Background: Early childhood education and care (ECEC) centers are ideal venues for food education. As smartphones and tablets are becoming increasingly popular in ECEC centers, technology can be used to deliver such pedagogical content. Evidence suggests that video games can affect fruit and vegetable (FV) consumption among 9-to 12-year-old children, but studies among preschoolers are scarce. Objective: This paper describes the development of the Mole's Veggie Adventures app and its effectiveness in increasing FV acceptance among Finnish and Polish preschoolers aged 3 to 6 years. Methods: A multiprofessional team created an app to be used in ECEC centers in groups of 3 to 10 children. The app aimed to increase vegetable acceptance, and it was built using elements that support the development of self-regulation and social skills. Altogether, 7 Finnish and 4 Polish ECEC centers participated in the study. Before randomization, parents reported background factors and their children's willingness to taste different FVs. The ECEC professionals in the intervention arm were instructed to use the app at least once a week during the 3-to 4-week intervention period. The main outcomes in this unblinded, cluster-randomized study were FV acceptance and relative FV acceptance. The first was calculated as a sum variable describing the children's willingness to taste 25 different FVs, the second as FV acceptance divided by the number of FVs served. We used analysis of covariance to compare the FV acceptance and relative FV acceptance scores between the intervention and control groups at follow-up. Results: A total of 221 children were included in the analysis. At follow-up, the intervention group (115/221, 52%) had higher FV acceptance scores (baseline adjusted difference of mean 7.22; 95% CI 1.41-13.03) than the control group (106/221, 48%). The intervention effect was parallel for relative FV acceptance scores (baseline adjusted difference of mean 0.28; 95% CI 0.05-0.52). Conclusions: The Mole's Veggie Adventures app has the potential to increase FV acceptance among preschoolers and can be a valuable tool in supporting food education in ECEC centers. Furthermore, the app can be feasibly incorporated into preschool routines in countries with different educational environments. Disturbances in insulin sensitivity and insulin secretion precede the manifestation of type 2 diabetes. Both genes and environment contribute to the diabetes risk. Heritability indicates the proportion to which genotype determines the variability of a trait, such as for example blood glucose concentration. The current work was done for the Hjelt institute and for the Finnish institute of Molecular Medicine (FiMM) in the University of Helsinki. Heritability of insulin sensitivity, insulin secretion and traits associated with type 2 diabetes were estimated by means of twin and family studies. The twins belonged to the Finnish Twin Cohort Study of the University of Helsinki and the families participated in the Botnia study, a project on type 2 diabetes genetics. The collecting of the material was made between the years 1992 - 2004. Insulin sensitivity was measured by means of hyperinsulinaemic euglycaemic clamp technique. Insulin secretion was measured both via oral (OGTT) and intravenous (IVGTT) glucose tolerance tests. The heritability of body mass index (BMI) and type 2 diabetes were estimated in a follow-up study of twins. Furthermore, heritability estimates of several glucose metabolism and insulin sensitivity related traits were obtained by examining Botnia study families where at least two family members had type 2 diabetes. Altogether 66 monozygotic (MZ) and 85 dizygotic (DZ) twin pairs underwent OGTT. A subset of the pairs participated also in IVGTT- and clamp studies. The follow-up study was based on information that was received from 10400 twin pairs during a maximum of 28 years of follow-up. From the Botnia-study, 5810 adults from 942 families were chosen: 1707 of them had type 2 diabetes. The average family size was 6,17. On the basis of the metabolic studies, it was concluded that the genotype determines the insulin secretion's early phase, which begins immediately after the ingestion or infusion of glucose. The heritability of this trait varied between 55% and 76%. The majority of variation in insulin sensitivity, instead, seemed to be due to non-hereditary factors, as the heritability of insulin sensitivity was only 37%. In the follow-up study, 6.3% of male and 5.1% of female twins got type 2 diabetes. BMI calculated on the basis of the data recorded at the start of the follow-up, predicted future diabetes well. On average, the hazard ratio for type 2 diabetes was 1,22 per each BMI unit and the risk began to increase already from the BMI-value of 20kg/m2. The hazard ratios for type 2 diabetes in normal weight, overweight, obese and morbidly obese twins were 0.59, 2.96, 6.80 and 13.64, respectively. When BMI and type 2 diabetes were modelled together, the heritability of BMI in men was 75%, and 71% in women. Correspondingly, the heritability of type 2 diabetes was 73% in men and 64% in women, respectively. The genetic factors influencing BMI explained only 16% of the risk for type 2 diabetes among men and 21% of the risk among women. In the Botnia study, the highest heritability estimate for type 2 diabetes, 69%, was observed among individuals aged 35 to 60 years. The heritability of the early phase of insulin secretion varied between 41% (all subjects) and 50% (non-diabetic subjects). The heritability of insulin sensitivity was the same as among twins, from 37% to 40%. Among diabetes-related traits, the highest heritability estimates were obtained for lean body mass (53% to 65%), serum HDL cholesterol concentration (52% to 61%) and suppression of free fatty acids during OGTT (63% to 76%). The variation of all insulin sensitivity and insulin secretion -related traits were smaller within than between families. It is stated as a summary of this doctoral thesis that the risk for type 2 diabetes seems to be an inherited trait. A relatively small share in this risk, on the other hand, seems to be due to genes which influence BMI. Finally, the early phase of the pancreatic insulin secretion appears to be an aspect of metabolism that encompasses promising phenotypes for genomic studies. Häiriöt sokeriaineenvaihdunnan insuliiniherkkyydessä ja insuliininerityksessä edeltävät tyypin 2 diabeteksen puhkeamista. Sekä perimä (geneettiset tekijät) että elintavat vaikuttavat diabetesriskiin. Periytyvyys eli heritabiliteetti ilmaisee, paljonko perimä määrää mitattavan ominaisuuden kuten esimerkiksi verensokerin vaihtelua. Helsingin yliopiston lääketieteellisen tiedekunnan alaisessa, Hjelt-instituutille ja Suomen molekyylilääketieteen instituutille (FiMM) tehdyssä väitöstutkimuksessa selvitettiin sokeriaineenaihdunnan, insuliininerityksen ja diabetesriskin periytyvyyttä kaksos- ja perhetutkimuksen keinoin. Kaksoset kuuluivat Helsingin yliopiston Kaksostutkimukseen, joka vuonna 2014 täyttää 40 vuotta. Perheaineisto koottiin suomalaisesta Botnia-tutkimuksesta, joka on maailman huomattavimpia tyypin 2 diabeteksen perimää selvittäviä projekteja. Aineiston kokoaminen ja seuranta tehtiin vuosien 1992 - 2004 aikana. Tutkimuksessa määritettiin insuliiniherkkyys clamp-tekniikan avulla: tutkittavan henkilön veren insuliinitaso nostettiin keinotekoisesti halutulle tasolle ja katsottiin, kuinka paljon sokeriliuosta on annettava laskimoon estämään insuliinin aikaansaama verensokerin lasku. Insuliinineritys mitattiin sekä tavallista (oraalista, OGTT) että laskimonsisäistä (IVGTT) sokerirasitusta käyttäen. Tyypin 2 diabeteksen ja kehon painoindeksin (BMI) periytyvyys selvitettiin pitkässä seurantatutkimuksessa. Lisäksi useille sokeriaineenvaihduntaan ja insuliiniherkkyyteen liittyville piirteille määritettiin periytyvyys tutkimalla Botnia-tutkimukseen osallistuneita perheitä, joissa vähintään kahdella perheenjäsenellä on tyypin 2 diabetes. Kaikkian 66 identtiselle ja 85 epäidenttiselle kaksosparille tehtiin OGTT ja osalle lisäksi sekä IVGTT että clamp. Seurantatutkimus perustui 10400 kaksosparista saatuihin tietoihin. Pisin seuranta-aika oli 28 vuotta. Botnia-tutkimuksesta valittiin 942 perhettä, joihin kuului 5810 aikuista. Heistä 1707:llä oli tyypin 2 diabetes. Keskimääräinen perhekoko oli 6,17. Kaksosille tehtyjen aineenvaihduntatutkimusten perusteella ilmeni, että perimä määrää vahvasti erityisesti insuliininerityksen varhaista vaihetta, joka alkaa välittömästi sokerin nauttimisen tai laskimoon annostelun jälkeen. Tämän ominaisuuden periytyvyys vaihteli 55 ja 76% välillä. Valtaosa insuliiniherkkyyden vaihtelusta sen sijaan näytti johtuvan ei-perinnöllisistä syistä ja insuliiniherkkyyden periytyvyydeksi saatiin vain 37%. Seurantatutkimuksessa 6,3% mies- ja 5,1% naiskaksosista sairastui tyypin 2 diabetekseen. Seurannan alussa kerättyjen tietojen perusteella laskettu painoindeksi ennusti tulevaa diabetesta erittäin hyvin. Keskimäärin riskisuhde oli 1,22 jokaista nousevaa BMI-yksikköä kohti. Diabetesriski alkoi lisääntyä jo BMI-arvosta 20 kg/m2 lähtien. Normaalipainoisten, ylipainoisten, lihavien ja sairaalloisen lihavien riskisuhteet olivat 0,59, 2,96, 6,80 ja 13,64. Kun sekä BMI että tyypin 2 diabetes huomioitiin yhtä aikaa, saatiin BMI:n periytyvyydeksi 75% miehillä ja 71% naisilla. Vastaavasti tyypin 2 diabeteksen periytyvyydeksi saatiin miehillä 73% ja naisilla 64%. Painoindeksin perintötekijät selittivät vain 16% miesten ja 21% naisten riskistä sairastua tyypin 2 diabetekseen. Botniatutkimuksessa tyypin 2 diabeteksen periytyvyys oli suurimmillaan 69 prosenttia, 35 - 60 -vuotiaiden keskuudessa. Insuliininerityksen varhaisen vaiheen periytyvyys vaihteli 41% (kaikki tutkittavat) ja 50% (tutkittavat, joilla ei diabetesta) välillä. Insuliiniherkkyyden periytyvyys oli sama kuin kaksostutkimuksessa, 37-40%. Diabetekseen liittyvistä aineenvaihdunnan ominaisuuksista korkein periytyvyys saatiin kehon rasvattomalle massalle (53-65%), seerumin HDL-kolesteroli-pitoisuudelle (52-61%) ja OGTT:n aikaiselle vapaiden rasvahappojen pitoisuuden laskulle (63-76%). Kaikkien insuliiniherkkyyteen ja insuliinineritykseen liittyvien ominaisuuksien vaihtelu perheiden sisällä oli pienempää kuin perheiden välillä. Väitöstutkimuksen yhteenvetona todetaan, että riski sairastua tyypin 2 diabetekseen on periytyvä. Painoindeksiin vaikuttavilla geeneillä on merkittävä, kenties tähän asti oletettua pienempi osuus tässä riskissä. Lisäksi todetaan, että insuliininerityksen varhainen vaihe kannattaa huomioida haettaessa tyypin 2 diabetekselle altistavia perintötekijöitä esimerkiksi koko genomin laajuisissa assosiaatiotutukimuksissa.

Nutrition plays an important role in athletic performance. Its full potential is rarely realized due to limited nutrition knowledge among athletes and their coaches. Misunderstandings and gaps in knowledge can lead to food choices that do not support athletic development. This thesis study was undertaken to gain further insight into the nutrition knowledge of young Finnish endurance athletes. Another aim was to evaluate how athletes’ knowledge and nutritional skills can be improved. Study I created and validated a questionnaire measuring knowledge in different areas of sports nutrition. This questionnaire was then used in Study II, which aimed to measure the nutrition knowledge of young Finnish endurance athletes and their coaches. The results of Study II were used to develop an education intervention, aiming to improve athletes’ nutrition knowledge and dietary intake, in Study III. The intervention compared the effects of participatory nutrition sessions alone to those enhanced by a mobile app. In Study II, a total of 312 athletes and 94 coaches completed the questionnaire. The athletes were 17.9±1.2 and the coaches 44.3±12.3 years old. Half of the athletes were women and half men; of the coaches 27% were women. Of the athletes 36% were cross country skiers, 35% orienteers and the rest other endurance athletes. Seventy-nine athletes took part in the intervention in Study III. Their mean age was 18.0±1.4 years, 56% of them were men and 44% were women, and 42% were cross-country skiers. The education in the intervention was based on the Self-Determination Theory and the concept of meaningful learning processes. The education sessions included discussions, tasks and goal setting, which were all intended to increase the athletes’ intrinsic motivation. This motivation in turn is a prerequisite for effective learning. The three sessions lasted 90 minutes each and were held fortnightly. The athletes filled in the questionnaire at baseline and a week and three months after the last session. The athletes in the mobile app group used the app for four days after each session and took photos of everything they ate or drank. Both groups completed a three-day food diary at baseline and three months after the last session and received personal feedback on it. On average, the athletes in Study II answered 73% and the coaches 81% of the items correctly. However, over a half of the athletes and 44% of the coaches scored below the mean knowledge score, at worst answering only 47% of the items correctly. The coaches scored better in all sections of the questionnaire. The questions in the ‘Dietary supplements’ section proved to be the most difficult for the athletes, and those in the ‘Nutrition recommendations for endurance athletes’ section for the coaches. The older the athletes were, the higher was their mean nutrition knowledge score. Among the coaches, the situation was the opposite. On average, the female athletes and coaches scored better than the men. The athletes who were part of a national team had higher knowledge scores than those who were not. The athletes’ nutrition knowledge improved significantly during the intervention. At baseline, their knowledge score was 78%. A week after the education sessions, the athletes in the mobile app group answered on average 87% of the questions correctly and three months later, 86%. In the group without the mobile app, the scores were 85% and 84%, respectively. There was no significant difference between the groups in any sections of the questionnaire. The mean energy intake of the athletes was below the estimated energy expenditure during Study III. The intake of protein and fat met the recommendations for endurance athletes, but the intake of carbohydrates was below them (6–10 g·kg-1·day-1) throughout the study, even though it slightly improved. At the end of the intervention, the athletes in the mobile app group consumed 5.4 g·kg-1·day-1 of carbohydrates and the athletes in the other group 5.0 g·kg-1·day-1. Many psychological, social and economic factors affect what we eat. Improved knowledge does not automatically lead to better food choices if the intention to perform the behavioural change is lacking. The duration of the intervention may also have been too short for notable behavioural changes. In addition, already at the beginning of the study the diet of the athletes was better than that of the general Finnish population, thus leaving less room for dietary improvements. Nutrition knowledge improved after only three education sessions and food diary feedback, but the mobile app did not further improve this learning. Thus, if sport clubs and other sport organizations dedicated even a relatively small amount of time and other resources to structured, targeted, motivational and science-based nutrition education, it may promote positive changes in nutrition knowledge. As athletes make use of the skills they learn during their sports careers in their everyday lives as well as when training other athletes, receiving influential nutrition education could also benefit their performance and health in the future. Hyvä ravitsemustila vaikuttaa urheilijan terveyteen, kehittymiseen, suorituskykyyn ja palautumiseen. Sen tärkeys voi kuitenkin jäädä huomioimatta urheilijoilta ja valmentajilta, sillä heidän ravitsemustiedoissaan on usein puutteita. Harhaluulot ja väärinymmärrykset voivat johtaa ruokavalintoihin, jotka eivät tue urheilemista. Tämän tutkimuksen perusteella nuorten urheilijoiden ravitsemustietoja voidaan parantaa ravitsemusohjauksella. Jo suhteellisen pienellä ajan- ja muiden resurssien käytöllä voidaan saada aikaan merkittäviä muutoksia ravitsemusosaamisessa. Tässä tutkimuksessa selvitettiin, millaista 16–20-vuotiaiden suomalaisten kestävyysurheilijoiden ravitsemusosaaminen on ja miten siihen voidaan vaikuttaa. Tutkimuksen alussa luotiin ja validoitiin kyselylomake mittaamaan osaamista urheiluravitsemuksen eri osa-alueilla. Kyselytutkimukseen osallistui 312 urheilijaa ja 94 valmentajaa. Sen tuloksiin pohjautuen suunniteltiin luentomallista ravitsemusohjausta sekä mobiilisovellusta hyödyntävä ravitsemusinterventio, jolla urheilijoiden ravitsemustietoihin ja ruokavalintoihin pyrittiin vaikuttamaan. Kyselytutkimukseen osallistuneista urheilijoista puolet oli naisia ja puolet miehiä. Osallistuneista valmentajista 27 % oli naisia. Interventioon osallistui 79 urheilijaa, joista 56 % oli miehiä ja 44 % naisia. Vuorovaikutteista ravitsemusohjausta annettiin kolmesti kahden viikon välein. Toinen ryhmä käytti jokaisen luennon jälkeen mobiilisovellusta, jolla kuvattiin kaikki syömiset neljän päivän ajan ja tehtiin näihin liittyviä tehtäviä. He saivat sovelluksen kautta palautetta ruokavaliostaan. Molemmat ryhmät täyttivät kolme vuorokautta ruokapäiväkirjaa tutkimuksen alussa ja lopussa ja saivat tästä palautetta. Kyselytutkimuksessa urheilijat vastasivat keskimäärin 73 % ja valmentajat 81 % kysymyksistä oikein, mutta vaihtelu oli suurta. Heikoimmillaan urheilijat vastasivat alle puoliin kysymyksistä oikein. Valmentajien ravitsemusosaaminen oli parempaa kaikilla kyselyn osa-alueilla. Haasteellisimmiksi osoittautuivat ravintolisiin ja urheilijoiden ravitsemussuosituksiin liittyvät väittämät. Nuoremmilla urheilijoilla oli tiedoissaan enemmän puutteita kuin vanhemmilla; valmentajilla tilanne oli päinvastainen. Naisurheilijat ja -valmentajat saivat keskimäärin miehiä parempia tuloksia. Urheilijoiden ravitsemustiedot paranivat merkittävästi intervention aikana. Tutkimuksen alussa urheilijat vastasivat 78 % kysymyksistä oikein. Viikko luentojen jälkeen oikeita vastauksia oli mobiilisovellusryhmässä keskimäärin 87 % ja kolmen kuukauden päästä 86 %; toisessa ryhmässä vastaavasti 85 ja 84 %. Urheilijoiden keskimääräinen energiansaanti jäi alle arvioidun kokonais-energiankulutuksen, vaikka se tutkimuksen aikana kasvoikin. Proteiineja ja rasvoja saatiin kestävyysurheilijoiden ravitsemussuositusten mukaisesti, mutta hiilihydraatinsaanti jäi suosituksista. Ruokavalintoihin vaikuttavat monet psykologiset, sosiaaliset ja taloudelliset tekijät. Parantuneet tiedot eivät suoraan johda parempiin ruokavalintoihin, jos riittävää motivaatiota käyttäytymisen muutokseen ei ole. Muutokset voivat myös vaatia aikaa, miksi intervention lyhyehkö kesto voi selittää sen, ettei ruokavalioissa tapahtunut tilastollisesti merkitseviä muutoksia. Lisäksi urheilijat söivät jo lähtötilanteessa paremmin kuin suomalaiset keskimäärin. Nuorten urheilijoiden ravitsemustietoja voidaan parantaa merkittävästi huolellisesti suunnitellulla, vuorovaikutteisella, ryhmämuotoisella ravitsemusohjauksella. Urheilijat voivat hyödyntää oppimiaan tietoja uransa aikana sekä sen jälkeen, mikä voi näkyä heidän suorituskyvyssään ja terveydessään. Heidän on lisäksi mahdollista toimia esimerkkeinä muille sekä auttaa ravitsemukseen liittyvien harhaluulojen torjumisessa.
