Abstract

Dermatitis artefacta or factitious disease may be unrecognized in children. We present a 12-year-old girl who had an unusual facial lesion on the chin, which was self-inflicted but histologically mimicked cutaneous T-cell lymphoma. Our report emphasizes both the potential diagnostic pitfalls and the importance of clinicopathologic correlation.

3. Geometric Cutaneous Melanoma: A Helpful Clinical Sign of Malignancy?

Abstract

Background. Malignant melanomas change shape in a random pattern, with ovoid, crescentic, or nodular shapes seen most frequently. We have observed a number of malignant melanomas that have presented with a geometric, angular shape and have noted that pigmented lesions with this configuration are often found to be malignant. We present 20 patients with malignant melanoma whose lesion displayed a geometric, angular shape. Methods. Before excision for formal histopathology, all lesions were scored using the seven-point checklist and ABCDE systems and were divided into low-risk or significant risk of melanoma. Results. Five different geometric shapes were observed. Depending on the scoring system employed, 20% to 40% of the geometric melanomas were considered to be of low risk of malignancy. Conclusion. The development of geometrical angular patterns in a malignant melanoma may represent a morphologic growth pattern that can be used as a clinical risk sign. Even apparently benign low-risk lesions with a geometric shape may pose a significant risk of malignant melanoma. By definition, the majority of lesions that are morphologically geometric are symmetrical in shape, which is more in favor of a benign diagnosis. This may increase the likelihood that early cutaneous melanomas with a geometric shape may be missed. Any pigmented lesion with a geometric configuration should raise the clinician's suspicion of malignancy even if considered otherwise to be of low risk by the standard melanoma checklists.

Abstract

We report an infant with severe combined immunodeficiency (SCID) who had cutaneous manifestations of acute graft-versus-host disease (GVHD) due to maternofetal lymphocyte engraftment. Histologic and ultrastructural examinations of the skin revealed intracellular vacuoles resembling lipid droplets in the epidermis and dermis, which is not a recognized feature of acute GVHD or SCID.

Abstract

background. Patients with severely damaged skin are more likely to develop skin malignancies. However, suturing following excision of tumors is difficult due to their fragile skin. objective. To evaluate the effectiveness of a technique using butyl-2-cyanoacrylate tissue adhesive in split skin grafts in patients with severely damaged skin. method. The technique was used in 19 patients with 24 skin tumors. Two patients also had multiple skin biopsies to assess the biodegradability of the adhesive. results. Split skin grafts were applied successfully in all patients. The adhesive was completely absorbed without a foreign body reaction within 6 months. conclusion. We recommend the use of butyl-2-cyanoacrylate in split skin grafting patients with severely damaged skin.

Abstract

Investigation of 92 patients with discoid lupus erythematosus, manifested initially by localized cutaneous lesions only, showed abnormal laboratory test results for 57 patients (62%) on admission and for 62 patients (67.4%) on review 16 to 20 years later. Patients with discoid lesions confined to the head and neck (DLE) showed fewer laboratory abnormalities than those patients with disseminated lesions involving trunk and limbs (disseminated discoid lupus erythematosus [DDLE]). Systemic lupus erythematosus (SLE) eventually developed in six (6.5%) of the patients, and all had shown persistent multiple abnormal laboratory findings from the beginning. Complete remission occurred in 46.7%. A persistent positive antinuclear factor of either speckled or homogeneous pattern with a titer greater than 1:50, leukopenia, thrombocytopenia, or a false-positive Wassermann reaction indicated those patients who may progress to DDLE or SLE.
