11. Successful medical management of Acanthamoeba keratitis

Abstract

Acanthamoeba keratitis is one of the most difficult corneal infections to treat. It is potentially devastating, but does not have a universally poor prognosis. The predominant risk factor for its development is the use of contact lenses. Contamination of the contact lens system by bacteria or fungal elements may be important for the survival of the Acanthamoeba. The symptoms of Acanthamoeba keratitis include severe eye pain and photophobia (abnormal sensitivity to light). In order to better understand the role of medical and surgical treatment, seven patients with this disease are reported. All patients were treated with extended and intensive triple anti-amoeba therapy: local application of neomycin-polymyxin B-gramicidin, propamidine isethionate and miconazole nitrate. Five patients, in addition to the above, also received locally applied steroids. This medical treatment cured six of the seven patients; this was true even when four of the cases did not receive treatment until five or more weeks after onset of symptoms. Although use of steroids may allow the organism to penetrate the cornea more deeply, in these cases the steroids were introduced carefully and were not associated with any clinical deterioration. One patient required corneal transplant (penetrating keratoplasty) to eliminate the infection. Two patients, cured of their infection by medical treatment, later underwent corneal transplant to improve their vision. It is concluded that extensive and intense medical treatment may be effective in treating Acanthamoeba keratitis. (Consumer Summary produced by Reliance Medical Information, Inc.)

16. Endothelial Rejection in Human Transplants

Abstract

The clinical appearance of endothelial rejection following penetrating keratoplasty was studied in three human cases. On the posterior surface of each graft was an endothelial rejection line formed by keratitic precipitates. In two cases, the rejection process was reversed with corticosteroids, while in one case, the endothelial rejection line progressed across the back of the graft, destroying the transplanted endothelium.This latter graft became available for study five weeks after the onset of rejection. Routine histologic and scanning electron microscopic examination of this specimen showed nearly complete destruction of endothelial cells and early retrocorneal membrane formation.The presence of an endothelial rejection line proved a common helpful sign in the diagnosis of early graft rejection.

25. Problems With Punctal Plugs

Abstract

TO THE EDITOR. —Punctal occlusion has proved to be a valuable adjunct in the treatment of moderate to severe dry-eye syndrome. Recently, silicon plugs have become available for reversible occlusion of the lacrimal punctum and canaliculus.1 Although this device has proved beneficial to many cases in our experience, its use is not without some potential hazard, as the following two case reports will indicate. REPORT OF CASES. —Case 1.—A 29-year-old woman suffered from moderate dry-eye syndrome that caused irritation, particularly of the right eye. Since she was helped by temporary occlusion of her canaliculi with collagen inserts, we elected to perform punctal occlusion using a punctal plug for her lower right punctum. The lower lid was grasped with forceps in the recommended manner and the punctum was dilated with the punctal dilator supplied by the manufacturer.
