Hence, this study assessed Dessie city resident’s knowledge, attitude, and practice on COVID-19.Methods: We used a facility-based cross-sectional study among 424 Dessie city residents from 17 to 21/05/2020. We dichotomized knowledge, attitude and practice scores based on the mean value. We entered the data into EpiData manager software 4.2 and exported to SPSS-20 for data analysis. We run three independent logistic regression analyses to determine factors associated with sufficient knowledge, a favorable attitude, and adequate practice. We defined significant association at a p-value of < 0.05.Results: Among 424 participants, 92.7% have sufficient knowledge about COVID-19, while 96% have a favorable attitude to prevent and control the pandemic. However, the practice was adequate only in 44.6% of the participants. Increasing educational status (AOR: 6.5, 95% CI: 2– 21.4), availability of television (AOR: 3.8, 95% CI: 1.4– 10.5), having a telephone (AOR: 3.4, 95% CI: 1.3– 9.1) and radio (AOR: 4.2, 95% CI: 1.1– 15.5) are the factors associated with sufficient knowledge, while sufficient knowledge (AOR: 5.4, 95% CI: 1.7– 17.2), is the only predictor identified for favorable attitude. Similarly, being a farmer (AOR: 0.14, 95% CI: 0.047– 0.4), availability of telephone (AOR: 3.1, 95% CI: 1.2– 8.2), and sufficient knowledge (AOR: 15.2, 95% CI: 1.9– 118) are the predictors of adequate practice.Conclusion: In the study area, the participant’s knowledge and attitude are found to be sufficient to halt coronavirus transmission. However, practice in the vast majority is not adequate to stop coronavirus transmission. Educational status, television, telephone, radio, occupation and knowledge are the significant factors for successful prevention and control of coronavirus. Despite escalating public knowledge, our finding suggests the government to follow some compulsory regulations for uniform implementation of preventive measures.Keywords: attitude, COVID-19, knowledge, practice, Dessie city

3. Waste disposal systems of Addis Ababa City, the African Capital, Ethiopia

Abstract

Industries have played vital roles for economic development that enable people to create new and better livelihoods, more income, and modern life systems. However, Ethiopia has little benefited from industry sector due to limited number of industries with less advanced technologies. This study was conducted in Addis Ababa, the Capital City of Africa. Datasets were collected from 2,204 institutions through structured interview, field observation and GPS points. Datasets were entered, cleaned and analyzed using a relational database system in Visual Basic and MS-access database environment. Spatial distribution of industries indicates that only 18% of industries were found in industry zones, while 82% are outside industry zones and of which 78% of them are found in residential areas. Industrial wastes are disposed either to nearest river or open field. The findings indicate that only 10% of industries treated and disposed their wastes whereas 90% didn’t treat industrial wastes but disposed to the environment. The major observed impacts were on: affecting human and animals’ health, pollute surface and groundwater, pollute soil and air, trigger firing, damage ditches, block canals and cause over floods, among others. It requires concerted efforts to minimize and avoid improper waste disposal systems so as to realize economically active, environmental suitable and socially conduce capital city for its residents, tourist and business people.

Abstract

Background: Understanding the magnitude of cancer burden attributable to potentially modifiable risk factors is crucial for development of effective prevention and mitigation strategies. We analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 to inform cancer control planning efforts globally. Methods: The GBD 2019 comparative risk assessment framework was used to estimate cancer burden attributable to behavioural, environmental and occupational, and metabolic risk factors. A total of 82 risk-outcome pairs were included on the basis of the World Cancer Research Fund criteria. Estimated cancer deaths and disability-adjusted life-years (DALYs) in 2019 and change in these measures between 2010 and 2019 are presented. Findings: Globally, in 2019, the risk factors included in this analysis accounted for 4·45 million (95% uncertainty interval 4·01-4·94) deaths and 105 million (95·0-116) DALYs for both sexes combined, representing 44·4% (41·3-48·4) of all cancer deaths and 42·0% (39·1-45·6) of all DALYs. There were 2·88 million (2·60-3·18) risk-attributable cancer deaths in males (50·6% [47·8-54·1] of all male cancer deaths) and 1·58 million (1·36-1·84) risk-attributable cancer deaths in females (36·3% [32·5-41·3] of all female cancer deaths). The leading risk factors at the most detailed level globally for risk-attributable cancer deaths and DALYs in 2019 for both sexes combined were smoking, followed by alcohol use and high BMI. Risk-attributable cancer burden varied by world region and Socio-demographic Index (SDI), with smoking, unsafe sex, and alcohol use being the three leading risk factors for risk-attributable cancer DALYs in low SDI locations in 2019, whereas DALYs in high SDI locations mirrored the top three global risk factor rankings. From 2010 to 2019, global risk-attributable cancer deaths increased by 20·4% (12·6-28·4) and DALYs by 16·8% (8·8-25·0), with the greatest percentage increase in metabolic risks (34·7% [27·9-42·8] and 33·3% [25·8-42·0]). Interpretation: The leading risk factors contributing to global cancer burden in 2019 were behavioural, whereas metabolic risk factors saw the largest increases between 2010 and 2019. Reducing exposure to these modifiable risk factors would decrease cancer mortality and DALY rates worldwide, and policies should be tailored appropriately to local cancer risk factor burden. 5. Development and Validation of the Amharic Version of Self-Efficacy and Outcome Expectancy Measures on Intention to Take Preventive Actions on Noncommunicable Disease.

Abstract

This study is aimed at developing and accessing the validity and reliability of an Amharic version of the self-efficacy and outcome expectancy measures on noncommunicable disease prevention strategies. The intentions to take protective measures on NCDs' self-efficacy and outcome expectancy scales were created in Amharic using a sequential nine-step process that included translation and contextualization of the items, content validity, pretesting of the questions, sampling, and survey administration. Principal component analysis was conducted on 829 university students which showed a one-factor solution for self-efficacy and a three-factor solution for outcome expectancy scales using split-half measures. Confirmatory factor analyses supported the factor structure, which also demonstrated good internal consistency (.828 self-efficacy, .766 outcome expectancy). The scales had a moderate level of correlation ( r = .35, p .001) between them. 6. Epidemiological and therapeutic studies on sheep lice in Sayint district, South Wollo Zone, Northeast Ethiopia.

Abstract

An epidemiological (cross-sectional) and therapeutic (randomized controlled field trial) study was conducted on sheep lice in Sayint district, South Wollo, Northeast Ethiopia. The aim of the study was to (i) quantify the magnitude of sheep lice burden and the prevailing lice species, (ii) identify and quantify risk factors affecting lice infestation in sheep, and (iii) evaluate the efficacy of commonly used acaricides (diazinon and ivermectin) against sheep lice infestation. A total of 232 randomly selected sheep, 15 naturally infested sheep, and 80 viable Bovicola ovis lice were used for epidemiological, in vivo , and in vitro based therapeutic studies, respectively. Three naturally infested treatment groups each with five sheep (Group I-treated with diazinon, Group II-treated with ivermectin, and Group III-untreated/control) were used for in vivo therapeutic study. Lice count for the corresponding treatment groups was conducted on weekly basis using clinical and parasitological examinations. We used logistic regression to quantify the association between different putative risk factors and lice infestation, and the independent t -test and one-way ANOVA to compare the within and between treatment group mean lice count variations. The overall prevalence of sheep lice in the study area was 48.3%, where Bovicola ovis (83%) was the dominant lice species. Hair length, body condition, agroecology, and season were significantly ( P < 0.05) associated with sheep lice infestation. Analysis of variance revealed that mean lice count significantly ( P < 0.05) varies between treatment groups. A significant ( P < 0.05) low mean lice cunt was recorded in diazinon- and ivermectin-treated groups when compared to untreated group. The in vivo efficacy of ivermectin (81%) was lower than diazinon (99%) when compared to the efficacy standard limit (98-100%). However, no significant mean lice count variation was recorded between the two groups. In vivo (99%) and in vitro (95%) assay evidence revealed that diazinon was highly effective for the treatment of sheep lice. According to this study, it can be concluded that the magnitude of sheep lice burden in Sayint district was found to be high and this could have a potential negative impact on sheep productivity and health performances. 7. Health Professionals' Knowledge, Attitude and Practices Regarding COVID-19 in Dessie City, Northeast Ethiopia: A Facility-Based Cross-Sectional Study.

Abstract

Background: Knowledge and attitudes are among the key drivers of social behavioral change. We assessed employed health professionals' (HPs) knowledge, attitude, and practice regarding COVID-19 in Dessie city, northeast Ethiopia., Methods: A facility-based cross-sectional study was conducted among 419 HPs working at Dessie city from 17 to 21 May 2020. The data were collected using a self-administered structured questionnaire. Knowledge, attitude, and practice are measured using 19, 16, and 8 questions, respectively. Knowledge and attitude scores are dichotomized at the 3rd quartile, while practice is using the mean value. Data entry and analysis were conducted using EpiData Manager 4.2 and SPSS 25, respectively. Three independent logistic regression analyses were carried out to determine the associated factors. We defined significant association at a p -value of < 0.05., Results: Out of 419 participants, 369 (88.1%) have sufficient knowledge regarding COVID-19 (95% CI: 85-91). The mean knowledge score is 16.8 with a ± 2.1 SD. Similarly, 355 (84.7%) of the HPs have a favorable attitude toward COVID-19 (95% CI: 81-87.9). The mean attitude score is 14 with ± 2.1 SD. However, practice regarding COVID-19 is adequate only in 69.7% (292) of the HPs (95% CI: 65.2-94). The mean practice score is 5.1 with a ± 1.3 SD. Sufficient knowledge is significantly associated with the type of health facility (AOR: 4.4, 95% CI: 1.4-13.3), degree and above education (AOR: 2.6, 95% Cl: 1.4-4.9), radio availability (AOR: 2.4, 95% CI: 1.3-4.7), and social media utilization (AOR: 2.3, 95% CI: 1.1-5.1). The predictors of favorable attitude are training (AOR: 3.1, 95% CI: 1.6-6.1), sufficient knowledge (AOR: 5.2, 95% Cl: 2.6-10.4), and type of health facility (AOR: 2.3, 95% CI: 1.1-5.2)., Conclusion: Most HPs have sufficient knowledge and a favorable attitude regarding COVID-19. However, practice is relatively low and there remains plenty to build assertive preventive behaviors. Abstract

Background: Successful health care and clinical services essentially depend on patients' realization of ones' rights, and health workers' and facilities' fulfillments and protections of these rights. However, little is documented about how patients and health workers perceive patients' rights during care-seeking practices., Methods: A qualitative study was conducted in four hospitals in Ethiopia through 8 focus group discussions with patients and 14 individual interviews with diverse groups of patients, caretakers, and 14 interviews with health workers. Participants were recruited through a purposive sampling method to meet the saturation of ideas about patients' rights. The sampled patients, caretakers, and professionals were enlisted from various departments in the hospitals. The data analysis was assisted by ATLAS.ti 7.1.4., Results: The study identified three major categories of healthcare rights (clinical, socio-cultural, and organizational), incorporating supporting elements of education, engagement, and empowerment. Study participants reported detailed rights the patients would have during hospital visits which included the right to timely access to care and treatment, adequate medications) with full respect, dignity, and without any discrimination. Patients widely perceived that they had the right to tell their illness history and know their illness in the language they can understand. It was also widely agreed that patients have the right to be educated and guided to make informed choices of services, procedures, and medications. Additionally, patients reported that they had the right to be accompanied by caretakers together with the right to use facilities and resources and get instructions on how to utilize these resources, the right to be protected from exposure to infections and unsafe conditions in hospitals, right to get a diet of their preference, and right to referral for further care. Nevertheless, there was a common concern among patients and caretakers that these rights were mostly non-existent in practice which were due to barriers related to patients (fear of consequence; a sense of dependency, feeling of powerlessness, perceptions of low medical literacy), health workers (negligence, lack of awareness and recognition of patient rights, undermining patients), and facilities' readiness and support, including lack of guiding framework., Conclusions: Perceived patients' rights in the context of hospital visits were profoundly numerous, ranging from the right to access clinical and non-clinical services that are humanely respectful, fulfilling socio-cultural contexts, and in a manner that is organizationally coordinated. Nonetheless, the rights were not largely realized and fulfilled. Engaging, educating, and empowering patients, caretakers, and health care providers supported with policy framework could help to move towards patient-centered and right-based healthcare whereby patients' rights are protected and fulfilled in such resource-limited settings., Competing Interests: The authors have declared that no competing interests exist.

Abstract

Background: Human papillomavirus (HPV) infection is considered as the major risk factor for the development of cervical cancer, second most frequent cancer in Ethiopia. However, the magnitude of the problem and the associated factors remain unrevealed in the Amhara region. This study aimed to determine the prevalence of HPV infection and factors contributing to the progression of HPV infection to cervical cancer., Methods: Facility-based cross-sectional study design was employed among women aged 21 to 49 years of age who came for routine cervical cancer screening to 4 randomly selected hospitals (2 general and 2 referral) of Amhara region from May to October, 2019. The sample size was calculated by using the single population proportion formula, proportionated to hospitals, and women were recruited consecutively. Socio demographic and clinical data were collected using a pretested questionnaire and detection of HPV infection was done using HPV test (OncoE6TM Cervical Test) specific to HPV16/18 in cervical swabs. Visual inspection with acetic acid (VIA) was used to determine cervical lesions (precancerous and cancerous). Descriptive statistics and bivariate and multivariate logistic regression were used to describe HR-HPV and cervical lesions burden and association between HR-HPV, and cervical lesions and potential risk factors., Results: Among 337 women 21 to 49 years (median age of 35 years ±SD = 7.1 years) of age enrolled in the study, The overall prevalence of oncogenic HPVs (HPV16/18) and the VIA-positivity rate, possible an indicative of cervical lesions, were 7.1% and 13.1%, respectively. Logistic regression analysis showed a significant association between early age of first sexual intercourse (COR = 2.26; 95% CI: 1.0-5.05) and level of education (COR = 0.31; 95% CI: 0.12-0.78) with cervical lesions. Higher odds of HPV positivity (COR = 1.56; 95% CI: 0.59-4.11, p = 0.36) and VIA positivity (COR = 1.39; 95% CI: 0.64-3.00, p = 0.39) were observed among participants who had a history of sexually transmitted illnesses (STIs)., Conclusions: There was a relatively low prevalence of oncogenic HPV 16/18 and VIA-positivity in women attending four hospitals in the Amhara Region. Early age sexual contact, high parity, and being uneducated/low level of education were independently associated factors with HR-HPV infection and development of cervical lesions, highlighting the importance of prioritizing the limited HPV testing to those risk groups., Competing Interests: The authors have declared that no competing interests exist.

Abstract

Background: In-service training programs should be evaluated and modified regularly to enhance training quality. However, in Ethiopia, there is no published evidence regarding its effectiveness. Therefore, we evaluated the Amhara Public Health Institute Dessie Branch (APHI&#95;DB) in-service training program using the Kirkpatrick model., Methods: In October 2019, a concurrent nested mixed method facility-based cross-sectional study was conducted among 107 laboratory stakeholders from 22 randomly selected government health facilities in the eastern part of the Amhara region. The qualitative part involved interviews with each of these key stakeholders. We collected data using a semi-structured questionnaire through face-to-face interviews. EpiData 3.1 and Microsoft Excel 2016 software were used for data entry and analysis respectively. The major qualitative findings were narrated and summarized based on four thematic areas to supplement the quantitative findings., Results: A total of 107 laboratory personnel were interviewed, which makes a response rate of 97.3%. At the reaction level, 82.1% of the participants agree/strongly agree with the course structures, training contents, and learning tools. Likewise, 85.4% of the participants agreed/ strongly agreed on the trainer's knowledge and their communication skills. In addition, 93.1% of the participants stated an improvement in knowledge and skills after attending the training. As a result, 65.6% of them were able to transfer their knowledge and skills into practice. Regarding the training set-ups and environment, 45.1% of the respondents disagree/strongly disagree with the training hall, toilet, cafe, tea and snacks, financial process, and accommodation perdiem., Conclusion: Generally, the laboratory in-service training program of APHI&#95;DB was more or less effective. Our findings suggest regular monitoring of each training event and evaluation of training programs against a clearly defined criterion. Abstract

Obesity, insulin resistance, and poor metabolic profile are hallmarks of a high-fat diet (HFD), highlighting the need to understand underlying mechanisms. Therefore, we sought to determine the effect of succinic acid (SA) on metabolism in high-fat diet (HFD)-induced obesity. Animals were randomly assigned to either low-fat diet (LFD) or a high-fat diet (HFD). Mice consumed their respective diets for 4.5 months and then assigned to the following groups: (LFD)+vehicle, LFD + SA (0.75 mg/ml), HFD + vehicle, or HFD + SA. Body weight (BW), food, and water intake, were tracked weekly. After 6 weeks, insulin, glucose, and pyruvate tolerance tests were completed, and spontaneous physical activity was assessed. Epididymal white adipose tissue (EWAT) mass and in vitro measurements of oxidative skeletal muscle (soleus) respiration were obtained. Expectedly, the HFD increased BW and EWAT mass, and reduced glucose and insulin tolerance. SA significantly reduced EWAT mass, more so in HFD (p < .05), but had no effect on any in vivo measurements (BW, insulin, glucose, or pyruvate tolerance, nor physical activity, all p > .05). A significant (p < .05) interaction was observed between mitochondrial respiration and treatment, where SA increased respiration, likely owed to greater mitochondrial content, as assessed by complex IV activity in both LFD and HFD. In HFD-induced obesity, coupled with insulin desensitization, we found no favorable effect of succinic acid on glucose regulation, though adiposity was attenuated. In oxidative skeletal muscle, there was a tendency for increased respiratory capacity, likely owed to greater mitochondrial content, suggestive of a succinic acid-induced mitochondrial biogenesis., (© 2020 The Authors. Abstract

Climate change is expected to increase weather variability and incidences of extreme events, which will have an impact on livelihoods and wellbeing. This study was intended to assess the role of livestock diversification in minimizing climate change adverse impacts on livelihood assets in Dangila district, Ethiopia. A random sampling technique was used, and 107 sample households were selected. Primary data were collected through field visit and interview, whereas secondary data were collected from the district agricultural office and meteorological stations. To analyze the data, descriptive statistics, correlation and multiple linear regressions were used for testing the hypotheses. Accordingly, the results revealed that weather shocks affected livelihood assets negatively and significantly (β = - .157, p < 0.05) than other shocks. There has been a significant positive interaction effect (β = .197, p < 0.05) between adaptive capacity and weather shock which implies that a household's adaptive capacity (through livestock diversification) counteracts the adverse effects of weather shocks on livelihood assets. Cattle population size has decreased by 19.8% from 2008 to 2017, which infers that reductions in rainfall amount and variation drive the downward trend in cattle number. To conclude, the efforts, which were achieved in terms of sustainable adaptation practices that enhance the resilience of household's livelihood assets, were not adequate. Therefore, to further enhance households' adaptive capacity, improvement in livestock diversification through the provision of a package of livestock species and access to credit for youths were recommended.

Abstract

Background: Evaluation of a surveillance system should be conducted on regular bases to ensure that the system is working as envisioned or not. Therefore, we evaluated Dangila district's public health surveillance system performance in line with its objectives., Methods: In August 2017, a concurrent embedded mixed quantitative/qualitative, facility-based cross-sectional study was conducted in Dangila district among 12 health facilities/sites. The qualitative part involved 12 purposively selected key stakeholders interview. A semi-structured questionnaire adapted from updated CDC guideline for evaluating public health surveillance system was used for data collection through face to face interview and record review. The major qualitative findings were narrated and summarized based on thematic areas to supplement the quantitative findings. The quantitative findings were analyzed using Microsoft Excel 2007., Results: All necessary surveillance guidelines, registers and reporting formats were distributed adequately to health facilities. Only the district health office has Emergency Preparedness and Response Plan (EPRP), but not supported by the budget required to respond in case an emergency occurred. There were no regular data analysis and interpretations in terms of time, place and person. Weekly report completeness and timeliness were 100 and 94.6% respectively. The information collected was considered relevant by its users to detect outbreaks early with high acceptability. All stakeholders agreed that the system is simple, easy to understand, representative and can accommodate modifications. Written feedbacks were not obtained in all health facilities. The supervision checklist obtained in the district was not adequate to assess surveillance activities in detail. The calculated positive predictive value for malaria was 11%., Conclusions: The surveillance system was simple, useful, flexible, acceptable and representative. Report completeness and timelines were above the national and international targets. However, the overall implementation of the system in the district was not satisfactory to achieve the intended objective of surveillance for public health action due to the lack of regular data analysis and feedback dissemination. To create a well-performing surveillance system, regular supervision and epidemiologically analyzed and interpreted feedback system is mandatory.

15. Effect of rice variety and blending proportion on the proximate compositions, minerals and phytic acid contents of bread from rice-teff blend.

Abstract

Development of bakery products containing rice ( Oryza sativa, Linn .) and teff ( Eragrostis tef ) could have potential health benefits due to their gluten free nature. Nine experimental runs were generated using custom design by JMP 8 software. The effect of two factors, rice variety (Edeget, X-jigna and Nerica-4) and blending proportions of rice and teff (0.5:0.5, 0.7:0.3 and 0.9:0.1) were studied. The data analysis was conducted using SAS software package for the mean comparison and custom design by JMP 8 software. Response surface methodology was applied to study the interaction effect of the main factors and to generate the predictive equations. An optimal value (1.60%) of fiber was obtained when the proportion of the blend was 50% Edeget and 50% teff because teff grain is high in fiber. A maximum value (10.75%) of protein was obtained when the proportion of the blend was 70% Nerica-4 and 30% teff. Carbohydrate was optimal (81.37%) when 90% Edeget and 10% teff were blended because rice grain is high in carbohydrate. Optimal iron content (12.97 mg/100g) was obtained when the proportion of the blend was 50% Nerica-4 and 50% teff because teff grain is high in iron. Optimal zinc content (4.14 mg/100g) was obtained when the proportion of the blend was 50% X-jigna and 50% teff. The optimal value (61.25 mg/100g) of calcium was obtained when the proportion of the blend was 50% Edeget and 50% teff.  Optimum (lower) value (0.31mg/g) of phytic acid was obtained when the proportion of the blend was 90% Nerica-4 and 10% teff because rice grain is lower in phytic acid content. It was concluded that rice variety and rice-teff blending proportion had a significant effect on the physico-chemical properties of rice-teff blend bread. An optimal nutrient blend (high in nutrients, low in anti-nutrients) was obtained when 70% Edeget rice variety was blended with 30% teff.
