Background and objective: Neurodegenerative diseases like Parkinson’s disease often take several years before they can be diagnosed reliably based on clinical grounds. Imaging techniques such as MRI are used to detect anatomical (structural) pathological changes. However, these kinds of changes are usually seen only late in the development. The measurement of functional brain activity by means of [18F]fluorodeoxyglucose positron emission tomography (FDG-PET) can provide useful information, but its interpretation is more difficult. The scaled sub-profile model principal component analysis (SSM/PCA) was shown to provide more useful information than other statistical techniques. Our objective is to improve the performance further by combining SSM/PCA and prototype-based generalized matrix learning vector quantization (GMLVQ). Methods: We apply a combination of SSM/PCA and GMLVQ as a classifier. In order to demonstrate the combination’s validity, we analyze FDG-PET data of Parkinson’s disease (PD) patients collected at three different neuroimaging centers in Europe. We determine the diagnostic performance by performing a ten times repeated ten fold cross validation. Additionally, discriminant visualizations of the data are included. The prototypes and relevance of GMLVQ are transformed back to the original voxel space by exploiting the linearity of SSM/PCA. The resulting prototypes and relevance profiles have then been assessed by three neurologists. Results: One important finding is that discriminative visualization can help to identify disease-related properties as well as differences which are due to center-specific factors. Secondly, the neurologist assessed the interpretability of the method and confirmed that prototypes are similar to known activity profiles of PD patients. Conclusion: We have shown that the presented combination of SSM/PCA and GMLVQ can provide useful means to assess and better understand characteristic differences in FDG-PET data from PD patients and HCs. Based on the assessments by medical experts and the results of our computational analysis we conclude that the first steps towards a diagnostic support system have been taken successfully.

Rationale In Parkinson’s disease (PD), spatial covariance analysis of 18F-FDG PET data has consistently revealed a characteristic PD-related brain pattern (PDRP). By quantifying PDRP expression on a scan-by-scan basis, this technique allows objective assessment of disease activity in individual subjects. We provide a further validation of the PDRP by applying spatial covariance analysis to PD cohorts from the Netherlands (NL), Italy (IT), and Spain (SP). Methods The PDRPNL was previously identified (17 controls, 19 PD) and its expression was determined in 19 healthy controls and 20 PD patients from the Netherlands. The PDRPIT was identified in 20 controls and 20 “de-novo” PD patients from an Italian cohort. A further 24 controls and 18 “de-novo” Italian patients were used for validation. The PDRPSP was identified in 19 controls and 19 PD patients from a Spanish cohort with late-stage PD. Thirty Spanish PD patients were used for validation. Patterns of the three centers were visually compared and then cross-validated. Furthermore, PDRP expression was determined in 8 patients with multiple system atrophy. Results A PDRP could be identified in each cohort. Each PDRP was characterized by relative hypermetabolism in the thalamus, putamen/pallidum, pons, cerebellum, and motor cortex. These changes co-varied with variable degrees of hypometabolism in posterior parietal, occipital, and frontal cortices. Frontal hypometabolism was less pronounced in “de-novo” PD subjects (Italian cohort). Occipital hypometabolism was more pronounced in late-stage PD subjects (Spanish cohort). PDRPIT, PDRPNL, and PDRPSP were significantly expressed in PD patients compared with controls in validation cohorts from the same center (P < 0.0001), and maintained significance on cross-validation (P < 0.005). PDRP expression was absent in MSA.

The technique of positron emission tomography (PET) is well established in the field of medical imaging. Non-medical applications have also been shown to be valuable in the measurement of dynamic chemical processes, specifically in the determination of the characteristics of small rock samples. Since rock is more dense than tissue, the problems of attenuation and scatter are accentuated. However, we are able to show that measurement of porosity in a large rock sample (21.5 cm diameter) is indeed practicable using three-dimensional (3-D) acquisition techniques. Due to multiple scattering of the photons in the rock and the cylindrical symmetry of the experiment, we measured a scatter distribution which is approximately homogeneous, allowing a correction for scattered radiation with a simple method. The problems of determining attenuation coefficients and applying a scatter correction makes absolute quantitation difficult. However, relative changes in porosity within the sample can be measured with a spatial resolution not appreciably different from that in water. Index Terms - PET, rock samples, scatter.

Various measurements can be used to study the energy metabolism of the brain; these include regional cerebral blood flow (CBF), oxygen extraction ratio (OER), oxygen utilization (CMRO2), and cerebral blood volume. Several earlier studies of healthy volunteers examined the effect of age on these parameters and sought to establish a range of normal values that would permit comparisons with ill patients, but the results were unclear. Under normal conditions, the CBF is directly related to the brain oxygen and glucose consumption, but the mechanisms responsible for this coupling are not known. A recent study of 34 healthy adults, 22 to 82 years of age, measured brain regional CBF, CMRO2, OER, and CBV using a method called oxygen steady-state inhalation, combined with a brain imaging technique called positron emission tomography (PET). In support of earlier studies, CBF was directly related CMRO2. OER or the amount of oxygen taken up from the blood was not related to brain CMRO2 or CBF. As expected, CBV was directly related to the CBF. The aging process decreased CMRO2, CBF, and CBV by 0.5 percent per year. The OER either remained the same or else slightly increased by an average of 0.35 percent per year. These results might indicate that neurotransmission, which is the process of how the brain communicates with itself, may decrease with age, but further studies are needed to confirm this. Furthermore, these observations give insight into the processes of normal brains and help researchers understand what happens in diseased brains. - Abstract
_Objective_ DNAJC6 mutations were recently described in two families with autosomal recessive juvenile parkinsonism (onset age < 11), prominent atypical signs, poor or absent response to levodopa, and rapid progression (wheelchair-bound within ∼10 years from onset). Here, for the first time, we report DNAJC6 mutations in early-onset Parkinson's disease (PD). _Methods_ The DNAJC6 open reading frame was analyzed in 274 patients with early-onset sporadic or familial PD. Selected variants were followed up by cosegregation, homozygosity mapping, linkage analysis, whole-exome sequencing, and protein studies. _Results_ We identified two families with different novel homozygous DNAJC6 mutations segregating with PD. In each family, the DNAJC6 mutation was flanked by long runs of homozygosity within highest linkage peaks. Exome sequencing did not detect additional pathogenic variants within the linkage regions. In both families, patients showed severely decreased steady-state levels of the auxilin protein in fibroblasts. We also identified a sporadic patient carrying two rare noncoding DNAJC6 variants possibly effecting RNA splicing. All these cases fulfilled the criteria for a clinical diagnosis of early-onset PD, had symptoms onset in the third-to-fifth decade, and slow disease progression. Response to dopaminergic therapies was prominent, but, in some patients, limited by psychiatric side effects. The phenotype overlaps that of other monogenic forms of early-onset PD. _Interpretation_ Our findings delineate a novel form of hereditary early-onset PD. Screening of DNAJC6 is warranted in all patients with early-onset PD compatible with autosomal recessive inheritance. Our data provide further evidence for the involvement of synaptic vesicles endocytosis and trafficking in PD pathogenesis.

5-bromodeoxyuridine (BUdR) provides in vitro measures of tumor cell proliferation. We used positron emission tomography to study tissue and plasma kinetics of [Br-76]BUdR in tumor-bearing animals. In order to account for the slow washout of the major plasma metabolite, [Br-76]bromide, a mathematical correction for the distribution volume of [Br-76]bromide was applied. However, following correction specific tumor tracer retention was low or even zero and did not correlate with independent measures of proliferation. - Abstract
Early-onset Parkinson's disease (EOPD) has been associated with recessive mutations in parkin (PARK2). About half of the mutations found in parkin are genomic rearrangements, i.e., large deletions or duplications. Although many different rearrangements have been found in parkin before, the exact breakpoints involving these rearrangements are rarely mapped. In the present study, the exact breakpoints of 13 different parkin deletions/duplications, detected in 13 patients out of a total screened sample of 116 EOPD patients using Multiple Ligation Probe Amplification (MLPA) analysis, were mapped using real time quantitative polymerase chain reaction (PCR), long-range PCR and sequence analysis. Deletion/duplication-specific PCR tests were developed as a rapid and low cost tool to confirmMLPA results and to test family members or patients with similar parkin deletions/duplications. Besides several different deletions, an exon 3 deletion, an exon 4 deletion and an exon 7 duplication were found in multiple families.
