Abstract

Promising initial studies on vitamin D (VD) supplementation as an adjunct treatment for major depressive disorder (MDD) require rigorously designed randomized controlled trials (RCTs). We aim to investigate the effects of augmenting standard MDD treatment with VD supplementation and examine factors influencing the treatment outcome. This article describes the study design, measures, and baseline characteristics. This multicenter RCT compares the efficacy of a six‐month VD intervention at 100 micrograms/day versus 10 micrograms/day (μg/day) (4000 IU (international units)/day vs. 400 IU/day) added to a standard treatment in outpatients aged 20–61 years with MDD. The primary outcome is change in the Montgomery–Åsberg Depression Rating Scale (MADRS) score. Secondary outcomes are other indicators of mental health and functionality (BDI, SOC, 15‐D, PSS10, LS‐4, LOT‐R, YSQ‐S2‐extended, CORE‐OM, TAS‐20, BRCS, TADS, SCL‐90, DIAD, GAF), and circulating biomarkers. Intervention assessments are conducted at baseline, 3, and 6 months, and follow‐ups at 18 months and 6 years post‐baseline. The baseline sample had 319 subjects (74% women; median age 31 (inter‐quartile range (IQR) 15), mean MADRS score 21.38 (SD 6.04)), with 281 assigned to the RCT. At present, the study continues as a follow‐up study. DepFuD project will provide extensive information regarding the potential benefits of VD and enables to identify various biopsychosocial depression‐associated risk factors. [ABSTRACT FROM AUTHOR]

3. Oppimisanalytiikkaa hyödyntävän suosittelutyökalun mahdolliset hyödyt ja rajoitteet uraohjauksessa

Abstract

Tiivistelmä. Tieto- ja viestintäteknologiaa sekä tekoälyä pidetään yhtenä ratkaisuna elinikäisen ohjauksen tavoitteiden saavuttamiseen. Teknologiaa hyödyntäessä ja kehittäessä tulisi kuitenkin huomioida myös teknologian rajoitukset sekä käyttäjäryhmän tarpeet. Tämän laadullisen pro gradu -tutkielman tavoitteena on tutkia uraohjaajien ja hallinnon edustajien näkemyksiä oppimisanalytiikkaa hyödyntävän suosittelutyökalun mahdollisista hyödyistä ja rajoitteista uraohjauksessa. Pro gradu -tutkielma on toteutettu laadullisena tutkimuksena. Tutkimusaineistona on EU-rahoitteisen Compleap-hankkeen yhteydessä vuonna 2019 kerätty haastatteluaineisto, joka on toteutettu puolistrukturoituna teemahaastatteluna. Aineisto koostuu kolmen ammatillisen oppilaitoksen ohjaajan haastatteluista sekä kolmen ammatillisen oppilaitoksen hallinnon edustajan haastatteluista. Aineiston analysointimenetelmänä on aineistolähtöinen sisällönanalyysi. Tutkielman tulosten perusteella suosittelutyökalu nähtiin hyödylliseksi niin ohjaajalle, ohjattavalle kuin organisaatiollekin. Ohjaajalle mahdolliset hyödyt näyttäytyvät etenkin ohjaajan työn tehostamisen kautta, sillä työkalu sisältää hyödyllistä tietoa ja voi auttaa säästämään aikaa ohjauksessa. Ohjattavalle työkalu nähtiin hyödylliseksi, sillä se on saavutettava, mahdollistaa elinikäisen oppimisen, sisältää hyödyllistä tietoa ja tekee päätöksenteosta mielekkäämpää. Organisaatiolle mahdolliseksi hyödyksi nähtiin kustannustehokkuus, sillä työkalu voisi auttaa asiakkaita tekemään onnistuneempia koulutusvalintoja, auttaa organisoimaan ohjaustilanteita sekä tekemään organisaation koulutustarjonnan näkyväksi hakijoille. Oppimisanalytiikkaa hyödyntävän suosittelutyökalun käyttämiseen liittyvät huolenaiheet koskivat ohjaajan tuen puutetta, työkalun toteutusta sekä ehdotusten ja tiedon laatua. Haastateltavat toivat esiin huolenaiheen siitä, millä perusteella työkalun ehdotukset on muodostettu ja ovatko ehdotukset luotettavia. Lisäksi huolta aiheutti työkalun mahdollisuudet ottaa huomioon käyttäjien erilaiset valmiudet hyödyntää teknologiaa ja sen sisältämää informaatiota. Tutkimus tuo esiin uraohjaajien ja hallinnon edustajien näkemyksiä suosittelutyökalun mahdollisista hyödyistä uraohjauksessa, mutta myös tärkeitä eettisiä näkökulmia, jotka tulee ottaa huomioon suosittelutyökalun kehitystyössä ja käytössä.The possible benefits and limitations of using learning analytics tool in career guidance. Abstract. Information and communication technology and artificial intelligence are considered as one solution for achieving the goals of lifelong guidance. However, when utilizing and developing technology, the limitations of the technology and the needs of the user group should be considered. The aim of this qualitative master’s thesis is to research the views of a career counsellors and administration representatives on the possible benefits and limitations of a recommendation tool in career guidance. The thesis has been carried out as a qualitative study. Data collection has been carried out with interviews as part of the EU funded project Compleap in 2019. The data consists of interviews with three career counsellors and three representatives of the administration from vocational educational institutions. The data analysis method is data-oriented content analysis. According to the results of the study, the recommendation tool was seen as useful for career counsellor, client, and organisation. For the career counsellor, the tool was seen as useful, because it might make counsellor’s work more efficient. Tool contains useful information about the client and might save time in career guidance. The tool could be useful for the client because it was evaluated as accessible, supporting lifelong learning, containing useful information, and making decision-making easier. From organization perspective cost efficiency was seen as a possible benefit of a tool because it might help their clients to make more suitable educational choices. It might also help to organise career guidance and make organization visible to potential students. Concerns related to using the recommendation tool in career guidance included the lack of instructor support, the implementation of the tool, and the quality of recommendations and information. Interviewees were concerned how the algorithm makes the recommendations and whether the recommendations are reliable. They were also concerned whether the tool would be able to consider people’s different capabilities to use technology and its information. The research brings out the views of career counselors and administration representatives on the possible benefits of recommendation tools, but also important ethical perspectives that should be considered in development process and the use of learning analytics tools

Abstract

Tiivistelmä Nikotiini on riippuvuutta aiheuttava, terveydelle haitallinen yhdiste. Se lisää monin tavoin sairastuvuutta ja kuolleisuutta sydän- ja verisuonitauteihin. Se aiheuttaa useilla mekanismeilla endoteelin toiminnan häiriöitä, ja sen sykettä tihentävä sekä verenpainetta nostava vaikutus välittyy sympaattisen hermoston aktivaation kautta. Nikotiini aktivoi reniini-angiotensiinijärjestelmää (RAS), jonka toiminnan häiriö liittyy moniin sydän- ja verisuonitauteihin. Nikotiini vaikuttaa hengitystä säätelevään kemorefleksiin. Nuorena alkanut nikotiinin käyttö kuormittaa sydän- ja verenkiertoelimistöä, ja pitkäaikaisvaikutukset kertautuvat aikuisiällä. Yhteiskunnan tulisi panostaa nikotiinittomuuteen ja kohdistaa toimet erityisesti nuoriin.

Abstract

Tiivistelmä. Diplomityönä suunnittelen rakennuksen sisään sijoittuvan ulkoilmatyyppisen kuntosalin ja hyvinvointikeskuksen Kuopion kaupunginkirjaston ja terveyskeskuksen väliselle nykyiselle pysäköintialueelle. Diplomityön pääpaino on julkisten rakennusten korttelialueen katutilan eheyttäminen ja kaupunkilaisia palvelevien toimintojen sovittaminen tiiviiseen ympäristöön. Diplomityön idea ja inspiraatio syntyivät pohtimalla keskellä kaupunkirakennetta olevaa aukkoa, jolle uusi asemakaava mahdollistaa tehokkaampaa käyttöä tulevaisuudessa. Suunnitelma on henkilökohtainen näkemys tontin parhaasta mahdollisesta käyttötavasta. Sijainti kulttuurilaitosten ja terveyskeskuksen välissä sopii kokonaisvaltaisen hyvinvoinnin parantamisen teemaan. Sekä koronapandemian että yleisen liikkumattomuuden ja elintapasairauksien vuoksi panostukset kaupunkilaisten terveyteen ja jaksamiseen ovat nykypäivänä tärkeitä. Kortteliin muodostuu julkisivusarja, jossa on edustettuna julkisten rakennusten arkkitehtuuria yli sadan vuoden ajalta. Etelästä lukien ensimmäisenä on kansallisromanttinen luonnonhistoriallinen museo vuodelta 1907 vierellään 2021 valmistunut lisärakennuksensa. Seuraavana jyhkeä ruskea tiilinen kirjasto tummalla jalustallaan. Sitten kuntosalin uudisrakennus, ja kadun toisella puolella vielä terveyskeskus vaaleansinisestä 4x4 ruudutetusta betonielementistä tehtynä. Suunnittelemani rakennuksen alemmassa kerroksessa on kaupunkilaisten vapaassa käytössä oleva kuntosali. Kuntosalin laitteet ovat tukevia metallirakenteisia ulkoliikuntapaikkoja varten valmistettuja laitteita, jotta ne kestäisivät jatkuvaa käyttöä ja muita keskikaupungin julkisen tilan haasteita mahdollisimman hyvin ja taloudellisesti. Kuntosalin julkisivu on maantasokerrokselta lasiseinää sekä tumman harmaata metallia ja puuta. Toisen kerroksen julkisivu on luonnollisen sävyinen puurima suurien ikkunoiden edessä. Toisessa kerroksessa on tiloja sekä julkisen sektorin terveydenhuollon kevyempää käyttöä varten (esim. fysioterapia, seniorien kuntouttaminen, opetus, luennot), että ryhmäliikuntatilaa esim. urheiluseuroille vuokrattavaksi. Toisen kerroksen pohjan ja käytön suunnitelma jää tarkoituksella konseptitasolle, suunnittelun pääpainon ollessa katutason toiminnoissa.

10. Aivojen tasavirtastimulaation teho masennuksen hoidossa

Abstract

Tiivistelmä Aivojen tasavirtastimulaatio (tDCS) on uusimpien meta-analyysien perusteella masennuksen hoidossa lumetta tehokkaampaa vaikutuksen koolla 0,3–0,7 (Hedgesin g). Teho lienee samaa luokkaa kuin lääkehoidon. Tasavirtastimulaation tehoa masennuksessa, jossa on liitännäissairauksia, on tutkittu vähän. Aivohaverin jälkeiseen masennukseen se näyttää olevan lumetta tehokkaampi. On viitteitä siitä, että hoitotulos on parempi potilailla, joilla masennuksen yhteydessä esiintyy ahdistuneisuusoireita. Tasavirtastimulaatio ei näytä soveltuvan hoitoresistentin masennuksen hoitoon. Hoito voidaan toteuttaa potilaan kotona. Abstract Transcranial direct current stimulation (tDCS) is a non-invasive form of brain stimulation, which modifies neuronal networks by increasing or decreasing cortical sensitivity. tDCS has been investigated for conditions such as depression, obsessive-compulsive disorder, schizophrenia, post-traumatic stress disorder, neuropathic pain and migraine. The majority of research has been conducted on the treatment of unipolar depression. Depression is the most common cause for psychiatric disability in Finland, but current forms of treatment remain insufficient. Approximately 20% of patients have treatment resistant depression. Recent meta-analyses suggest that tDCS is as effective as pharmacotherapy. tDCS can be combined with pharmacotherapy and psychotherapy without significant side effects and it can also be performed in the patient’s home. Additional research on tDCS should be carried out regarding optimal treatment dosage, electrode placement, optimal patient selection, as well as maintenance treatment.

Abstract

Background: Maternal depression during pregnancy increases the risk for adverse developmental outcomes in children. However, the underpinning biological mechanisms remain unknown. We tested whether depression was associated with levels of and change in the inflammatory state during pregnancy, if early pregnancy overweight/obesity or diabetes/hypertensive pregnancy disorders accounted for/mediated these effects, and if depression added to the inflammation that typically accompanies these conditions. Methods: We analyzed plasma high-sensitivity C-reactive protein (hsCRP) and glycoprotein acetyls at three consecutive stages during pregnancy, derived history of depression diagnoses before pregnancy from Care Register for Healthcare (HILMO) (N = 375) and self-reports (N = 347) and depressive symptoms during pregnancy using the Center for Epidemiological Studies Depression Scale completed concurrently to blood samplings (N = 295). Data on early pregnancy body mass index (BMI) and diabetes/hypertensive pregnancy disorders came from medical records. Results: Higher overall hsCRP levels, but not change, during pregnancy were predicted by history of depression diagnosis before pregnancy [HILMO: mean difference (MD) = 0.69 standard deviation (s.d.) units; 95% confidence interval (CI) 0.26–1.11, self-report: MD = 0.56 s.d.; 95% CI 0.17–0.94] and higher depressive symptoms during pregnancy (0.06 s.d. per s.d. increase; 95% CI 0.00–0.13). History of depression diagnosis before pregnancy also predicted higher overall glycoprotein acetyls (HILMO: MD = 0.52 s.d.; 95% CI 0.12–0.93). These associations were not explained by diabetes/hypertensive disorders, but were accounted for and mediated by early pregnancy BMI. Furthermore, in obese women, overall hsCRP levels increased as depressive symptoms during pregnancy increased (p = 0.006 for interaction). Abstract

Aims: To evaluate risk factors for major adverse cardiac event (MACE) after the first acute coronary syndrome (ACS) and to examine the prevalence of risk factors in post-ACS patients. Methods: We used Finnish population-based myocardial infarction register, FINAMI, data from years 1993–2011 to identify survivors of first ACS (n = 12686), who were then followed up for recurrent events and all-cause mortality for three years. Finnish FINRISK risk factor surveys were used to determine the prevalence of risk factors (smoking, hyperlipidaemia, diabetes and blood pressure) in post-ACS patients (n = 199). Results: Of the first ACS survivors, 48.4% had MACE within three years of their primary event, 17.0% were fatal. Diabetes (p = 4.4 × 10−7), heart failure (HF) during the first ACS attack hospitalization (p = 6.8 × 10−15), higher Charlson index (p = 1.56 × 10−19) and older age (p = .026) were associated with elevated risk for MACE in the three-year follow-up, and revascularization (p = .0036) was associated with reduced risk. Risk factor analyses showed that 23% of ACS survivors continued smoking and cholesterol levels were still high (>5mmol/l) in 24% although 86% of the patients were taking lipid lowering medication. Conclusion: Diabetes, higher Charlson index and HF are the most important risk factors of MACE after the first ACS. Cardiovascular risk factor levels were still high among survivors of first ACS.

Methods: Standardized methods were used by trained technicians to collect information on 7824 patients from 130 centers in 27 countries, from the medical records and at a visit at least 6 months after hospitalization for a coronary event. All lipid measurements were performed in one central laboratory. Patients were divided into three groups: on high-intensity LDL-C-lowering-drug therapy (LLT), on low or moderate-intensity LLT and on no LLT. Results: At the time of the visit, almost half of the patients were on a high-intensity LLT. Between hospital discharge and the visit, LLT had been reduced in intensity or interrupted in 20.8% of the patients and had been started or increased in intensity in 11.7%. In those who had interrupted LLT or had reduced the intensity, intolerance to LLT and the advice of their physician were reported as the reason why in 15.8 and 36.8% of the cases, respectively. LDL-C control was better in those on a high-intensity LLT compared to those on low or moderate intensity LLT. LDL-C control was better in men than women and in patients with self-reported diabetes. Conclusions: The results of the EUROASPIRE V survey show that most coronary patients have a less than optimal management of LDL-C. More professional strategies are needed, aiming at lifestyle changes and LLT adapted to the need of the individual patient.

Abstract

Tiivistelmä. Tässä tutkielmassa tarkastellaan alkuopetuksen luokanopettajien näkemyksiä tunnekasvatuksesta ja sen merkityksestä alkuopetuksessa. Lisäksi tutkielmassa paneudutaan siihen, miten luokanopettajat toteuttavat tunnekasvatusta työssään ja millaisiksi he mieltävät omat valmiutensa toteuttaa tunnekasvatusta. Tutkimuksen aihetta pidetään tärkeänä tutkia, sillä Perusopetuksen opetussuunnitelman perusteet 2014 velvoittaa opettajia tunnekasvatukseen. Aiheeseen liittyvää tutkimusta on kuitenkin tehty kotimaassa varsin vähän, mikä luo tarpeen sen tutkimiselle. Tutkimuskysymykset ovat seuraavat: Minkälaisia näkemyksiä luokanopettajilla on tunnekasvatuksesta alkuopetuksessa? Mitä keinoja luokanopettajilla on toteuttaa tunnekasvatusta alkuopetuksessa? Millaiseksi opettaja mieltää omat valmiutensa toteuttaa tunnekasvatusta? Luokanopettajien näkemysten tutkimista tunnekasvatuksesta alkuopetuksessa pidetään tärkeänä, sillä aikaisemmat tutkimukset ovat osoittaneet, että opettajan oma asenne tunnekasvatusta kohtaan on yhteydessä tunnekasvatuksen onnistumiseen. Tutkimusongelma on rajattu alkuopetukseen, sillä aiempien tutkimusten mukaan lapsen ensimmäiset kouluvuodet ovat tunnetaitojen oppimisen kannalta kriittisiä. Opettajan toteuttama tunnekasvatus on alkuopetusikäisten oppilaiden kohdalla erityisen merkittävää, mistä johtuen pidetään tärkeänä, että luokanopettajien näkemyksiä omista valmiuksistaan toteuttaa tunnekasvatusta tuodaan esiin. Tutkimuksen teoreettinen viitekehys rakentuu tunteiden, tunneälyn, tunnetaitojen ja tunnekasvatuksen teoriaan, minkä lisäksi tutkielmassa esitellään aikaisempia tunnekasvatukseen liittyviä tutkimuksia. Tutkimus on luonteeltaan kvalitatiivinen ja sen lähestymistapa on hermeneuttisfenomenologinen. Aineistonkeruumenetelmänä toimii teemahaastattelu. Haastateltavia ovat viisi alkuopetuksessa työskentelevää luokanopettajaa. Aineiston analysointikeinona on teoriaohjaava sisällönanalyysi. Tutkimus osoittaa, että tunnekasvatusta pidetään tärkeänä osana alkuopetusta, mutta vähemmistö haastatelluista kokee sen vahvuudekseen. Luokanopettajat ilmaisevat tarpeen lisäkoulutukselle. Tunnekasvatuksen toteutustavat jakautuvat tilannesidonnaisiin ja systemaattisiin keinoihin. Tutkimuksesta saatavan tiedon avulla luodaan kuvaa tunnekasvatuksen toteutuksesta suomalaisen peruskoulun alkuopetuksessa. Sen avulla voidaan kehittää luokanopettajien koulutusta. Sitä voivat hyödyntää myös kaikki kasvatusalan ammattilaiset oman tunnekasvatustyönsä tukena.

Abstract

This is the second iteration of the European Society of Cardiology (ESC) and European Association for the Study of Diabetes (EASD) joining forces to write guidelines on the management of diabetes mellitus (DM), pre-diabetes, and cardiovascular disease (CVD), designed to assist clinicians and other healthcare workers to make evidence-based management decisions. The growing awareness of the strong biological relationship between DM and CVD rightly prompted these two large organizations to collaborate to generate guidelines relevant to their joint interests, the first of which were published in 2007. Some assert that too many guidelines are being produced but, in this burgeoning field, five years in the development of both basic and clinical science is a long time and major trials have reported in this period, making it necessary to update the previous Guidelines.

Abstract

Associations between adverse childhood experiences (ACEs) and cholesterol in depressed patients are unclear. Therefore, we compared 78 adult outpatients with major depressive disorder (MDD) with (n = 24) or without (n = 54) experiences of physical violence in childhood. Background data were collected with questionnaires, and total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) were measured from fasting blood samples. Patients with a history of childhood physical violence had lower levels of TC than the control group. No differences were observed in HDL-C, LDL-C, or low-grade inflammation levels between the two groups. In multivariate models, decreased levels of TC were associated with childhood physical violence, and these associations remained significant after adjustments for age, gender, lifestyle, metabolic condition, socioeconomic situation, psychiatric status, suicidality, low-grade inflammation, the chronicity of depression, medications used and somatic diseases. At the 8-month follow-up, the results were essentially the same when the Trauma and Distress Scale (TADS) was used as the measure of ACEs. The specific mechanisms underlying cholesterol alterations associated with ACEs are a topic for future studies. Better understanding of these mechanisms might lead to possible new interventions in the prevention of adverse health effects resulting from ACEs.

Abstract

Background: Idiopathic normal pressure hydrocephalus (iNPH) is a progressive and potentially treatable neurodegenerative disease affecting elderly people, characterized by gait impairment and ventricular enlargement in brain imaging. Similar findings are seen in some patients with schizophrenia (SCZ). Objective: To determine the prevalence of SCZ among patients suffering from probable or possible iNPH and the specific effects of comorbid SCZ on the outcome of the cerebrospinal fluid (CSF) shunting. Methods: All medical records of the 521 iNPH patients in the NPH registry were retrospectively analyzed from 1991 until 2017. The prevalence of comorbidity of SCZ was determined and compared to that of general aged (≥65 yr) population in Finland. Results: We identified a total of 16 (3.1%) iNPH patients suffering from comorbid SCZ. The prevalence of SCZ among the iNPH patients was significantly higher compared to the general population (3.1% vs 0.9%, P < .001). All iNPH patients with comorbid SCZ were CSF shunted and 12 (75%) had a clinically verified shunt response 3 to 12 mo after the procedure. The CSF shunt response rate did not differ between patients with and without comorbid SCZ. Conclusion: SCZ seems to occur 3 times more frequently among iNPH patients compared to the general aged population in Finland. The outcome of the treatment was not affected by comorbid SCZ and therefore iNPH patients suffering from comorbid SCZ should not be left untreated. These results merit validation in other populations. In addition, further research towards the potential connection between these chronic conditions is warranted.

47. Positiivinen pedagogiikka ja sen vaikutus oppilaan sosiaaliseen kompetenssiin

Tiivistelmä. Kandidaatin tutkielmamme tavoitteena oli tutkia positiivista pedagogiikkaa ja sen käytön vaikutuksia oppilaan sosiaaliseen kompetenssiin. Koulujen luokkakoot kasvavat ja oppilaiden käytöshäiriöt ovat lisääntyneet. Oppilaiden keskinäisten sekä opettajan ja oppilaiden välisten suhteiden toimivuuden tärkeys korostuu luokan toiminnan ja oppimisen kannalta. Halusimme tutkia työssämme erityisesti positiivisen pedagogiikan soveltamista opetuksessa ja sen vaikutuksia oppilaiden sosiaalisiin taitoihin, suhteisiin ja luokan ilmapiiriin. Päätimme lähestyä ongelmaa sosiaalisen kompetenssin kautta sen kattavuuden vuoksi. Positiivinen pedagogiikka on positiivisen psykologian pohjalta syntynyt kasvatus- ja opetussuuntaus. Perehdyimme työssämme erityisesti vahvuusopetukseen, joka on positiivisen pedagogiikan sovellus. Tutkimuksemme myötä selvisi, että positiivisen pedagogiikan ja vahvuusopetuksen kautta voidaan vaikuttaa oppilaan sosiaaliseen kompetenssiin. Sekä opettaja että luokan muut oppilaat voivat tukea oppilaan sosiaalista kompetenssia esimerkiksi vahvuusopetuksen kautta ja ylläpitämällä myönteistä ja hyväksyvää koulukulttuuria. Toteutimme tutkimuksemme parityönä teoreettisena kirjallisuuskatsauksena. Etsimme sekä kansainvälisiä että kotimaisia lähteitä ja pyrimme löytämään ajankohtaista ja keskeistä lähdemateriaalia, jotta työmme olisi relevantti. Viittasimme lähteisiin asiaankuuluvalla viittaustekniikalla. Koemme, että työn toteuttaminen parityönä lisäsi tutkimuksen luotettavuutta, sillä ylläpidimme keskustelemalla työn luotettavuutta koko työskentelyprosessin ajan. Tutkimme aihetta yleisellä tasolla rajoittamatta tutkimuskohdetta esimerkiksi tiettyyn ikäryhmään. Lisäksi halusimme tutkimuksemme keskustelevan kansainvälisen tutkimuksen kanssa. Tulokset ovat mielestämme hyvin sovellettavissa käytännön työssämme luokanopettajina.Abstract. The number of students in school classes increases along with misbehavior of the students. The importance of the quality of relationships between the students’ and the teacher is emphasized. The goal of our bachelor’s thesis was to investigate whether using positive pedagogy in teaching would impact students social skills and the quality of the relationships between the class members. We chose to approach the research problem using the concept of social competence for its comprehensive nature. Positive pedagogy is an educational orientation based on positive psychology. We orientated ourselves particularly in strength-based education, which is practical application of positive pedagogy. Based on the research we found that students’ social competence, not limited to a particular age group, can be impacted through the use of positive pedagogy and strengthbased education in teaching. Through proper application of strength-based education, both the teacher and other students can promote one student’s social competence. The thesis was conducted between the two of us using the methods of theoretical literature review. In this paper we reviewed the most recent national and international references to ensure that our research will be relevant and could be beneficial for teachers worldwide, and we referenced to it in an appropriate manner. Our research-related conversations between each other increased the validity and reliability of our research. In our opinion the results can easily be adapted into the future job of ours as teachers.

Abstract

Aims: The objective of the study is to determine the demographics and the in-hospital outcome of diabetic and non-diabetic patients treated with percutaneous coronary interventions (PCI) in Europe, to report the type of equipment and technology used for PCI procedures in diabetics and to clarify whether the treatment of diabetic patients complies with current European Society of Cardiology (ESC) guidelines. Methods and results: A total of 14,458 patients treated with PCI were enrolled from 29 member countries of the ESC between June 2005 and January 2006. Data were collected on patient characteristics and treatment, using new Cardiology Audit and Registration Data standards. In total, 3,603 patients (24.9%) were diabetic. Diabetics were older, more often female and had a higher body mass index than non-diabetics. Diabetics had higher rates of hypercholesterolaemia and hypertension, while current smokers were more frequent in the non-diabetics. Diabetics also had significantly higher rates of previous cardiovascular events. Clopidogrel was administered only in 48.1% of diabetic patients before PCI, while IIb/IIIa inhibitors were 22.9% during PCI. At discharge, there was a major adjustment of treatment with increases in the use of Beta-blocker (80.4%), angiotensin converting enzyme inhibitor (ACEI, 71.3%) and statins (89.8%) compared with on admission (Beta-blocker 60.9%, ACEI 55.0%, statin 63.1%). Inhospital mortality was higher in diabetics (1.8% vs 1.2%) although the in-hospital MACCE rate was not significantly different (3.6% vs 3.0%, p=0.09). Conclusions: Diabetic patients treated with PCI were older with more comorbidity. According to ESC guideline, the under-usage of clopidogrel, GP IIb/IIIa inhibitors should be improved. PCI is now taken as a good opportunity to adjust the use of appropriate medication. © Europa Edition. All rights reserved.
