Abstract

In the decommissioning of nuclear facilities, it is necessary to assess the radioactivity inventory for waste classification and management. For nuclear fusion experiments or accelerator-driven neutron sources, the main contribution to the radiological inventory comes from activation products and tritium. When water is used as coolant of components, it may be activated by neutrons with the consequent production of tritium, and short-lived oxygen and nitrogen isotopes. In most cases several chemical elements may be in solution or suspension in water, because of metallic pipes corrosion when occurring, or functional reasons, e.g. chemical conditioning, addition of antifreeze agent, etc. In this case other activation products could be present and need to be characterized. In this case study, a sample of the coolant of the ENEA Frascati Neutron Generator (FNG) has been radiologically, chemically and physically characterized. In such a facility, during the operation, a beam of deuterons is accelerated up to 300 keV and impacts on a tritiated target for producing 14 MeV neutrons exploiting the fusion reaction T(d,n)α. To avoid overheating in the area around the target, a water-cooling loop is used. After several years of operation, a sample of the fluid has been extracted to undergo a qualitative and quantitative analysis of the radionuclides inventory. Gamma spectrometry has not shown any presence of gamma emitters. On the other hand, Liquid Scintillation Counting (LSC) has provided evidence of the presence of beta emitters. It is known that tritium is expected to occur in the sample because of releases from the tritiated titanium target. This work concerns the development of a method to discriminate the tritium contribution with respect to the total beta activity within the FNG coolant. Since it is known that an unspecified percentage of an unknown anticorrosive agent was added to the water coolant, vacuum distillation has been used to extract purified water (containing only tritium) from the coolant sample leaving any (activated) non-volatile compound in the undistilled fraction. After distillation, any fraction has been investigated by Raman spectroscopy, to determine: a) the chemical composition of the colored anticorrosive agent, b) the efficiency of the separation. Afterwards, both fractions have been analysed by means of LSC, and results showed that the main contribution to sample activity is due to tritium and the activity values are consistent with the theoretical H atomic concentration in both fractions.

Abstract

The MICADO (Measurement and Instrumentation for Cleaning And Decommissioning Operations) Project of the H2020 Research and Innovation Programme aims to propose a cost-effective solution for non-destructive characterization of nuclear waste, implementing a digitization process that could become a referenced standard facilitating and harmonizing the methodology used for the in-field Waste Management and Dismantling & Decommissioning operations. It employs instruments based on different technologies: an active and passive neutron measurement system, a photo-fission facility, and the ‘Integrated Gamma Station’. Within the MICADO Project, an entire work package has been dedicated to the design and realization of the ‘Integrated Gamma Station’ obtained by combining different gamma detection technologies supporting each other to a comprehensive and effective non-destructive gamma characterization, able to accommodate Radioactive Waste Packages of different sizes. The techniques implemented are i) dosimetry measurements, count rate inspection and raw spectroscopy with the CAEN RadHAND, ii) gamma imaging in open geometry with the CEA Nanopix gamma camera, iii) high resolution gamma spectrometry with the ENEA Tomographic Gamma Scanner (TGS). The latter able to carry out different characterization methodologies, i.e., Open Geometry, Segmented Gamma Scanning, Angular Scanning, and Tomography. This paper describes the layout of the Integrated Gamma Station conceived, its features and detection capabilities, and part of the measurement campaign realized during the MICADO Project.

3. Defected fuel rods identification in TRIGA Reactors: The experience at the ENEA Casaccia TRIGA RC-1 reactor

Abstract

Experience in running TRIGA reactors all around the world has shown that fuel cladding failures can occur. Fission products, especially in gaseous physical form, can exit the defected fuel rods being dispersed within the water primary coolant. Suspects of a cladding failure event can be confirmed by detection of short-lived fission products, i.e., Krypton, Xenon, and even Iodine isotopes in primary water, or within the ionic-exchange resins tank installed for the purification of the primary coolant loop. The magnitude of the release of those ‘key-indicators’ from the defected fuel element(s) is the driving method by which the defected rod(s) can be identified. ‘Significant’ releases can be detected with a direct online sampling of the water from the top of the suspected fuel rod with reactor at power, leading this water to an online high-resolution gamma spectrometry analysis system. By considering delays due to the water velocity in tubes and decay time of radionuclides identified within the gamma spectrum, it is possible to calculate the concentration of those radionuclides just emerging from the inspected fuel rod. Releases lower than the minimum detection capabilities of the previous online experimental configuration push to modify the detection method with an indirect identification of the release. This is the case when the ‘normal’ radioactive background of the activated water, when reactor is on power, is the dominant component in the gamma spectrum of the sampled water, and fission gases (even produced) are not identified promptly, i.e. a relationship to a specific fuel rod by the sampling circuit before could not be identified. The paper describes the experience carried out at the Italian ENEA TRIGA RC-1 reactor, deepening the technical aspects and solutions applied to solve the issue. In particular, a ‘significant’ release has been found in the instrumented fuel rod within ring B, i.e. the inner fuel ring, exposed to the maximum neutron flux of the reactor. The leaking element was found within a week, in two days of operation, being the sampling system designed on detection of minutes-shortlived fission gases (no need to ‘cool’ down the primary loop by waiting decay of hours-lived radionuclides). After the removal of the instrumented fuel rod in ring B, further days in searching other ‘significant’ release with the sampling circuit before have reported nothing detected. But in samples of water taken after reactor shutdown, some iodine emerged after decaying of ‘normal’ radioactive background of the activated water. This was sufficient evidence of another defected fuel rod in the pool. Identification of the latter defected fuel element is still ongoing, being based on: 1) selective removal of rod(s) from the reactor core, 2) run power of the reactor, 3) take samples of water after shutdown and measure iodine after decaying of ‘normal’ radioactive-background of the activated water, 4) identify the rod responsible of the remaining leakage when no further iodine is detected.

Abstract

An accelerator-driven 14 MeV neutron source of new concept, denominated SORGENTINA-RF, will be installed in ENEA Brasimone Research Centre, to test the feasibility of producing radionuclides of medical relevance using fusion neutrons. The main goal of the facility is generating 99Mo as precursor of 99mTc, a radionuclide widely used in nuclear medicine diagnostic procedures, using the 14 MeV fusion neutrons produced by the plant. This work describes the study performed for the design of a proper shielding structure that aims at fulfilling the requirement of 0.01 mSv/h dose rate limit on the external surface of the shielding during beam-on operations. The proposed shielding consists of a layered structure composed of 2 m standard concrete and 1 m baritic concrete. The design is still in the preliminary phase to assess the feasibility and the economic issues as well as structural impact of the shielding structure.

Abstract

Background: Presenting symptoms of childhood cancers might mimic those of rheumatic diseases. However, the evidence available to guide differential diagnosis remains scarce. Preventing wrong or delayed diagnosis is therefore important to avoid incorrect administration of glucocorticoid or immunosuppressive therapy and worsening of prognosis. As such, we aimed to assess the prevalence and characteristics of presenting musculoskeletal manifestations in patients at cancer onset and to identify the factors that differentiate childhood malignancies with arthropathy from juvenile idiopathic arthritis. Methods: We did a multicentre, cross-sectional study at 25 paediatric haemato-oncology centres and 22 paediatric rheumatology centres in Italy. We prospectively recruited patients who were younger than 16 years that were newly diagnosed with cancer or juvenile idiopathic arthritis. We excluded patients with glucocorticoid pre-treatment (>1 mg/kg per day of oral prednisone or equivalent for ≥2 consecutive weeks). We collected data for patients with a new diagnosis of cancer or juvenile idiopathic arthritis using an electronic case report form on a web-based platform powered by the Cineca Interuniversity Consortium. The primary outcome was to describe the frequency and characteristics of musculoskeletal manifestations at cancer onset; and the secondary outcome was to identify factors that could discriminate malignancies presenting with arthropathy, with or without other musculoskeletal symptoms, from juvenile idiopathic arthritis using multivariable logistic regression analysis. Findings: Between May 1, 2015, and May 31, 2018, 1957 patients were eligible, of which 1277 (65%) had cancer and 680 (35%) had juvenile idiopathic arthritis. Musculoskeletal symptoms occurred in 324 (25% [95% CI 23·0–27·8]) of 1277 patients with cancer, of whom 207 had arthropathy. Abstract

In Italy, malaria continues to be one of the most common imported parasitoses; therefore, continuous surveillance of epidemiological data and clinical management is needed. In 2016, the National Institute for Infectious Diseases 'Lazzaro Spallanzani' in Rome promoted a retrospective questionnaire-based survey to assess the clinical management of imported malaria cases in Italy in 2015. The questionnaire was sent to 104 Tropical and/or Infectious Diseases Units in the country, and 37 of them filled out and returned the questionnaires. A total of 399 malaria cases were reported in 2015, mostly caused by Plasmodium falciparum and imported from Africa. Malaria chemoprophylaxis was correctly used by a minority of patients. Most patients presented with uncomplicated malaria and were treated orally. In severe cases, intravenous artesunate or quinine alone or in combination were administered, although one third of these severe cases received oral treatment. This retrospective survey reveals a lack of homogeneity in management of malaria-imported cases in Italy. Improvement of malaria chemoprophylaxis, standardization of clinical management of malaria cases and harmonization of oral and intravenous drug availability are needed throughout the country.

Abstract

We report an unusual and rare case of infection from methicillin resistant Staphylococcus aureus (MRSA) producing Panton-Valentine leukocidin in a preterm neonate in NICU. On day of life 8, a preterm baby boy suddenly developed arthritis, giant cutaneous abscesses and an osteomyelitic focus with pour clinical condition. This very aggressive presentation of infection from MRSA push us to test Panton- Valentine leukocidin resulted positive and to test contacts to discover the bearer of the germ. MRSA producing Panton-Valentine leukocidin is an unusual case of infection in preterm neonate that has not been reported elsewhere. A very aggressive sepsis in neonates from Staphilococcus aureus should evoke the need to test Panton-Valentine leukocidin to rapidly establish an appropriate treatment. Abstract

Prophylactic low molecular weight heparin (pLMWH) is currently recommended in COVID-19 to reduce the risk of coagulopathy. The aim of this study was to evaluate whether the antinflammatory effects of pLMWH could translate in lower rate of clinical progression in patients with COVID-19 pneumonia. Patients admitted to a COVID-hospital in Rome with SARS-CoV-2 infection and mild/moderate pneumonia were retrospectively evaluated. The primary endpoint was the time from hospital admission to orotracheal intubation/death (OTI/death). A total of 449 patients were included: 39% female, median age 63 (IQR, 50–77) years. The estimated probability of OTI/death for patients receiving pLMWH was: 9.5% (95% CI 3.2–26.4) by day 20 in those not receiving pLMWH vs. 10.4% (6.7–15.9) in those exposed to pLMWH; p-value = 0.144. This risk associated with the use of pLMWH appeared to vary by PaO2/FiO2 ratio: aHR 1.40 (95% CI 0.51–3.79) for patients with an admission PaO2/FiO2 ≤ 300 mmHg and 0.27 (0.03–2.18) for those with PaO2/FiO2 > 300 mmHg; p-value at interaction test 0.16. pLMWH does not seem to reduce the risk of OTI/death mild/moderate COVID-19 pneumonia, especially when respiratory function had already significantly deteriorated. Data from clinical trials comparing the effect of prophylactic vs. therapeutic dosage of LMWH at various stages of COVID-19 disease are needed.

Abstract

Background: Coronavirus disease 2019 (COVID-19), the disease caused by the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that originated in China in December 2019, was recently recognized as pandemic threat by the World Health Organization, with the potential of rapidly overloading health care systems and causing substantial mortality worldwide (www.who.int/dg/speeches/detail/who-director-general -s-opening-remarks-at-the-media-briefing-on-covid-19 —11-march -2020). Human-to-human transmission occurs mainly through respiratory droplets, but other routes are under investigation, because SARS-CoV-2 has been detected in several body fluids (1). So far, few data are available on ocular samples from patients with COVID-19, although conjunctivitis has been occasionally reported among COVID-19 symptoms, similar to infections caused by other human coronaviruses (2, 3). During the SARS epidemic, eye exposure to infectious fluids was associated with an increased risk for SARS-CoV transmission to health care workers (3, 4). Although SARS-CoV RNA was occasionally found in ocular specimens during the early phase of illness, its infectivity is unknown (2, 3). With regard to COVID-19, unprotected ocular exposure was thought to be responsible for infections that occurred in the Wuhan Fever Clinic in January 2020 (3, 4); in addition, SARS-CoV-2 RNA was detected in conjunctival secretions collected from the only patient with conjunctivitis out of 30 patients with COVID-19 from a hospital in China (5). However, further studies are needed to evaluate the infectious potential of the SARS-CoV-2 RNA detected in the ocular specimens and to determine whether transmission may occur through ocular secretions (3, 4). Objective: To present the early detection of infectious SARS-CoV-2 in ocular fluids from a patient with the first confirmed case of COVID-19 in Italy, who had been hospitalized at the National Institute for Infectious Diseases “L. Spallanzani” (INMI) in Rome. Methods and Findings: The patient, a 65-year-old woman, travelled from Wuhan, China, to Italy on 23 January 2020 and was admitted on 29 January 2020, 1 day after symptom onset. At admission to the high isolation unit at INMI, she presented with nonproductive cough, sore throat, coryza, and bilateral conjunctivitis. She had no fever until day 4, when fever (38 °C), nausea, and vomiting began. Infection with SARS-CoV-2 was confirmed by performing real-time reverse transcription polymerase chain reaction (RT-PCR) assay on sputum samples (cycle threshold value [Ct], 16.1) on the admission day, followed by viral M gene sequencing (GenBank accession number MT008022), and virus isolation on Vero E6 cell line (2019-nCoV/Italy-INMI1). The full genome sequence was obtained from either clinical sample or culture isolate (GISAID accession numbers EPI_ISL_410545 and EPI_ISL_410546). At admission, no other respiratory infections were detected (QIAstat-Dx Respiratory Panel; Qiagen).

Abstract

Little is known about long-term maintenance of virologic suppression in HIV migrants in Italy. The study aims to compare virologic failure rates and associated factors among antiretroviral therapy (ART)-naïve migrants and natives enrolled in the ARCA database since 2007 who achieved virologic suppression within 18 months from the beginning of the ART. Kaplan-Meier method assessed the probability of virologic suppression and failure. Cox regression model was used for multivariate analysis. Of 2515 patients, 2020 (80.3%) were Italian, 286 (10.6%) migrants from low-income countries, of whom 201 (75.0%) from Africa, and 227 (9.0%) from high-income-countries. The median follow-up was 4.5 years (IQR 2.5–7). No difference was observed in the time of achievement of virological suppression in the three groups (log-rank: p = 0.5687). Higher probability of virologic failure was observed in Africans compared to Italians, to patients from high-income-countries and from low-income-countries other than Africans (Log-rank = p < 0.001). In the adjusted analysis, a higher virologic failure risk was found in Africans only compared to Italians. [HR 4.01; 95% CI 2.44–6.56, p < 0.001]. In Italy, African migrants are less likely to maintain virologic suppression compared to natives and other migrants. Targeted interventions could be needed for foreigners, especially for Africans.

Abstract

Glyphosate (CAS 1071-83-6) is one of the most widely applied herbicides sprayed against emerged annual, perennial and biennial weeds in all crops including root and tuber vegetables, pulses, oil seeds, and cereals. A new Flow Injection - Tandem Mass Spectrometry (FI-MS/MS) method for the determination of Glyphosate in different matrices was developed and validated. For FI-MS/MS analysis, milled samples were extracted with deionized water, then the extract was cleaned-up by passing it through a polymeric solid phase extraction column. The purified extract was diluted with deionized water prior to FI-MS/MS analysis. FI-MS/MS analysis was performed on a triple quadrupole detector operating in multiple reaction monitoring. Validation data for FI-MS/MS method were satisfactorily compared with acceptability criteria established by Document No. SANTE/11813/2017. To confirm the trueness of data obtained with the FI-MS/MS method, described above, incurred samples, previously characterized for glyphosate by QuPPe method, were analyzed. The applied QuPPe method, was validated according to SANTE/11813/2017 and accredited ISO 17025. Chromatographic separation was conducted by Hypercarb(TM) (100×2.1 mm; 5 ?m) column from Thermo Scientific (Waltham, MA, USA). Incurred chickpea (n=5) and wheat samples (n=6) were selected among samples from official controls of imported products performed by Istituto Zooprofilattico Sperimentale of Umbria and Marche Regions. In particular, chickpeas and wheat samples were from outside EU (Canada, Ukraine, Iran, Russian Federation, USA) and were collected in different ports in central and southern Italy. For incurred chickpea samples a Glyphosate contamination ranging from 16 to 36 mg/Kg was found with both methods, whereas for wheat samples glyphosate concentration ranged from 0.3 to 2 mg/Kg. Good agreement between results obtained by the two methods indicated the FI-MS/MS approach as a valuable alternative to QuPPe method for rapid Glyphosate detection, ensuring high quality of the generated data, and therefore appearing suitable for broad monitoring programs as well as risk assessment purposes.

Abstract

Objectives This study aims to identify the risk factors for intensive care (IC) in severe malaria patients admitted to the "Lazzaro Spallanzani" National Institute for Infectious Diseases, Rome, Italy. Methods All patients with confirmed severe malaria and hospitalized between 2007 and 2015 were included in the analysis and stratified into two groups: those requiring IC and those who did not. Five prognostic malaria scores were estimated; clinical severity at IC unit admission was assessed using the Sequential Organ Failure Assessment and the quick-Sequential Organ Failure Assessment scores. Univariate and multivariate analysis were performed to assess factors independently associated to IC. Results A total of 98 severe malaria patients were included; 10 of them required IC. There were no deaths or sequelae. Patients requiring IC had higher severity scores. At the multivariate analysis, only the number of World Health Organization criteria and the aspartate aminotransferase value were independently associated with the need of IC. Conclusions An early and accurate assessment of the severity score is essential for the management of severe malaria patients.

Abstract

Nel presente lavoro si esamina il caso ipotetico di un incendio sviluppatosi in un laboratorio di ricerca in cui sono presenti quattro scatole a guanti, nelle quali sono avvenute lavorazioni di fabbricazione di combustibile a ossidi misti U-Pu Mixed OXide (MOX). L’analisi parte dalla valutazione del rischio incendio, che si basa sulla quantificazione del carico di incendio all’interno del laboratorio e tiene inoltre conto dell’efficacia dei presidi antincendio presenti (impianti di rilevazione, presidi mobili di spegnimento, impianti automatici di spegnimento). L’incendio, coinvolgendo le quattro scatole a guanti, rappresenta una potenziale fonte di contaminazione radioattiva con dispersione di particolato. Sulla base di questi presupposti si valuta la dose efficace impegnata ai gruppi di riferimento dei lavoratori (secondo i requisiti dell’art. 115-ter D.Lgs. 230/95) in relazione alla specifica granulometria del particolato contenuto nell’aerosol, stimando la velocità di rilascio, la concentrazione del contaminante in aria e la frazione respirabile del particolato. Specifica attenzione è rivolta al compartimento polmonare che, nello scenario incidentale considerato, rappresenta il distretto più critico in termini di potenziale conferimento di dose al soggetto esposto, a seguito di inalazione del particolato radioattivo
