Aims: To investigate how training intensity influences HRQoL in people classified as obese, if HRQoL changes are related to changes in weight or aerobic capacity, and to define minimal clinically important difference (MCID).Methodology: Participants (BMI ≥35 kg/m2) randomized into high-intensity training (n = 49), moderate intensity training (n = 39), or no training (n = 22), completed the SF-36 questionnaire, performed a maximal exercise test, and were weighed, before and after a 16-week intervention.Major findings: High-intensity training showed a significantly greater increase in Physical Summary Scale (PCS), Physical Functioning (PF) and General Health (GH) compared to control, and in Vitality (VT) compared to moderate intensity. Within-group analysis showed that high-intensity training improved PCS 2.0 (0.0–4.4) points (mean (95%CI)), significantly improved mental summary scale (MCS) 3.8 (1.0–6.4) points (mean (95%CI)), PF, GH, VT, mental health, aerobic capacity, and reduced body weight. Moderate intensity training significantly increased PF, GH, aerobic capacity, and reduced weight. No correlations were found between changes in HRQoL and changes in weight or changes in aerobic capacity.Conclusion: High-intensity training improved HRQoL evaluated with SF-36. HRQoL changes were not correlated with changes in weight or aerobic capacity. Proposed MCIDs are 1.3 points for PCS and 2.0 points for MCS. (Less)

Since its creation nearly a decade ago, the Digital Curation Centre (DCC) Curation Lifecycle Model has become the quintessential framework for understanding digital curation. Organizations and consortia around the world have used the DCC Curation Lifecycle Model as a tool to ensure that all the necessary stages of digital curation are undertaken, to define roles and responsibilities, and to build a framework of standards and technologies for digital curation. Yet, research on the application of the model to large-scale digitization projects as a way of understanding their efforts at digital curation is scant. This paper reports on findings of a qualitative case study analysis of Indiana University Bloomington’s multi-million-dollar Media Digitization and Preservation Initiative (MDPI), employing the DCC Curation Lifecycle Model as a lens for examining the scope and effectiveness of its digital curation efforts. Findings underscore the success of MDPI in performing digital curation by illustrating the ways it implements each of the model’s components. Implications for the application of the DCC Curation Lifecycle Model in understanding digital curation for mass digitization projects are discussed as well as directions for future research.

The purpose of this study was to compare oxygen uptake estimated from submaximal Astrand tests (VO2est) with measured peak oxygen uptake (VO2peak) from maximal tests, and to compare estimated maximal HR (HRest) with measured HRpeak in obese subjects. Seventy obese subjects performed a maximal exercise test starting with a 4-min period of constant load. VO2peak and HRpeak were measured. VO2est was based on HR at constant load. HRest was calculated by the formula 220 − age. The mean± SD of VO2est 2.05 ± 0.39 l/min (20 ± 5 ml/kg/min) was lower than VO2peak 2.42 ± 0.57 l/min (17 ± 4 ml/kg/min) (p < 0.001, p < 0.001, respectively). The mean difference ± 2SD was − 0.4 ± 1.22 l/min (− 3 ± 10 ml/kg/min) with an uneven distribution of the individual variation. The correlation between the methods was low (r = 0.24, r = 0.38 respectively). The mean± SD of HRest 176 ± 10 bpm was higher than the HRpeak 161 ± 17 bpm (p < 0.001). We describe IoTCloud, a platform to connect smart devices to cloud services for real time data processing and control. A device connected to IoTCloud can communicate with real time data analysis frameworks deployed in the cloud via messaging. The platform design is scalable in connecting devices as well as transferring and processing data. With IoTCloud, a user can develop real time data processing algorithms in an abstract framework without concern for the underlying details of how the data is distributed and transferred. For this platform, we primarily consider real time robotics applications such as autonomous robot navigation, where there are strict requirements on processing latency and demand for scalable processing. To demonstrate the effectiveness of the system, a robotic application is developed on top of the framework. The system and the robotics application characteristics are measured to show that data processing in central servers is feasible for real time sensor applications.

A comprehensive assessment of Quality of Life (QOL) in the Marine Corps, using a life domain framework, in which were included the domains of residence, neighborhood, leisure and recreation, health, friends and friendships, marriage/intimate relationship, relations with children, relations with other relatives, income/standard of living, job, and self, was conducted in 1993. This report focuses on data for COMCABEAST only. Global quality of life was shown to have significant effects on organizational outcomes. Junior enlisted personnel were more negative on all measures of global QOL. Detailed discussions of each life domain are included.

1 A detergent-solubilized pig intestinal microvillus aminopeptidase (microsomal aminopeptidase, EC 3.4.11.2) was purified approximately 150 times with a yield of about 20% by a rapid and simple immunoadsorbent technique. The purified enzyme was shown to be homogeneous by crossed immunoelectrophoresis and by polyacrylamide gel electrophoresis in dodecyl sulphate. Furthermore it was essentially free from other known peptidases and disaccharidases. 2 By charge-shift crossed immunoelectrophoresis it was demonstrated that the hydrophilic form was continuously generated both during a more time-consuming purification procedure and during storage. The development of a rapid purification procedure allowed the preparation of an amphiphilic form free from the hydrophilic one, which could be stabilized by 0.5 mM HgCl2. 3 Intestinal aminopeptidase isolated from a pig having the pancreatic duct ligated three days before sacrifice migrated in polyacrylamide gel electrophoresis in dodecyl sulphate as a single polypeptide chain with a molecular weight of 162000. In contrast, the enzyme from a pig with intact pancreatic duct when purified even in the presence of Kunitz trypsin inhibitor consisted of three polypeptide chains (molecular weights 162000, 123000 and 61000). Trypsin treatment in vitro converted the large polypeptide chain into the two smaller ones. It is suggested that the membrane-bound enzyme after biosynthesis is modified by trypsin and/or chymotrypsin. 4 The conversion of the amphiphilic form to the hydrophilic one and the hydrolysis of the large polypeptide chain into the two smaller ones represented different reactions.

Overall transvascular escape rate of albumin [TERalb, i.e. the fraction of intravascular mass of albumin (IVMalb) passing to the extravascular space per unit time] was determined from the disappearance of i.v. injected radioiodinated serum albumin. Patients with tense ascites due to liver cirrhosis and pigs with posthepatic portal hypertension and intraperitoneally instilled fluid were studied before and after abdominal paracentesis in order to evaluate the effect of ascitic fluid hydrostatic pressure on the transvascular escape rate of albumin. TERalb of the ascitic patients (n = 6) were on average 7.8% IVMalb.h-1, which is somewhat higher but not significantly above normal (mean 5.6% IVMalb.h-1). After paracentesis and removal of the ascitic fluid, TERalb rose significantly to an average of 11.9% IVMalb.h-1 (P less than 0.05). The fraction of IVMalb passing into the peritoneal cavity was on average 0.21 and 0.38% IVMalb.h-1 before and after paracentesis, respectively (n = 4). In ascitic, posthepatic portal-hypertensive pigs (n = 6), TERalb averaged 15.1% IVMalb.h-1, a value not being significantly different from non-ascitic, normotensive controls (mean 16.1% IVMalb.h-1). After paracentesis, TERalb rose significantly to an average of 24.3% IVMalb.h-1. The increased albumin extravasation rate after removal of ascites is best explained by an increased sinusoidal-tissue pressure difference caused by a decreased hydrostatic fluid pressure in the liver interstitium (portal and subcapsular spaces) due to the hydrostatic effect of the removed ascitic fluid.

Transvascular escape rate of albumin [TERalb, i.e. the fraction of the intravascular mass of albumin (IVMalb) passing to the extravascular space per unit time] was determined from the disappearance of i.v. injected radioiodinated serum albumin in anaesthetized pigs during control conditions and during regional venous congestion in the infradiaphragmatic area. Balloon catheters were placed in the portal vein (infrahepatic portal congestion) and in the inferior vena cava above (suprahepatic caval congestion) and below (infrahepatic caval congestion) the outlets of the hepatic veins. TERalb was on the average 13% IVMalb.h-1 under basal pressure conditions. TERalb rose significantly (p less than 0.01) during suprahepatic caval and infrahepatic portal congestion to an average of 29 and 19% IVMalb.h-1, respectively. TERalb was positively correlated to the portal pressure (r = 0.75, P less than 0.001). Only a minor increment in TERalb was found during infrahepatic caval congestion. The hepatic share of the increased TERalb during stasis above the hepatic veins was estimated to be threefold that of the extrahepatic splanchnic area. Our results point to filtration of protein, predominantely through the lining of the sinusoids and perisinusoidal space of the liver into the interstitial space around the portal vessels and further into the lymphatics, as the main mechanism of the previously demonstrated marked increase in TERalb in patients with portal venous hypertension due to cirrhosis of the liver.

Stricture formation after transurethral prostatectomy was studied in a randomized clinical trial including 185 patients. The patients were allocated to either a 2-day urethral catheter-dilation or internal Otis urethrotomy and to an operation with either a polytetrafluoroethylene coated or an uninsulated metal resectoscope. Urethral stricture was defined as an obstruction resulting in a maximum urine flow rate of less than 15 ml. per second and not permitting the passage of a 21F cystoscope. The frequency of urethral strictures was significantly lower after Otis internal urethrotomy (4 per cent) than after a 2-day urethral catheter dilation (16 per cent). The incidence of stricture formation was similar in patients operated on with a polytetrafluoroethylene coated (8 per cent) and with an uninsulated metal (12 per cent) resectoscope sheath. The incidence of stricture formation was unrelated to age, duration of preoperative and postoperative catheterization, operating time and presence of urinary tract infection.

The ocular function of 14 non-alcoholic, high icteric patients with recent occlusion of the common bile duct and 3 patients with viral hepatitis with a cholestatic pattern was studied. By means of a colour vision test panel including the Farnsworth-Munsell 100-hue test, 12 patients were initially classified as colour defective with a pattern of acquired colour vision deficiency (ACVD), predominantly of a tritan type. Visual acuity, visual field, slit lamp microscopy, intraocular pressure, ophthalmoscopy and tear secretion tests were normal, and all patients had normal levels of serum vitamin A. Retesting of 4 initially colour defective patients after disappearance of the obstructive jaundice showed a complete normalisation of the ACVD's. It is concluded, that the colour perception in patients with obstructive jaundice is related to the serum bilirubin level, and not to a deficiency of vitamin A.

In a prospective randomized study 8 patients were peroperatively allocated to closure of ureterotomies with a fibrin adhesive (Tisseel) and 8 patients to closure with Dexon sutures to evaluate whether the duration of postoperative urinary leakage could be diminished by the use of fibrin adhesive. The median drainage period was 6.5 days (range 3-15 days) and 5.5 days (2-16 days), respectively. Closure of ureterotomies with a fibrin adhesive cannot be recommended.
