Background: The lumbar extensor muscles (LEMs) play an important role in body posture and physical function in older adults. Because lumbar extensor strength decreases more rapidly than limb muscle strength with age, it should be evaluated to aid healthy aging. This study investigated the association between LEM strength and physical performance in community-dwelling older adults. Methods: This prospective observational cohort study of spinal sarcopenia (SarcoSpine) was conducted at a single center. One hundred and ten consecutive individuals who completed the baseline survey, including conventional sarcopenic indices, lumbar spine three-dimensional magnetic resonance imaging, isokinetic lumbar extensor strength, and physical performance tests (Short Physical Performance Battery, Berg Balance Scale, and Back Performance Scale [BPS]), were enrolled. A multivariate linear regression analysis was performed to determine the variables for evaluating their association with LEM strength. Results: Among the conventional sarcopenic indices, gait speed was significantly correlated with Short Physical Performance Battery results and Berg Balance Scale score in men and women. Handgrip strength was significantly correlated with the BPS score for both sexes. In the multivariable linear regression, age (β = −2.12, p <.01) and BPS score (β = −3.54, p =.01, R2 =.29) were independent indicators of LEM strength. Conclusions: Our findings reveal the substantial association between LEM strength and BPS score in older women. The targeted intervention aimed at improving the LEMs strength would be needed to enhance physical performance in the aging population. [ABSTRACT FROM AUTHOR]

Background: The evaluation of gait function and severity classification of stroke patients are important to determine the rehabilitation goal and the level of exercise. Physicians often qualitatively evaluate patients' walking ability through visual gait analysis using naked eye, video images, or standardized assessment tools. Gait evaluation through observation relies on the doctor's empirical judgment, potentially introducing subjective opinions. Therefore, conducting research to establish a basis for more objective judgment is crucial. Objective: To verify a deep learning model that classifies gait image data of stroke patients according to Functional Ambulation Category (FAC) scale. Methods: Gait vision data from 203 stroke patients and 182 healthy individuals recruited from six medical institutions were collected to train a deep learning model for classifying gait severity in stroke patients. The recorded videos were processed using OpenPose. The dataset was randomly split into 80% for training and 20% for testing. Results: The deep learning model attained a training accuracy of 0.981 and test accuracy of 0.903. Area Under the Curve(AUC) values of 0.93, 0.95, and 0.96 for discriminating among the mild, moderate, and severe stroke groups, respectively. Conclusion: This confirms the potential of utilizing human posture estimation based on vision data not only to develop gait parameter models but also to develop models to classify severity according to the FAC criteria used by physicians. To develop an AI-based severity classification model, a large amount and variety of data is necessary and data collected in non-standardized real environments, not in laboratories, can also be used meaningfully. This study analyzes the shortcomings of South Korea's current evaluation method of ventilation performance for apartment buildings and suggests improvements. The current Korean regulations rely on the air change rate method, which is a prescriptive method and thus inadequately measures indoor air quality practically. Therefore, this study reviews various standards, finding that these standards can be categorized into those evaluating the mechanical performance of ventilators and those assessing indoor ventilation performance. This study highlights that the standards evaluating indoor ventilation performance are based on the tracer gas method but lack clear testing procedures and boundary conditions. This research also reviews the various previous research articles, noting that Korean research places emphasis on system design parameters, while international research focuses on architectural factors. It also identifies inconsistencies in the experimental setups across studies. To improve the current evaluation methods, the research suggests enhancing the tracer gas method with clear testing procedures and introducing indicators such as the age of air and uniformity coefficient together. Since the air change rate method does not consider the actual airflows and distribution in indoor spaces, this method is limited to deriving improvements in indoor ventilation performance. However, the suggested tracer gas method and indicators can be used to discover the optimal locations of vents for better indoor air quality or to drive a better building design to achieve better indoor ventilation performance. In other words, these enhancements aim to provide more accurate and comprehensive insights into the effectiveness of indoor ventilation systems, helping engineers, designers, and residents better understand and improve indoor air quality. Objective: Metabolic syndrome (MetS) and sarcopenia together pose significant health risks, increasing frailty, falls, and fractures in older adults. This study compared muscle mass measurements obtained using two different dual-energy X-ray absorptiometry (DXA) machines and bioelectrical impedance analysis (BIA), and evaluated the accuracy of these measurements in these older adults. Methods: In this prospective multicenter cohort study, patients aged ≥ 65 years with MetS had their muscle mass assessed using both BIA and DXA. Two DXA devices, Hologic Horizon and GE Lunar Prodigy, were used as clinical standards for sarcopenia diagnosis. Statistical analyses generated equations for transforming BIA results to match those from DXA, enhancing comparability. Results: Participants had a mean age of 73.2 ± 5.3 years. The mean appendicular skeletal muscle mass (ASM) measured by BIA and DXA was 19.7 ± 3.1 kg (BIA) and 18.1 ± 2.9 kg (DXA) for males, and 13.7 ± 2.2 kg (BIA) and 12.6 ± 1.8 kg (DXA) for females. Device-specific equations were developed to estimate DXA-measured ASM based on BIA results. These equations are presented for all participants and for each DXA device, highlighting significant differences in prediction models between the two DXA machines. Conclusion: The study developed device-specific equations for sarcopenia diagnosis in older adults with MetS, highlighting substantial differences between Hologic and GE Lunar devices. While BIA may offer a more accessible alternative to DXA, the variation in prediction formulas underscores the need for standardized equipment to ensure consistency in sarcopenia diagnosis. Diabetes mellitus has a global impact, necessitating surgical intervention when conservative methods fail. Transtibial amputation (TTA) is commonly performed on diabetic patients, yet surgical site complications can lead to more procedures. This study aimed to identify factors linked to wound healing issues post-TTA in diabetics. A total of 181 patients who underwent TTA between 2004 and 2021 at a single hospital were included in the study. Exclusion criteria comprised trauma, non-diabetic mellitus, follow-up duration of less than 1 year, incomplete medical records, and surgeries performed by different surgeons. The comparison focused on underlying diseases other than diabetes between the group with wound problems and the group without. Additionally, factors impacting blood flow, such as presurgery hemoglobin levels, intraoperative blood transfusion, the use of antithrombotic or anticoagulant drugs, and the presence of procedures like percutaneous transluminal angioplasty (PTA) and bypass surgery, were analyzed. Among the 181 cases, 22.1% experienced problems at the surgical site while 77.9% did not. Statistical analysis revealed that age was a significant variable affecting wound healing problems after TTA in diabetic patients (p =.007). However, there were no significant differences in wound problems based on comorbidities other than diabetes (p =.209), gender (p =.677), preoperative anemia (p =.102), intraoperative blood transfusion (p =.633), the use of antithrombotic or anticoagulant medications (p =.556), and the performance of PTA or bypass surgery (p =.6). In conclusion, this study found that age was a significant variable affecting wound healing problems after TTA in diabetic patients. Although no association was observed between underlying diseases and wound healing problems, further investigation and cautious management of factors such as preoperative anemia, intraoperative blood transfusion, the use of antithrombotic or anticoagulant drugs, and the performance of PTA or bypass surgery are warranted to prevent complications and optimize wound healing outcomes in diabetic patients undergoing TTA. Achieving this necessitates bioink formulations that possess highly printable properties and provide a cell-friendly microenvironment mimicking the native extracellular matrix. Rapid advancements in printing techniques continue to expand the capabilities of researchers, enabling them to overcome existing biological barriers. This review offers a comprehensive examination of ultraviolet-based 3D bioprinting, renowned for its exceptional precision compared to other techniques, and explores its applications in inducing angiogenesis across diverse tissue models related to hypoxia. The high-precision and rapid photocuring capabilities of 3D bioprinting are essential for accurately replicating the intricate complexity of vascular networks and extending the diffusion limits for nutrients and gases. Addressing the lack of vascular structure is crucial in hypoxia-related diseases, as it can significantly improve oxygen delivery and overall tissue health. Consequently, high-resolution 3D bioprinting facilitates the creation of vascular structures within three-dimensional engineered tissues, offering a potential solution for addressing hypoxia-related diseases. Emphasis is placed on fundamental components essential for successful 3D bioprinting, including cell types, bioink compositions, and growth factors highlighted in recent studies. The insights provided in this review underscore the promising prospects of leveraging 3D printing technologies for addressing hypoxia-related diseases through the stimulation of angiogenesis, complementing the therapeutic efficacy of cell therapy.
