons, we identified 938 strains detected in a System of Universal Active Surveillance of MRSA in clinical samples during the period 2009-2010, obtaining the clonal distribution of MRSA at the Hospital Universitario de Canarias (Tenerife, Spain) and the relationship between antimicrobial susceptibility and three major clones present. The antibiotypes that best defined the ST5-MRSA-IV (Pediatric) clone showed resistance to tobramycin and susceptibility to clindamycin, erythromycin, gentamicin, rifampin, trimethoprim-sulfamethoxazole, vancomycin, quinupristin/dalfopristin, and linezolid, whereas the ST22-MRSA-IV clone (EMRSA-15) showed susceptibility to these antibiotics, and finally, the ST36-MRSA-II clone (EMRSA-16) was resistant to clindamycin, erythromycin, and tobramycin and susceptible to the remaining antimicrobials. β2‐Adrenergic agonists have been shown to regulate Na,K‐ATPase in the alveolar epithelium by recruiting Na,K‐ATPase‐containing vesicles to the plasma membrane of alveolar epithelial cells (AEC). Here, we provide evidence that β2‐agonists induce store‐operated calcium entry (SOCE) in AECs. This calcium entry is necessary for β2‐agonist‐induced recruitment of Na,K‐ATPase to the plasma membrane of AECs. Specifically, we show that β2‐agonists induce SOCE via stromal interaction molecule 1 (STIM1)‐associated calcium release‐activated calcium (CRAC) channels. We also demonstrate that the magnitude of SOCE affects the abundance of Na,K‐ATPase at the plasma membrane of AECs.β2‐Agonists elicit store‐operated Ca2 + entry (SOCE) in alveolar epithelial cells. Ca2+ entry is necessary for the β2‐agonist‐induced upregulation of Na,K‐ATPase. STIM1‐associated CRAC channels mediate the β2‐agonist‐induced SOCE. Objective: Repetitive transcranial magnetic stimulation (rTMS) is an approved treatment for depression. The clinical relevance of its efficacy is unclear. The clinical relevance of findings in the rTMS literature was assessed by translating Hamilton Depression Rating Scale (HAMD) data into Clinical Global Impression-Improvement scale (CGI-I) scores. Method: We performed electronic searches of MEDLINE, Embase, PsycINFO, PubMed and Cochrane Central Register of Controlled Trials for RCTs and non-RCT trials on rTMS using Hamilton Depression Rating Scale (HAMD). Articles were included if published in English before January 2014. We translated HAMD scores into nominal CGI-I scores for rTMS for depression and for treatment-resistant depression (TRD). Results: About 960 abstracts were retrieved. Sixty-three studies were included, yielding 130 study arms. For depression, the mean percentage change in HAMD scores in all sham-controlled rTMS treatment arms was 35.63 (SD 16.35) and for sham-rTMS 23.33 (SD 16.51). For TRD, active rTMS in sham-controlled studies showed a mean HAMD percentage reduction of 45.21 (SD 10.94) versus 25.04 (SD 17.55) for sham-rTMS. When aggregated scores were translated into notional CGI-I scores, for the treatment of depression, the notional CGI-I score difference between rTMS and sham-rTMS was 0.5 in favour of rTMS; for TRD, it was 0.75 in favour of rTMS. Differences between rTMS and sham-rTMS were bigger when all study arms were combined. Conclusion: Whilst rTMS appears to be efficacious for both non-refractory and treatment-resistant depression, the clinical relevance of its efficacy is doubtful. [ABSTRACT FROM AUTHOR]

Abstract: Though electroconvulsive therapy (ECT) requires a close cooperation between anesthesiology and psychiatry, literature lacks of approaches that consider both disciplines in parallel. Special problems might be posed by patients with complicated features or ECT-indications other than treatment-refractory depression (TRD). Considering these patients there is a particular paucity of data, especially regarding anesthesiological aspects. Therefore, we sought (1) to discuss special issues of the peri-interventional management of non-TRD-cases from a combined psychiatric–anesthesiological point of view and (2) to assess the efficacy of ECT in the classical indication of TRD as compared to cases undergoing ECT for other indications or under difficult conditions (non-TRD) by means of Clinical Global Impression-Improvement (CGI-I) scale scores. A retrospective chart analysis of patients treated with ECT between the years 2009 and 2011 at the University of Ulm, Department of Psychiatry, was conducted. Special anesthesiological efforts were necessary in cohort non-TRD. There was no difference in the clinical outcome between cohort non-TRD (n=7) and TRD (n=22) with a median CGI-I score of 2 (“much improved”) in both groups. Close cooperation between psychiatry and anesthesiology is indispensable in non-TRD patients. Abstract: Tranylcypromine (TCP) is a non-selective and irreversible monoamine oxidase inhibitor and an effective agent in the treatment of major depression. It features a complex pharmacologic profile and overdoses might induce severe intoxications. To identify typical clinical presentations of TCP-intoxications, range of associated TCP-dosages and possible differences between fatal and non-fatal intoxications a systematic review of all previously published cases of TCP-intoxications was conducted. We detected n=20 reports of TCP-intoxications in the literature (fatalities n=10). Mean age was 36.7 years (median 37); the majority of patients were female (60%). Frequent findings in patients with TCP-intoxications were disturbance of consciousness/cognitive dysfunction (90%), cardio-vascular symptoms (55%), hyperthermia (50%), respiratory distress (45%), delirium (45%), muscular rigidity (30%) and renal failure (20%). Suicidal intent was present in n=18 (90%) patients. First clinical symptoms related to TCP-intoxication developed on average in less than 1 day. The average dosage related to TCP-intoxication was 677mg. The highest survived TCP-dosage was 4000mg and the lowest fatal dosage was 170mg. Patients with fatal intoxications were on average older (40.5 vs. 32.8 years) and developed a more rapid onset of symptoms (0.2 vs. 0.8 days). Death occurred after a mean time of 0.6 days; symptom relief in patients with non-fatal intoxications developed on average after 3.2 days. Considering the large dose spectrum between survived and lethal TCP-dosages individual susceptibility factors might play a role regarding the severity of clinical symptoms independently of the ingested dosage. Abstract: This paper reviews relevant issues on solar cooking in order to define and evaluate an innovative layout of a portable solar cooker of the standard concentrating parabolic type that incorporates a daily thermal storage utensil. This utensil is formed by two conventional coaxial cylindrical cooking pots, an internal one and a larger external one. The void space between the two coaxial pots is filled with a phase change material (PCM) forming an intermediate jacket. The ensemble is thermally simulated using 1-D finite differences. A lumped elements model with convective heat transfer correlations is used for the internal behavior of the utensil, subjected to external radiation. This numerical model is used to study its transient behavior for the climatic conditions of Madrid, and validated with experimental data. Two options have been checked as possible PCMs: technical grade paraffin and erythritol. The results indicate that cooking the lunch for a family is possible simultaneously with heat storage along the day. Keeping afterwards the utensil inside an insulating box indoors allows cooking the dinner with the retained heat and also the next day breakfast. This expands the applicability of solar cooking and sustains the possibility of all the day around cooking using solar energy with a low inventory cost. [Copyright &y& Elsevier]

Tranylcypromine (TCP) is an effective antidepressant with a complex pharmacological profile and a relevant risk of abuse and dependence. Withdrawal phenomena (WP, in the case of TCP-abuse/dependence) or discontinuation phenomena (DP, in the case of absent TCP-abuse/ dependence) subsequent to abrupt termination of TCP are a potentially severe clinical syndrome. We conducted a systematic review of all previously published WP/DP cases following abrupt termination of TCP in order to identify typical clinical presentations and risk factors of WP/ DP and frequency of TCP abuse or dependence within these patients. By searching the Medline and Scopus databases we identified n = 25 cases (cohort WP: n = 18, cohort DP: n = 7). Delirium was found in n = 13 patients (cohort WP: 10/55.6 %; cohort DP: 3/42.9 %), n = 6 demonstrated WP/DP without delirium (WP: 6/33.3 %; DP: 0/0 %) and n = 5 rapid relapse in depression (WP: 1/5.6 %; DP: 4/57.1 %). Mean time until development of WP/DP was 1.9 (WP) and 2.2 (DP) days. Mean duration of WP/DP was 5.7 (WP) and 11.3 (DP) days. All patients of cohort WP were described to feature TCP-abuse/dependence. Patients with delirium were on average older (41.8 years vs. 37.8 years) and featured higher mean prescribed (71.0 mg vs. 38.3 mg) and actually taken daily TCP dosages (285.8 mg vs. 187.7 mg). In conclusion, even termination of lower daily dosages of TCP may result in delirium. Thrombocytopenia features diagnostic value in patients with deliria of unknown etiology. Certain guidelines for the prevention of ventilator-associated pneumonia (VAP) recommend oral care with chlorhexidine, but none refer to the use of a toothbrush for oral hygiene. The role of toothbrush use has received scant attention. Thus, the objective of this study was to compare the incidence of VAP in critical care patients receiving oral care with and without manual brushing of the teeth. This was a randomized clinical trial developed in a 24-bed medical-surgical intensive care unit (ICU). Patients undergoing invasive mechanical ventilation for than 24 h were included. Patients were randomly assigned to receive oral care with or without toothbrushing. All patients received oral care with 0.12 % chlorhexidine digluconate. Tracheal aspirate samples were obtained during endotracheal intubation, then twice a week, and, finally, on extubation. There were no significant differences between the two groups of patients in the baseline characteristics. We found no statistically significant differences between the groups regarding the incidence of VAP (21 of 217 [9.7 %] with toothbrushing vs. 24 of 219 [11.0 %] without toothbrushing; odds ratio [OR] = 0.87, 95 % confidence interval [CI] = 0.469-1.615; p = 0.75). Antimicrobial-impregnated catheters are more expensive than standard catheters (S-C). A higher incidence of catheter-related bloodstream infection (CRBSI) has been found in jugular venous access with tracheostomy than without tracheostomy. The objective of this study was to determine central venous catheter (CVC)-related costs (considering only the cost of the CVC, diagnosis of CRBSI, and antimicrobial agents used to treat CRBSI) using rifampicin-miconazole-impregnated catheters (RM-C) or S-C in jugular venous access with tracheostomy. We performed a retrospective cohort study of patients admitted to the intensive care unit (ICU) with tracheostomy who received one or more jugular venous catheters. RM-C showed a lower incidence of CRBSI compared with S-C (0 vs. 20.16 CRBSI episodes/1,000 catheter-days; odds ratio = 0.05; 95% confidence interval = 0.001-0.32; p < 0.001) and lower CVC-related costs (including the cost of the CVC, diagnosis, and treatment of CRBSI) (€11.46 ± 6.25 vs. €38.11 ± 77.25; p < 0.001) in jugular venous access with tracheostomy. The use of RM-C could reduce CVC-related costs in jugular venous access with tracheostomy. The results of our study may contribute to clinical decision-making and selection of those patients who could benefit from the use of antimicrobial-impregnated catheters. [ABSTRACT FROM AUTHOR]

Abstract: The boll weevil (Anthonomus grandis) is the main pest of cotton in the Americas. The aim of this work was to evaluate isolates of the entomopathogenic fungi Beauveria bassiana sensu lato and Metarhizium anisopliae sensu lato virulent against A. grandis. Screening was performed to evaluate the pathogenicity of 28 isolates of M. anisopliae s.l. and 66 isolates of B. bassiana s.l. against boll weevil adults. To select the isolates, LC50 values of the most virulent isolates were calculated, and compatibility between the fungi and insecticides was studied. In addition, the effects of these isolates on the feeding behavior of the adults were evaluated. Isolates Ma 50 and Ma 20 were the most virulent against A. grandis and their LC50 values were 1.13×107 and 1.20×107 conidia/ml, respectively. In addition, these isolates were compatible with pyrethroid insecticides, but none with endosulfan. On the other hand, infected females reduced the damage caused by feeding on the cotton squares and their weight gain. This shows that entomopathogenic fungi cause mortality in the insects, but also these fungi could influence the feeding behavior of the females. In summary, these results indicate the possibility of the use of M. anisopliae s.l. as a microbiological control agent against boll weevils. Inflammatory myofibroblastic tumor (IMT) is a rare neoplasm with unknown malignant potential that has been described in most organ systems. We present the case of a 3-year-old boy who was referred with lower urinary tract symptoms and macroscopic hematuria. An IMT was suspected after clinical, radiological, and surgical work-up, and the diagnosis was confirmed after a partial cystectomy was performed. A bladder-preserving approach is the treatment of choice, but close clinical follow-up is recommended because of the unknown biological behavior of these tumors. [ABSTRACT FROM AUTHOR]

Elevated CO2 concentrations (hypercapnia) occur in patients with severe lung diseases. Here, we provide evidence that high CO2 levels decrease O2 consumption and ATP production and impair cell proliferation independently of acidosis and hypoxia in fibroblasts (N12) and alveolar epithelial cells (A549). Cells exposed to elevated CO2 died in galactose medium as well as when glucose-6-phosphate isomerase was knocked down, suggesting mitochondrial dysfunction. High CO2 levels led to increased levels of microRNA-183 (miR-183), which in turn decreased expression of IDH2 (isocitrate dehydrogenase 2). The high CO2-induced decrease in cell proliferation was rescued by α-ketoglutarate and overexpression of IDH2, whereas proliferation decreased in normocapnic cells transfected with siRNA for IDH2. Also, overexpression of miR-183 decreased IDH2 (mRNA and protein) as well as cell proliferation under normocapnic conditions, whereas inhibition of miR-183 rescued the normal proliferation phenotype in cells exposed to elevated levels of CO2. Accordingly, we provide evidence that high CO2 induces miR-183, which down-regulates IDH2, thus impairing mitochondrial function and cell proliferation. These results are of relevance to patients with hypercapnia such as those with chronic obstructive pulmonary disease, asthma, cystic fibrosis, bronchopulmonary dysplasia, and muscular dystrophies.
