Objective To investigate the effect of f fine nursing management for patients undergoing simultaneous bilateral lobectomy. Methods Totally 52 patients under simultaneous bilateral lobectomy were randomly divided into the control group and observation group, with 26 cases in each group. The control group received routine nursing interventions during perioperative period. The observation group was given fine nursing management. The indwelling time of catheter, length of postoperative hospital stay, hospitalization costs and postoperative complications were recorded. Results The indwelling time of drainage tube and length of postoperative hospital stay in the observation group were shorter than those in the control group(P＜0. 05). There was a case of pneumothorax, 1 case of stage I pressure injury, 5 cases of arrhythmia, 2 cases of breathing difficulties and 2 cases of incision infection in the observation group. There was a case of pneumothorax, 3 cases of stage I pressure injury, 5 cases of arrhythmia, 1 case of breathing difficulties, 5 cases of incision infection and 1 case of death in the control group. Conclusion Implementation of fine nursing management is effective to shorten indwelling time of drainage tube and length of postoperative hospital stay of patients undergoing simultaneous bilateral lobectomy. (目的 探讨同期双侧肺叶切除围手术期实施精细化护理干预的效果。方法 选取2015年1月—2021年12月医院收治的行同期双侧肺叶切除患者52例, 随机分为对照组和观察组, 各26例。对照组围手术期采取常规护理, 观察组围术期采取精细化护理干预。记录和分析两组患者引流管留置时间、术后平均住院日、住院费用和术后并发症。结果 观察组患者引流管留置时间、术后平均住院日较对照组缩短, 差异有统计学意义(P＜0. 05)。观察组术后发生气胸1例, I期压疮1例, 心律失常5例, 呼吸困难2例, 切口感染2例; 对照组术后气胸1例, I期压疮3例, 心律失常5例, 呼吸困难1例, 切口感染5例, 死亡1例。结论 同期双侧肺叶切除围手术期实施精细化护理干预, 能缩短患者引流管留置时间和术后平均住院日, 对促进患者术后康复具有积极意义。)

To analyze the potential causes of peritoneal dialysis-associated refractory peritonitis and summarize related nursing measures. The clinical treatment and nursing process of two cases of peritoneal dialysis-associated refractory peritonitis were retrospectively analyzed. The operating procedure of peritoneal dialysis was not strictly followed in these two patients with slim body type and hypoproteinemia, and treatment was delayed due to insufficient awareness about peritoneal dialysis -associated peritonitis. Anti-infective treatment was administrated for intra-abdominal and peripheral infection, and treatment was switched to hemodialysis as no obvious improvement of bacteriological examination results. The treatment environment, operation procedure, intestinal factor and awareness of disease were main factors influencing the peritoneal dialysis -associated refractory peritonitis, and efforts should be made to enhance the self-management skills of peritoneal dialysis patients. (探讨腹膜透析相关性难治性腹膜炎潜在原因及拔管后相应的护理措施。回顾性分析科室2例难治性腹膜炎患者的临床诊疗及护理经过, 总结疾病临床特征及护理经验。2例患者均体型偏瘦, 且存在低蛋白血症, 日常未严格按操作规程进行, 同时未引起对腹膜透析相关性腹膜炎的重视度, 导致延时就诊。入院后给予腹腔及外周抗感染治疗后10～20 d, 腹腔细菌学检查无明显改善, 给予拔管, 改行血液透析。环境、操作、肠道原因、营养状态、对腹膜炎的认识及重视度是导致难治性腹膜炎发生的原因, 应加强对腹膜透析患者的自我管理。)

The emerging challenges of global health constantly puts forward new requirements for cultivating medical talents. From the perspective of great health, the First Affiliated Hospital of Sun Yat-sen University aims at cultivating top-notch and innovative medical talents for the future. Since 2018, the hospital has been inheriting and innovating the model of training medical elites and has explored and implemented the clinical postdoctoral program. This program relies on the platform of progressive medical training of the relatively mature 'college system' initiated by our hospital. By regrouping the high-quality interdisciplinary faculty team, the platform optimizes the 'outcome-based' advanced training system and builds effective evaluation and the assessment system, to comprehensively strengthen clinical postdoctoral trainees' abilities of clinical and scientific research and teaching. The program provides reference for the cultivation of high-level medical elites.

Receptor-based biosensors often suffer from slow analyte diffusion, leading to extended assay times. Moreover, existing methods to enhance diffusion can be complex and costly. In response to this challenge, we presented a rapid and cost-effective technique for fabricating concave magnetic-responsive hydrogel discs (CMDs) by straightforward pipetting directly onto microscope glass slides. This approach enables immediate preparation and customization of hydrogel properties such as porosity, magnetic responsiveness, and embedded particles and is adaptable for use with microarray printers. The concave design increased the surface area by 43% compared to conventional hemispherical hydrogels, enhancing diffusion rates and accelerating reactions. By incorporating superparamagnetic particles, the hydrogels become magnetically responsive, allowing for stirring within reagent droplets using magnets to improve mixing. Our experimental results showed that CMDs dissolved approximately 2.5 times faster than hemispherical ones. Numerical simulations demonstrated up to a 46% improvement in diffusion speed within the hydrogel. Particles with lower diffusion coefficients, like human antibodies, benefited most from the concave design, resulting in faster biosensor responses. The increased surface area and ease of fabrication make our CMDs efficient and adaptable for various biological and biomedical applications, particularly in point-of-care diagnostics where rapid and accurate biomarker detection is critical.
